Classifying Diabetic and Healthy β-Cells in Type 2 Diabetes Using Machine Learning on Single-Cell RNA Sequencing Data.

Diabetes is a chronic metabolic disorder characterized by elevated blood glucose levels due to impaired insulin production or function. Two main forms are recognized: type 1 diabetes, which involves the autoimmune destruction of insulin-producing β-cells, and type 2 diabetes (T2D), which arises from insulin resistance and progressive β-cell dysfunction. Understanding the molecular mechanisms underlying these diseases is essential for the development of improved therapeutic strategies, particularly those targeting β-cell dysfunction. To investigate these mechanisms in a controlled and biologica

Addisonian Crisis Mimicking Acute Kidney Injury in Dogs: A Retrospective Study of 34 Dogs Diagnosed with Acute Kidney Injury in Romania.

Primary hypoadrenocorticism (Addison's disease) is an uncommon but potentially life-threatening endocrine disorder in dogs. Affected animals may present with clinicopathological features mimicking acute kidney injury (AKI). The challenge in diagnosing hypoadrenocorticism arises from its highly heterogeneous and non-specific clinical presentation, including acute kidney injury (AKI). This retrospective observational study aimed to evaluate dogs presenting with AKI and to identify cases in which primary hypoadrenocorticism was the underlying etiology. Thirty-four dogs diagnosed with acute kidney

Addisonian crisis following osteoprotective therapy in a patient with secondary adrenal insufficiency.

Addisonian crisis secondary to parenteral therapy with zoledronic acid has been rarely reported in the literature. The case report herein presents a case of a 27-year-old female patient with a known history of panhypopituitarism and secondary adrenal insufficiency who developed an Addisonian crisis following her first intravenous infusion of zoledronic acid. This highlighted the role of zoledronic acid as a potential trigger. This is probably mediated by an acute-phase reaction with the lack of a sufficient cortisol response. Therefore, close monitoring of patients with adrenal insufficiency u

Vena Caval Hematoma Secondary to Adrenal Hemorrhage From an Addisonian Crisis in a Dog.

An 11-year-old spayed female American Staffordshire Terrier was evaluated for an Addisonian crisis. Abdominal CT scan revealed a mass-like lesion arising from the right adrenal gland and invading into the vena cava. The patient was treated for an Addisonian crisis, and no further treatment for the right adrenal gland mass was pursued by the clients. The patient presented 1 year later for re-evaluation of the right adrenal gland mass, and an abdominal CT scan was performed, which showed the previous right adrenal gland mass had resolved, and both adrenal glands were now atrophied. Adrenal hemor

Emergency management of hyperkalemia in dogs and cats - Part 2: Diagnosis and treatment.

OBJECTIVE: Hyperkalemia in dogs and cats can cause rapid cardiac and neuromuscular compromise. Fast recognition, ECG-guided stabilization, and cause-directed therapy improve survival. ANIMALS AND PROCEDURE: Part 2 of this 2-part review gives a patient-side approach for recognition, confirmation, and emergency treatment of hyperkalemia. It integrates current veterinary evidence and core physiology into step-by-step guidance for ECG interpretation, point-of-care testing, drug selection, and monitoring. It also maps 1st-hour priorities to common etiologies such as urethral obstruction and hypoad

When physiology fails: Endocrine and clinical perspectives on complicated pregnancy and parturition outcomes in dogs.

In this review, metabolic, endocrine and molecular dysregulation of canine pregnancy and parturition complications are discussed in a clinical context. The focus on metabolic and hormonal abnormalities of pregnancy includes gestational diabetes mellitus, pregnancy toxemia, hypocalcemia, luteal insufficiency and hypothyroidism, all with potential negative maternal and fetal outcomes. Gestational pseudo-Addisonian crisis, a rather infrequent but severe complication of canine pregnancy with yet unexplained pathophysiology is also presented. While most of these pregnancy complications have long be

Pharmacokinetics of inhaled prednisolone for adrenal crisis: an exploratory study.

OBJECTIVE: An adrenal crisis is a potentially life-threatening medical emergency that requires rapid treatment with glucocorticoids. Guidelines advise immediate self-administration of hydrocortisone by intramuscular injection using an emergency management kit in case an adrenal crisis is suspected. However, self-injection is often not performed due to administration complexity or patient anxiety. Pulmonary administration of glucocorticoids may be a more patient-friendly and suitable alternative, especially when administered using a dry powder inhaler. Before advancing to the development of a d

The Adrenals Under Silent Siege: Diffuse Large B-Cell Lymphoma Presenting as Addisonian Crisis.

