The Infection Timeline in Solid Organ Transplantation: Lessons from the Swiss Transplant Cohort.

BACKGROUND: Dupuytren's disease (DD) is a common fibrotic disorder of the hand, characterized by progressive thickening and contracture of the palmar and digital fascia. Surgical excision remains the primary treatment; however, there are currently no therapies to prevent disease progression or recurrence. This study aims to develop a 3D in vitro model to test novel antifibrotic therapies. The model is based on decellularized pathological DD tissue seeded with patient-derived fibroblasts, capturing the role of both cellular and extracellular matrix components in disease progression. METHODS: F

Magnetically ordered yet topologically robust phases emerging in concurrent Kitaev spin liquids.

Peroxisome proliferator-activated receptors (PPARs) are members of the nuclear receptor superfamily that regulate lipid, glucose, and amino acid metabolism. More recently, PPARs and corresponding ligands have been shown in skin and other organs to regulate important cellular functions, including cell proliferation and differentiation, as well as inflammatory responses. These new functions identify PPARs and corresponding ligands as potential targets for the treatment of various skin diseases and other disorders. It has been shown that in inflammatory skin disorders, including hyperproliferativ

Inappropriately normal aldosterone levels as the norm for hyperaldosteronism: Use of a modified aldosterone-to-renin ratio to enhance primary aldosteronism detection.

BACKGROUND: The aldosterone-to-renin ratio is the preferred screening test for primary aldosteronism, but its performance may be limited by assay variability and threshold definitions. We evaluated the aldosterone-to-renin ratio in a contemporary cohort of primary aldosteronism and nonsecretory tumors. METHODS: Patients diagnosed with primary aldosteronism were retrospectively identified at our center from January 2016 to January 2024 and compared with nonsecreting adrenal tumors. An aldosterone-to-renin ratio of 5.7 ng/dL/ng/L was used based on previously established thresholds. Plasma aldos

Bilateral adrenal adenomas presenting with ACTH-independent Cushing's syndrome and primary aldosteronism: diagnostic insights from multi-steroid adrenal venous sampling.

BACKGROUND: Primary aldosteronism (PA) is the most prevalent cause of secondary hypertension (HTN), and adrenocorticotropic hormone (ACTH)-independent Cushing's syndrome (CS) represents a distinct form of adrenal-derived secondary HTN. Their coexistence resulting from aldosterone- and cortisol-producing adenomas (A/CPAs) is an extremely rare clinical entity. This is particularly true for the subtype characterized by bilateral adrenocortical adenomas, in which one adenoma predominantly secretes aldosterone (ALD) and the other predominantly secretes cortisol (COR), for which only limited clinica

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Epidemiological and Clinical Profiles with Prognostic Implications: A Five-Year Retrospective Study of Head and Neck Cancer in Visakhapatnam, India - A Single Center Study.

BACKGROUND: In 1987, a report from this unit described the changing indications for open adrenalectomy over a 15-year period. The indications for adrenalectomy had switched from it being the principal therapeutic procedure used in advanced breast cancer in the early 1970s, to being predominately performed for Cushing's disease or incidental, asymptomatic, adrenal masses by the early 1980s. The aim of the present study was to evaluate the changes in the presentation and management of adrenal disease in the last 15 years and to compare these findings with our previously published results. Increased Salt-Sensitive Blood Pressure in Women vs Men: Is Relative Hyperaldosteronism the Mechanism?

CONTEXT: Women vs men have more salt-sensitive blood pressure (SSBP) and higher stimulated aldosterone (ALDO) levels, suggesting that their increased SSBP is secondary to a relative hyper-ALDO state. Contrariwise, men vs women have higher sedentary ALDO levels. OBJECTIVE: The present project was designed to address the question are women vs men in a relatively hyper-ALDO state. METHODS: A total of 363 women and 483 men were selected from the HyperPATH cohort to assess the potential underlying mechanism for observed sex differences. RESULTS: Women had greater SSBP, greater ALDO and vasculatu

Transient Ischaemic Attack in a Patient With Conn Syndrome: A Case Report and Literature Review on the Importance of Identifying Secondary Hypertension.

BACKGROUND/AIM: Transient ischaemic attack (TIA) is characterised by a temporary neurological dysfunction resulting from focal ischaemia in the brain, spinal cord or retina without acute infarction. These episodes typically last less than 24 hours and are significant predictors of subsequent ischaemic strokes. Hypertension is a major risk factor for cerebrovascular events, and primary aldosteronism (PA) is recognised as a common cause of secondary hypertension. This case report presents a male patient with secondary hypertension due to Conn Syndrome, a form of PA, who experienced a TIA manifes

Outcomes after medical treatment for primary aldosteronism: an international consensus and analysis of treatment response in an international cohort.

BACKGROUND: Primary aldosteronism can be treated medically but there is no standardised method to evaluate treatment outcomes. We aimed to develop criteria for assessing the outcomes of targeted medical treatment of primary aldosteronism, analyse outcomes across an international cohort, and identify factors associated with a complete treatment response. METHODS: An international panel of 31 primary aldosteronism experts used the Delphi method to reach consensus on the definition of complete, partial, or absent biochemical and clinical outcomes of medical treatment of primary aldosteronism. EGR1 regulates oxidative stress and aldosterone production in adrenal cells and aldosterone-producing adenomas.

