Optimising patient selection for adrenal vein sampling in primary aldosteronism: role of biochemical markers and scoring systems.

BACKGROUND: Adrenal vein sampling (AVS) is the gold standard for subtyping primary aldosteronism (PA), but the role of adrenocorticotropic hormone (ACTH) stimulation during this procedure remains debated. AVS is technically challenging and is available at limited centres; strategies that limit its use to selected patient groups are needed. METHODS: We retrospectively analysed 98 patients with biochemically confirmed PA who underwent AVS between 2018 and 2025. Outcomes were compared between unstimulated (n = 31) and ACTH-stimulated (n = 67) protocols, including bilateral cannulation success, l

Physiological uptake patterns of gastric lymph node stations on 68Ga-pentixafor positron emission tomography (PET): station-specific patterns to aid interpretation.

BACKGROUND: The C-X-C motif chemokine receptor 4 (CXCR4)-targeted tracer, 68Ga-pentixafor, has shown promise for imaging hematologic malignancies including mucosa-assisted lymphoid tissue (MALT) lymphoma involving lymph nodes (LNs). Its physiological uptake patterns in gastric LN stations remain undefined. This retrospective study aimed to evaluate gastric LN positivity in individuals without known malignancy, establish region-specific references, and explore factors influencing uptake. METHODS: Data of patients with hyperaldosteronism who underwent 68Ga-pentixafor positron emission tomograph

Biochemical and clinical remission after adrenalectomy in patients with primary aldosteronism: a retrospective two-center study.

BACKGROUND: Primary hyperaldosteronism (PHA) is a common condition with a prevalence of around 11% in the hypertensive population. For unilateral PHA, the preferred treatment approach is adrenalectomy with the objective to normalize aldosterone levels and its associated harmful effects. However, the reported biochemical outcomes after adrenalectomy vary widely within the literature. This study aimed to evaluate the biochemical and clinical remission rates after unilateral adrenalectomy in patients with PHA. METHODS: We conducted a retrospective study in two academic centers in Switzerland to

Comprehensive evaluation of secondary causes of hypertension: Integrated screening for obstructive sleep apnea and primary aldosteronism.

INTRODUCTION: Obstructive sleep apnea (OSA) and primary aldosteronism (PA) are common and treatable conditions that are often overlooked in the evaluation of hypertension. We assessed the prevalence and relative contribution of identifiable causes of hypertension using a comprehensive diagnostic approach. METHODS AND MATERIALS: In this cross-sectional study, consecutive adults (aged 18-80 years) with hypertension from the endocrinology OPD were screened for secondary causes. All participants underwent standardized blood pressure assessment and systematic evaluation for major causes of seconda

Severe Hypokalemia and Life-Threatening Ventricular Arrhythmias: A Case of Normotensive Primary Aldosteronism.

Primary aldosteronism (PA), the most common cause of secondary hypertension, is characterized by autonomous adrenal aldosterone overproduction independent of the renin-angiotensin system, resulting in sodium retention, potassium wasting, and cardiovascular sequelae that exceed the risk attributable to blood pressure elevation alone. Although hypertension is the predominant clinical manifestation, PA may also present without hypertension or hypokalemia, making atypical presentations a significant diagnostic challenge. Aldosterone-mediated myocardial fibrosis combined with hypokalemia-driven ele

Primary aldosteronism.

Primary aldosteronism (PA) results from excessive aldosterone production by one or both adrenal glands and is an important cause of hypertension, leading to increased cardiovascular and renal morbidities. PA is primarily caused by a spectrum of somatic or germline mutations in aldosterone-driver genes and superimposed aberrant adrenal expression of various G-protein-coupled receptors and their ligands, leading to dysregulated aldosterone production. PA remains underdiagnosed, and simplified testing by measuring renin and aldosterone is recommended in all people with hypertension to maximize th

Pseudo-Hyperaldosteronism After the Ingestion of Licorice Tea: A Case Report.

Severe hypokalemia is a common electrolyte disorder with a broad spectrum of etiologies. We report the case of a 44-year-old woman admitted to the emergency department with nausea, vomiting, chills, and chronic diffuse paresthesia. On admission, the patient was hypertensive. Laboratory evaluation revealed profound hypokalemia (2.3 mmol/L), severe hypophosphatemia, metabolic alkalosis, and electrocardiographic abnormalities, including U waves and a prolonged corrected QT interval, a key marker of severity associated with a high risk of life-threatening arrhythmias. Further investigations demons

Refractory hypokalemia associated with levetiracetam: a case report.

Levetiracetam (LEV) is frequently prioritized in epilepsy management due to its minimal drug-drug interactions and perceived metabolic neutrality. However, its potential involvement in severe electrolyte disturbances remains scarcely documented. We describe a 36-year-old male admitted for status epilepticus and initiated on a dose-escalated LEV regimen (2000 mg/day). The clinical course was complicated by the development of severe, recalcitrant hypokalemia (nadir 2.8 mmol/L). This electrolyte depletion proved remarkably refractory to intensive parenteral and enteral potassium chloride suppleme

Robotic Excision and Pathologic Profiling of Nonfunctioning Juxtaglomerular Cell Tumor of the Kidney: First Reported Case From the Indian Subcontinent.

