This paper describes the LH and FSH responses to synthetic LH-RH in 81 anovulatory women and 37 ovulatory women at various estradiol (Ed) levels. Besides, effects of mestranol of releases of LH and FSH in response to LHrh were examined in 4 anovalatory women. Subjects were divided into 3 groups: low Ed group, moderate Ed group and high Ed group. Three young women with primary ovarian failure, secondary amenorrhea or oligo-ovulation, and normal gonadotropin levels are reported on here. Vaginal cytology and serum estradiol levels indicated the two women to be persistently hypoestrogenic. Variation in the severity of keratosis follicularis relative to states of homonal change was noted in eight women with the disease. Observations included frequent onset near puberty, puberty, exacerbation during menses, and improvement while patients are pregnant or on oral contraceptive agents. It is proposed that states of relative estrogen excess are associated with quiescence of this skin disorder. In support of this hypothesis, three postpubertal women with Darier desease were observed to undergo significant improvement with the use of estrogen-dominant birth control pills.

The origin and quantity of estrogen and androgen were measured in a postmenopausal woman with clinical signs of estrogen excess and a nonendocrine tumor of the ovary. The plasma androstenedione production rate was elevated 5-fold. Seven oophorectomized patients were administered a total of 60,000 units of human chorionic gonadotropin over a 3-day period, and plasma testosterone levels were obtained. One of the 7 patients demonstrated a definitive response to the hormone but repeat testing proved to be negative. The potential role of luteinizing hormone in the pathogenesis of polycystic ovarian syndrome is discussed and current evidence linking LH to adrenal androgenesis is reviewed. Modifications that may be employed for future study are suggested.

A case of maternal virilization associated with bilateral luteomas of pregnancy is described. Urinary 17-ketosteroids and plasma testosterone fell from markedly abnormal values to normal limits within 2 weeks of delivery. However, symptoms of virilization persisted in the postpartum state. A possible relation between polycystic ovarian syndrome in the nonpregnant state and luteoma of pregnancy is proposed.

A virilizing adrenocortical adenoma was demonstrated in a yound female. Urinary 17-ketosteroid excretion and subfractions, plasma testosterone level and urinary 17-ketogenic steroid excretion were markedly increased. Dehydroepiandrosterone was the main constituent of the androgen excess. Hirsute women pose a diagnostic dilemma when urinary 17-ketosteroid and serum testosterone levels are normal. Unilateral ovariectomy was performed on 3 mares affected with granulosa cell tumors. Tumor fluid in each mare was found to contain estrogen, testosterone, and progesterone. Large multiple cystic ovarian follicles were induced in three sows and small multiple cystic ovarian follicles were induced in three other sows by injections of adrenocorticotrophic hormone (ACTH) during the follicular phase of the oestrous cycle. Serum testosterone levels were monitored in female subjects who received therapy with human gonadotropins of urinary origin (menotropins) and human chorionic gonadotropin (hCG). Endocrine features of anovulation are reviewed, with particular emphasis on methods of investigation. The value of, and indications for, measurements of serum gonadotrophin concentrations are discussed: it appears that FSH measurements allow better discrimination than LH measurements in diagnosing patients with primary ovarian failure. The results of stimulation tests with the synthetic form of gonadotrophin-releasing hormone are described and the relationship of tests with exogenous progestogens and clomiphene is discussed.
