Primary Aldosteronism: An Endocrine Society Clinical Practice Guideline.

BACKGROUND: Primary aldosteronism (PA), a primary adrenal disorder leading to excessive aldosterone production by one or both adrenal glands, is a common cause of hypertension. It is associated with an increased risk of cardiovascular complications compared with primary hypertension. Despite effective methods for diagnosing and treating PA, it remains markedly underdiagnosed and undertreated. OBJECTIVE: To develop an updated guideline that provides a practical, clinical approach to identifying and managing PA to improve diagnosis rates and encourage targeted treatment. METHODS: The Guideline

A Case of Hypokalemia Caused by the Consumption of Licorice-Containing Herbal Tea.

Hypokalemia can have many etiologies, of which licorice toxicity is notable but often overlooked. Licorice is being increasingly used as a component in herbal formulations and holistic remedies. We present a case of licorice-induced hypokalemia due to herbal tea formulation in a patient with a history of type 2 diabetes mellitus and metastatic adenocarcinoma. This case highlights a unique presentation of pseudo-hyperaldosteronism due to licorice consumption with concurrent use of a sodium-glucose cotransporter (SGLT2) inhibitor, requiring a high index of suspicion for diagnosis. Glycyrrhetinic

Superselective Adrenal Artery Embolization as a Novel Alternative Treatment for Aldosterone-Producing Adenomas: A Case of Refractory Hypertension With Concomitant Autonomous Cortisol Secretion.

Primary aldosteronism (PA) is characterized by an overproduction of aldosterone, leading to hypertension and hypokalemia. Although surgery is an effective treatment for unilateral PA, some patients may be unwilling or unable to undergo this invasive procedure. This case report presents a 51-year-old female patient with refractory hypertension, diagnosed with right-sided hypersecretion of aldosterone and concomitant with autonomous cortisol secretion. The patient declined surgery and was treated with spironolactone as an alternative. Despite optimized medication over 3 months, her blood pressur

Primary Hyperaldosteronism Presenting as Cardiac Arrest: A Case Report.

Primary hyperaldosteronism is an underdiagnosed cause of hypertension despite its increasing prevalence and strong association with cardiovascular morbidity and mortality. Early recognition is critical. Individuals with primary aldosteronism generally present with treatment-resistant hypertension and hypokalemia. Here, we present the case of a patient who experienced cardiac arrest due to severe hypokalemia and was subsequently diagnosed with primary hyperaldosteronism, confirmed through adrenal vein sampling. The condition was successfully treated with a left adrenalectomy.

Perirenal Adipose Tissue and Hypertension: Observational and Genetic Analyses.

BACKGROUND: Perirenal adipose tissue (PRAT) consists of white and brown adipocytes with good vascularization and dense innervation, which could influence the blood pressure. We aim to investigate the association of PRAT thickness with risks of overall and specific forms of hypertension. METHODS: We measured PRAT thickness in the UK Biobank and CONPASS (Chongqing Primary Aldosteronism Study). We prospectively examined the correlation between PRAT thickness and incident hypertension in the UK Biobank. We cross-sectionally explored associations between PRAT thickness and common forms of hyperten

Machine learning-based classification of adrenal tumors using clinical, hormonal, and body composition data.

OBJECTIVE: Accurate diagnosis of adrenal tumors, including mild autonomous cortisol secretion (MACS), adrenal Cushing's syndrome (ACS), primary aldosteronism (PA), pheochromocytoma (PCC), and nonfunctioning adrenal adenomas (NFAs), is crucial but challenging. We aimed to develop a machine learning (ML)-based single-step diagnostic method for differentiating adrenal tumors by integrating clinical data, serum adrenal hormone profiles (SAPs), and body composition data. METHODS: A total of 641 patients with adrenal tumors (MACS = 141, ACS = 64, PA = 265, PCC = 78, and NFA = 93), excluding adrenal

The role of the mineralocorticoid receptor in skin.

The mineralocorticoid receptor (MR) plays a pivotal role in skin homeostasis, inflammation, and repair, interacting closely with the glucocorticoid receptor (GR) to regulate various physiological and pathological processes. Dysregulation of MR signaling has been implicated in several skin disorders, including psoriasis, atopic dermatitis, and impaired wound healing. Furthermore, studies have shown that patients with primary hyperaldosteronism exhibit epidermal hyperplasia, impaired differentiation, increased immune cell infiltrates, and elevated pro-inflammatory cytokines due to MR overactivat

Cosyntropin-Stimulated Aldosterone Reserve of the Nondominant Adrenal Predicts Surgical Outcomes in Primary Aldosteronism.

BACKGROUND: Adrenal vein sampling (AVS) is commonly used to direct therapy of primary aldosteronism (PA). We hypothesized that cosyntropin stimulation during AVS exposes the reserve of aldosterone-producing cells in the nondominant adrenal gland and might predict postoperative outcomes. METHODS: Patients with PA who underwent successful AVS before and after cosyntropin stimulation between January 2015 and December 2024 in a tertiary-referral center were included. The nondominant adrenal aldosterone reserve was assessed with (1) contralateral aldosterone reserve ratio (=aldosterone postcosyntr

Somatic Mutations in MCOLN3 Are Associated With Aldosterone-Producing Adenomas.

