BACKGROUND/OBJECTIVES: Primary aldosteronism (PA), the leading cause of secondary hypertension, results from autonomous aldosterone hypersecretion. It is characterized by increased extracellular volume, elevated cardiac output, and greater arterial stiffness compared with essential hypertension, reflecting aldosterone-mediated hemodynamic dysregulation. The prevalence and morbidity of PA are increasingly acknowledged; however, PA continues to be underdiagnosed because of limited screening and diagnostic complexity.
METHODS: A narrative review was conducted using PubMed (2015-2025), with terms

BACKGROUND: Adrenal venous sampling (AVS) interpretation distinguishes unilateral from bilateral primary aldosteronism (PA), which determines surgical versus lifelong medical management. Data is scant regarding the effects of mineralocorticoid receptor antagonist (MRA) use on the risk of AVS misclassification.
OBJECTIVES: To review the data in the literature about the effect of uninterrupted MRA use on the success of AVS lateralization and postoperative clinical and biochemical outcomes in patients with PA.
METHODS: A systematic search of five different databases (PubMed, Embase, Scopus, Web

Primary aldosteronism (PA) is the most common cause of secondary hypertension in many populations worldwide. Although hypertension with recurrent hypokalemia from renal potassium wasting and simultaneously elevated plasma aldosterone with suppressed renin is classic and pathognomic of PA, there are numerous instances when patients show unexpected, atypical, and confusing biochemical nuances. A thorough understanding of the appropriate diagnosis based on varied plasma aldosterone-renin-potassium concentrations is therefore necessary. We have derived a mathematical model to teach medical student

Adrenal vein sampling (AVS) is the cornerstone diagnostic procedure for subtype differentiation in primary hyperaldosteronism, guiding therapeutic decision-making and offering the possibility of curative surgery to a subset of patients. However, technical challenges limit its use, and success rates vary substantially across centers, restricting equitable access for all patients with primary hyperaldosteronism. To evaluate AVS success rates in major hospitals across Andalucía and to assess the clinical evolution of patients who underwent surgery, with the goal of identifying referral centers an

Hypertension is common in autosomal-dominant polycystic kidney disease (ADPKD) but may occasionally reflect a secondary etiology. We report a 71-year-old man with ADPKD and end-stage renal disease who developed persistent, refractory hypertension following renal transplantation despite multidrug therapy and bilateral native nephrectomy. Evaluation revealed an elevated aldosterone-to-renin ratio, and adrenal venous sampling confirmed unilateral aldosterone excess consistent with a left aldosterone-producing adenoma. The patient underwent robotic adrenalectomy with subsequent improvement in bloo

BACKGROUND: The prevalence of adrenal incidentalomas is increasing substantially with cross-sectional imaging, with reported prevalence from 1% to 5% on computed tomography and up to 8.7% at autopsy. While the majority of incidentalomas are benign nonfunctional adenomas, clinicians face the critical challenge of identifying the rare adrenocortical carcinoma (0.5-2 per million annually), which carries an extremely poor prognosis with 5-year survival of only 50% and <15% if metastatic. Functioning adrenal tumors occur in 27.5% of incidentalomas, including mild autonomous cortisol secretion (MACS

Adrenal venous sampling (AVS) is the gold standard for identifying aldosterone-producing adenoma and bilateral adrenal hyperplasia. This study reports the success rate of AVS and the distribution characteristics of the right adrenal vein orifice (RAVO). The data of 166 patients with primary hyperaldosteronism who underwent AVS in Fuwai Central China Cardiovascular Hospital were retrospectively reviewed. A single experienced cardiologist with 8 years of experience in cardiovascular intervention performed AVS on the included patients and separately drew 3 mL of blood from each adrenal vein and i

BACKGROUND: Cardiac magnetic resonance imaging (CMR) is increasingly used for detailed evaluation of cardiac structures and function. Its wide field of view also allows identification of extracardiac abnormalities, some of which may significantly impact clinical management.
CASE SUMMARY: We present a case of a 44-year-old woman referred for evaluation of prolonged QT interval and hypertension. Investigations revealed mild left ventricular dilatation, and suspicion of long QT syndrome (LQTS). CMR clarified the underlying cardiac abnormalities, identifying a patent ductus arteriosus (PDA) as th

