Which bot is best? Accuracy and readability of generative artificial intelligence assistant responses on adrenal nodules.

INTRODUCTION: Patients increasingly use generative artificial intelligence assistants (chatbots) for medical information. We examined surgeon-perceived accuracy, reliability, and readability of chatbot responses to adrenal nodule queries. METHODS: Six commonly asked adrenal nodule questions were input into five chatbots. Blinded answers were reviewed by 10 endocrine surgeons for correctness and reliability (6-point Likert scale) and content structure (3-point Likert scale). One-way ANOVAs and Tukey adjusted post hoc analyses tested differences. Inter-rater reliability across surgeon ratings w

Hemodynamic and microvascular abnormalities in P450 oxidoreductase deficiency: evidence for COX-dependent dysfunction.

OBJECTIVE: Cytochrome P450 oxidoreductase deficiency (PORD) is a rare form of congenital adrenal hyperplasia caused by impaired electron transfer to multiple microsomal enzymes. Hypertension in PORD has been attributed to mineralocorticoid excess; however, experimental evidence suggests that POR may also regulate endothelial vasodilatory pathways. The vascular effects of PORD in humans remain poorly defined. We evaluated blood pressure regulation, microvascular structure, and serum vasoactive mediators in patients with PORD. DESIGN: Observational, cross-sectional study. METHODS: Ten patients

[Diagnostics and treatment of pheochromocytomas and paragangliomas].

Pheochromocytomas and paragangliomas (PPGL) are rare neuroendocrine tumors arising from chromaffin cells and are capable of producing catecholamines. The diagnostics rely on measurements of free plasma and urinary fractionated metanephrines, complemented by computed tomography (CT) or magnetic resonance imaging (MRI) for anatomical imaging. Up to 35% of PPGLs are hereditary and the underlying genetic background significantly influences both tumor biology and surgical strategy. Surgery remains the cornerstone of treatment. For adrenal tumors, laparoscopic or retroperitoneoscopic adrenalectomy r

High-Resolution 3D reconstruction and visualization of normal adrenal vasculature and sympathetic nerves in mice.

PURPOSE: The vasculature and sympathetic nervous system are critical for adrenal homeostasis, but conventional imaging limits characterization of the adrenal glands’ 3D structure. This study aimed to delineate the mouse adrenal glands’ 3D architecture and key component spatial relationships via advanced tissue-clearing imaging. METHODS: We used FDISCO (fluorescence-preserving 3D imaging of solvent-cleared organs) for whole-mouse adrenal imaging, complemented by 2D cortical and medullary marker co-staining and 3D density quantification. RESULTS: 3D imaging revealed three key features of mouse a

The pivotal role of etiology in managing Non-CAH primary adrenal insufficiency in pediatric and adolescent populations: A Single-Center study.

BACKGROUND: Primary adrenal insufficiency (PAI) in children, outside of congenital adrenal hyperplasia (CAH), represents a rare and genetically diverse group of disorders often leading to diagnostic delays. Advances in molecular genetics have uncovered a wide spectrum of non-CAH etiologies, yet real-world data from pediatric cohorts remain limited. METHODS: We retrospectively reviewed 11 children diagnosed with non-CAH PAI at a single tertiary care center between March 2015 and December 2024. Clinical, biochemical, and genetic findings were evaluated using targeted Sanger sequencing or next-ge

From hypercortisolism to remission: impact of Cushing's disease on eating patterns.

BACKGROUND: Cushing's disease (CD) is characterized by chronic hypercortisolism and is associated with persistent metabolic, psychological, and neurocognitive disturbances. While metabolic consequences are well described, the impact of disease activity and remission on eating behavior remains insufficiently explored. OBJECTIVE: This study aimed to compare multidimensional eating behavior patterns among patients with active Cushing's disease, patients in biochemical remission, and healthy controls, and to examine their associations with cortisol biomarkers. METHODS: In this cross-sectional st

Steroid-induced adrenal insufficiency in children with nephrotic syndrome: a systematic review.

BACKGROUND: Idiopathic nephrotic syndrome (NS) is the most common glomerular disease in children. While corticosteroids remain the first-line treatment for inducing remission, their prolonged or frequent use can suppress the hypothalamic-pituitary-adrenal (HPA) axis, potentially leading to adrenal insufficiency (AI). However, the true burden of this complication remains poorly understood and inconsistently reported. This study aimed to systematically evaluate the prevalence, diagnostic methods, risk factors, and clinical implications of steroid-induced AI in children with NS. METHODS: A syste

From Tissue Architecture To Genetic Signature: Artificial intelligence-based Analysis of Reticulin Framework and Clinical Variables Predicts Molecular Cluster in Paragangliomas.

