Heart failure in a 10-year-old girl: incidental discovery of adrenal adenoma by cardiac MRI-case report.

BACKGROUND: Functional adrenal adenoma is uncommon in children, and cardiovascular complications, such as dilated cardiomyopathy and heart failure (HF), are rare as initial manifestations. CASE SUMMARY: Here, we report a 10-year-old girl presenting with acute HF, mild troponin rise and severe left ventricular dysfunction in echocardiography. The Cardiac magnetic resonance imaging (CMR) demonstrated dilated cardiomyopathy, with a left ventricular ejection fraction (LVEF) of 17% with concentric left ventricular hypertrophy and a focal myocardial infarction. Extracardiac findings in CMR showed a

Clinical implications of aldosterone responsiveness to adrenocorticotropic hormone stimulation in two major subtypes of primary aldosteronism.

OBJECTIVE: This study examined differences in aldosterone responsiveness to adrenocorticotropic hormone (ACTH) between the unilateral aldosterone-producing adenoma (APA) and bilateral idiopathic hyperaldosteronism (IHA) subtypes of primary aldosteronism (PA) and the impacts on vascular function and treatment outcomes. METHODS: This retrospective study enrolled 97 patients with PA (63 with APA and 34 with IHA) who underwent adrenal venous sampling (AVS) with ACTH stimulation. During AVS, baseline and ACTH-stimulated plasma aldosterone concentration (PAC) and cortisol levels were also assessed.

7-T Potassium (39K) MRI to Assess Muscle K+ Depletion in Primary Aldosteronism.

Background Although 98% of potassium (K+) resides intracellularly, current clinical diagnostics assess only extracellular K+ concentrations. Noninvasive imaging of tissue K+ distribution could provide novel insights into pathophysiologic processes in diseases such as primary aldosteronism (PA). PA is characterized by excessive aldosterone production, which leads to electrolyte imbalances, hypertension, and increased cardiovascular risk. Purpose To determine whether potassium 39 (39K) MRI combined with sodium 23 (23Na) MRI can help detect alterations in skeletal muscle K+ and Na+ distribution i

Big Data Approach to Assessment of an Aldosterone-Renin Ratio for Detection of Potential Surgical Cases of Primary Aldosteronism.

BACKGROUND: Prior retrospective studies of aldosterone-renin ratio (ARR) measures were confounded by selection bias. METHODS: To define ARR sensitivity independent of case definition, administrative health and diagnostic imaging repositories in Alberta, Canada, were used to construct a 5-component definition for primary aldosteronism (PA) for the most easily recognizable cases. Components included hypertension, hypokalemia, adrenal mass, suppressed renin, and nonsuppressed aldosterone. ARR was omitted from the definition, permitting independent evaluation of the ARR using receiver operating c

Fewer Pills, Lower Bills: Antihypertensive and Cost Outcomes of Adrenalectomy in Primary Aldosteronism.

BACKGROUND: Primary aldosteronism (PA) can be treated surgically or medically depending on disease lateralization and surgical candidacy. There is a dearth of data directly comparing antihypertensive medication trajectories and costs between these strategies. PATIENTS AND METHODS: We performed a retrospective cohort study of patients with new PA diagnoses and adrenal vein sampling to assess antihypertensive medication outcomes and treatment costs using Optum's de-identified Clinformatics® Data Mart Database (2004-2022). Patients were stratified by receipt of adrenalectomy versus medical manag

Biochemical success after selective adrenal arterial embolization predicts long-term regression of aldosterone-mediated cardiac remodeling.

BACKGROUND: Selective adrenal arterial embolization (SAAE) is a minimally invasive, vascular intervention targeting aldosterone-producing tissue in primary aldosteronism (PA). While surgery and mineralocorticoid receptor antagonists are standard therapies, long-term cardiac outcomes after SAAE remain poorly defined. METHODS: In this prospective cohort study, 129 patients with adrenal vein sampling-confirmed unilateral PA who declined adrenalectomy underwent SAAE. Biochemical outcomes were classified by Primary Aldosteronism Surgical Outcome (PASO) criteria as complete (CS, n = 71) or incomple

Partial pressure of oxygen-guided adrenal venous sampling in primary aldosteronism.