Juxtaglomerular cell tumor (JGCT), also known as reninoma, is an exceptionally rare benign renal neoplasm arising from modified smooth muscle cells of the afferent arteriole of the juxtaglomerular apparatus. The tumor classically presents in young patients with secondary hypertension due to autonomous renin secretion, hypokalemia, and hyperaldosteronism.  A 56-year-old asymptomatic female with known hypothyroidism was incidentally detected to have a right renal mass on a routine health check-up. Contrast-enhanced computed tomography (CECT) confirmed a 2.7 × 2.5 cm homogeneously enhancing medul

An extrarenal renin-producing tumor associated with a 3 cm mature cystic teratoma.

Extrarenal renin-producing tumors are a cause of secondary hypertension and are most commonly reported to arise in the adrenal gland, lung, and pancreas; however, they rarely occur in the ovary, and renin-producing ovarian tumors are mainly larger than 5 cm or malignant. We report a case of a 3 cm renin-producing ovarian tumor. A 27-year-old woman presented with newly diagnosed hypertension. Laboratory evaluation revealed hyperreninemic hyperaldosteronism. Renal artery ultrasonography and abdominal magnetic resonance angiography showed no renal artery stenosis or adrenal abnormalities, making

Bilateral Adrenal Primary Aldosteronism Is Uniformly Present in Hypertensive Afro-Caribbean First-Generation Immigrants to Western Canada.

BACKGROUND: Uncontrolled hypertension is prevalent in sub-Saharan Africa and a major contributor to premature morbidity. The reasons may not be identical to those reported in studies of Black patients in Western countries. Recent African immigration to western Canada permits the study of hypertension mechanisms in African Black patients. METHODS: Retrospective study of 15 years' experience (2010-2025) with hypertensive first-generation African immigrants to a resistant hypertension clinic in Alberta, Canada. RESULTS: With uniform screening for primary aldosteronism (PA), PA was found in 93%

Clinical phenotypes and disease outcomes of patients with systemic lupus erythematosus meeting indication for primary aldosteronism testing.

BackgroundHypertension is prevalent and contributes significantly to cardiovascular risk in patients with systemic lupus erythematosus (SLE). Primary aldosteronism (PA), the most common endocrine cause of hypertension, confers higher cardiovascular risk than essential hypertension and has been linked to autoimmune diseases including SLE. While the prevalence of PA in SLE is unknown, a third of patients in a tertiary lupus clinic have been demonstrated to have clinical indications for PA testing.ObjectivesTo compare SLE phenotypes and outcomes in patients attending the clinic with and without i

Selective Cell Death of Adrenocortical Cells by Pharmacological Activation of Mutant KCNJ5.

BACKGROUND: Primary aldosteronism is frequently caused by mutations in the KCNJ5 (potassium inwardly rectifying channel subfamily J member 5) gene, encoding a G-protein inwardly rectifying potassium channel. These mutations disrupt the channel's selectivity filter, permitting abnormal sodium influx. We aimed to identify compounds that selectively activate mutant KCNJ5 channels. By further increasing sodium conductance, such compounds could induce lethal sodium influx in mutant adrenal cells, providing a targeted therapeutic strategy for primary aldosteronism caused by KCNJ5 mutations. Primary aldosteronism and obstructive sleep apnea in hypertension: interplay, target organ damage, and clinical management.

Primary aldosteronism (PA) and obstructive sleep apnea (OSA) are common but often unrecognized contributors to hypertension. When they coexist, aldosterone excess, fluid redistribution, intermittent hypoxia, sympathetic activation, and shared metabolic risk may increase hypertension-mediated organ damage. This narrative review examines the evidence linking PA and OSA, with emphasis on mechanisms, cardiovascular and renal consequences, diagnostic limitations, and clinical management. Aldosterone-related sodium retention and nocturnal rostral fluid shift may worsen upper-airway obstruction. Conv

Defining the potential role of the mineralocorticoid receptor in musculoskeletal health and bone crosstalk with other tissues.

Excessive mineralocorticoid receptor (MR) activation in the heart and vasculature leads to pathological effects such as extracellular matrix accumulation, oxidative stress, and sustained inflammation. While MR's role in cardiovascular and renal systems is well understood, MR signaling has also been implicated as a key driver of homeostasis and pathological changes in several other body systems, including skeletal muscle and adipose tissue. The glucocorticoid receptor (GR) and MR are structurally and functionally linked, sharing 95% similarity in DNA-binding domains and recognizing many of the

Gut microbiome dysbiosis is associated with aldosterone overproduction in idiopathic hyperaldosteronism.

Idiopathic hyperaldosteronism (IHA) is a significant cause of secondary hypertension. Emerging evidence links the gut microbiota to endocrine and cardiovascular diseases. We integrated human genetics, multi-omics, and fecal microbiota transplantation (FMT) to investigate the association between IHA and gut microbiome. Two-sample Mendelian randomization identified seven bacterial taxa associated with IHA. Metagenomic analysis of 30 patients with IHA and 30 healthy control subjects revealed lower microbial diversity and depletion of protective genera identified by Mendelian randomization. Plasma

Increased prevalence of abdominal aortic aneurysm in biochemically confirmed primary aldosteronism: a new complication?

BACKGROUND: Aldosterone excess in primary aldosteronism (PA) has been implicated in vascular remodeling and aortic pathology. However, the prevalence of abdominal aortic aneurysm (AAA) in PA patients remains largely unexplored. This study aimed to evaluate AAA prevalence among patients with biochemically confirmed PA and to explore potential aldosterone-mediated vascular effects. METHODS: In this single-center, case-control study, 73 PA patients and 74 age- and sex-matched individuals with essential hypertension were included. Patients with smoking history, connective tissue disorders, vasculi
