Metabolic effects of testosterone therapy in transgender and gender diverse individuals with polycystic ovary syndrome.

BACKGROUND: Polycystic ovary syndrome (PCOS) affects 10% to 13% of individuals assigned female at birth (AFAB), although a greater prevalence is reported in transgender/gender diverse individuals (TGD) (23.8-38.8%). Testosterone therapy (TT) in TGD individuals is linked to worsening cardiovascular risk factors (CVRFs). Whether TT induces more adverse cardiometabolic changes in TGD with PCOS than TGD without PCOS remains uncertain. OBJECTIVE: To compare the effects of TT on CVRF in TGD with and without PCOS. METHODS: We retrospectively reviewed 373 records to identify 62 TGD with PCOS (withes

Pyroptosis in Ovarian Aging, and Its Influence on Female Reproductive Health in Aged Ovaries.

Pyroptosis is a lytic cell death mechanism mediated by the gasdermin family of proteins, resulting in the release of certain pro-inflammatory molecules to the extracellular space. Ovaries serve as the source of oocytes and main producer of steroid sex hormones, making them essential with respect to the maintenance of fertility and endocrine homeostasis during reproductive lifespan in females. Ovaries exhibit early-onset aging-associated dysfunction, compared to most other tissues, with dramatic functional declines after only 30 years of age in women. In this review, I covered studies reporting

Polycystic ovary syndrome (PCOS): current insights, emerging therapeutics, and future treatment strategies.

Polycystic ovary syndrome (PCOS) is a common gynaecological disorder, clinically characterized by chronic anovulation and hyperandrogenism. Despite its high prevalence, no curative treatment exists, and managing the syndrome remains challenging. A strong association between PCOS and metabolic syndrome, particularly insulin resistance, adversely influences fertility, leading to an increased worldwide demand for advanced treatment options. Although the exact aetiology and pathophysiology of the syndrome remain unclear, an intricate interaction of ovarian, endocrine, and metabolic factors is high

[Adrenocortical carcinoma (ACC): current surgical treatment strategies].

Adrenocortical carcinoma (ACC) is among the rarest solid malignancies, but is characterized by pronounced biological aggressiveness and a high risk for recurrence. Complete tumor resection (R0) remains the only potentially curative treatment and is the primary determinant of long-term survival. Given the functional heterogeneity of ACC and the frequent presence of hypercortisolism or androgen excess, preoperative evaluation and perioperative management require close interdisciplinary management. Open adrenalectomy continues to be regarded as the gold standard, whereas minimally invasive techni

Adult and perimenopausal acne and the nurse's role in management.

Adult female acne is increasingly recognised as a chronic inflammatory skin disorder affecting women well beyond adolescence. Hormonal fluctuation, inflammatory signalling, skin barrier impairment, and microbiome shifts contribute to its development and persistence. Perimenopause and menopause introduce further complexity due to declining oestrogen levels and relative androgen excess. This article provides an evidence-based overview of adult and menopausal acne, integrating epidemiology, underlying mechanisms, clinical presentation, assessment, management, and the crucial role nurses play in p

MASLD, diabetes and PMOS across the female life stages.

Metabolic dysfunction-associated steatotic liver disease (MASLD) and type 2 diabetes, which affect approximately 38% and 10.5%, respectively, of adults globally, intersect critically in women through polyendocrine metabolic ovarian syndrome (PMOS), previously known as polycystic ovary syndrome (PCOS), a prevalent but under-recognised cardiometabolic disorder affecting 10-13% of reproductive-age women. Beyond its reproductive consequences, PMOS promotes hepatic disease through hyperandrogenism, insulin resistance and chronic inflammation. Androgens directly upregulate hepatic lipogenic gene exp

Iron overload disorders in adults: a comprehensive review of gonadal function, reproductive, and sexual health.

BACKGROUND: Iron overload (IO) disorders, including thalassaemias, hereditary haemochromatosis, and transfusion-dependent anaemias, represent a growing clinical challenge with widespread systemic implications. Reproductive dysfunction remains severely underappreciated despite its high prevalence. Hormonal changes due to iron toxicity are frequently reported, yet are seldom the focus of reproductive medicine, causing fragmented knowledge, inconsistent clinical approaches, and a lack of consensus guidelines. OBJECTIVE AND RATIONALE: This review synthesizes evidence on the impact of IO on male a

Lipid dysregulation in mouse ovarian antral follicles after oral exposure to a human relevant mixture of three phthalates.

