Human 3D Cardiac Microtissue Model to Investigate Aldosterone-Induced Fibrosis and Electrical Dysfunction.

BACKGROUND: Aldosterone induces cardiac fibrotic remodeling and arrhythmogenic alterations. The lack of suitable preclinical models has hampered an in-depth investigation of the molecular mechanisms involved in aldosterone-induced cardiac damage. Our aim was to evaluate the effects of aldosterone on 3D human microtissue (hMT) cardiac organoids. METHODS: hMT were generated by coculturing human cardiac fibroblasts, aortic endothelial cells and induced pluripotent stem cell-derived cardiomyocytes. hMT were treated with aldosterone, the mineralocorticoid receptor antagonist eplerenone, and serum

Lipid Profile in Primary Aldosteronism: Cross-Sectional and Post-Treatment Analyses.

The relationship between primary aldosteronism (PA) and lipid metabolism remains controversial, with inconsistent findings reported in the literature. This study aimed to clarify PA's impact on the lipid profile using both a cross-sectional comparison with essential hypertension (EH) controls and a longitudinal within-patient pre-post treatment analysis in a North African tertiary-center cohort. This retrospective study included 112 patients with PA and 115 contemporaneously hospitalized patients with EH in whom secondary hypertension had been excluded. Sociodemographic, clinical, and biochemi

Real world experience with [68Ga]PentixaFor PET/CT in Primary Aldosteronism using newly developed harmonized diagnostic criteria.

BACKGROUND: Positron emission tomography (PET) of the adrenal glands emerged as a non-invasive alternative to adrenal vein sampling (AVS) for treatment selection in primary aldosteronism (PA). The C-X-C-chemokine-receptor type 4 targeting tracer [68Ga]PentixaFor showed potential in visualizing aldosterone-producing adenomas. However, standardized diagnostic criteria and experience in European cohorts remain scarce. Here, we report harmonized PET/CT interpretation criteria, assess their impact on diagnostic agreement, and present a large European tertiary center's experience with [68Ga]PentixaF

Increased Frequency and Distinct Genotypic Patterns of Somatic Aldosterone-Driver Mutations in Aldosterone-Producing Micronodules from Primary Aldosteronism Patients.

Primary aldosteronism (PA) is the leading cause of endocrine hypertension. With the development of highly specific human aldosterone synthase (CYP11B2) antibodies for immunohistochemistry (IHC), the presence of microscopic subcapsular foci of CYP11B2-postive cells – now termed aldosterone-producing micronodules (APM) – has been documented in normal and PA adrenal glands, however, there continues to be debate regarding the role of APM in the pathogenesis of PA. In this study, CYP11B2 IHC-guided targeted next-generation sequencing (NGS) was utilized to characterize the frequency and spectrum of

Germline and somatic genetic drivers of adrenocortical tumours.

Adrenal tumours affect 1-4% of the population, with a prevalence increasing with age, tumours arising from the adrenal cortex being the most frequent, unlike the adrenomedullary tumours. Adrenocortical tumours are defined by their benign or malignant potential, and unilateral or bilateral, hormone-producing or non-functioning nature. As any tumoral formation, all adrenocortical tumours are clonal proliferations driven by genetic events, whether by a germline predisposition or by somatic alterations. By summarizing the genetic findings of the last 30 years of adrenocortical research, this revie

Dexamethasone Nonsuppression in Primary Aldosteronism: Associations With Adrenal Imaging Findings.

BACKGROUND: The 1-mg dexamethasone suppression test (DST) nonsuppression is increasingly recognized beyond adrenal incidentaloma, but its prevalence and clinical significance in patients evaluated for primary aldosteronism (PA) remain unclear, particularly in those without adrenal nodules. METHODS: In this prospective observational study, consecutive patients with hypertension suspected of PA underwent standardized hormonal evaluation, including the 1-mg DST. DST nonsuppression was defined as post-DST serum cortisol >1.8 μg/dL in the absence of overt Cushing's syndrome. Patients were classifi

Sex Differences in Cardiac Remodeling and Dysfunction in Primary Aldosteronism.

BACKGROUND: Sex differences influence cardiovascular risk assessment and management; however, their role in aldosterone-mediated cardiac remodeling in primary aldosteronism remains incompletely understood. METHODS: We conducted a retrospective study of 547 patients with primary aldosteronism, including 249 men and 298 women. Clinical and echocardiographic data were collected at baseline and 1 year following aldosterone-targeted therapies. RESULTS: The mean age was 53.8 years in men and 54.6 years in women. At baseline, men had a higher left ventricular mass index (LVMI), whereas women had a

Prospective randomised trial comparing thermal ablation With laparoscopic Adrenalectomy as an alternatiVE treatment for unilateral asymmetric primary aldosteronism: a protocol for the WAVE trial.

INTRODUCTION: Unilateral aldosterone-producing adenomas (>1 cm) and aldosterone-producing nodules (<1 cm) are a common cause of primary aldosteronism (PA) and hypertension. Adrenal surgery can potentially cure or significantly improve the condition. However, fewer than 1% of patients with PA undergo adrenalectomy. For some, this is due to surgical risks, service capacity or patient preference against surgery. In these individuals, thermal ablation may provide an alternative. This paper describes the protocol for the WAVE trial, designed to compare thermal ablation with the current standard, la

DPA/AA ratio as a potential biomarker of primary aldosteronism in young and middle-aged adults.

