NNMT enhances hyperandrogenism in polyendocrine metabolic ovarian syndrome through epigenetic remodelling.

Polyendocrine metabolic ovarian syndrome (PMOS) features intertwined reproductive, endocrine and metabolic defects, yet its initiating mechanisms remain incompletely understood. Here we show that an epigenetic mechanism driven by nicotinamide N-methyltransferase (NNMT) in granulosa cells contributes to disease progression in PMOS. We observe that Nnmt overexpression in mouse granulosa cells reduces histone H3 lysine 27 trimethylation (H3K27me3), leading to increased retinoic acid receptor beta (RARB) expression. RARB, in turn, amplifies ovarian androgen synthesis. The resulting androgen excess

Microglia as candidate links between hormonal signals and neuronal circuits in polycystic ovary syndrome: evidence gaps and a research agenda.

Polycystic ovary syndrome (PCOS) combines reproductive endocrine dysfunction with metabolic, autonomic, and neurobehavioral phenotypes, yet the contribution of brain microglia remains uncertain. We conducted a structured narrative review that separates human PCOS evidence, PCOS-like animal findings, PCOS-related cellular observations, and non-PCOS mechanistic bridges. Only three heterogeneous PCOS-like studies directly assessed microglial outcomes, and none established a conserved microglial state, microglial necessity, a complete hormone-microglia-neuron causal chain, or human relevance. The Gut-Insulin Axis in Polycystic Ovary Syndrome: A Narrative Review of Gut Dysbiosis, Inflammation, and Emerging Therapeutic Strategies.

Polycystic ovary syndrome (PCOS) is a complicated, endocrine-endemic disorder with a prevalence rate of 8-13% in reproductive-age women in the world, which manifests through insulin resistance, hyperandrogenism, and reproductive dysfunction. Despite its high global prevalence, existing therapeutic interventions focus predominantly on symptom management rather than targeting the heterogeneous and multifactorial pathophysiology of the syndrome. Intestinal barrier integrity is destroyed by dysbiosis of the gut microbiota and induces endotoxemia and chronic low-grade inflammation through signallin

Adrenocortical Carcinoma with Suspected Dual Cortisol and Aldosterone Secretion: A Case Report.

Adrenocortical carcinoma (ACC) is a rare malignancy, accounting for 0.05-2% of all malignant tumors. Most cases are sporadic and asymptomatic, often discovered incidentally via imaging. However, 50-60% present with autonomous hormonal secretion, with 45% producing excess cortisol, and only 1% exhibiting mineralocorticoid excess. We report a 43-year-old Filipino female with a 6-month history of abdominal bloating, proximal muscle weakness, edema, and hypertension. Laboratory tests showed hypokalemia (serum potassium 2.5-3.05 mmol/L; normal 3.3-5.0 mmol/L). Imaging revealed a neoplastic right su

Physiopathology of PMOS from a Neuroendocrine Perspective.

Polyendocrine metabolic ovarian syndrome (PMOS) is the most common reproductive and cardiometabolic disorder in women of reproductive age. Once viewed as primarily an ovarian disease, it is now recognized as a neuroendocrine-metabolic disorder driven by dysregulation of the hypothalamic-pituitary-gonadal axis. A central feature of the syndrome is increased gonadotropin releasing hormone (GnRH) pulsatility, which drives excess luteinizing hormone (LH) secretion, disrupts the LH/follicle stimulating hormone (FSH) balance, and promotes ovarian androgen excess and anovulation. Hypothalamic Kisspep

Factors associated with chronic kidney disease in patients with gout: a two-centre study incorporating ultrasonographic features.

OBJECTIVES: Chronic kidney disease (CKD) is a significant comorbidity in patients with gout, yet the specific phenotypes associated with its prevalence remain insufficiently characterised. This study aimed to evaluate the association between clinical indicators and ultrasonographic features with CKD using a transparent, interpretable machine learning approach. METHODS: This cross-sectional bicentric study analysed a cohort of 666 patients with gout, who were partitioned into a derivation cohort (n=500) for model development and internal validation, and an independent validation cohort (n=166)

Divergent effects of hyperandrogenemia on the female reproductive neuroendocrine axis under different physiological conditions.

The hypothalamic-pituitary-gonadal (HPG) axis coordinates reproductive function and incorporates gonadal sex steroid feedback to regulate gonadotropin secretion in a homeostatic manner. In females, ovarian estrogens and progestins act as the primary negative feedback signals to suppress luteinizing hormone (LH) release, whereas in males, androgens, such as testosterone (T), typically provide this feedback. However, androgens can also exert feedback effects on the female HPG axis, particularly in conditions where circulating T levels exceed that of the normal female physiological range. Three c

Multi-omics factor analysis v2 (MOFA+) reveals specific co-variation patterns in women with PMOS that are strongly influenced by obesity.

STUDY QUESTION: What are the mechanisms underlying the complex interactions between polyendocrine metabolic ovarian syndrome (PMOS) and obesity as studied by a multi-omics integration approach? SUMMARY ANSWER: Multi-Omics Factor Analysis v2 (MOFA+) suggested that PMOS was characterized by an androgen-driven metabolic reprogramming in which sexual dimorphism was reshaped in an obesity- and omics-specific manner, consisting of strong associations of proinflammatory proteins and adipokines with gut microbiota genera and metabolomics variables that were amplified by obesity. From Polycystic Ovary Syndrome to Polyendocrine Metabolic Ovarian Syndrome: A Systematic Search and Historical Account of the Renaming of PCOS.