Dibutyl phthalate, benzyl butyl phthalate, and di-2-ethylhexyl phthalates leach from consumer and medical products, leading to chronic daily exposure in women. Phthalates are associated with impaired ovarian function and metabolic syndrome in women. In mice, oral exposure to human relevant levels of a mixture of these phthalates disrupted the ovarian follicle proteome causing dysregulation of lipid metabolism proteins. This study aimed to establish the consequences of those alterations on the follicular lipid profile and identify relevant systemic impacts of phthalates. Adult CD-1 female mice

BDNF- Dysregulation as a Neurobiological Bridge between Polycystic Ovarian Syndrome and Autism Spectrum Disorder.

Polycystic ovary syndrome (PCOS), a prevalent endocrine disorder characterized by hyperandrogenism, has been increasingly associated with a high risk of autism spectrum disorder (ASD) in offspring. The emerging interaction between reproductive endocrinology and neurodevelopmental biology suggests that excessive androgen exposure during gestation may perturb neurotrophic signaling and impair neural circuit formation. Brain-derived neurotrophic factor (BDNF) acts through tropomyosin receptor kinase B receptor to activate downstream phosphoinositide 3-kinase/protein kinase B and extracellular sig

Long-Term Consequences of Perinatal Asphyxia in the Development of Alzheimer's Disease.

Despite remarkable progress in medicine, perinatal asphyxia (PA) remains a significant clinical problem, and concerns regarding its long-term complications are increasingly being raised. Emerging evidence indicates that similar pathological pathways are activated following PA and during neurodegeneration in Alzheimer's disease (AD). These similarities involve various mechanisms, including alterations in AD-related proteins and their gene expression. Disturbances in neurotransmitter function can lead to excitotoxicity and cell death via necrosis and delayed apoptosis. Additionally, as observed

Age- and sex-specific reference intervals for 11-oxygenated androgens from infancy throughout childhood and adulthood.

OBJECTIVES: The adrenal glands are the main source of 11-oxygenated androgens (11-OAs), including the potent 11-ketotestosterone (11-KT) and the weaker metabolites 11β-hydroxyandrostenedione (11-OHA4), 11β-hydroxytestosterone (11-OHT) and 11-ketoandrostenedione (11-KA4). Due to their adrenal origin, 11-OAs allow the differentiation of adrenal and extra-adrenal-produced androgens. However, their clinical use is limited due to the lack of robust age- and sex-specific reference intervals (RIs). METHODS: To establish RIs from infancy throughout childhood and adulthood, we performed simultaneous q

Efficacy and safety of SarogLItazar as an add-on medication to Metformin and lifestyle intervention in women with PolyCystic Ovary Syndrome (SLIM-PCOS trial): study protocol for a randomized, open-label, parallel group clinical trial.

BACKGROUND: Emerging concepts on the role of insulin resistance (IR) in the pathogenesis of polycystic ovary syndrome (PCOS) have driven a shift in the therapeutic approach towards interventions targeting IR. SarogLItazar as an add-on to Metformin in PolyCystic Ovary Syndrome (SLIM-PCOS) trial aims to test the efficacy and safety of saroglitazar, a dual peroxisome proliferator-activated receptor (PPAR α/γ) agonist belonging to "glitazar" class of drugs, on top of metformin and lifestyle intervention in PCOS. METHODS: SLIM-PCOS is an investigator-initiated, randomized, open label, parallel gro

Intra-Abdominal Compartment Syndrome Secondary to Massive Pelvic Müllerian Tumor.

Abdominal compartment syndrome (ACS) is a rare surgical emergency characterized by end-organ dysfunction resulting from sustained elevation of intra-abdominal pressure. Gynecological malignancies are an uncommon cause of ACS. We report the case of a 61-year-old woman who presented following an unwitnessed fall with hypotension, tachycardia, confusion, and marked abdominal distension. Initial laboratory investigations demonstrated severe acute kidney injury (AKI), hyperkalemia, anemia, and markedly elevated inflammatory markers. Computed tomography (CT) imaging revealed a massive septate cystic

Divergent pathophysiological drivers of polycystic ovary syndrome: insulin resistance independently fuels the hyperandrogenic phenotype whilst neuroendocrine factors dominate non-hyperandrogenic presentations.

