IGF2R: a new player in the IGF2 loop sustaining adrenocortical carcinogenesis.

BACKGROUND: Most of adrenocortical carcinomas (ACC) are characterized by IGF2 overexpression; therefore, several studies have focused on its two oncogenic effectors, the tyrosine-kinase receptors IGF1R and IR. However, the specific IGF2 receptor, IGF2R, due to its scavenging activity, was considered a tumour suppressor and has never been fully investigated in this context. Nevertheless, recent evidence from other tumours identified IGF2R as a pro-tumorigenic actor able to exert its function through the downstream activation of the pro-mitotic sphingosine kinases (SphK) enzymes responsible for

Steroid Use, Adrenal Suppression, and Emergency Department Visits in COPD Patients: A Cross-Sectional Study.

INTRODUCTION: This study aims to investigate the relationship between steroid use, adrenal suppression, and frequent emergency department (ED) visits in patients with Chronic Obstructive Pulmonary Disease (COPD). Systemic glucocorticoids are commonly prescribed in the management of COPD exacerbations; however, prolonged or repeated steroid use may lead to adrenal suppression. Although the standard steroid regimen for COPD exacerbations is short-term, frequent ED visits may result in cumulative steroid exposure, raising concerns about adrenal insufficiency and its clinical consequences. FKBP5 Methylation in Adrenal Insufficiency: New Insights Into Assessing the Quality of Glucocorticoid Replacement.

CONTEXT: Methylation of FKBP5, a glucocorticoid (GC)-receptor co-chaperone, negatively correlates with cortisol levels in both healthy individuals and Cushing patients, potentially serving as an indicator of GC exposure. OBJECTIVE: We explored whether GC replacement correlates with FKBP5 methylation in patients with adrenal insufficiency (AI), aiming to assess the adequacy of therapy. METHODS: In an explorative cross-sectional analysis, we analyzed FKBP5 gene methylation at 54 CpG sites using bisulfite pyrosequencing in 120 patients with chronic primary (n = 72) and secondary (n = 48) AI on

Basal Cortisol as a Predictor of Passing Short Synacthen Test and the Utility of the 60-Minute Poststimulation Cortisol.

CONTEXT: Short Synacthen testing (SST) is resource intensive and may be unnecessary if adrenal sufficiency (AS) can be predicted reliably using basal cortisol (BC). OBJECTIVE: This work aimed to identify the BC threshold predictive of AS and evaluate the utility of 30- and 60-minute stimulated cortisol measurements in response to SST. METHODS: This research took place at a public tertiary pathology service (PathWest) in Western Australia. Of 1216 SSTs performed, 704 adult results were eligible. Exclusions were based on incomplete data or protocol deviations. Intervention was intramuscular Sy

Fatal TAFRO syndrome in which adrenal hemorrhage preceded thrombocytopenia.

TAFRO syndrome is a rare systemic inflammatory disease characterized by thrombocytopenia, anasarca, fever, renal dysfunction, and myelofibrosis, and its follows a rapidly fatal clinical course. A few cases of TAFRO syndrome complicated by adrenal hemorrhage were recently reported; however, its pathogenetic significance remains largely unclear. The present case was a 59-year-old man presenting with left lower back pain. Imaging studies revealed left adrenal hemorrhage. Although an elevated C-reactive protein level was observed in the initial visit, there were no obvious abnormalities in the plat

Cushing's Syndrome Presenting with Depression as the Initial Manifestation: A Case Report and Literature Review.

BACKGROUND: In approximately 12% of Cushing's syndrome (CS) cases, psychiatric symptoms, such as depression and anxiety, are the initial manifestation, obscuring the diagnosis and leading to delayed treatment. CASE SUMMARY: This case report describes a patient whose initial symptom of CS was low mood. Eighteen months later, typical clinical features of CS emerged, and the diagnosis was confirmed through dexamethasone suppression tests and adrenal computed tomography. This report also reviews relevant literature, analyzing the clinical features, pathogenesis, treatment, and prognosis of CS. C

Cognitive Assessment in Adults With Adrenal Cortisol Insufficiency: Challenges and Opportunities.

