CT and MRI in the Characterization and Preoperative Assessment of Adrenocortical Carcinoma: Current Concepts and Perspectives.

Adrenocortical carcinoma (ACC) is a rare and aggressive malignancy arising from the adrenal cortex. This tumor shows suggestive imaging features on computed tomography (CT), which include a large size, internal heterogeneity and various degrees of internal necrosis. Accurate characterization of ACC with imaging is essential to ensure prompt and appropriate management and avoid useless biopsy or delayed surgery. Magnetic resonance imaging (MRI) provides additional information with respect to the internal content of ACC by comparison with CT. Recent studies evaluating the capabilities of radiomi

Pretreatment with phenoxybenzamine improves short-term survival and hemodynamic stability in dogs undergoing adrenalectomy for pheochromocytoma.

OBJECTIVE: To evaluate indices of hemodynamic instability (HDI) in dogs and evaluate the effects of phenoxybenzamine (PBZ) on HDI and short-term survival in dogs undergoing adrenalectomy for pheochromocytoma. METHODS: A retrospective medical record review of all dogs that underwent adrenalectomy at a single institution between 1996 and 2024. Dogs were included if the mass was confirmed to be a pheochromocytoma on histopathology and excluded if they received α antagonists other than PBZ, had incomplete medical or anesthetic records, or were treated with laparoscopy. RESULTS: 52 dogs met the i

Prenatal androgens and female-advantaged cognition: A systematic review and Bayesian meta-analysis in congenital adrenal hyperplasia.

Females with Congenital Adrenal Hyperplasia (CAH) are exposed to elevated levels of androgens during the prenatal period, offering insights into the effects of early hormonal influences on brain and cognition. Although spatial abilities, which typically favor males, have received considerable attention in the context of CAH, this is the first review to systematically examine cognitive domains that generally show a female advantage. By meta-analyzing 43 effect sizes from 11 studies, we assessed whether elevated prenatal androgens are associated with shifts toward male-typical performance across

Hippocampal volume and white matter impairments in patients with congenital adrenal hyperplasia.

CONTEXT: Patients with congenital adrenal hyperplasia (CAH) have lifelong imbalances in adrenal hormones that may result in changes in structure and function of the brain that can impair cognitive performance. The hippocampus might be especially sensitive to cortisol and androgen disturbances but has not been systematically investigated in this patient group. OBJECTIVE: In this study, we assessed hippocampal subfield volumes, mean diffusivity, and resting-state functional connectivity along with memory performance in patients with CAH and healthy controls using structural magnetic resonance i

The utility of genetic testing for pheochromocytoma and paraganglioma.

BACKGROUND: Pheochromocytoma and paragangliomas are unique neuroendocrine tumors that can be a significant source of morbidity and mortality. It was previously thought that these tumors were largely sporadic. There is a paucity of literature to support routine genetic testing. The aim of this study was to determine the utility of preoperative genetic testing for these tumors. METHODS: This study was a retrospective review of all patients >18 years of age with a diagnosis of a pheochromocytoma or paraganglioma who were evaluated in our surgical oncology clinic from January 1, 2014, through Jan

Surgical decision-making and referral patterns for adrenal vein sampling in primary aldosteronism: a national survey of endocrine surgeons.

BACKGROUND: This study aimed to discover and explore factors associated with endocrine surgeons' (ES) referral patterns to Interventional Radiologists (IR) for adrenal vein sampling (AVS) for patients with primary aldosteronism (PA). METHODS: Following IRB approval, an electronic survey was distributed to members of the American Association of Endocrine Surgeons (AAES). This cross-sectional, descriptive survey was designed to generate hypotheses about AVS referral behavior rather than test prespecified causal relationships. Data collected included demographics, practice type, referral pattern

Endocrine Evaluation of Hyperandrogenism in Women.

Hyperandrogenism in women encompasses a spectrum of clinical manifestations including hirsutism, acne, androgenetic alopecia, menstrual dysfunction, and virilization. Although polyendocrine metabolic ovarian syndrome accounts for the majority of cases, a systematic endocrine evaluation is essential to identify less common but clinically significant etiologies including androgen-secreting tumors, ovarian hyperthecosis, congenital adrenal hyperplasia, and cortisol excess. Total testosterone liquid chromatography-tandem mass spectrometry measurement in women has refined thresholds for further eva

Diagnostic value of saline infusion test-derived biochemical parameters for primary aldosteronism subtyping.

BACKGROUND: Current clinical practice guidelines recommend adrenal venous sampling (AVS) for subtype classification in patients with primary aldosteronism (PA) who are candidates for adrenalectomy; however, AVS may be considered unnecessary in selected young patients with a high probability of unilateral disease. The additional diagnostic information provided by baseline and saline infusion test (SIT)-derived biochemical parameters when evaluated alongside clinical and radiological features remains uncertain. We aimed to compare the diagnostic performance of baseline and SIT-derived biochemica

Use of plasma chromogranin a epitopes vasostatin and catestatin as biomarkers for catecholamine-producing tumours in dogs.

BACKGROUND: Differentiation of catecholamine-producing tumours from other adrenal tumours remains difficult. Chromogranin A (CgA) is a marker for catecholamine secretion in the body. An assay for quantification of the CgA epitopes vasostatin and catestatin has been validated for use in dogs but not yet evaluated in dogs with catecholamine-producing tumours. The aims were to evaluate the assays foranalysis of plasma concentrations of the CgA epitopes vasostatin and catestatin in dogs with adrenal tumours and to compare concentrations between catecholamine-producing tumours and adrenocortical tu

Androgen driven organoids for the male urogenital system.

