Baxdrostat: A Next-Generation Aldosterone Synthase Inhibitor Offering New Hope in Resistant Hypertension.

Hypertension is a leading global cause of cardiovascular disease and mortality, with resistant hypertension (RH) posing treatment challenges. Aldosterone synthase inhibitors (ASIs) are a novel drug class that reduce blood pressure by lowering aldosterone levels. Baxdrostat is a selective ASI that inhibits the CYP11B2 enzyme, responsible for aldosterone synthesis, without affecting cortisol production. This selectivity minimizes hormonal side effects. Clinical trials have shown that baxdrostat reduces plasma aldosterone in a dose-dependent manner while preserving cortisol levels. In the Phase 2

Prevalence and risk of thyroid disease among adult primary aldosteronism patients: a systematic review, meta-analysis, and trial sequential analysis.

BACKGROUND: Primary aldosteronism (PA), the most prevalent curable secondary hypertension, and thyroid diseases (the second most common endocrine disorder) are increasingly linked, yet their mechanistic connections remain unclear. DESIGN AND METHODS: Four databases (PubMed, Embase, Web of Science, Cochrane Library) were searched for case-control studies. Random-effects meta-analysis and subgroup analyses for thyroid disease subtypes were performed. Sensitivity/trial sequential analyses and Begg's test evaluated robustness/publication bias. RESULTS: Our meta-analysis included five case-contro

Molecular investigation of primary aldosteronism: exploring genetic heterogeneity in understudied populations.

OBJECTIVE: Genetic abnormalities in ion channels that regulate the depolarization of adrenal glomerular cell plasma membranes have been identified as a cause of primary aldosteronism (PA) due to aldosterone-producing adenoma (APA). This study aimed to evaluate somatic variants in the KCNJ5, CACNA1D, CLCN2, ATP1A1, ATP2B3, GNAQ, GNA11, and CTNNB1 genes, assess the genotype-phenotype correlation, and analyze the outcomes in patients with APA from a heterogenic ethnic population. SUBJECTS AND METHODS: Clinical, biochemical, and molecular data were obtained from 32 patients. RESULTS: Pathogenic

Adrenal vein sampling with continuous infusion of cosyntropin for identifying surgically curable cases of primary aldosteronism.

PURPOSE: Cosyntropin (ACTH) bolus stimulation prior to adrenal vein sampling (AVS) increases sampling selectivity but reduces sensitivity for lateralization, predictive of surgical cure. We examined effectiveness of an alternative continuous cosyntropin stimulation, without bolus, specifically via rates of sampling adequacy, lateralization, and surgical cure among patients with primary hyperaldosteronism. MATERIALS AND METHODS: A single institution retrospective review was performed of a cohort of consecutive patients with primary aldosteronism who underwent AVS from 2014 to 2021 with cosyntr

Testing for Primary Aldosteronism Underutilized Following Stroke: A Multicenter Cohort Study.

BACKGROUND: Primary aldosteronism, the most common endocrine cause of hypertension, is associated with an increased risk of stroke compared with essential hypertension. Targeted treatment with mineralocorticoid receptor antagonists, or surgical resection of an aldosterone-producing adrenal adenoma, can effectively reduce blood pressure and mitigate cardiovascular risk. However, despite being a highly modifiable cardiovascular risk factor, primary aldosteronism is grossly underdiagnosed, even in high-risk populations. METHODS: We conducted a multicenter retrospective cohort study to assess the

Regulation of Klotho Production by Mineralocorticoid Receptor Signaling in Renal Cell Lines.

Through the mineralocorticoid receptor, aldosterone controls extracellular volume and arterial blood pressure by stimulating Na+ absorption and K+ secretion in epithelial cells of the kidney, colon, and several glands. Hyperaldosteronism promotes fibrosis and inflammation in epithelial and non-epithelial tissues, thereby favoring loss of kidney and heart function. Mineralocorticoid receptor blockade therefore gains relevance especially in renal and cardiac disease. Kidney-derived Klotho is a powerful anti-aging protein with anti-fibrosis and anti-inflammatory effects providing cardio- and neph

Unmasking Secondary Hypertension: Renal Artery Stenosis Concealing the Diagnosis of Primary Hyperaldosteronism.

