Optimization of Permeable Cryoprotectants for Neisseria meningitidis Based on Assessment of Ice Inhibition and Cytotoxicity.

INTRODUCTION: Neisseria meningitidis, the primary pathogen of epidemic meningococcal meningitis, is a typical fastidious bacterium with stringent nutritional requirements and high environmental sensitivity, making routine preservation challenging. Current cryopreservation methods lack quantitative efficacy evaluation and suffer from low viability. OBJECTIVE: This study aimed to screen optimal permeable cryoprotectants (CPAs) for fastidious bacteria typified by N. meningitidis via cytotoxicity, cryopreservation viability, and ice crystal inhibition assays and elucidate the underlying protectiv

Purulent meningococcal pericarditis with cardiac tamponade in a Hajj pilgrim: A diagnostic challenge of anchoring bias on MERS-CoV.

We report a rare case of purulent pericarditis with cardiac tamponade due to Neisseria meningitidis serogroup W135 in a 15-year-old pilgrim returning from the Hajj. Initial suspicion of Middle East respiratory syndrome coronavirus (MERS-CoV) was prompted by pneumonia and camel milk exposure, misdirecting the diagnosis. The patient developed cardiac tamponade requiring urgent pericardiocentesis; 16S rRNA PCR confirmed N. meningitidis W135 despite negative cultures. He recovered fully with ceftriaxone and colchicine. This case highlights diagnostic anchoring bias in travel medicine and the need

Post-COVID-19 pandemic inequities in routine childhood immunization in Canada: racial disparities in uptake of polio, meningococcal conjugate, and varicella vaccines.

BACKGROUND: Decline in routine childhood vaccination coverage in Canada has accelerated following the COVID-19 pandemic, increasing risks of vaccine-preventable diseases. Yet, little is known about how this affects different racial groups. This study examines racial disparities in uptake of childhood polio, meningococcal, and varicella vaccines, and the roles of health literacy and conspiracy beliefs. METHODS: We conducted a national survey that included 2,528 Canadian parents of children aged 0-12 years. Parents reported their child's vaccination status and completed measures of health liter

Disseminated meningococcal infection presenting with constrictive pericarditis and bilateral septic arthritis without meningitis: A case report.

Invasive meningococcal disease is a life-threatening infection that classically presents as meningitis or meningococcemia. Extra-meningeal manifestations, including pericarditis and septic arthritis, are uncommon and may complicate diagnosis, particularly in the absence of meningeal involvement. A previously healthy vaccinated man in his 50s presented with acute bilateral knee pain and swelling following a short history of fever, productive cough, fatigue, and dyspnea. Blood cultures grew Neisseria meningitidis serogroup W, whereas cerebrospinal fluid, synovial fluid, and pericardial fluid cul

Serum bactericidal activity against meningococcus in patients with systemic lupus erythematosus.

BACKGROUND: Patients with systemic lupus erythematosus (SLE) are susceptible to infectious diseases owing to various immunosuppressive treatments and disease characteristics. Meningococcal infections progress rapidly with a high incidence of severe complications and mortality; therefore, meningococcal vaccination is needed. However, there is limited evidence regarding the immunity and immunogenicity of patients with SLE. PURPOSE: This study aimed to analyze the serum bactericidal activity against meningococci in patients with SLE in 2 domestic institutions in Korea. METHODS: Serum samples we

Successfully treated myopericarditis and acute heart failure due to severe Neisseria meningitidis infection: a case report.

Neisseria meningitidis is a known cause of bacterial meningitis and sepsis. It is rarely associated with serious cardiac manifestations. A 39-year-old woman diagnosed with meningococcal meningitis/septicemia developed myopericarditis leading to severe acute heart failure. Diagnosis was established by clinical, laboratory and echocardiographic findings and was later confirmed by cardiac MRI. Management included antibiotics, vasopressors and early intubation. Her course was further complicated with disseminated intravascular coagulation and unilateral hearing loss. Cardiac dysfunction resolved c

Comparative immunologic profiling of mRNA and protein-conjugated vaccines: acute inflammatory responses and anti-PEG antibody production.