Paragangliomas (PGLs), encompassing pheochromocytomas and extra-adrenal paragangliomas, are genetically heterogeneous non-epithelial neuroendocrine neoplasms that segregate into molecular clusters with distinct biological and clinical behavior. Architectural correlates of genotype have not been systematically investigated. This study aimed to assess the reticulin framework as a potential morphologic structural surrogate of molecular background and to introduce a novel deep learning model for its spatially resolved quantitative analysis and genotype prediction. A total of 104 adrenal and extra-

Impact of Obesity on Outcomes of Laparoscopic Adrenalectomy: A Single-Center Retrospective Study.

AIM: Obesity is a well-known risk factor for surgical complications. Laparoscopic adrenalectomy is now preferred over open surgery for adrenal masses of <6 cm without high malignancy suspicion. This study evaluates the impact of obesity on surgical complication rates in laparoscopic adrenalectomy. MATERIALS AND METHODS: Patients who underwent laparoscopic transabdominal adrenalectomy between 2013 and 2024 were retrospectively analyzed. They were divided into two groups: obese (BMI ≥30 kg/m 2 ) and nonobese (BMI <30 kg/m 2 ). Demographic data, perioperative, and postoperative outcomes were com

Histopathological and Molecular Characteristics of Pediatric Adrenocortical Tumors: where Do we Stand?

Pediatric adrenal cortical carcinoma (ACC) is a rare heterogeneous cancer type that is incompletely understood and differs from its adult counterpart in clinical presentation, histopathological characteristics, genomic landscape, and prognosis. Pediatric ACC has a bimodal age distribution for children with ACC, with the highest incidence in young children and adolescents. Tumors commonly present with an endocrine syndrome such as virilization or Cushing’s syndrome, associated with hormone overproduction. Surgical resection is the primary treatment. Adjuvant therapies such as mitotane or chemot

The left adrenal mass is difficult to differentiate from a hemorrhagic gastric diverticula: a case report.

BACKGROUND: The complex periadrenal anatomy complicates differentiation between adrenal tumors and masses of extradrenal origin. Given the nonspecific clinical and radiological presentations, misdiagnosis occurs frequently. We report a case where a gastric diverticula (GD) was initially misdiagnosed as a left adrenal tumor on imaging. CASE PRESENTATION: A 50-year-old male presented with epigastric pain and discomfort. Gastroscopy showed gastric retention. Non-contrast CT suggested a left adrenal tumor, while abdominal MRI indicated a left adrenal neoplasm, with possible adenoma or pheochromocy

Effects of catecholamines on bone and mineral metabolism in patients with pheochromocytoma and paraganglioma.

Pheochromocytomas and paragangliomas (PPGLs) are rare catecholamine-producing neuroendocrine tumors with significant systemic effects. While catecholamine-related cardiovascular manifestations are well-characterized as a prominent component of the clinical presentation, other systemic effects, particularly those on bone and mineral metabolism, remain much less characterized. Bone physiology and health in patients with PPGLs can be affected through catecholamine-mediated changes, skeletal metastases, or specific skeletal abnormalities associated with syndromic PPGL forms. The catecholamine effe

Metabolic Risk Assessment Using the Triglyceride-Glucose Index in Patients with Adrenal Insufficiency on Glucocorticoid Therapy.

PURPOSE: Adrenal insufficiency (AI) is an endocrine condition requiring lifelong glucocorticoid replacement treatment. Long-term exposure to glucocorticoids may result in significant metabolic problems, such as insulin resistance, abdominal obesity, dyslipidemia, and increased cardiometabolic risk. The triglyceride-glucose (TyG) index is a simple, inexpensive marker of insulin resistance and metabolic syndrome (MetS), increasingly used in endocrine and metabolic research, but its role in AI remains insufficiently researched. This study aimed to assess the clinical utility of the TyG index in p

Urinary free cortisol analyses: Enhancing their clinical performance in Cushing's syndrome management by means of LC-MS/MS.

24 h urinary free cortisol (UFC) analysis constitutes one of the three first level recommended tests in Cushing's syndrome (CS) diagnostic confirmation work up. However, it occasionally leads to inaccurate results due to the use of immunoassays (IAs) or the concomitant administration of exogenous glucocorticoids, among others. This study aimed to develop a rapid and accurate LC-MS/MS method which may ultimately replace the use of IAs, and also provide relevant clinical information through the simultaneous monitoring of UFC, cortisone, and dexamethasone. An LC-MS/MS method based on direct injec

Standardizing the Seated Saline Suppression Test for Primary Aldosteronism.

BACKGROUND: Primary aldosteronism is the most common secondary endocrine form of hypertension. Specific treatment directly against aldosterone excess improves control of (and may cure) hypertension and reduces long-term cardiovascular and renal morbidity in patients with primary aldosteronism. Optimal treatment is guided by the results of adrenal venous sampling, with unilateral adrenalectomy being the preferred management for those with lateralizing forms and medical treatment with a mineralocorticoid antagonist for those with bilateral forms.