BACKGROUND: Primary aldosteronism (PA) is a common cause of secondary hypertension, yet its diagnosis in young and middle-aged adults remains challenging due to low screening rates and complex testing procedures. OBJECTIVE: This study aimed to investigate serum polyunsaturated fatty acid (PUFA) profiles, particularly the docosapentaenoic acid/arachidonic acid (DPA/AA) ratio, as novel biomarkers of PA in young adults. METHODS: This 1:1 matched case-control study was performed on 138 young and middle-aged patients with PA and 138 essential hypertension (EH) controls. Serum PUFA levels were mea

Coexistence of fibromuscular dysplasia and primary aldosteronism : an underrecognized association.

This study describes a unique cohort of patients with concomitant fibromuscular dysplasia (FMD) and primary aldosteronism (PA), two distinct causes of secondary hypertension rarely reported together. By combining clinical, biochemical, and imaging data, we characterize their overlapping features and discuss potential shared hormonal and molecular mechanisms. Our findings suggest that coexistence of FMD and PA may increase vascular complication risk and warrants systematic cross-screening in selected hypertensive patients.

Histopathology and Molecular Profiling of Primary Aldosteronism.

Primary aldosteronism is the most common surgically treatable form of hypertension. Surgically amenable forms are characterized by lateralized aldosterone overproduction, which may occur along a spectrum ranging from true unilateral hypersecretion to asymmetrical bilateral production. Historically attributed to solitary aldosterone-producing adenomas, lateralized primary aldosteronism is now recognized to encompass a range of histopathologic lesions, including aldosterone-producing micronodules, reflecting a continuum from subclinical autonomous secretion to overt primary aldosteronism. This r

Aldosterone fuels the progression of cardiovascular-kidney -metabolic syndrome: focus on primary aldosteronism spectrum.

In 2023, the American Heart Association (AHA) introduced the Cardiovascular-Kidney-Metabolic (CKM) syndrome concept to address the substantial burden of interrelated cardiovascular, kidney, and metabolic disorders. The framework highlights that chronic kidney disease (CKD) significantly accelerates CKM syndrome progression and increases cardiovascular risk, an effect that may be closely paralleled by aldosterone excess. Excess aldosterone can arise from renin-dependent aldosteronism (RDA), a primarily physiological state (not discussed in this review), or from renin-independent aldosteronism (

A Practical Strategy for Primary Aldosteronism Subtyping: Leveraging the Unilateral Adrenal Vein Sampling Index When Bilateral Cannulation Fails.

OBJECTIVE: Adrenal vein sampling (AVS) is the standard procedure for identifying unilateral primary aldosteronism (uPA) in surgical candidates. However, many patients undergo nondiagnostic or partially successful AVS, which may preclude them from curative surgery. This study aimed to evaluate the diagnostic utility of the unilateral AVS index for identifying uPA. METHODS: In this multicenter, retrospective study, we enrolled PA patients who underwent at least unilateral selective AVS, regardless of adrenocorticotropic hormone (ACTH) stimulation. The diagnostic accuracy of the relative aldoste

Gut microbiota signatures in primary aldosteronism and functional identification of an aldosterone-degrading gut bacterium.

Primary aldosteronism (PA), a major cause of secondary hypertension, is characterized by autonomous aldosterone overproduction. Although the gut microbiota is closely linked to blood pressure regulation, its role in PA remains unclear. We performed metagenomic sequencing on fecal samples from 13 patients with essential hypertension (EH), 57 with unilateral PA (UPA), and 51 with bilateral PA (BPA). Despite comparable overall microbial diversity, gut microbial compositional differences were observed among EH and PA subtypes, particularly at finer taxonomic levels. We next identified 39 microbial

Respiratory triggered 3D bSSFP MRI enables accurate noninvasive localization of the right adrenal vein confluence in primary aldosteronism.

Catheterization of the right adrenal vein (RAV) is a technically challenging step in adrenal venous sampling (AVS) for primary aldosteronism. Accurate preprocedural evaluation of RAV anatomy is essential for procedural success. To assess the feasibility of respiratory-triggered three-dimensional balanced steady-state free precession (RT-3D bSSFP) magnetic resonance imaging for visualizing the confluence of the RAV with the inferior vena cava (IVC), in comparison with contrast-enhanced computed tomography (CECT). In this retrospective study, we evaluated RT-3D bSSFP, CECT, and CV for RAV identi

High-gradient magnetic separation chip-based small extracellular vesicle isolation for noninvasive subtyping of primary aldosteronism.

Primary aldosteronism (PA), a leading cause of secondary hypertension, remains vastly underdiagnosed due to unreliable screening tools. We developed an integrated platform combining a high-gradient magnetic separation three-dimensional (HGMS-3D) chip and locked nucleic acid (LNA)-enhanced droplet digital polymerase chain reaction (ddPCR) for noninvasive detection of potassium inwardly rectifying channel subfamily J member 5 (KCNJ5) mutations in plasma small extracellular vesicles (sEVs). The HGMS-3D chip uses a nickel mesh-based stereoscopic immunoaffinity capture system, achieving a vesicle i

Clinical implications of 68Ga-pentixafor PET/CT in surgically treated primary aldosteronism.

BACKGROUND: CXCR4-targeted positron emission tomography/computed tomography has demonstrated potential utility in functional characterization and lateralization assessment of primary aldosteronism. However, clinical implications for postoperative outcomes after adrenalectomy and biologic correlations with adrenal histopathology remain incompletely defined in surgical cohorts. METHODS: We retrospectively analyzed data from 188 patients with primary aldosteronism who underwent CXCR4-targeted 68Ga-pentixafor positron emission tomography/computed tomography before adrenalectomy.
