Male Pattern Androgenetic Alopecia Linked to an Adrenal Tumor: A Case Report.

INTRODUCTION: Androgenetic alopecia (AGA) is a type of hair loss that often manifests as male or female pattern baldness. In women, AGA resembling male pattern baldness can occur in conditions of hyperandrogenism, such as congenital adrenal hyperplasia or ovarian and adrenal tumors, which are exceedingly rare, occurring in approximately 0.2% of cases. CASE PRESENTATION: We report a 62-year-old female exhibiting hair loss and hirsutism that began over a decade ago. The medical examination reveals hirsutism on the face, abdomen, and chest, and hair loss in the fronto-parieto-occipital region, w

Prevalence of Acne Vulgaris Among Women with Polycystic Ovary Syndrome: A Systematic Review and Meta-Analysis.

BACKGROUND: Acne vulgaris is one of the most common dermatological conditions worldwide, particularly affecting women of reproductive age. It is often linked to underlying hormonal imbalances, including those seen in polycystic ovary syndrome (PCOS). PCOS is a prevalent endocrine disorder characterized by hyperandrogenism and insulin resistance, both of which contribute to acne vulgaris development. This systematic review and meta-analysis aimed to estimate the global prevalence of acne vulgaris among women with PCOS and identify contributing factors, thereby highlighting the burden of this sk

Carvone attenuates cardiac fibrosis and inflammation by modulating GSK-3β/PKC-α signalling pathway in experimentally induced polycystic ovarian syndrome.

BACKGROUND: Beyond the reproductive function, polycystic ovarian syndrome (PCOS) affects cardiovascular integrity and recognized as a risk-enhancing factor for cardiovascular disease, reducing the quality of life of reproductive-aged women. Studies have attributed PCOS to cardiovascular risk profile and altered lipid/glucose metabolism/systemic inflammation, though the pathophysiological mechanisms remain unclear. Carvone (CAV), a monoterpene ketone exhibits antibacterial, antioxidant and ant-inflammatory effects, making it a potential therapeutic agent for abating PCOS-associated cardiometabo

IDO Inhibition by 1-Methyltryptophan: Unlocking New Paths to Treat Ovarian Dysfunction and Hormonal Imbalance in PCOS.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a common endocrine disorder characterized by chronic inflammation, as well as metabolic and reproductive dysfunction. While insulin resistance affects many tissues, ovarian tissue exhibits insulin hypersensitivity, which promotes androgen excess and worsens PCOS symptoms. The kynurenine pathway (KP), a major route of tryptophan metabolism regulated by indoleamine 2,3-dioxygenase (IDO), is implicated in ovarian dysfunction in PCOS. OBJECTIVES: To investigate the effects of IDO inhibition on ovarian morphology and insulin signaling in a PCOS rat m

Therapeutic perspective of postbiotics in managing health of women with polycystic ovarian syndrome: a holistic approach.

Polycystic Ovary Syndrome (PCOS) is an amalgamation of several factors such as complex combination of endocrine and metabolic disorder caused due to insulin resistance, hyperandrogenism and chronic inflammation. Research shows that postbiotics and their metabolic byproducts represent a new therapeutic opportunity for PCOS treatment. Postbiotics produce various therapeutic properties including anti-inflammatory, antioxidant and immunomodulatory effects that assist in balancing the metabolism. Postbiotics strengthen gut wall integrity and reduces inflammation while it affects the interaction bet

Measurement of twenty-one serum steroid profiles by UPLC-MS/MS for the diagnosis and monitoring of congenital adrenal hyperplasia.

BACKGROUND: Congenital adrenal hyperplasia (CAH) represents a group of inherited disorders affecting steroidogenesis. Early and accurate diagnosis is crucial for effective treatment, particularly for preventing adrenal insufficiency and minimizing androgen excess. This study aims to develop and validate an ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) method for the simultaneous quantification of 21 steroid hormones, including 11-oxygenated androgens, which are critical for diagnosing and monitoring various forms of CAH. METHODS: We utilized a microbore column

Perception of women with classic congenital adrenal hyperplasia and their parents on genital surgery and a diagnosis of differences of sex development: a retrospective survey.