Aldosterone-producing adenomas (APAs) are a major cause of primary aldosteronism, a common form of endocrine hypertension. Here, we demonstrate that Early Growth Response 1 (EGR1) plays a dual role in adrenal cell biology, regulating both oxidative stress and aldosterone production. Using RNA sequencing of RSL3-treated human adrenal cells and spatial transcriptomics of adrenal glands from patients with primary aldosteronism, we identify EGR1 as a key gene associated with RSL3-related oxidative stress and APAs. We show that EGR1 silencing decreases oxidative stress and increases CYP11B2 gene ex

A Phase I/IIa Clinical Trial to Evaluate Safety and Adrenal Uptake of Para-Chloro-2-[18F]Fluoroethyletomidate in Healthy Volunteers and Patients with Primary Aldosteronism.

Primary aldosteronism (PA) is a common, potentially reversible, cause of hypertension. Distinguishing unilateral from bilateral PA is critical when deciding who should be offered surgery (unilateral adrenalectomy). Recent studies have shown that PET/CT with [11C]metomidate can accurately identify unilateral PA, with localization of the causative aldosterone-producing adenoma (APA). However, the availability of [11C]metomidate is limited to centers with an on-site cyclotron. Here, we report an early-phase human study with the 18F-labeled analog, para-chloro-2-[18F]fluoroethyletomidate ([18F]CET

High prevalence of primary aldosteronism in a tertiary care hospital in Lebanon.

Primary aldosteronism (PA) is a frequent cause of secondary hypertension. Studies on the prevalence of PA are lacking in the Middle East region. To determine the proportion of patients with high aldosterone-to-renin ratio (ARR) among Lebanese patients with hypertension screened for PA and to compare their clinical and biological characteristics with patients with essential hypertension. A retrospective review of medical charts of patients with hypertension undergoing sampling of serum aldosterone and plasma renin concentration at a tertiary care center between October 2022 and 2023 was conduct

Endoscopic, ultrasound-guided, radiofrequency ablation of aldosterone-producing adenomas (FABULAS): a UK, multicentre, prospective, proof-of-concept trial.

BACKGROUND: Unilateral aldosterone-producing adrenal adenomas (APAs) are the potentially curable cause of 5% of all cases of hypertension. Their localisation (via adrenal vein sampling [AVS]) and removal (by laparoscopic adrenalectomy) require invasive procedures that are unattractive to patients. Molecular imaging-located, endoscopic ultrasound-guided trans-gastric radiofrequency ablation (EUS-RFA) is a potential novel, minimally invasive alternative to AVS and total adrenalectomy for the treatment of APAs in the left adrenal gland, which is very close to the stomach. We aimed to determine wh

Unmasking primary aldosteronism: Transforming disease management with advanced steroid profiling.

Primary aldosteronism (PA) is traditionally thought to result from the overproduction of aldosterone which is unregulated by the renin-angiotensin system. It leads to a significantly increased risk of cardiovascular and metabolic complications as compared to primary hypertension. However, approximately one-third of patients diagnosed with PA according to the traditional guidelines have been found to exhibit low aldosterone levels and other steroid hormones may also play a potentially critical role in PA development. Early diagnosis of PA remains challenging due to low screening rates and the c

Prevalence and main characteristics of primary aldosteronism in bilateral macronodular adrenal disease: a systematic review of the literature.

CONTEXT: Bilateral macronodular adrenal disease (BMAD) typically presents with bilateral benign adrenocortical macronodules and variable cortisol excess. Anecdotal evidence suggests oversecretion of other adrenal steroids, including mineralocorticoids. HYPOTHESIS: We hypothesized that primary aldosteronism (PA) can occur in BMAD, resulting in a distinct, more severe phenotype compared to BMAD with isolated cortisol hypersecretion or unilateral PA (uPA). OBJECTIVE: To assess the prevalence and characteristics of PA in BMAD. METHODS: We systematically reviewed case reports and series of BMAD

Nationwide, Pragmatic, Direct-to-Patient Primary Aldosteronism Testing Program.

BACKGROUND: Primary aldosteronism, an endocrinopathy present in ≥10% to 25% of patients with hypertension, confers excess cardiovascular risk that can be mitigated with aldosterone-directed therapy. However, only 2% of eligible patients undergo guideline-recommended screening. This study aimed to bypass clinical inertia and identify people with primary aldosteronism using pragmatic, direct-to-patient testing. METHODS: Hypertensive adults were recruited via online platforms and underwent virtual consent and local phlebotomy. Using a standardized diagnostic algorithm, laboratory results with in

Novel ultrasound-guided adrenal venous sampling in iodine contrast intolerant patients: two case reports.

Adrenal venous sampling (AVS) is an important procedure for determining primary aldosteronism (PA) subtypes. To guide the catheter to the adrenal veins and confirm cannulation, an iodinated contrast medium (ICM) is typically used. As an ultrasound-based device, the AcuNav intracardiac echocardiography (ICE) catheter allows for real-time visualization of vessels, offering a safer and less invasive option for patients at risk of using ICM. This report introduces a novel AVS technique using the AcuNav ICE catheter in two patients with PA with contraindications to ICM. One patient had a documented
