Inter- and intratumoral heterogeneity of aldosterone-producing adenomas: recent advances and clinical implications.

Primary aldosteronism is the most common form of secondary hypertension, and aldosterone-producing adenoma (APA) is the most prevalent surgically curable subtype of primary aldosteronism. Over the past decade, the identification of recurrent somatic mutations, most notably in KCNJ5, has transformed the molecular understanding of APA and has revealed substantial heterogeneity across tumors. In parallel, advances in transcriptomic, epigenomic, and metabolomic profiling have further expanded insights into genotype-dependent differences in steroidogenic activity, cellular morphology, and clinical

A Ten Year Overview of Adrenal Vein Sampling in Croatia: Insights and Outcomes.

BACKGROUND: Adrenal vein sampling (AVS) is the gold standard for subtype differentiation in primary aldosteronism (PA), yet it remains technically demanding and resource intensive. However, data on long-term performance, learning curves, and concordance with imaging in real-world clinical practice are limited. METHODS: We conducted a retrospective single-centre study including patients with PA who underwent AVS between January 2016 and June 2025. AVS was performed under continuous adrenocorticotropic hormone stimulation using a sequential sampling protocol. Procedural success, learning curve,

[Stratified and graded diagnosis and treatment strategies for patients with primary aldosteronism].

Primary aldosteronism (PA) is the most common endocrine cause of secondary hypertension and carries a greater risk of cardiorenal complications than essential hypertension. Recent domestic and international guidelines and consensus statements have proposed screening all patients with hypertension for PA. To cope with the massive patient base resulting from universal screening, this article explores practical schemes for the stratified and graded diagnosis and treatment of PA. By employing "non-drug withdrawal" and "minimal-drug withdrawal" testing protocols, PA screening can be feasibly implem

[Efficacy of bidirectional switching therapy between adrenal ablation and adrenalectomy in patients with primary aldosteronism].

To investigate the efficacy in patients with primary aldosteronism (PA) who converted from adrenal ablation to surgical intervention or from surgical intervention to adrenal ablation. A retrospective analysis was performed on the clinical data of patients with PA who sequentially underwent transcatheter adrenal ablation and surgical intervention (regardless of treatment sequence) admitted to Daping Hospital, Army Medical University between June 2018 and June 2025. Patients were divided into the post-ablation surgery group and post-surgery ablation group according to the treatment sequence. Cli

Case Report: Primary aldosteronism coexisting with adrenal insufficiency in a 73-year-old woman with paradoxical hyponatremia.

INTRODUCTION: Primary aldosteronism (PA) and autoimmune polyglandular syndrome type 2 (APS-2) represent opposing ends of adrenal pathology-hyperfunction versus hypofunction. Their coexistence is exceptionally rare and diagnostically challenging. CASE PRESENTATION: A 73-year-old woman with a 30-year history of refractory hypertension and recent hyponatremia was diagnosed with PA (aldosterone-to-renin ratio 29.11, positive captopril challenge, left adrenal hyperplasia). Spironolactone controlled blood pressure but paradoxically worsened hyponatremia. Two independent circadian cortisol profiles

From low renin to the primary aldosteronism spectrum: aldosterone synthase inhibitors and the endocrine management of hypertension.

BACKGROUND: Hypertension remains a major contributor to global cardiovascular morbidity despite decades of available therapy. AIM: This review advances an endocrinological framework in which chronic hypertension, including resistant hypertension, is understood as a consequence of dysregulated hormonal control of renal sodium handling, mediated by excess renin-angiotensin II activity, inappropriate aldosterone excess, or both. RESULTS: Within this framework, the traditional binary distinction between "essential" and "secondary" hypertension gives way to a mechanistic continuum characterized b

Aldosterone synthase inhibitors may alter how primary care treats hypertension.

Hypertension guidelines recommend adding antihypertensive therapies until the blood pressure (BP) is below a specific target. If the BP is not below target with 3 medications, including a diuretic, then it is labeled resistant hypertension, and then we investigate for secondary causes, such as primary aldosteronism (PA). PA testing seems complex with 3 results: aldosterone, renin, and the aldosterone-renin ratio. Hence, these tests are ordered infrequently, and the diagnosis of PA is often delayed or never made. Also, heart failure, chronic kidney disease, and other conditions can have elevate

Etomidate vaping: A new form of anesthetic abuse.

