Sampling variability and concordance between sequential and simultaneous adrenal venous sampling in primary aldosteronism subtype diagnosis.

OBJECTIVE: Accurate subtyping of primary aldosteronism (PA) via adrenal venous sampling (AVS) is pivotal for guiding treatment decisions. However, the optimal AVS approach-simultaneous versus sequential bilateral sampling-remains controversial, necessitating a rigorous evaluation of diagnostic consistency between these 2 techniques. METHODS: Consecutive PA patients who underwent bilateral simultaneous unstimulated AVS via the antecubital vein between December 2024 and January 2026 were prospectively enrolled. Blood samples were collected from the bilateral adrenal veins and a peripheral vein

Prenatal diagnosis of antenatal Bartter syndrome: Another four case reports.

OBJECTIVE: Bartter syndrome (BS) is a general term for a group of rare genetic disorders in which the defective kidneys have impaired ability to reabsorb salt, resulting in salt wasting, hypokalemia, and metabolic alkalosis. Prenatal diagnosis of antenatal BS is challenging because of the absence of structural anomalies in the fetus. We here report four cases of antenatal BS identified in utero by fetal exome analysis. CASE REPORT: A total of four cases of antenatal BS were detected. All cases presented with idiopathic polyhydramnios, with no evidence of anomalies in the fetuses and placentae

Impact of the 2025 Endocrine Society clinical practice guideline on the diagnosis and management of primary aldosteronism for the clinical laboratory.

Primary aldosteronism (PA) is a prevalent yet under-recognised condition affecting up to 14% of patients with hypertension in primary care and up to 30% with resistant hypertension. PA is associated with an increased rate of cardiovascular disease compared to patients with similar blood pressure. The diagnosis of PA involves multiple steps; first, screening with the aldosterone-to-renin ratio (ARR), then if indicated, confirmatory dynamic function testing, and depending on the outcome of confirmation testing, subtyping with imaging and adrenal vein sampling (AVS). The United States Endocrine S

Association of the Primary Aldosteronism Severity Classification with Lateralization and Treatment Outcomes.

CONTEXT: The 2025 clinical guideline for primary aldosteronism (PA) recommends a diagnostic and therapeutic pathway stratified by lateralization risk. The PA severity classification (PASC) integrates biochemical and clinical features and is intended to inform graded recommendations for adrenal venous sampling (AVS). OBJECTIVE: We examined whether PASC is associated with AVS-defined lateralization and treatment outcomes. DESIGN: A retrospective multicenter cohort study. SETTING: Eight tertiary centers. PATIENTS: A total of 833 patients with PA who underwent AVS. PASC severity was classified

Effects of adrenal ablation on blood pressure and target organ damage in deoxycorticosterone acetate-salt hypertensive rats.

BackgroundPrimary aldosteronism is a common and underdiagnosed form of secondary hypertension. However, preclinical investigations of adrenal ablation as a potential therapeutic approach for primary aldosteronism treatment are limited.MethodsA deoxycorticosterone acetate-salt hypertensive rat model was established, and the animals were treated with either unilateral adrenal ablation or oral spironolactone for 12 weeks. Blood pressure was continuously monitored throughout the study period. The key endpoints included blood pressure regulation, circulating hormone levels, and target organ damage.

Adrenal venous sampling: Indications and technical challenges.

Adrenal venous catheterisation is a procedure used in the aetiological diagnosis of primary aldosteronism (PA). Its purpose is to measure aldosterone and cortisol levels in blood samples obtained from both adrenal veins and compare them with peripheral levels, allowing for the identification of potential lateralisation in hormone secretion. Owing to its high sensitivity and specificity, it is a key-though technically demanding-technique that should be performed in specialised centres. Accurate diagnosis leads to better clinical outcomes by avoiding unnecessary surgical interventions. The aim o

Resistant Hypertension Variants Link to Hyperaldosteronism and Potassium Levels.

BACKGROUND: We aimed to characterize the genetic architecture of resistant hypertension (rHTN), which affects up to 18% of hypertensive individuals and increases cardiovascular disease risk. METHODS: We conducted a genome-wide association study on rHTN, defined as use of 3 or more concomitant antihypertensive drugs for at least 6 months without reaching blood pressure target (in the 3-drug case), comparing it to controlled hypertension (cHTN), in which persons on 1 or 2 antihypertensives for at least 6 months reach target BP after 30 days of therapy initiation. The study included 23 508 rHTN

[Expert consensus on the subtyping diagnosis of primary aldosteronism (2026)].

