One hundred and one patients with polycystic ovary syndrome (PCO) were treated during an 11-year interval. Sixty-seven cases were classified as typical PCO with enlarged ovaries of probable ovarian origin (Type I) and 34 as atypical PCO of probable adrenal origin (Type II). Fifty-five patients were treated for anovulatory infertility with clomiphene citrate or other endocrine therapy as indicated. Basal gonadotrophin and sex steroid levels and responses to an intravenous injection of 100 mug gonadotrophin releasing hormone (Gn-RH) have been studied in 15 patients with polycystic ovaries. Mean basal LH concentration was raised and an excessive, exaggerated and prolonged response was observed after Gn-RH treatment, but patients could further be subdivided into two functional groups on the basis of their basal LH values and LH response patterns. Evidence was also produced which suggested a breakdown in the negative feedback mechanism in these patients.

Women commonly complain of excess facial or body hair. There are generally no other abnormal physical findings and the relevance of other symptoms, especially those related to the menstrual cycle, may be unclear. The theme of androgen excess provides an explanation for the sign and various symptoms, and suggests certain pathology. Rational therapy can be based on and understanding of the hormonal processes involved in the development of secondary sexual hair. Treatment of simple hirsutism is well tolerated even in prolonged courses, and in most cases produces a satisfactory cosmetric result.