Background/Objectives: The Seated Saline Suppression Test (SSST) is widely used for investigation of primary hyperaldosteronism (PA); however, the impact of a one-hour sitting period prior to SSST (as recommended by the Endocrine Society, with a 7.8 ng/dL aldosterone cut-off) on the test outcome remains unclear. Methods: In 82 individuals (23 males) aged 57.3 ± 12.6 years, BMI 27.1 ± 5.1 kg/m2, where the majority were diagnosed with adrenal incidentalomas (n = 75), we performed an SSST preceded by a 120 min postural test (first protocol), or immediately after waking (second protocol, a cross-o

INTRODUCTION: Liddle's syndrome is a rare inherited disorder associated with early-onset and resistant hypertension due to hyperactivity of epithelial sodium channels (ENaCs). The condition is caused by mutations in the genes SCNN1A, SCNN1B, and SCNN1G. Liddle's syndrome leads to hypertension, hypokalemia, and metabolic alkalosis.
CASE PRESENTATION: A previously healthy 21-year-old male patient complained of severe central chest pain radiating to both arms and had a hypertensive emergency with a BP reading of 267/170 mmHg. The patient had marked elevation in troponin I (13.9 ng/mL), hypokalem

Primary hyperaldosteronism (PHA) is a cause of secondary hypertension, often due to adrenal adenoma. The diagnosis and management of multiple unilateral adrenal adenomas in PHA represent a unique challenge. Adrenal venous sampling (AVS) has become a pivotal diagnostic tool in these cases, providing critical information for accurate localization of aldosterone production. Here, we describe a rare case of a 52-year-old female with a history of resistant essential hypertension who presented with multiple unilateral adrenal adenomas concomitant with adrenal hyperplasia in a case of primary hyperal

Primary hyperaldosteronism is a common and potentially curable cause of hypertension (HTN), affecting 10 to 15% of hypertensive patients. Underdiagnosed, it is associated with a higher cardiovascular risk than essential HTN. Recent recommendations from the Endocrine Society (2025) and the European Society of Cardiology (2024) advocate screening in all patients with newly diagnosed HTN. The diagnosis is based on an increased aldosterone-to-renin ratio. A new classification helps assess disease severity and the likelihood of a unilateral form,. Treatment relies on mineralocorticoid receptor anta

In young patients with resistant hypertension and hypokalemia, primary hyperaldosteronism should be suspected. Biochemical assessment and adrenal imaging could identify surgically curable aldosterone-producing adenomas, and a normalization of blood pressure, potassium levels, and symptom resolution can be achieved through adrenalectomy.

SUMMARY: Bardet-Biedl syndrome (BBS) is a rare autosomal recessive ciliopathy characterized by marked clinical and genetic heterogeneity, classically presenting with retinal dystrophy, obesity, polydactyly, renal dysfunction, and hypogonadism. Although arterial hypertension is frequently observed in affected individuals, it is not currently included in diagnostic criteria and remains insufficiently studied. We report the case of a 35-year-old man with clinically diagnosed BBS who developed severe, treatment-resistant hypertension and hypokalaemia. A comprehensive endocrine evaluation revealed

Refractory hypertension in young adults warrants evaluation for secondary causes. Chronic nonsteroidal anti-inflammatory drug (NSAID) use, though common, is an underrecognized cause of chronic kidney disease that may lead to biochemical abnormalities, including electrolyte disturbances and hormonal dysregulation. A 37-year-old male presented with headache, vomiting, and abdominal pain. Examination revealed severe hypertension (210/190 mmHg) with epigastric tenderness. Electrocardiography (ECG) showed a Wellens type A pattern with Q waves. Initial laboratories revealed hyponatremia, hypokalemia

Bartter syndrome (BS) represents a group of rare, autosomal recessive renal tubular disorders characterized by hypokalemic hypochloremic metabolic alkalosis, secondary hyperaldosteronism, and normal to low blood pressure. The underlying pathophysiology is primarily driven by defects in critical ion transport proteins or channels localized within the thick ascending limb of the loop of Henle, leading to impaired salt reabsorption. Recent advances in molecular genetics have refined the classification of Bartter syndrome. Current evidence supports SLC12A1, KCNJ1, CLCNKB, BSND, and MAGED2 as the c

Compound glycyrrhizin tablets, derived from licorice root, are widely used for their anti-inflammatory and hepatoprotective properties. However, their active metabolite, glycyrrhetinic acid, can induce acquired apparent mineralocorticoid excess (AME) - a clinical syndrome where cortisol abnormally activates mineralocorticoid receptors due to the inhibition of 11β-hydroxysteroid dehydrogenase type 2 (11β-HSD2), mimicking primary aldosteronism. We report a case of a 57-year-old male who developed severe hypertension and hypokalemia after 30 days of treatment with compound glycyrrhizin (150 mg, t