Androgen signalling plays a central role in the development and function of the male urogenital system, including the adrenal gland, prostate, and testis. Disruption of the androgen axis underlies diseases ranging from infertility to castration-resistant prostate cancer, yet current models fail to fully display its multi-organ and hormone-driven complexity. Organoid systems provide a promising platform to model androgen-responsive tissues while preserving three-dimensional structure and cellular heterogeneity, with the ability for long-term culture and cryopreservation. Recent advances have en

Inverse association between the extracellular matrix protein Tenascin-X and concurrent abdominal aortic calcification in patients with primary aldosteronism.

BACKGROUND: Patients diagnosed with primary aldosteronism (PA) are at significantly increased risk of abdominal aortic calcification (AAC) and subsequent cardiovascular complications. Notably, abdominal aortic calcification is independently associated with adverse cardiovascular outcomes. However, to date, no serum proteomic profiling has been specially carried out in PA patients complicated with AAC, leaving an unaddressed research gap. This study was designed to identify serum differentially expressed proteins (DEPs) via DIA-based untargeted proteomics. METHODS: In this translational resear

[Adrenal ganglioneuroma as an incidentaloma in a patient with Sjögren's syndrome].

Ganglioneuroma is a benign, non-functional tumor, most of which are asymptomatic and discovered incidentally through imaging studies or at autopsy. We present the case of a 47-year-old woman with Sjögren's syndrome. Computed tomography scan of the abdomen showed an ovoid tumor with well-defined borders, measuring 11 x 8 cm, located in the left adrenal gland. Robotassisted adrenalectomy was performed, and microscopic examination revealed a mature ganglioneuroma. Although imaging studies such as computed tomography and magnetic resonance imaging are effective in the diagnostic approach to adrena

Defining the learning curve of laparoscopic adrenalectomy: a single-center experience.

BACKGROUND: Laparoscopic adrenalectomy has become the standard approach for most benign adrenal lesions. However, the number of cases required for a surgeon to achieve proficiency remains debated, as learning curves vary according to patient characteristics, lesion factors, and surgeon experience. This study aims to evaluate learning curve for laparoscopic adrenalectomy and identified factors influencing operative duration in a single-surgeon experience. METHODS: This is a retrospective observational cohort study that evaluated 70 consecutive unilateral laparoscopic adrenalectomies performed

Diabetes in Cushing's disease: age, hypercortisolism, central adiposity, and β-cell stress as determinants of glycaemia.

OBJECTIVE: Diabetes is a frequent complication of Cushing's disease, but metabolic differences between normal glucose tolerance, prediabetes, and diabetes remain incompletely characterised. We aimed to characterise glucose metabolism across these groups, assess abnormalities in patients with normal glucose tolerance, and identify clinical, hormonal, and body-composition factors associated with glycaemia. DESIGN: Cross-sectional analysis of a prospective cohort with matched healthy controls. MATERIAL AND METHODS: The study included 50 patients with Cushing's disease and 52 age- and sex-matche

Venous thromboembolism in Cushing syndrome: current evidence and a practical thromboprophylaxis algorithm.

Cushing syndrome (CS) is a rare endocrine disorder associated with a markedly increased risk of venous thromboembolism (VTE). Chronic hypercortisolism induces a prothrombotic state through activation of coagulation pathways, impaired fibrinolysis, and endothelial dysfunction. VTE represents a major source of morbidity and mortality in this population, yet optimal strategies for prevention and treatment remain insufficiently standardized. To summarize current evidence on the epidemiology, pathophysiology, risk factors, prevention, and treatment of VTE in patients with endogenous CS and to propo

Case Report: Veno-Arteriovenous Extracorporeal Membrane Oxygenation for Management of Pheochromocytoma-Induced Cardiac Arrest in a Post-Partum Patient.

A 34-year-old woman, 2 days post-partum, was admitted to the intensive care unit (ICU) for management of hypertensive crisis and respiratory failure. An 18-cm left adrenal mass was found on computed tomography (CT), suspicious for a pheochromocytoma. After cardiac arrest, the patient was placed on veno-arteriovenous extracorporeal membrane oxygenation (V-AV ECMO) given recurrent destabilizing arrhythmias. After successful ECMO decannulation, the patient underwent an uncomplicated resection of her adrenal mass. Although VA-ECMO has been used to support patients with pheochromocytoma, this appea

Low Anterior Access Approach for Single-Port Retroperitoneal Robot-Assisted Adrenalectomy: Surgical Technique and Early Outcomes.

INTRODUCTION: The da Vinci single-port (SP) system has been utilized across various surgical procedures. However, a standardized technique for SP robot-assisted adrenalectomy (SP-RAA) remains unestablished. Herein, we report our technique for retroperitoneal SP-RAA via the low anterior access (LAA) approach. PATIENTS AND SURGICAL TECHNIQUE: Nine patients underwent SP-RAA using our standardized procedure. Patients were placed in a modified supine position. The primary incision was typically located two fingerbreadths cranial to a point two-thirds laterally along the spino-umbilical line. The o

Preoperative Preparation With Selective versus Nonselective Alpha-Adrenoceptor Blockade for Pheochromocytomas and Paragangliomas: A Systematic Review and Updated Meta-Analysis.

OBJECTIVE: Preoperative preparation with nonselective alpha-blockers (NABs) or selective alpha-blockers (SABs) is crucial for the maintenance of intraoperative hemodynamic stability during surgical resection of pheochromocytomas and paragangliomas (PPGLs). This study aims to compare the efficacy and safety of NAB versus SAB for preoperative preparation in patients with PPGL. METHODS: We systematically searched PubMed, Embase, Web of Science, and Cochrane CENTRAL databases (up to August 2, 2025) for randomized and observational studies directly comparing NAB with SAB.