Adults with adrenal cortisol insufficiency (ACI) often report cognitive dysfunction, especially in memory processing and executive function. Only a few studies have objectively compared cognitive function as the primary outcome between patients with ACI and controls, and these efforts have yielded inconsistent results. In this review, we examine the challenges facing researchers studying cognitive function in adult patients with ACI. We consider the effect of dysregulated cortisol on cognition in patients with ACI, and the inability of current guideline-recommended glucocorticoid (GC) treatmen

Neurofilament light chain as a prognostic marker in cerebral adrenoleukodystrophy.

Cerebral adrenoleukodystrophy (C-ALD) is a rapidly progressing inflammatory neurodegenerative disease with unpredictable onset in males with ALD. It must be treated at an early stage of demyelination, determined by MRI, to preserve neurocognitive function. Validation of biochemical markers that can aid in the prediction of functional outcomes is needed for optimizing disease management and therapeutic development. Our objective was to determine whether plasma neurofilament light chain (NfL), a biomarker of neuroaxonal injury, corresponds to functional outcomes after standard of care treatment

Insights into diagnosis and management of giant adrenal pseudocysts.

Adrenal cysts are rare lesions that may coexist with, or mimic, malignant adrenal tumours. Their shared imaging characteristics may cause misdiagnosis with potentially harmful outcomes. We report the case of a young man who presented with several months of headaches and mild anaemia. Non-contrast CT imaging revealed a giant left adrenal cystic lesion measuring 19×15×15 cm with indeterminate CT attenuation and internal debris. 18F-fluorodeoxyglucose-positron emission tomography demonstrated low peripheral uptake favouring a benign aetiology. The multidisciplinary recommendation was for excision

Primary adrenal lymphoma presenting as primary adrenal insufficiency.

Although lymphoma may involve the adrenal glands as part of a generalised disease process, primary adrenal lymphoma is a rare entity. We present the case of a man in his late 60s who was admitted via the emergency department with clinical features consistent with adrenal failure. Initial investigations demonstrated anaemia, hypercalcaemia and adrenal insufficiency. Further biochemical work-up showed no evidence of an autoimmune or congenital aetiology of adrenal failure, and diagnostic imaging revealed bilateral adrenal enlargement. Adrenal biopsy revealed a diagnosis of diffuse large B-cell l

Trajectory-based analysis of growth in congenital adrenal hyperplasia: therapeutic implications for final height preservation.

UNLABELLED: Congenital adrenal hyperplasia (CAH) is an autosomal recessive genetic disorder resulting in a dysfunction of the steroidogenesis pathway and glucocorticoid deficiency. Proper care demands not only preventing acute complications but also optimizing growth and sexual development. This study was set out to analyze final height metrics and the factors affecting them as a long-term outcome measure in CAH patients. This observational retrospective study evaluated almost 3000 visits of 63 CAH patients treated in one center between 1996 and 2025, who reached their near-final height (nFH).

Diagnostic challenges in primary aldosteronism evaluation and finerenone use.

INTRODUCTION: Primary aldosteronism (PA) is a common but frequently ignored cause of hypertension, particularly in patients with normal serum potassium and aldosterone levels. CASE PRESENTATION: We report the case of a 70-year-old Han Chinese man with a 20-year history of hypertension who presented with normokalemia and normal plasma aldosterone levels. Despite this, laboratory evaluation revealed suppressed direct renin concentration (DRC) and a markedly elevated aldosterone-to-renin ratio (ARR), raising suspicion for PA. Adrenal computed tomography demonstrated bilateral nodular adrenal thi

Newborn screening for congenital adrenal hyperplasia: a 3 years study embedded in 25 years of regional experience in Italy.

PURPOSE: Newborn screening for congenital adrenal hyperplasia (CAH) using 17-hydroxyprogesterone (17-OHP) is hampered by high false-positive rates, especially in preterm and low birth weight infants. Second-tier strategies aim to improve specificity, but population-based data are limited. The aim was to evaluate CAH screening approach using birth weight-adjusted cut-offs and repeated sampling in a large regional cohort. METHODS: We analyzed data from 90,092 infants screened in Piedmont, Italy (2019-2022). 17-OHP was measured in dried blood spots, with birth weight-adjusted thresholds and repe