CONTEXT: Adrenal venous sampling (AVS) plays a pivotal role in treatment optimization for primary aldosteronism (PA) to minimize cardiovascular risks. However, technical difficulties often hinder accurate cannulation to the adrenal veins (AVs). OBJECTIVE: This study aimed to explore the distribution of partial pressure of oxygen (pO2) in the AV and neighboring veins, inspired by our awareness of the lighter red color of adrenal venous blood than the others. METHODS: We enrolled 179 PA patients who underwent AVS from 2021 to 2024. During AVS, we collected residual blood samples from bilateral

Effect of collection timing on selectivity index in unstimulated adrenal venous sampling: discovery and validation.

This study is to determine peripheral sample collection timing impact on selectivity index (SI) and success of unstimulated adrenal venous sampling (AVS) for primary aldosteronism (PA) subtyping. In this study, a retrospective discovery and a prospective validation arm were conducted. 74 patients undergoing AVS before and after-ACTH stimulation were reviewed. Discovery dataset was divided into 1) "pre" group, peripherals collected 30-min before AVS and 2) "post" group, peripherals collected 1-min after AVS. SIs were calculated using 30-min-pre and 1-min-post values. Patients with samples havin

Improvement of Health-Related Quality of Life and Mental Health After Unilateral Adrenalectomy for Primary Aldosteronism.

BACKGROUND: Primary aldosteronism (PA) has been linked with reduced health-related quality of life (QoL) and increased prevalence of anxiety and depression. While most studies compared patients with PA to healthy subjects, this study focused on comparison with hypertensive patients without PA to identify PA-specific effects. Furthermore, the impact of targeted therapy on QoL and mental health was evaluated. METHODS: Mental and physical health-related QoL, anxiety and presence of depressive and panic symptoms were assessed using the SF-12, GAD-7 and PHQ questionnaires in 925 patients with arte

Primary aldosteronism in premature coronary artery disease: A cross-sectional study from a tertiary center in India.

BACKGROUND: Primary aldosteronism (PA) is the most prevalent cause of secondary hypertension and is associated with adverse cardiovascular outcomes. While its prevalence is well established among patients with resistant hypertension and hypokalemia, evidence regarding its occurrence in premature coronary artery disease (CAD) remains limited, particularly in South Asian populations with a high burden of early cardiovascular disease. OBJECTIVE: To determine the prevalence of PA in patients with premature CAD (≤45 years) and to describe their clinical and biochemical characteristics. METHODS: I

Hypertension and primary aldosteronism diagnosis in hospitalized patients: an observation from public hospitals in Victoria, Australia.

BACKGROUND: Current efforts of increasing diagnosis of primary aldosteronism (PA), a potentially curable but under-diagnosed form of hypertension, have been limited to primary care and specialist clinics, leaving out additional opportunities within healthcare. This study estimated hypertension and PA diagnosis rates during hospital admission. METHODS: Public hospital admissions in Victoria, Australia, between 2007/8 and 2018/19 from patients aged ≥ 15years were retrospectively analysed to estimate rates of hypertension as principal diagnosis and PA as principal and secondary diagnosis to hyper

Can the size of adrenal tumor impact hormonal secretion? A retrospective analysis of 170 adrenalectomies in a single center.

PURPOSE: Adrenal tumors are a heterogeneous group of neoplasms that may be hormonally active or inactive. Tumor size is a key factor in evaluating and managing these lesions. Understanding how size impacts hormonal activity, surgical outcomes, and complications can help guide treatment and follow-up. This study investigates the relationship between tumor size, hormonal secretion, and clinical outcomes in adrenal tumors. METHODS: We retrospectively analyzed data from patients who underwent adrenalectomy between 2007 and 2023 at the University Hospital of Basel. Tumors were categorized by pathol

Aldosterone and Impaired Nocturnal Blood Pressure Decline in Primary Aldosteronism.

