Comparison of Perioperative Outcomes of Laparoscopic and Single-Port Robotic Adrenalectomy.

Background and Objectives: Laparoscopic adrenalectomy is the standard treatment for adrenal tumors, providing reduced postoperative pain, shorter recovery, and improved cosmetic outcomes compared with open surgery. Robotic adrenalectomy was developed to overcome issues resulting from its restricted instrument mobility and two-dimensional vision by providing three-dimensional visualization, tremor elimination, and enhanced dexterity. The single-port (SP) robotic system further reduces invasiveness and improves ergonomics. In this study, we aimed to compare perioperative outcomes between SP robo

Aldosterone-identified targets for optimal sodium and potassium supplementation in intestinal failure.

BACKGROUND: Parenteral support (PS) is a life-saving organ replacement therapy for patients with intestinal failure (IF). However, practical strategies to establish optimal PS sodium and potassium remain poorly defined. We investigated whether elevated plasma renin and aldosterone reflect suboptimal electrolyte supplementation and could help guide physiologically optimal sodium and potassium supplementation in patients with IF. METHODS: Real-world monitoring data from adult IF patients receiving long-term PS were analyzed using mixed-effects models for repeated measurements. Associations betw

Validation of a novel risk stratification model for primary aldosteronism.

Primary aldosteronism (PA) is an under-diagnosed cause of resistant hypertension. In this study, we validated a PA risk stratification model in an independent cohort. Patients screened for PA were identified and risk scores were calculated for each patient. The predicted and observed proportions of PA-positive patients at each risk score were assessed and performance, accuracy, and cut-point analyses were calculated. Among 652 patients screened, 13% were positive for PA. Rates of PA increased significantly (p < 0.001) with increase in risk score and varied from 0% (1 point) to 29% (9 points).

Bilateral superselective adrenal artery embolization for primary aldosteronism: A comparative evaluation of clinical efficacy and safety.

ObjectiveSuperselective adrenal arterial embolization with ethanol has demonstrated promising efficacy in patients with primary aldosteronism and lateralized aldosterone secretion. However, approximately 40% of patients have nonlateralized disease, defined as bilateral primary aldosteronism based on adrenal venous sampling.MethodsIn this prospective observational cohort study, 30 patients with primary aldosteronism underwent superselective adrenal arterial embolization, including 16 patients with unilateral primary aldosteronism and 14 patients with bilateral primary aldosteronism confirmed by

[Primary aldosteronism as an independent factor of kidney damage. Potential therapeutic strategies and research prospects].

Primary aldosteronism (PA) is the most common curable cause of secondary hypertension and an independent risk factor for chronic kidney disease (CKD). This review focuses on the pathophysiological mechanisms of aldosterone-induced renal injury, including mineralocorticoid receptor activation, nongenomic signaling via GPER, oxidative stress, and the upregulation of profibrotic and inflammatory mediators such as TGF-β1, NF-κB, and NLRP3. Chronic aldosterone excess leads to podocyte injury, microvascular dysfunction, and interstitial fibrosis, resulting in decreased glomerular filtration rate and

Metagenomic and metabolomic profiling in primary aldosteronism with coexisting obstructive sleep apnea.

BACKGROUND: Primary aldosteronism (PA) frequently coexists with obstructive sleep apnea (OSA), and this comorbidity is associated with increased cardiometabolic risk. Although both PA and OSA have been individually linked to gut microbiome alterations, it remains unclear which layer of gut microbiome-associated variation best reflects clinical heterogeneity in PA with coexisting OSA. METHODS: In this prospective observational study, we performed shotgun metagenomic sequencing and untargeted fecal metabolomic profiling in 29 adults with clinically confirmed PA, who were stratified according to

Prevalence and cardiometabolic impact of mild autonomous cortisol secretion in primary aldosteronism: a systematic review and meta-analysis.

Many primary aldosteronism (PA) patients harbor concomitant mild autonomous cortisol secretion (MACS), termed "Connshing syndrome." The prevalence and cardiometabolic impact of this co-secretion have not been quantitatively synthesized. To determine the pooled prevalence of MACS in PA and evaluate its association with cardiovascular (CV) events, type 2 diabetes mellitus (T2DM), and obesity. Prevalence and clinical correlates of positive aldosterone-to-renin ratio screening in obese hypertensive patients with obstructive sleep apnea: a Southeast Asian cross-sectional study.

Although routine screening for primary aldosteronism (PA) is increasingly recognized among patients with obstructive sleep apnea (OSA), data regarding aldosterone-to-renin ratio (ARR) screening positivity among Southeast Asian OSA populations remain limited, particularly in resource-limited settings. This study aimed to determine the prevalence and clinical correlates of ARR positivity screening among obese hypertensive OSA patients. In this cross-sectional study, obese hypertensive adults with confirmed OSA were recruited. Blood samples were analyzed for plasma aldosterone concentration (PAC)

Performance of Primary Aldosteronism Screening Tests Before Washout of Dihydropyridine Calcium Channel Blockers: An Exploratory Retrospective Study.

This retrospective cohort study preliminarily investigated the feasibility of implementing primary aldosteronism (PA) screening without discontinuing dihydropyridine calcium channel blockers (DHP-CCBs). We screened all patients undergoing diagnostic testing for secondary hypertension from January 2017 to May 2022 at authors' center. For inclusion, patients must be on DHP-CCBs monotherapy for hypertension. Aldosterone and renin concentration were measured pre- and post-washout. Confirmatory tests were conducted in patients with positive screen [combination of ARR >2.5 (ng/dL)/(μIU/mL), aldoster

Cross-trait genetic analysis maps shared polygenic architecture between primary aldosteronism and blood pressure to adrenal cell states.