A 24-year-old woman with no prior comorbidities presented with severe, treatment-resistant hypertension characterized by episodic headaches and persistent systolic blood pressures above 140 mm Hg despite lifestyle modifications. Initial evaluation revealed hypokalemia and metabolic alkalosis with an aldosterone-to-renin ratio suggestive, but not diagnostic, of primary hyperaldosteronism. Secondary workup excluded pheochromocytoma, Cushing's syndrome, and thyroid disease. Imaging identified right renal artery fibromuscular dysplasia with angiographic narrowing of approximately 30%. Despite maxi

Effectiveness of Adrenal Venous Sampling in the Management of Primary Aldosteronism: Single-Centered Cross-Sectional Study at a Tertiary Care Hospital in Sri Lanka.

INTRODUCTION: Adrenal venous sampling (AVS) is the gold standard procedure to discriminate unilateral primary aldosteronism (UPA) from bilateral disease (BPA). AVS is technically demanding and is only performed in a limited number of centers in Sri Lanka. This study aimed to evaluate the effectiveness of AVS in the management of primary aldosteronism. METHODOLOGY: Thirty-two patients who underwent AVS at the National Hospital of Sri Lanka from April 2021 to April 2023 were enrolled. Continuous and categorical variables were summarized with mean ± SD and proportions, respectively. Mean and sta

A cardiac event triggers an endocrine investigation: a case report of primary hyperaldosteronism revealed by postpartum cardiomyopathy.

BACKGROUND: Hypertensive heart disease due to primary hyperaldosteronism as a cause of heart failure (HF) during pregnancy is a rare and challenging condition. This situation may be confused with other clinical entities, particularly with postpartum cardiomyopathy, a pregnancy-related pathology that is characterized by the development of HF secondary to left ventricular systolic dysfunction towards the end of pregnancy. CASE SUMMARY: The case describes a 41-year-old female with severe preeclampsia, who presented for evaluation of severe congestive HF felt to be secondary to postpartum cardiom

Impact of Age, Sex, and Race on Primary Aldosteronism Test Interpretations.

BACKGROUND: New guidelines recommend testing for primary aldosteronism (PA) in all people with hypertension. Interpreting population-based results for PA will require understanding the influence of demographic characteristics. METHODS: 858 adults from across the US meeting traditional guideline criteria for PA testing underwent testing. The influence of demographic factors on test result interpretations was assessed after multivariable adjustment. RESULTS: Mean age was 62±11 years, with 54.6% women, 14% Black, and 22.9% Hispanic. Independent of antihypertensive medications and clinical comor

GWAS and Replication Analysis of Apparent Treatment-Resistant Hypertension.

BACKGROUND: Resistant hypertension (RH), in which blood pressure remains elevated on ≥3 medications or controlled on ≥4 medications, increases the risk of adverse cardiovascular events nearly 50% more than primary hypertension. We sought to identify genetic drivers of RH in a reliable and generalizable manner. METHODS: We utilized FinnGen (discovery) and UKBB (UK Biobank, replication) data sets to identify potential genetic drivers of RH. Using standard RH definitions, we developed cohorts in each data set and performed genome-wide (genome-wide association studies) and transcriptome-wide asso

Partially Successful Adrenal Vein Sampling With and Without Cross-Sectional Imaging in Primary Aldosteronism Subtyping.

BACKGROUND: Adrenal vein (AV) sampling (AVS) is used to guide therapy in primary aldosteronism (PA). When a single AV is successfully cannulated, the relative aldosterone secretion index (RASI), which compares the aldosterone/cortisol ratio in that AV versus the periphery, has been proposed as sufficient for PA subtyping, particularly when <1. Data on RASI reliability have, however, been inconsistent. METHODS: This retrospective cohort study included patients with PA who underwent AVS before and after cosyntropin stimulation at a referral center between January 2015 and December 2024. To simu

Efficacy and Safety of Non-surgical Versus Surgical Management of Primary Hyperaldosteronism: A Systematic Review and Meta-Analysis.