BACKGROUND: The loss of physiological nocturnal blood pressure (BP) decline is a major contributor to cardiovascular risk, yet its endocrine underpinnings in primary aldosteronism (PA) remain underexplored. This study aimed to elucidate the dose-dependent relationship between aldosterone excess and circadian BP rhythm disruption and assess whether targeted aldosterone suppression could ameliorate nocturnal dipping. METHODS: We prospectively analyzed 24-hour ambulatory BP data from 681 patients with confirmed PA, stratified by plasma aldosterone concentration. A subset of 99 patients with adre

Baxdrostat for the treatment of primary aldosteronism: current evidence.

Primary aldosteronism (PA) is a common form of secondary hypertension, estimated to affect up to 20% of individuals with resistant hypertension. High levels of aldosterone induce several genomic and nongenomic effects, which lead to comorbidities beyond high blood pressure (BP), including excess cardiovascular and renal risk. Adrenalectomy is the treatment of choice for unilateral PA, while mineralocorticoid receptor antagonists (MRAs) represent the standard medical treatment for PA. However, many individuals fail to achieve complete medical treatment responses. This may be partly due to treat

The Pharmacological Properties and Safety Profile of Mineralocorticoid Receptor Antagonists in Heart Failure with Reduced Ejection Fraction.

Among the established mineralocorticoid receptor antagonists (MRAs), spironolactone and eplerenone have demonstrated significant clinical utility in managing conditions such as chronic heart failure, resistant hypertension, and hyperaldosteronism. Spironolactone, the first steroidal MRA, is known for its broad receptor affinity, contributing to both therapeutic benefits and endocrine-related side effects. Eplerenone, a more selective agent, offers improved tolerability with reduced hormonal adverse effects. This review explores the pharmacokinetic and pharmacodynamic profiles of these agents,

Circulating soluble CXCR4 fails to differentiate subtypes of primary aldosteronism.

Accurate subtyping of primary aldosteronism (PA) is essential for treatment selection. This study examined whether soluble CXC chemokine receptor type 4 (sCXCR4) in peripheral blood reflects adrenal CXCR4 expression assessed by 68Ga-pentixafor PET/MR, and whether sCXCR4 aids PA classification. A total of 160 subjects were enrolled, including 88 patients with PA (52 aldosterone-producing adenoma [APA], 36 idiopathic hyperaldosteronism [IHA]) and 72 healthy volunteers. Serum sCXCR4 was measured by enzyme-linked immunosorbent assay. Group comparisons, correlation analyses, and receiver operating

KCNJ5-Negative Primary Aldosteronism: Histopathology and Surgical Outcomes.

BACKGROUND: Unilateral primary aldosteronism (uPA) is the most common surgically curable secondary hypertension. KCNJ5 mutations are frequent in aldosterone-producing adenomas, but the clinical significance of KCNJ5-negative uPA remains unclear. We investigated whether histopathologic subtypes defined by the HISTALDO (Histopathology of Primary Aldosteronism) consensus are associated with postoperative outcomes in KCNJ5-negative uPA. METHODS: We analyzed 171 KCNJ5-negative uPA patients from the Taiwan Primary Aldosteronism Investigation registry who underwent adrenalectomy. Somatic mutations w

The Importance of Molecular Testing in the Diagnosis of Genetic Syndromes with Chronic Kidney Disease: Genotype-Phenotype Correlations.

Globally, chronic kidney disease (CKD) affects over 800 million individuals and is characterized by significant genetic complexity. More than 600 genes are associated with hereditary kidney disease, which may manifest as isolated kidney issues or as part of a syndrome that also includes extrarenal manifestations. The aim of this study was to identify genetic variants in a group of ten patients who presented with clinical signs suggestive of genetic syndromes associated with CKD, or who were asymptomatic but had a positive family history of CKD. Extensive genetic testing (targeted gene panels a