Messenger ribonucleic acid (mRNA) vaccines have become a prevalent immunization method, even as the coronavirus disease 2019 (COVID-19) pandemic recedes. However, the potential adverse effects using mRNA vaccines need to be explored in this evolving landscape. In this study, 60 participants were randomly assigned to receive either an mRNA vaccine, specifically for COVID-19, or a conventional vaccine for meningococcal disease. Symptom records and blood samples were collected on Days 0, 3, and 7 after vaccination. Results showed that recipients of mRNA vaccines exhibited elevated levels of serum

Long-term survival after invasive pneumococcal disease: a matched cohort study using electronic health records in England.

BACKGROUND: Invasive pneumococcal disease (IPD) is associated with increased long-term mortality, but it is unclear if this is explained by pre-existing comorbidities. We aimed to estimate the long-term survival following IPD in comparison with the general population, adjusting for potential confounders such as underlying comorbidities. METHODS: We conducted a matched cohort study comparing long-term survival (>120 days after infection) in individuals with IPD and comparators without IPD. Cases were individuals aged 65 years or older with laboratory-confirmed IPD (2012-19) identified through

Culture-negative infective endocarditis due to Neisseria bacilliformis identified via 16S rRNA gene analysis from resected valve tissue: Case report and review of the literature.

Blood culture-negative infective endocarditis (BCNE) has a poorer prognosis than culture-positive cases. Thus, it is crucial to determine the pathogenic microorganism using molecular diagnostic techniques, in addition to conventional techniques, including cultures of blood and/or resected valve tissue. Herein, we report a case of culture-negative infective endocarditis (IE) caused by Neisseria bacilliformis, as identified by 16S rRNA analysis of valve tissue. N. bacilliformis a non-gonococcal and non-meningococcal Neisseria species that partially comprises the oropharyngeal microbiome, and rep

Vaccine-Preventable Infections Among Solid Organ Transplant Recipients in Switzerland.

IMPORTANCE: Vaccine responses are decreased in solid organ transplant (SOT) recipients, and given the complexity of implementation, vaccination programs may be suboptimal. The actual burden of vaccine-preventable infections (VPIs) among SOT recipients remains unclear. OBJECTIVES: To assess the incidence rate of VPIs among SOT recipients and to evaluate whether SOT recipients are at increased risk for specific VPIs compared with the general population. DESIGN, SETTING, AND PARTICIPANTS: This nationwide cohort study used data from the Swiss Transplant Cohort Study on VPIs in individuals who un

Increasing human monoclonal antibody cloning efficiency with a whole-cell modified immunoglobulin-capture assay (mICA).

Expression cloning of fully human monoclonal antibodies (hmAbs) is seeing powerful utility in the field of vaccinology, especially for elucidating vaccine-induced B-cell responses and novel vaccine candidate antigen discovery. Precision of the hmAb cloning process relies on efficient isolation of hmAb-producing plasmablasts of interest. Previously, a novel immunoglobulin-capture assay (ICA) was developed, using single protein vaccine antigens, to enhance the pathogen-specific hmAb cloning output. Here, we report a novel modification of this single-antigen ICA using formalin-treated, fluorescen

Neisseria meningitidis accumulate in large organs during meningococcal sepsis.

BACKGROUND: Neisseria meningitidis (Nm) is the cause of epidemic meningitis and fulminant meningococcal septicemia. The clinical presentations and outcome of meningococcal septic shock is closely related to the circulating levels of lipopolysaccharides (LPS) and of Neisseria meningitidis DNA (Nm DNA). We have previously explored the distribution of Nm DNA in tissues from large organs of patients dying of meningococcal septic shock and in a porcine meningococcal septic shock model. OBJECTIVE: 1) To explore the feasibility of measuring LPS levels in tissues from the large organs in patients wit

Ethylmalonic encephalopathy masquerading as meningococcemia.

Ethylmalonic encephalopathy (MIM #602473) is a rare autosomal recessive metabolic condition caused by biallelic variants in ETHE1 (MIM #608451), characterized by global developmental delay, infantile hypotonia, seizures, and microvascular damage. The microvascular changes result in a pattern of relapsing spontaneous diffuse petechiae and purpura, positional acrocyanosis, and pedal edema, hemorrhagic suffusions of mucous membranes, and chronic diarrhea. Here, we describe an instructive case in which ethylmalonic encephalopathy masqueraded as meningococcal septicemia and shock. Ultrarapid whole-