Primary adrenal insufficiency (Addison's disease) is a potentially life-threatening disorder most commonly caused by autoimmune adrenalitis, though rare etiologies such as infiltrative malignancies must be considered, particularly when bilateral adrenal involvement leads to clinical manifestations. Diffuse large B-cell lymphoma (DLBCL), an aggressive subtype of non-Hodgkin lymphoma, may rarely involve the adrenal glands and precipitate adrenal crisis. We report a 58-year-old male patient who presented with giddiness, vomiting, fatigability, hyperpigmentation, weight loss, and hypotension. Labo

Addisonian Crisis in a 39-Year-Old Woman With Primary Adrenal Insufficiency.

Primary adrenal insufficiency, or Addison's disease, is a rare endocrine disorder characterized by inadequate cortisol and aldosterone production. Addisonian crisis is a life-threatening complication triggered by physiological stress, infection, or medication non-adherence. We present the case of a 39-year-old Caucasian woman with a history of Addison's disease, Hashimoto's thyroiditis, celiac disease, vitiligo, and alopecia areata who presented with hypotension, bradycardia, and nausea secondary to an Addisonian crisis, likely precipitated by cyclic nausea and vomiting due to viral gastroente

Pseudo-Addisonian Crisis With Hyponatremia and Hyperkalaemia in a Late Pregnant Bitch.

CASE PRESENTATION: A 3-year-old pregnant Golden Retriever bitch was presented 52 days after ovulation for nausea, hyporexia, regurgitation, polyuria, and polydipsia. Reproductive ultrasound demonstrated several viable foetuses, and the bitch was treated conservatively. Two days later, the bitch was hospitalized due to continued hyporexia, vomiting, abdominal distention, and an acute onset of haematochezia. Blood work showed regenerative mild anaemia and neutrophilia with a left shift, metabolic acidosis, hypocalcaemia, hyponatremia, hypochloraemia, hyperkalaemia, and Na:K ratio of 24:1, raisin

A Case of Diagnostic Delay: Autoimmune Adrenalitis Mimicking Functional Gastrointestinal Disorder.

Autoimmune adrenalitis is an under-recognized cause of primary adrenal insufficiency in young individuals. Its nonspecific presentation can often mimic gastrointestinal or psychiatric conditions, leading to delayed diagnosis and life-threatening adrenal crisis. We report the case of a 23-year-old male having known treated tuberculous meningitis history who presented with recurrent episodes of vomiting, generalized weakness, and altered behavior. He was hypotensive, drowsy, and had metabolic derangements, including hyponatremia and hyperkalemia. Laboratory investigations revealed low serum cort

Congenital adrenal hyperplasia, ovarian adrenal rest tumours and Addison's disease: an exceptional clinical constellation.

We report a woman in her 20s with congenital adrenal hyperplasia (CAH) due to a clinical diagnosis of 11β-hydroxylase deficiency, bilateral ovarian adrenal rest tumours (OARTs), autoimmune Addison's disease and premature ovarian insufficiency. Despite the combination of CAH and OARTs, she conceived spontaneously twice, with complete regression of the tumours under glucocorticoid therapy. Years later, she developed an Addisonian crisis confirmed by positive 21-hydroxylase antibodies and subsequently manifested ovarian failure and mineralocorticoid deficiency. This case illustrates the rare coex

Characteristics of Successful Third-Line Eradication of Helicobacter pylori based on Antibiotic Susceptibility.

BACKGROUND: Regimens using vonoprazan (VPZ), amoxicillin (AMX), and sitafloxacin (STFX) have been reported to be effective for the third-line eradication of Helicobacter pylori and are widely used in Japan. However, the outcomes of these therapies have only been reported by a limited number of institutions. We aimed to investigate the backgrounds and treatment outcomes of patients who underwent successful third-line eradication of H. pylori. METHODS: This was a single-center, retrospective, observational study included patients who underwent antibiotic susceptibility testing and third-line er

Transient Third-Degree Atrioventricular Block in a Dog with Addisonian Crisis.

A 3-year-old spayed male mixed-breed Labrador presented to the Emergency and Critical Care Unit with lethargy, loss of appetite, vomiting, a recent history of presyncopal episodes, and severe exercise intolerance. On admission, the patient had bradycardia, low blood pressure, and mild abdominal pain. Serum biochemistry information revealed severe hyperkalemia, hyponatremia, hypoglycemia, and mildly increased liver and kidney parameters. Upon taking these findings into consideration, hypoadrenocorticism was suspected, and a basal cortisol level was determined; it was below 1.0 µg/dL. Upon exami

Does pre-emptive dexamethasone provide prophylaxis against sugammadex-induced bradycardia? A retrospective study.

Sugammadex is a cyclodextrin used to reverse neuromuscular block with amino-steroid nondepolarizing muscle relaxants, rocuronium and vecuronium. Sugammadex-induced bradycardia was recently demonstrated in a single-blind, placebo-controlled study in patients receiving rocuronium for neuromuscular block. It has also been hypothesized that the bradycardia and rare instances of cardiac arrest occurring after the use of sugammadex may be due to a transient decrease in circulating corticosteroids, causing a temporary 'mini Addisonian crisis.' It was proposed that the administration of corticosteroid