BACKGROUND: Primary aldosteronism is a common but underdiagnosed cause of endocrine hypertension that contributes to global cardiovascular morbidity and mortality. It is characterized by renin-independent hyperaldosteronism that originates from adrenal lesions-the majority of which are found to harbor aldosterone-driver somatic mutations in genes encoding ion-transporting proteins. These mutations disrupt intracellular calcium homeostasis, facilitating a pathological increase in aldosterone synthase expression and aldosterone production. Elucidating the exact mechanisms causing aldosterone exc

Discordance and shortcomings of aldosterone suppression tests in primary aldosteronism.

BACKGROUND: The saline suppression test (SST) and the captopril challenge test (CCT) have traditionally been used to confirm or exclude primary aldosteronism (PA). New guidelines recommend using these tests to predict the likelihood of unilateral PA. This study evaluated the diagnostic accuracy, consistency, and clinical implications of these tests. METHODS: We conducted a retrospective study of 531 patients with high-probability features of PA who underwent both SST and CCT to evaluate their accuracy and ability to predict unilateral PA. Association of Plasma Aldosterone Concentration With Early Renal Injury Biomarkers in Primary Aldosteronism: A Propensity-Matched Comparative Study.

Primary aldosteronism (PA) independently increases renal impairment risk beyond blood pressure effects. Although hyperaldosteronism is known to mediate renal injury, associations between plasma aldosterone concentration (PAC) and early kidney damage biomarkers such as retinol-binding protein (RBP) and β2-microglobulin (β2-MG) remain insufficiently explored. We investigated the association of PAC with renal function indicators-including RBP, β2-MG, albumin-to-creatinine ratio (ACR), and estimated glomerular filtration rate (eGFR)-comparing matched patients with PA and essential hypertension (EH

Use of NaCl 9‰ as a cheap diluent for cortisol assay in lateralized samples from adrenal veins catheterisation.

Among the tools necessary for the diagnosis of primary hyperaldosteronism, catheterization of the adrenal veins represents the major examination to determine the presence of lateralized aldosterone secretion. To ensure that the catheter is well positioned at the level of the adrenal veins, a cortisol dosage is carried out in parallel with the aldosterone dosage. At the Hormonology and Tumor Markers laboratory of the Bordeaux University Hospital, this cortisol assay is carried out on an Abbott Architect i2000, making it possible to extend the calibration range up to > 3,300 nmol/L. Beyond that,

Primary bilateral macronodular adrenal hyperplasia (PBMAH): from rare to common cause of Cushing syndrome in clinical practice.

Primary Bilateral Macronodular Adrenal Hyperplasia (PBMAH) is considered a rare cause of Cushing syndrome (CS). Despite progress in understanding the pathogenesis, clinical evaluation of the disease and optimal treatment remain relevant. Data were retrieved from institutional/hospital databases. The first group of PBMAH patients comprised 34 cases from a total of 634 adrenalectomies (including 166 CS cases) performed 2009-2015. The second group included 51 patients from 356 adrenalectomies (including 160 CS cases) conducted 2016-2023. Follow-up results are available for 79 patients. PBMAH is a

An automated Immunoaffinity liquid chromatography-tandem mass spectrometry assay for quantification of aldosterone in human plasma.

The diagnosis of primary aldosteronism (PA) relies on the accurate determination of aldosterone. Liquid chromatography-tandem mass spectrometry (LC-MS/MS) has long been considered the gold standard for aldosterone quantification but it is hindered by labor-intensive sample preparation. To address this, we developed an immunoaffinity-mass spectrometry (iMS) assay on a fully automated device combining anti-aldosterone antibodies with stable isotope-labeled internal standards (IS). This method completes sample preparation within 15 min for at least six parallel samples in parallel with minimal ma

Mass Spectrometric Measurements of 11-Deoxycortisol, Androstenedione and Dehydroepiandrosterone Are Superior to Cortisol to Assess Selectivity of Non-Stimulated Adrenal Vein Sampling.

OBJECTIVE: Successful adrenal venous sampling (AVS) is traditionally assessed using the ratio of cortisol in adrenal to peripheral venous plasma to calculate the selectivity index. With mass spectrometry other steroids can be simultaneously measured that may improve numbers of apparent successful sampling procedures. DESIGN: Cross-sectional multicenter study. PATIENTS: The study involved 229 patients who underwent unstimulated AVS for subtyping primary aldosteronism. MEASUREMENTS: Adrenal and peripheral venous plasma cortisol, 11-deoxycortisol, androstenedione and dehydroepiandrosterone wer

Contralateral metastatic papillary thyroid carcinoma and complicated by primary hyperaldosteronism: A case report.

BACKGROUND: Thyroid cancer is a common malignancy, often found in women. It is the second most common malignant tumor, second only to breast cancer, and it most frequently occurs as papillary thyroid carcinoma (PTC), representing over 90% of cases. PTC frequently presents with lymph node metastases, though in rare cases, patients may experience dysphagia, dyspnea, or hoarseness. In PTC and other differentiated thyroid cancers, direct invasion into major local veins is uncommon, and simultaneous involvement of the vagus nerve is even rarer. Herein, we report a case involving a 50-year-old male
