Neutrophils and NETosis in polycystic ovary syndrome: unraveling the immuno-metabolic thromboinflammatory axis.

Polycystic ovary syndrome (PCOS), traditionally defined by its endocrine and reproductive hallmarks-hyperandrogenism, oligo-anovulation, and polycystic ovarian morphology-is increasingly viewed as a complex disorder associated with chronic low-grade immuno-metabolic inflammation. Emerging evidence suggests that neutrophils, the most abundant innate immune cells, may play a contributory role in linking metabolic stress, inflammatory signaling, and vascular dysfunction in PCOS. In women with PCOS, hyperandrogenism, insulin resistance, and adipose-derived inflammatory cues have been associated wi

Polycystic ovary syndrome: quantifying the psychosocial burden and the predictive role of sexual trauma history in clinical outcomes.

Polycystic ovary syndrome (PCOS) is a complex endocrine disorder that needs to be addressed through holistic management of metabolic, hormonal, and lifestyle factors. This research study, a cross-sectional examination, reports on clinical and psychological aspects of PCOS and PCOD in a group of 75 educated women analyzed with descriptive statistics, non-parametric tests, and binary logistic regressions. To carry out this analysis, we integrated the symptom severity index (SSI) and a binary measure of past sexual trauma history. The study observed irregular menstruation in 85.3% and weight gain

Role of Liraglutide in weight management and reproductive health in women with obesity and PCOS- A critical review of the evidence.

Obesity and polycystic ovary syndrome (PCOS) in women are associated with significant reproductive dysfunction, manifesting as hormonal imbalances, anovulation, and subfertility. These conditions not only impair reproductive outcomes but also contribute to a reduced quality of life. Crucially, obesity and PCOS share a bidirectional relationship. While obesity can trigger and worsen PCOS manifestations, the convergence of these two conditions has contributed significantly to the growing incidence of female infertility. This review critically examines liraglutide's mechanistic role and therapeut

Expanded Steroid Profiling Identifies Novel Newborn Screening Markers for Congenital Adrenal Hyperplasia.

CONTEXT: Simultaneous measurement of multiple steroids in dried bloodspots facilitates accurate newborn screening (NBS) for classic congenital adrenal hyperplasia (CAH), although false-positive (FP) tests are encountered. Some babies with milder forms of CAH, who may benefit from early detection and treatment, can be missed by screening. OBJECTIVE: This work aims to evaluate a method for expanded steroid profiling in NBS bloodspots to identify novel sensitive and specific markers that will inform improvements for screening for CAH, and to identify the metabolic routes to excessive androgen sy

[Neuroendocrine regulation of the reproductive system: integration of research of neuropeptide and experimental pathophysiology of amenorrhea of different genesis (literature review)].

Neuroendocrine regulation of reproductive function represents a complex system based on the integration of signals between the central nervous system and peripheral organs. In recent years, particular attention has been given to the role of neuropeptides - such as kisspeptin, brain-derived neurotrophic factor (BDNF), and orexins - in the pathogenesis of disorders associated with menstrual irregularities. This review provides a detailed analysis of the molecular mechanisms underlying neuropeptide regulation in functional hypothalamic amenorrhea (FHA), primary ovarian insufficiency (POI), and po

Affective Sensitivity to Ovarian Steroid Hormone Flux Across the Menstrual Cycle: Manifestations and Biopsychosocial Risk Factors.

Hormone sensitivity is a heterogeneous phenomenon involving multiple dimensions of sensitivity to estrogen and progesterone fluctuations across the menstrual cycle. While the majority of menstruating individuals do not experience any significant impact from these hormone changes on mood or behavior, rates of hormone sensitivity in clinical populations, particularly affective disorders, are substantially elevated, suggesting potential shared psychosocial or physiological mechanisms or moderators of risk. In this review, we provide an overview of menstrually related mood disorders and dimensions

Comparison of suboptimal versus adequate ovarian function suppression in premenopausal women with early breast cancer treated with adjuvant endocrine therapy: An exploratory analysis of two prospective studies.