PURPOSE: Classic congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency leads to adrenal androgen excess, regularly causing prenatal virilisation of the external genitalia in affected females, commonly corrected by early genital surgery. As this practice is controversial, this study retrospectively assessed patients' and parents' perspective on this matter. METHODS: Adult female patients with classic CAH who had undergone genital surgery and their parents participated in this single-centre, cross-sectional survey study. Patients completed the female Sexual Function Index (fSFI)

Targeting Polycystic Ovary Syndrome (PCOS) Pathophysiology with Flavonoids: From Adipokine-Cytokine Crosstalk to Insulin Resistance and Reproductive Dysfunctions.

Polycystic ovary syndrome (PCOS) represents one of the most prevalent endocrine-metabolic disorder in women of reproductive age, which includes but not restricted to reproductive disruptions, insulin resistance (IR), hyperandrogenism, and chronic low-grade inflammation. Its heterogeneous pathophysiology arises from the interplay of metabolic, endocrine, and immune factors, including dysregulated adipokine secretion, cytokine-mediated inflammation, oxidative stress (OS), and mitochondrial dysfunction. Current pharmacological therapies, such as metformin, clomiphene, and oral contraceptives, oft

Data-driven subtypes of polycystic ovary syndrome and their association with clinical outcomes.

Polycystic ovary syndrome (PCOS) is a common and heterogeneous endocrine disorder that affects 11%-13% of women worldwide, with profound implications for fertility and long-term metabolic health. Here we identify four reproducible subtypes-PCOS with hyperandrogen, with obesity, with high-sex hormone-binding globulin and with high-luteinizing hormone-anti-Müllerian hormone-through unsupervised clustering of 9 clinical variables in 11,908 affected women, validated across 5 international cohorts. Prospective 6.5-year follow-up and in vitro fertilization treatment data revealed distinct reproducti

POLYCYSTIC OVARY SYNDROME: ORIGINS AND IMPLICATIONS: PCOS in adolescents: ontogeny of neuroendocrine and metabolic disturbances.

IN BRIEF: Early clinical manifestations of PCOS may begin during puberty. This paper reviews the initial characteristics of PCOS in the adolescent girl. ABSTRACT: Polycystic ovary syndrome (PCOS) is a heterogeneous multi-factorial familial disorder characterized by androgen excess, irregular menses, and chronic anovulation. The key features, hyperandrogenism, insulin resistance, and hyperinsulinemia, can be identified in peripubertal girls. From a pediatric/adolescent point of view, the ability to identify girls with PCOS or those 'at-risk for PCOS' is crucial to target educational and medica

The Triple Hormone Enigma: Pituitary Macroadenoma With Bilateral Cortisol-Androgen Secreting Adenomas.

The coexistence of a pituitary macroadenoma co-secreting growth hormone (GH) and adrenocorticotropic hormone (ACTH) with bilateral adrenal adenomas autonomously producing cortisol and androgens represents an extremely rare and diagnostically complex clinical entity. We describe a 54-year-old woman who presented with acral enlargement, hirsutism, hyperpigmentation, and poorly controlled diabetes and hypertension. Hormonal evaluation revealed concurrent elevations in GH, ACTH, cortisol, dehydroepiandrosterone sulfate, and testosterone. Contrast-enhanced magnetic resonance imaging of the brain sh

Assessment of mitochondrial peptide humanin in women with polycystic ovary syndrome: serum and skeletal muscle profile.

PURPOSE: Humanin is a mitochondria-derived peptide (MDP) secreted in response to oxidative stress and linked to diverse cellular processes including inflammation and insulin resistance (IR). In a PCOS rat model, humanin has been shown to be downregulated in polycystic ovaries, and exogenous humanin supplementation has attenuated IR and ovarian morphological abnormalities. We aimed to investigate the role of humanin in the pathophysiology of PCOS by comparing serum and skeletal muscle tissue profiles in women with PCOS and healthy women. METHODS: Forty women with PCOS [(mean ± SD) age:21.8 ± 2.

Forebrain AR Deletion Restores PR Expression but not Reproduction in Prenatally Androgenized Female Mice.

Prenatal androgen excess (PNA), an etiologic factor for polycystic ovary syndrome (PCOS), is implicated in programming long-term reproductive deficits in females such as anovulation, subfertility, and hyperandrogenism. Impaired steroid hormone feedback is a key neuroendocrine feature suspected to underpin the development of reproductive dysfunction in both clinical PCOS and in PNA mice exposed to dihydrotestosterone during late gestation. PNA is suspected to act in the brain to program the impaired sensitivity of the GnRH neuronal network to progesterone negative feedback, centrally dysregulat

The Complex Spectrum of 11β-Hydroxylase Deficiency: A Case of Precocious Puberty, Hypertension, and Testicular Adrenal Rest Tumors (TARTs).