BACKGROUND: Polycystic Ovary Syndrome (PCOS) manifests as a heterogeneous disorder, yet the extent to which metabolic dysfunction drives specific phenotypes independent of obesity remains debated. This study aimed to delineate the distinct clinical and pathophysiological characteristics of Hyperandrogenic (HA) versus 38 Non-Hyperandrogenic (Non-HA) phenotypes, with a specific focus on disentangling the roles of insulin resistance and adiposity in driving androgen excess. METHODS: A retrospective cross-sectional study was conducted involving 301 women with PCOS and 144 controls. Patients were

Oncofertility outcomes in early-onset patients with non-breast solid malignancies: a systematic review.

BACKGROUND: The combination of a rising incidence of early-onset cancer and the trend towards delayed parenthood has made the impact of treatments on fertility highly significant. While there is abundant evidence on oncofertility outcomes of anti-cancer treatment protocols used in people with breast cancer and haematological malignancies, data regarding gonadal toxicity and fertility preservation in young people with other cancers are lacking. We aimed to systematically review gonadal toxicity, fertility outcomes, and preservation strategies in people with gastrointestinal, sarcoma, lung, and

A Decade-Long Diagnostic Challenge: A Case of Nonclassical 21-Hydroxylase Deficiency Mistaken for Polycystic Ovary Syndrome.

We report a rare case of nonclassical 21-hydroxylase deficiency (NC21-OHD) diagnosed in adulthood after being misdiagnosed as polycystic ovary syndrome (PCOS) for 9 years. A 27-year-old Japanese woman presented with longstanding amenorrhea, hirsutism, and polycystic ovarian morphology on ultrasonography but did not exhibit withdrawal bleeding after standard estrogen/progestin (E/P) therapy. Hormonal evaluation revealed a low luteinizing hormone (LH) level, elevated testosterone (T), and a significantly increased 17α-hydroxyprogesterone level, which further increased following adrenocorticotrop

Clinical evidence on integrated Chinese-Western medicine for perimenopausal syndrome.

BACKGROUND: Perimenopausal Syndrome (PS) results from estrogen fluctuations due to ovarian dysfunction, leading to autonomic and neuropsychological symptoms. Existing therapeutic methods have certain limitations, and integrated Chinese-Western medicine (ICWM) treatment can achieve complementary advantages. METHODS: We defined 16 clinical questions and outcomes for PS and searched CNKI, PubMed, the Cochrane Library, and other databases and relevant websites from inception to August 29, 2025. Relevant clinical practice guidelines/consensus statements (CPGs/CSs), systematic reviews (SRs), and ra

Sixteen Years of Clinical Data Including Genetic Analysis to Explain Delayed Puberty in a Chinese Boy with 21-Hydroxylase Deficiency: A Case Report.

INTRODUCTION: In 21-hydroxylase deficiency (21-OHD), impaired 21-hydroxylase activity causes 17-hydroxyprogesterone (17-OHP) accumulation and androgen excess, typically manifesting as hyperandrogenism. The POR gene encodes cytochrome P450 oxidoreductase (POR), the essential electron donor for all microsomal cytochrome P450 enzymes. While POR deficiency impairs multiple steroidogenic enzymes, its phenotypic impact on 21-OHD patients remains poorly characterized. CASE PRESENTATION: We report a male 21-OHD patient with homozygous CYP21A2 variant (c.293-13C>G) who presented atypically with absent

Endocrine modulation of stimulant use: bidirectional interactions within the hypothalamic-pituitary-gonadal axis.

Stimulant use disorders are chronic relapsing conditions that differentially affect men and women. Growing evidence has demonstrated that endocrine systems play critical, bidirectional roles in regulating vulnerability to drug use, escalation, withdrawal, and relapse, particularly for women. In the current review we discuss the bidirectional relation between gonadal hormones; focusing on estradiol (E2), progesterone (P4), and testosterone (T); and stimulant rewards and reinforcement; focusing on cocaine, amphetamine, and methamphetamine. Most research to date has focused on cocaine and less so

Evaluating the PCOS risk algorithm (PriskA): a pilot study.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a heterogeneous disorder with reproductive, metabolic, and psychological features, often underdiagnosed due to diagnostic inaccuracies and inconsistent knowledge among providers. These gaps highlight the need for improved diagnostic approaches to identify patients at risk earlier. This pilot study aimed to evaluate the validity of the PCOS risk algorithm (PriskA), a digital tool designed to assess PCOS risk in symptomatic women. RESULTS: A total of 144 women were referred for standardized endocrine screening at the Erasmus Medical centre and were