PURPOSE: Ovarian function suppression (OFS) combined with tamoxifen or aromatase inhibitor (AI) are standard treatment options for young women with hormone receptor-positive breast cancer. Suboptimal OFS may reduce the efficacy of the endocrine treatment (ET). This study evaluated factors associated with suboptimal OFS induced by gonadotropin-releasing hormone agonist (GnRHa). METHODS: PREFER (NCT02895165) and GIM 23-POSTER (NCT05730647) are multicentric Italian prospective, observational studies enrolling premenopausal women undergoing (neo)adjuvant chemotherapy and/or ET. The present analys

From silos to synergy: the interdisciplinary discovery potential in redefining polyendocrine metabolic ovarian syndrome.

Polyendocrine metabolic ovarian syndrome (PMOS) affects one of every eight women worldwide and is associated with high rates of cardiovascular disease, type 2 diabetes, and infertility. Polycystic ovarian syndrome (PCOS), another term for this condition, reflects a historical focus on its effects on the female reproductive system. Recently, however, experts and patient groups have advocated renaming the condition PMOS, conveying increasing recognition that hyperandrogenism and insulin resistance contribute to its pathology. In this issue, Nguyen et al. took a systems genetic approach that comb

Nutrition care and Dietetic Practice in Managing Metabolic and Endocrine Dysfunction in Polyendocrine Metabolic Ovarian Syndrome (PMOS): Implications and Opportunities in the Context of the Name Change of Polycystic Ovarian Syndrome (PCOS) to PMOS.

Polyendocrine metabolic ovarian syndrome (PMOS) affects 8-13% of women across the lifespan. Those with PMOS have reduced quality of life and increased risk of co-morbidities including elevated weight, eating disorders, gestational diabetes, type 2 diabetes and cardiovascular disease. PMOS is a major population in clinical dietetic practice and dietary intervention is the first-line approach recommended to target metabolic risk in the international evidence-based guidelines for assessment and management of PCOS. The past name, polycystic ovary syndrome (PCOS) was ovarian-centric and did not acc

Mechanisms of Phthalate-Induced Accelerated Ovarian Aging in Experimental Models.

PURPOSE OF REVIEW: Accelerated ovarian aging is associated with early infertility as well as other adverse health outcomes. Little is known about the factors that accelerate ovarian aging, but several studies indicate that exposure to phthalates accelerates ovarian aging. This is a significant human health concern because humans are ubiquitously and unavoidably exposed to phthalates. Thus, it is imperative to study the mechanisms of phthalate-induced accelerated ovarian aging so that strategies can be developed to prevent phthalate-induced ovarian aging. This review focuses on the mechanisms b

Evaluating the Therapeutic Potential of Antioxidants in Polyendocrine Metabolic Ovarian Syndrome (PMOS)-Associated Infertility.

INTRODUCTION: Polyendocrine Metabolic Ovarian syndrome (PMOS) formerly known as polycystic ovarian syndrome (PCOS) constitutes the most common endocrinopathy in women of reproductive age group. Evidence from numerous studies in PMOS patients suggests that antioxidants can modulate the ovarian microenvironment, promote folliculogenesis, and increase oocyte competence and yield. This review aims to summarize the existing clinical evidence regarding the application of antioxidants as adjunctive treatment options in the management of PMOS-related infertility. MATERIALS AND METHODS: This is a stru

Hodgkin Lymphoma-The Effect of Chemotherapy on Gonadal Function and Fertility Is Strongly Related to the Treatment Regimen, Age, and Sex: A Systematic Review and Meta-Analysis.

Background/Objectives: Hodgkin lymphoma (HL) primarily affects individuals of reproductive age, making gonadal dysfunction after chemotherapy a critical survivorship concern. While fertility preservation options including gamete and gonadal tissue cryopreservation are available before treatment, evidence-based counseling requires regimen-specific risk estimates accounting for patient age and sex. Therefore, a meta-analysis was performed to assess presumed infertility in HL patients, stratified by chemotherapy regimen, age, and sex. Methods: This systematic review and meta-analysis, conducted w

Hydrogen-generating nanomodulators for metabolic repair and functional recovery in ovarian tissue transplantation.

Ischemia-reperfusion (I/R) injury occurring during ovarian tissue cryopreservation and transplantation (OTC-T) induces mitochondrial dysfunction and oxidative stress, leading to pronounced follicular loss and compromised graft performance-key challenges that limit transplantation success. Here, we present a hydrogen-releasing nanomodulator composed of potassium borohydride nanoparticles embedded within a thermosensitive hydrogel (KBH₄@Gel), designed to achieve sustained in situ hydrogen (H₂) generation for metabolic repair and ovarian function restoration. Upon local administration, the thermo