11β‑hydroxylase deficiency (11β‑OHD), a rare form of congenital adrenal hyperplasia (CAH) accounting for roughly 1 in 100,000 live births, often presents with androgen excess and hypertension. It manifests in males as precocious puberty, often accompanied by hypertension, and in females as 46XX disorders of sex development with virilization. Long-term complications such as adult hypertension, impaired final height, and bilateral testicular adrenal rest tumors (TARTs) are infrequently reported. We describe a 22-year-and-10-month-old male with a history of early-onset puberty beginning at 2.5 ye

Evaluation of dihydrotestosterone levels and total testosterone to dihydrotestosterone ratio with clinical symptoms and metabolic parameters in patients with polycystic ovary syndrome.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a clinically heterogeneous endocrine disorder with lifelong health risks. Androgen excess, particularly testosterone, plays a key role in the pathophysiology of PCOS. Dihydrotestosterone (DHT) is an active metabolite derived from testosterone, and the total testosterone (TT) to DHT ratio is known as a potential marker of androgen metabolism, which can be used for evaluation of metabolic phenotype. This exploratory study aims to evaluate the association between DHT levels and TT/DHT ratio with the metabolic parameters and clinical features of PCOS

Spontaneous Pregnancy in Genetically Confirmed 11-Beta Hydroxylase Deficiency: A Case Series and Literature Review.

11-beta hydroxylase deficiency (11βOHD) is a rare variant of congenital adrenal hyperplasia (CAH) with autosomal recessive inheritance, resulting in androgen excess and mineralocorticoid precursor accumulation. Fertility is often impaired due to hyperandrogenism and anatomical abnormalities, and spontaneous successful pregnancy in classic form is rare. We describe two cases. Case 1 is a 27-year-old woman with genetically confirmed classic 11βOHD (homozygous CYP11B1 splice-site variant c.240-2A>G), initially misdiagnosed as 21-hydroxylase deficiency, who achieved spontaneous conception despite

Oral prolonged-release dienogest 2 mg and ethinylestradiol 0.02 mg in a 24/4-day regimen for polycystic ovary syndrome-associated hirsutism: a double-blind, randomised, placebo-controlled trial.

BACKGROUND: Current guidelines for polycystic ovary syndrome (PCOS) recommend combined oral hormonal contraceptives as first-line pharmacological treatment to reduce androgen excess and to treat clinical hyperandrogenic skin manifestations, such as hirsutism. However, these are used off-label. We aimed to demonstrate the efficacy of a prolonged-release oral formulation in a 24/4-day regimen for up to nine 28-day consecutive cycles compared with placebo for the clinical management of hirsutism in women with PCOS. METHODS: This was a multinational, multicentre, double-blind, placebo-controlled

Unraveling PCOS therapies: Pharmacotherapeutic strategies and emerging therapeutic targets.

Polycystic ovary syndrome (PCOS) is a common health problem that can affect the reproductive health of women. A typical characteristic of PCOS is infertility, androgen excess, and anovulation. In addition, a disorder also accompanies other pathologies like obesity and hyperinsulinemia, with an increased threat of cardiovascular complications. Current treatment strategies focus on the reduction of PCOS symptoms primarily through pharmacological medications such as metformin, oral contraceptives, and anti-androgenic agents. Since these medications do not encompass all the outcomes of PCOS, there

CRISPR as a Tool to Uncover Gene Function in Polycystic Ovary Syndrome: A Literature Review of Experimental Models Targeting Ovarian and Metabolic Genes.

Polycystic ovary syndrome (PCOS) is a complex disorder characterized by reproductive abnormalities such as hyperandrogenism, ovulatory dysfunction, and polycystic ovarian morphology, and is frequently accompanied by metabolic disturbances such as insulin resistance, obesity and dyslipidemia. Genome-wide association studies (GWASs) have identified several susceptibility loci, yet little is known about their functional implications. Clustered regularly interspaced short palindromic repeats (CRISPR)/CRISPR-associated protein 9 (CRISPR/Cas9) has emerged as a powerful gene editing tool in bridging