Background: The emergence of etomidate-containing e-cigarettes represents a significant and rapidly evolving public health challenge, marking a transition of this clinical anesthetic into illegal use of an anesthetic agent. Originally utilized as a short-acting intravenous anesthetic for intubation induction, etomidate's high lipophilicity allows it to be easily aerosolized for inhalation via vaping devices. Misuse is primarily driven by its rapid-onset effects, including relaxation, dissociation, and transient euphoria, as well as a pervasive "illusion of safety" among users who perceive hosp

Pathophysiology of aldosterone dysregulation in difficult-to-control hypertension.

Aldosterone, a key component of the renin-angiotensin-aldosterone system (RAAS), plays a central role in sodium balance, vascular tone, blood pressure control, and cardiovascular remodeling. Dysregulation of aldosterone mechanism, including excessive production, deficiency, and inappropriate receptor activation, represents a significant contributor to cardiovascular and renal disease which is often underrecognized. Aldosterone dysregulation may manifest as primary aldosteronism that is increasingly identified as a common cause of secondary hypertension, or by secondary hyperaldosteronism that

Klotho Deficiency Impairs Distal Potassium Homeostasis Through mTORC1-Dependent Suppression of ROMK.

AIM: Hyperkalemia is prevalent in the elderly, and underlying molecular mechanisms remain incompletely understood. We hypothesized that Klotho deficiency, a feature of aging, impairs renal potassium homeostasis and contributes to hyperkalemia. METHODS: Klotho heterozygous knockout (KLhet) mice, characterized by reduced Klotho levels, were used to investigate determinants of potassium handling, including renal outer medullary potassium channel (ROMK)1, the epithelial sodium channel (ENaC), which provides the driving force for ROMK-mediated potassium secretion, and plasma aldosterone concentrat

Optimizing adrenal vein sampling interpretation for primary aldosteronism subtyping.

CONTEXT: Guidelines recommend adrenal vein sampling (AVS) for subtyping primary aldosteronism (PA); however, the optimal sample for determining lateralization when multiple successful adrenal vein samples are obtained is unclear. OBJECTIVE: To compare the impact of different adrenal vein sample-selection strategies on PA subtyping. METHODS: A retrospective study was conducted in PA patients who underwent bilaterally successful AVS with multiple samples obtained from each adrenal vein. LI was calculated using four different sample-selection strategies: (1) Maximum Aldosterone (Max Aldo), the

Reninoma in an adolescent boy with negative selective renal vein sampling: a case report and review of the literature.

Juxtaglomerular cell tumor (reninoma) is a rare benign renal neoplasm causing secondary hypertension via autonomous renin over-secretion, which is extremely rare in children and adolescents. Here we report a 13-year-old boy with a two-year misdiagnosis of resistant hypokalemic hypertension, who underwent unnecessary left adrenalectomy based on ambiguous adrenal computed tomography (CT). Biochemical tests revealed hyperreninemic hyperaldosteronism with normal aldosterone-to-renin ratio. Renal artery CT identified a tiny low-density lesion in the upper pole of the left kidney, while contrast-enh

Diagnostic concordance of CXCR4-PET/CT and adrenal vein sampling in subtyping primary aldosteronism and its guiding value for percutaneous adrenal artery embolisation: a prospective diagnostic test accuracy study protocol.

INTRODUCTION: Primary aldosteronism (PA) is the most common cause of secondary hypertension, and accurate differentiation between unilateral and bilateral disease is critical for selecting appropriate therapy. In patients undergoing adrenalectomy, histopathological classification based on the International Histopathology Consensus for Unilateral Primary Aldosteronism provides a definitive reference for unilateral disease. Although adrenal vein sampling (AVS) remains the diagnostic gold standard, it is invasive, technically demanding and often limited by variable success rates. CXCR4-targeted p

A NOVEL KCNJ5 GENE MUTATION ASSOCIATED WITH FAMILIAL ALDOSTERONISM TYPE III WITHOUT HYPERTENSION.