Primary aldosteronism (PA) is an increasingly recognised cause of treatment-resistant secondary hypertension. However, rates of diagnosis and awareness remain low due to under-screening and the challenges of lateralising aldosterone secretion. Adrenal venous sampling (AVS), the gold standard for subtype differentiation, is technically demanding and not widely available, contributing to delayed or suboptimal management. Laparoscopic adrenalectomy (LA) is the current gold-standard treatment for unilateral aldosterone-producing adenomas, but many patients are unfit for or decline surgery. Minimal

Hyperaldosteronism Presenting as Pre-syncope: A Case Report.

Primary Aldosteronism (PA) is a common but often underrecognized cause of secondary hypertension. While typically associated with hypokalemia, profound muscle weakness is rarely a presenting symptom, especially in the absence of significant electrolyte derangement. We present a case of a 65-year-old male with hypertension and dyslipidemia who presented with transient acute attacks of profound weakness and presyncope, without loss of consciousness or other focal neurologic signs. Initial evaluation, including cardiac, neurologic, and metabolic testing, was unrevealing. Mild hypokalemia (potassi

Aldosterone: Synthesis and regulation.

Aldosterone, the principal mineralocorticoid, is exclusively synthesized in the glomerular zone of the adrenal cortex. Its primary function is to maintain water and electrolyte balance and regulate blood pressure. The synthesis of aldosterone begins with the transport of cholesterol to the mitochondria, a process that is mediated by the StAR protein. This is followed by a sequence of reactions that are catalyzed by specific enzymes. CYP11A1, 3β-HSD2, CYP21A2, and finally CYP11B2. The regulation of its production is classified as acute, dependent on the rapid activation of StAR, and chronic, in

An unusual case of neuroblastoma presenting with prolonged watery diarrhea in a pediatric patient.

Neuroblastomas represent a diverse group of neuroblastic tumors characterized by variability in their clinical progression and degree of differentiation. In rare cases, patients with neuroblastoma may present with paraneoplastic syndromes, such as watery diarrhea, hypokalemia, and achlorhydria (WDHA syndrome), linked to the secretion of vasoactive intestinal peptide (VIP). We report a case of a 14-month-old girl presented with a three-week history of watery diarrhea and signs of dehydration with no other symptoms. The patient's medical history was unremarkable, and no medication use was report

Aldosterone-Producing Adrenal Oncocytoma: A Rare Cause of Primary Aldosteronism.

Adrenal oncocytomas are rare, usually non-functional tumors that are often found incidentally. In some cases, however, they may present with endocrine hyperfunction, leading to diagnostic and therapeutic challenges. We report the case of a 63-year-old male patient with a long-standing history of hypertension, managed with antihypertensive therapy. Due to persistently elevated blood pressure despite treatment, the patient underwent an evaluation for secondary causes of hypertension. Hormonal assessment demonstrated a high concentration of plasma aldosterone, with concurrently suppressed renin l

The mechanisms of ferroptosis and its role in atherosclerosis.

Ferroptosis is a newly identified form of non-apoptotic programmed cell death, characterized by the iron-dependent accumulation of lethal lipid reactive oxygen species (ROS) and peroxidation of membrane polyunsaturated fatty acid phospholipids (PUFA-PLs). Ferroptosis is unique among other cell death modalities in many aspects. It is initiated by excessive oxidative damage due to iron overload and lipid peroxidation and compromised antioxidant defense systems, including the system Xc-/ glutathione (GSH)/glutathione peroxidase 4 (GPX4) pathway and the GPX4-independent pathways. Multifocal, Asymmetric Bilateral Primary Aldosteronism Cannot be Excluded by Strong Adrenal Vein Sampling Lateralization: An International Retrospective Cohort Study.

BACKGROUND: Primary aldosteronism (PA) has been broadly dichotomized into unilateral and bilateral forms. Adrenal vein sampling (AVS) lateralization indices (LI) ≥2 to 4 are the standard-of-care to recommend unilateral adrenalectomy for presumed unilateral PA. We aimed to assess the rates and characteristics of residual PA after AVS-guided adrenalectomy. METHODS: We conducted an international, retrospective, cohort study of patients with PA from 7 referral centers who underwent unilateral adrenalectomy based on LI≥4 on baseline and/or cosyntropin-stimulated AVS.