Polyendocrine metabolic ovarian syndrome (PMOS) is one of the most common endocrine conditions affecting hundreds of millions of women globally. The former name polycystic ovary syndrome (PCOS) was inaccurate, underpinning the robust nomenclature change process. This review historically traces renaming efforts up to, including and beyond the PMOS global renaming process. A systematic search of Medline and Embase screened according to PRISMA 2020 methodology, identified 30 eligible papers (1995-2026). The name PCOS was repeatedly challenged on scientific grounds, with the absence of increased o

The Ontogeny of Distinct Reproductive Phenotypes in Girls at Risk for PCOS During the Postmenarchal Transition.

CONTEXT: Daughters of women with polycystic ovary syndrome (PCOS-d) and girls with overweight/obesity (OW-g) have hyperandrogenemia (HA) beginning in childhood. However, other features of their early reproductive phenotypes differ, suggesting that there are distinct mechanisms conferring increased risk for PCOS. OBJECTIVE: We performed a cross-sectional study of adolescent girls during the early postmenarchal transition. DESIGN, SETTING, AND PARTICIPANTS: PCOS-d (n = 15), OW-g (n = 12), and lean control girls (LC; n = 17) were studied within .2 to 1.2  years of menarche. Metabolic and reprod

Expanded Steroid Profiling Identifies Novel Newborn Screening Markers for Congenital Adrenal Hyperplasia.

CONTEXT: Simultaneous measurement of multiple steroids in dried bloodspots facilitates accurate newborn screening (NBS) for classic congenital adrenal hyperplasia (CAH), although false-positive (FP) tests are encountered. Some babies with milder forms of CAH, who may benefit from early detection and treatment, can be missed by screening. OBJECTIVE: This work aims to evaluate a method for expanded steroid profiling in NBS bloodspots to identify novel sensitive and specific markers that will inform improvements for screening for CAH, and to identify the metabolic routes to excessive androgen sy

Enigma of postmenopausal virilisation: bilateral Leydig cell tumours as the culprit.

A postmenopausal woman in her 50s presented with progressive hirsutism, virilisation and markedly elevated serum testosterone levels (>1500 ng/dL). Routine laboratory, adrenal and imaging evaluations were largely unremarkable except for a small left adnexal mass on MRI. With normal dehydroepiandrosterone sulphate (DHEAS) and cortisol levels, an ovarian source of androgen excess was suspected. The patient underwent total laparoscopic hysterectomy with bilateral salpingo-oophorectomy. Histopathology revealed bilateral Leydig cell tumours, an exceptionally rare finding. Postoperatively, serum tes

Nonhormonal Pharmacological Interventions in Adolescent Polycystic Ovary Syndrome (PCOS): A Systematic Review.

BACKGROUND: Adolescents with polycystic ovary syndrome (PCOS) experience metabolic dysfunction, reproductive disturbance, and psychosocial burden. While combined hormonal contraceptives (CHC) are first-line pharmacologic treatment, concerns regarding side effects and long-term safety have spurred interest in nonhormonal alternatives. However, most evidence is derived from adult cohorts, leaving guidance specific to adolescents limited. STUDY OBJECTIVES: To systematically review the efficacy of nonhormonal pharmacotherapies including metformin, glucagon-like peptide-1 receptor agonists (GLP-1R

Computed Tomography Angiography-Assisted Ovarian Vein Sampling for Diagnosing an Androgen-Producing Leydig Cell Tumor: A Case Report.

BACKGROUND: Ovarian Leydig cell tumors, rare androgen-producing neoplasms, may present with rapidly progressive virilization. Preoperative localization is frequently challenging, particularly when bilateral ovarian abnormalities coexist or radiologic findings are equivocal. To avoid unnecessary bilateral oophorectomy and preserve fertility in reproductive-aged females, accurate localization is crucial. CASE PRESENTATION: We describe the case of a 42-year-old reproductive-aged female who presented with hirsutism, voice deepening, amenorrhea, and biochemical evidence of severe hyperandrogenism.

Metabolic effects of testosterone therapy in transgender and gender diverse individuals with polycystic ovary syndrome.

BACKGROUND: Polycystic ovary syndrome (PCOS) affects 10% to 13% of individuals assigned female at birth (AFAB), although a greater prevalence is reported in transgender/gender diverse individuals (TGD) (23.8-38.8%). Testosterone therapy (TT) in TGD individuals is linked to worsening cardiovascular risk factors (CVRFs). Whether TT induces more adverse cardiometabolic changes in TGD with PCOS than TGD without PCOS remains uncertain. OBJECTIVE: To compare the effects of TT on CVRF in TGD with and without PCOS. METHODS: We retrospectively reviewed 373 records to identify 62 TGD with PCOS (withes

Polycystic ovary syndrome (PCOS): current insights, emerging therapeutics, and future treatment strategies.

Polycystic ovary syndrome (PCOS) is a common gynaecological disorder, clinically characterized by chronic anovulation and hyperandrogenism. Despite its high prevalence, no curative treatment exists, and managing the syndrome remains challenging. A strong association between PCOS and metabolic syndrome, particularly insulin resistance, adversely influences fertility, leading to an increased worldwide demand for advanced treatment options. Although the exact aetiology and pathophysiology of the syndrome remain unclear, an intricate interaction of ovarian, endocrine, and metabolic factors is high