Primary aldosteronism (PA) is a significant cause of secondary hypertension. While accurate subtyping diagnosis is crucial for determining the treatment strategy for this condition, performing this subtyping remains highly challenging in clinical practice. Therefore, based on current evidence and clinical practice in China, a panel of domestic experts in the field developed this consensus through multiple rounds of Delphi discussions, focusing on the target population, methods, and PA subtyping process.

Cytosolic calcium oscillations in human adrenocortical cells causing primary aldosteronism.

BACKGROUND: Intracellular calcium [Ca2+]i is pivotal in driving aldosterone secretion. However, knowledge of the [Ca2+]i dynamics underlying aldosterone excess in primary aldosteronism (PA), the most common form of endocrine hypertension, is lacking due to the difficulty of obtaining aldosterone-producing adenoma (APA) cells ex vivo. METHODS: We used the Ca2+-sensitive Fura-2 dye and live confocal microscopy to investigate variations in [Ca2+]i under basal conditions and after angiotensin II (Ang II) stimulation in primary aldosterone-producing CD56+ cells, immunomagnetically isolated ex vivo

Adrenocortical oncocytoma manifesting as primary aldosteronism: a case report.

BACKGROUND: Adrenocortical oncocytomas are rare adrenal neoplasms and are most often reported as non-functioning adenomas. A minority, however, exhibit steroid hormone hypersecretion and may present with Cushing's syndrome, pheochromocytoma-like syndromes, feminisation in males or virilization in females. Adrenocortical oncocytoma presenting with primary aldosteronism is an exceptionally uncommon entity. CASE PRESENTATION: We report a 44-year-old man with severe hypertension and hypokalaemia, accompanied by a right adrenal lesion on computed tomography. A comprehensive renin-angiotensin-aldos

Novel gain-of-function CLCN2 germline mutation associated with nonclassical lateralizing primary aldosteronism.

Primary aldosteronism (PA) is commonly caused by somatic and germline mutations in various genes. Patients with unilateral PA may display classical or nonclassical histopathologic features in adrenal tumors. Somatic and germline mutations are linked to classical unilateral and familial PA, respectively. In this study, we aim to search for novel gene mutations causing nonclassical lateralizing PA. Surgically resected adrenal tumor tissues from patients enrolled in a PA cohort were subjected to histopathological evaluation. Genetic analyses were performed for genomic DNA isolated from peripheral

Current and Novel Biomarkers for Diagnosis and Subtyping of Primary Aldosteronism.

Primary aldosteronism is a common and potentially curable cause of secondary hypertension, yet it remains substantially underdiagnosed. Current diagnostic pathways rely on the aldosterone-to-renin ratio, confirmatory tests, and subtype classification by imaging and adrenal venous sampling. However, these approaches are limited by assay variability, medication effects, imperfect diagnostic accuracy, and the absence of a true reference standard, particularly for bilateral disease. Recent molecular imaging and histopathologic data further challenge the traditional binary distinction between unila

Assessment of markers of primary aldosteronism in systemic sclerosis and their relationships with renal and cardiovascular outcomes.

BACKGROUND: Whether primary aldosteronism (PA), the most common endocrine cause of hypertension (HTN), contributes to HTN and cardiovascular disease burden in systemic sclerosis (SSc) has never been explored. We aimed to assess the prevalence and management of HTN, prevalence of positive PA screening test results and the associations between markers of PA with HTN and SSc-related outcomes in SSc. METHODS: Data from adult SSc patients and normotensive healthy controls (HC) were analysed in this single-centre study. HTN was defined as blood pressures ≥140/90 mm Hg on two visits or as documented

Rare confluence: clinically diagnosed Bartter syndrome with sensorineural hearing loss in a young endosulfan-exposed patient.

Bartter syndrome, a rare autosomal recessive disorder (1 in 1 000 000), causes salt wasting due to defects in the ion channels of the loop of Henle. In this case report, a young adolescent girl, who is a known case of Bartter syndrome and a recognised endosulfan exposure victim, presented with sensorineural hearing loss and multiple episodes of vomiting. She was treated with intravenous fluids, potassium supplementation and indomethacin. This case highlights the importance of early recognition and routine biochemical investigations to prevent complications of inherited tubulopathies.

Resistant hypertension: Epidemiology, mechanisms and contemporary therapeutic strategies.

Resistant hypertension (RH), defined as uncontrolled blood pressure on three or more antihypertensive agents (including a diuretic) or controlled blood pressure on four or more, poses a major clinical challenge. It disproportionately drives adverse cardiovascular outcomes, including stroke, heart failure and chronic kidney disease. Accurate diagnosis first requires distinguishing true RH from pseudo-resistance, which stems from inaccurate measurement, poor medication adherence and clinical inertia. The pathophysiology is multifactorial, driven by mechanisms such as sympathetic nervous system o