BACKGROUND: Primary aldosteronism (PA) is a major cause of endocrine hypertension and contributes substantially to blood pressure (BP)-related morbidity. Although aldosterone excess directly raises BP, the extent to which genetic susceptibility to PA overlaps with the broader polygenic architecture of BP remains unclear. We aimed to characterise the shared genetic architecture between PA and BP traits and to place shared genetic signals within adrenal tissue context. METHODS: We analysed genetic overlap between PA and BP using two independent PA datasets from the French COMETE Network and the

Subclinical primary aldosteronism: toward a conceptual framework linking primary aldosteronism and low-renin hypertension.

Primary aldosteronism (PA) has traditionally been considered a rare cause of secondary hypertension, but growing evidence supports a broader continuum of renin-independent aldosterone production extending into normotensive and mildly hypertensive populations. Subclinical primary aldosteronism (SPA) has emerged as a conceptual framework describing this milder phenotype. However, a universally accepted definition of SPA remains lacking, and consensus diagnostic criteria have yet to be established. This review synthesizes current evidence regarding SPA, including its epidemiology, pathophysiology

Regression of Left Ventricular Hypertrophy in Primary Aldosteronism.

BACKGROUND: Left ventricular hypertrophy (LVH) is frequently identified in patients with primary aldosteronism; determinants of LVH regression after targeted treatment are debated. We assessed LVH prevalence before and after adrenalectomy or mineralocorticoid receptor antagonist (MRA) therapy, identified determinants of LVH regression, and evaluated the relationship between patients' outcomes and cardiac remodeling. METHODS: We retrospectively analyzed an international cohort of patients with primary aldosteronism from 36 referral centers in Spain, Italy, and Germany. Echocardiographic record

Dissociation between nocturnal hypoxemic burden and renin suppression: evidence of sympathetic activation in primary aldosteronism.

PURPOSE: Primary aldosteronism (PA) is frequently accompanied by sleep-disordered breathing, yet the physiological consequences of nocturnal hypoxemia in this population remain poorly understood. Although intermittent hypoxemia is known to activate both the sympathetic nervous system and the renin-angiotensin-aldosterone system in obstructive sleep apnea, whether these responses are preserved in PA, where plasma renin activity (PRA) is chronically suppressed, remains unclear. We investigated the associations between nocturnal hypoxemic burden and autonomic, hormonal, and cardiac markers in pat

Association between primary aldosteronism and type 2 diabetes and identification of potential molecular mechanisms.

BACKGROUND: Primary aldosteronism (PA) is a common cause of secondary hypertension and is associated with an increased risk of cardiovascular and metabolic complications. Emerging evidence suggests a close relationship between PA and type 2 diabetes mellitus (T2DM), with a higher prevalence of impaired glucose metabolism observed in PA patients. However, the underlying molecular mechanisms linking PA and T2DM remain unclear. PURPOSE: This study aimed to investigate the causal relationship between PA and T2DM and to explore the underlying molecular mechanisms and potential therapeutic targets.

Peritoneal dialysis-associated peritonitis in two patients with Bartter syndrome and end-stage kidney disease: a case report.

Bartter syndrome is a rare inherited salt-wasting disorder affecting the thick ascending limb of the loop of Henle. It is characterized by hypokalemic metabolic alkalosis, polyuria, and secondary normotensive hyperaldosteronism. A minority of patients may progress to end-stage kidney disease while maintaining polyuria and normokalemia. Peritoneal dialysis may represent a suitable option for these patients when kidney transplantation is not available. However, due to its rarity, there are no data on PD-related outcomes. Here we report two cases of PD treatment with BS from two Italian referral

Reduced Expression of Magnesium Transport Proteins in the Distal Convoluted Tubule of Clcnkb-Deficient Mice May Explain Urinary Magnesium Wasting in Classical Bartter Syndrome.

AIM: The kidney thick ascending limb (TAL) plays a key role in the transport of sodium, chloride, potassium, calcium, and magnesium. Bartter syndrome is a hypokalemic, salt-losing tubulopathy caused by impaired TAL function. Pathogenic variants in SLC12A1 or KCNJ1 cause antenatal Bartter syndrome, and variants in CLCNKB result in classical Bartter syndrome. Although all variants impair TAL electrolyte transport, their effects on mineral handling differ. In contrast to antenatal forms, classical Bartter syndrome is frequently associated with hypomagnesemia, a feature also found in Gitelman synd

Predicting primary aldosteronism: risk stratification-based and machine learning-based models.

AIMS: Primary aldosteronism (PA) screening is difficult in an unselected population of hypertensive patients, as it can be challenging. We report new algorithms based either on risk stratification or machine learning models to improve the screening of PA. METHODS AND RESULTS: 15,507 patients with confirmed hypertension aged 18-65 years with full workup for PA screening were included in this study. We developed PAstrat, a risk stratification-based algorithm by generating patient groups of interest and assess probability of PA in each group, and two different machine learning models, logistic r

Tissue corticosteroid rhythms are dysregulated predominantly during sleep in primary aldosteronism.

Primary aldosteronism affects 5 to 20% of the hypertensive population and is associated with increased cardiovascular and metabolic risk compared with primary hypertension. Alterations in the dynamics of aldosterone secretion are key, but these are not well captured by current diagnostic pathways, and the disease is often missed. In this proof-of-concept study, we used ambulatory microdialysis sampling to continuously monitor tissue corticosteroid dynamics in primary aldosteronism over 24 hours. In a cohort of 60 patients, we found pulsatile nocturnal hypersecretion of aldosterone and the hybr
