A young man with chest pain and fever.

A young man with an unremarkable medical history was diagnosed with meningococcal endocarditis and was treated with penicillin and gentamycin. Further history was complicated by a fistulisation between the left ventricle and right atrium and aortic and tricuspid valve insufficiency. Aortic valve replacement, correction of the fistulisation and annuloplasty of the tricuspid valve were performed with good results. A search of the literature revealed only ten cases of meningococcal endocarditis without any report concerning the tricuspid valve.

[Analysis of the epidemiological characteristics and prognostic factors in probable or confirmed invasive meningococcal disease in a cohort of adolescents and adults during an epidemic outbreak].

OBJECTIVE: To describe the profile of people suffering Invasive Meningococcal Disease in Andalusia and the Canary Islands, and identify the risk factors for death. MATERIAL AND METHODS: A retrospective study was designed, recruiting cases from week 41 of 1995 to week 40 2000. Cases were probable or definite, and were extracted from the databases of the hospital by examining diagnosis at discharge or death. RESULTS: 167 cases were identified, with a mortality rate of 7.2%. Mean age was 28.88 years, this being greater in those who died (p = 0.041). There was no previous contact with the Health

Systemic manifestations in the course of meningococcal disease.

BACKGROUND/AIM: Meningococcal disease most often manifests itself as meningitis or sepsis. During the course of these diseases, other clinical events sometimes develop such as pneumonia, pericarditis, arthritis, and they are referred to as extrameningeal or systemic manifestations of the meningococcal disease. The aim of this study was to investigate the type and the incidence of particular extrameningeal/systemic manifestations among patients with meningococcal meningitis and sepsis, including time of their onset and the influence on the disease outcome. METHODS: The retrospective study of t

Self assessment exercises in Intensive Care Medicine.

Intensive Care Medicine (ICM) is no longer the exclusive preserve of anaesthetists as both emergency medicine and general medicine trainees now also train in this increasingly important speciality. This edition of the JRAMC self assessment question series serves to cover some of the current 'hot topics' in ICM and enables readers with experience of ICM to test their knowledge as well as educating junior trainees in relevant subjects that they may be unfamiliar with. Similarly, the Focus On .... series of papers elsewhere in this journal demonstrate the increasing importance of ICM in the deplo

A case of primary meningococcal pericarditis caused by Neisseria meningitidis serotype Y with rapid evolution into cardiac tamponade.

Primary meningococcal pericarditis (PMP) is a rare form of acute purulent pericarditis that often evolves into cardiac tamponade and usually requires pericardial drainage. PMP is a rare, difficult to diagnose, rapidly progressive form of meningococcal infections. PMP is typically caused by Neisseria meningitidis of serotype C, or, less commonly, B or W135. We report the second case of PMP caused by Neisseria meningitidis of serotype Y. Our patient was not critically ill at presentation, and her presentation could have been consistent with viral uncomplicated pericarditis with the exception of

[Purulent pericarditis in a patient with knee pain].

A 40-year-old man with pain in his left, swollen knee that persisted for 6 weeks presented with chest pain, dyspnoea and subfebrile temperature. The pain worsened during inspiration and was relieved by sitting up straight. The electrocardiogram showed pericarditis. The patient was treated with high-dose carbasalate calcium. Initially, echocardiography revealed a 2-cm pericardial effusion with no signs of influx inhibition. Blood cultures were positive for Neisseria meningitidis, and treatment was expanded to include antibiotics. Based on a deterioration in patient condition and the tamponade i

Eosinophilic myocarditis temporally associated with conjugate meningococcal C and hepatitis B vaccines in children.

We report the first cases of tissue-proven eosinophilic myocarditis after single vaccine administration of conjugate meningococcal C and hepatitis B vaccine, respectively. The nature of histopathologic findings strongly supports hypersensitivity reaction and negates viral etiology, which is typically characterized by a lymphocytic infiltrate. Both episodes resolved with corticosteroid therapy. To enhance discussion of our cases, we performed a systematic review of the literature on postimmunization myocarditis or pericarditis, and identified 37 publications, reporting 269 cases during the sear

Meningococcal serotype Y myopericarditis.

Involvement of the pericardium in meningococcal disease is a well-recognized but rare complication. Isolated meningococcal pericarditis is defined as purulent pericarditis without clinical evidence of meningococcemia. Neisseria meningitidis serotypes C, B, and W135 have been previously described to cause pericarditis. This is the 1st case report of isolated meningococcal myopericarditis due to N. meningitidis serotype Y.

The role of echocardiography in diagnosis and management of isolated meningococcal pericarditis.

Although pericarditis may complicate the course of meningococcemia, it is distinctly unusual as a presenting sign. Herein we report a case of a previously healthy 16-year-old male with isolated meningococcal pericarditis, in which transthoracic echocardiography was of great importance for the initial diagnosis and for guiding the therapeutic approach during the hospitalization period. The patient presented with symptoms of chest pain and fever that deteriorated into cardiac tamponade. Pericardiocentesis was successful and Neisseria meningitidis was identified as the causative agent in the peri

[Serogroup C meningococcemia presenting as pericarditis in a 55-year-old woman without medical history].

In case of meningococcemia, isolated pericarditis is a rare event with a high mortality rate. We report the clinical and paraclinical signs that lead us to the diagnosis in a 55-year-old woman with favourable evolution. Previously described predisposing factors were no found.

Cardiac tamponade as a delayed presentation of Neisseria meningitidis infection in a 5-month-old infant.

This is a case of a 5-month-old female infant diagnosed with primary meningococcal pericarditis. Pericarditis is a well-recognized but uncommon complication of meningococcal infection. Primary meningococcal pericarditis, defined as purulent pericarditis without any clinical evidence of disseminated meningococcemia, meningitis, or other foci of meningococcal infection, is exceedingly rare, with only 21 reported cases since the first case was reported in 1939. This case report of primary meningococcal pericarditis is the youngest case and only the second case reported in an infant in the English

Stroke in patients with human immunodeficiency virus infection.

OBJECTIVE: To report the nature of stroke in patients infected with human immunodeficiency virus (HIV) in a region with high HIV seroprevalence and describe HIV associated vasculopathy. METHODS: Patients with first ever stroke, infected with HIV and prospectively included in the stroke register of the Groote Schuur Hospital/University of Cape Town stroke unit were identified and reviewed. RESULTS: Between 2000 and 2006, 67 of the 1087 (6.1%) stroke patients were HIV infected. Of these, 91% (n = 61) were younger than 46 years. Cerebral infarction occurred in 96% (n = 64) of the HIV positive p

Giant coronary artery aneurysm formation following meningococcal septicaemia.

We report a case of coronary vasculitis with formation of giant coronary artery aneurysms in a child following meningococcal septicemia. We suggest that coronary vasculitis is a rare complication of meningococcal septicemia that should be included in the list of conditions known to elicit a Kawasaki-like inflammatory response.

Acute myopericarditis after multiple vaccinations in an adolescent: case report and review of the literature.

We report a case of postvaccination acute myopericarditis in an adolescent. The patient presented with acute chest pain, diffuse ST-segment elevation, and elevated cardiac enzyme levels. Cardiac MRI was consistent with acute myocarditis. He recovered within a few days with nonsteroidal antiinflammatory treatment and remains clinically stable, with improvement of MRI findings at the 10-week follow-up. Postvaccination cases of myopericarditis reported in the pediatric literature are also reviewed.