BACKGROUND: Primary aldosteronism (PA) involves excessive autonomous aldosterone (ALD) secretion by the adrenal cortex, leading to water and sodium retention and renin-angiotensin system inhibition, causing hypertension and K+ imbalance. Familial hyperaldosteronism type III (FH-III) is a rare subtype, characterized by hypokalemia with hypertension and adrenal hyperplasia. Eight associated pathogenic potassium inward rectifying channel protein subfamily J member 5 (KCNJ5) mutations have been reported. This study reports a Chinese family with FH-III associated with a rare KCNJ5 variant (c.536A>G

The Rare Case of a Sertoli Cell Tumor of the Adrenal Gland in a Man With Pathological Dexamethasone Suppression Test: A Case Report.

Sertoli cell tumors are rare gonadal stromal tumors. Extragenital manifestations, particularly in the adrenal gland, are exceptionally uncommon. We present a case of an adrenal Sertoli cell tumor in a male patient, presenting with a pathological (1 mg overnight) dexamethasone suppression test. An 82-year-old otherwise healthy man was referred for evaluation of an incidentally detected left adrenal mass. The patient was asymptomatic with no clinical signs of catecholamine excess, hypercortisolism, hyperaldosteronism, or B-symptoms. Initial CT scan revealed a 44 × 42 mm lesion, which increased t

Integrative Analysis of Metabolomic and Transcriptomic Data Reveals Metabolic Signatures and Major Metabolic Pathways in Primary Aldosteronism.

OBJECTIVE: Primary aldosteronism (PA) is the most common secondary hypertension. In this study, we performed the pathway enrichment analysis based on metabolomics and transcriptomic data to find the metabolic perturbations in PA, which could provide new targets for PA and further understand the biology of PA. METHODS: 24 PA patients and 24 healthy adults served as the control group in this study. Six participants were chosen from each group to have their peripheral blood and serum samples analyzed for omics investigations. Another eighteen participants' peripheral blood samples were selected

Adrenocortical Carcinoma with Suspected Dual Cortisol and Aldosterone Secretion: A Case Report.

Adrenocortical carcinoma (ACC) is a rare malignancy, accounting for 0.05-2% of all malignant tumors. Most cases are sporadic and asymptomatic, often discovered incidentally via imaging. However, 50-60% present with autonomous hormonal secretion, with 45% producing excess cortisol, and only 1% exhibiting mineralocorticoid excess. We report a 43-year-old Filipino female with a 6-month history of abdominal bloating, proximal muscle weakness, edema, and hypertension. Laboratory tests showed hypokalemia (serum potassium 2.5-3.05 mmol/L; normal 3.3-5.0 mmol/L). Imaging revealed a neoplastic right su

Diagnostic challenges in primary aldosteronism evaluation and finerenone use.

INTRODUCTION: Primary aldosteronism (PA) is a common but frequently ignored cause of hypertension, particularly in patients with normal serum potassium and aldosterone levels. CASE PRESENTATION: We report the case of a 70-year-old Han Chinese man with a 20-year history of hypertension who presented with normokalemia and normal plasma aldosterone levels. Despite this, laboratory evaluation revealed suppressed direct renin concentration (DRC) and a markedly elevated aldosterone-to-renin ratio (ARR), raising suspicion for PA. CHAC1 aggravates aldosterone-induced glomerular mesangial cell ferroptosis in diabetic kidney disease.

In diabetic kidney disease (DKD), overactivation of the renin-angiotensin-aldosterone system (RAAS) promotes ferroptosis. However, the specific role of aldosterone-the terminal effector of the RAAS-in regulating ferroptosis pathways in DKD remains poorly understood. In this study, we investigated how hyperaldosteronism modulates ferroptosis in glomerular mesangial cells. Using db/db mice and high-fat diet (HFD)-induced mice as models, we characterized the metabolic disturbances associated with DKD and the concurrent elevation of endogenous aldosterone. In vitro, MES13 SV40 mesangial cells were
