Combined androgen excess polycystic ovary syndrome and obesity are associated with elevated beat-to-beat blood pressure variability in adult women.

Androgen excess polycystic ovary syndrome (AE-PCOS) is associated with elevated risk for hypertension and cardiovascular disease. Greater beat-to-beat blood pressure variability (BPV) has emerged as a novel marker of cardiovascular risk. Little is known about beat-to-beat BPV in AE-PCOS, but prior studies suggest blunted baroreflex sensitivity (BRS), a major contributor to beat-to-beat BP regulation. Both obesity and androgen excess are implicated as driving factors of cardiovascular risk in AE-PCOS. We hypothesized that 1) beat-to-beat BPV would be greater in AE-PCOS compared with both women

Neurohormonal-Immune Dysregulation in Rosacea: Emerging Perspectives from the Skin-Gut-Brain Axis.

Rosacea is a chronic inflammatory skin disease with a global prevalence of approximately 5.1%. Existing evidence suggests that its pathogenesis may be associated with dysregulation of the neuro-hormone-immune network within the skin-gut-brain axis framework. This review summarizes the potential roles of stress hormones, sex hormones, and insulin-like growth factor-1 in the four major subtypes. In erythematotelangiectatic rosacea, glucocorticoid resistance within keratinocytes may induce CCL20-driven Th17 inflammation, while catecholamines released by sympathetic nerves are associated with neur

Determinants of pubertal progression and final height in premature pubarche.

BACKGROUND: Premature pubarche (PP), defined as the early onset of pubic hair and often associated with mild androgen excess, is a common reason for referral to a pediatric endocrinologist. A small proportion of cases may progress to central precocious puberty (CPP). Data on the factors determining pubertal tempo and associated growth outcomes in these individuals are limited. OBJECTIVE: To identify predictors of pubertal progression and final height (FH) in girls with PP and to evaluate growth outcomes in those progressing to CPP. DESIGN-METHODS: We retrospectively analyzed 88 girls with PP

Clinical and pathological features of oncocytic adrenocortical carcinoma: a retrospective comparative study.

PURPOSE: Adrenocortical carcinoma (ACC) is a rare tumor with highly aggressive malignancy. Its two main pathological subtypes, conventional ACC (CAC) and oncocytic ACC (OAC), are hypothesized to differ in clinical behavior, but comparative studies are limited due to the rarity of OAC. This study aimed to compare the clinical characteristics, pathological features, and prognosis between CAC and OAC. METHODS: A retrospective review was conducted on 128 ACC patients (105 CAC, 23 OAC) during the period of October 2015 and October 2025. Data on clinical presentation, hormonal status, pathological

Relationship Between Adipose Tissue and Liver Dysfunction in Women with Polycystic Ovary Syndrome and Metabolic Syndrome.

Polycystic ovary syndrome (PCOS) is frequently accompanied by visceral obesity, insulin resistance, low-grade chronic inflammation, and metabolic syndrome (MetS). These alterations promote significant dysfunction in adipose tissue and liver metabolism through cytokine production. Growing evidence indicates that the interaction between hepatokines and adipokines plays a central role in the development of metabolic and hepatic abnormalities in women with PCOS. This narrative review was conducted to analyze the relationship between adipose tissue dysfunction and liver metabolic impairment in wome

Pharmacotherapeutic strategies for the management of congenital adrenal hyperplasia.

INTRODUCTION: Classical congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency (21OHD) is characterized by impaired cortisol synthesis and excess adrenal androgen production. Current management relies on adrenal hormone replacement, however, achieving adequate androgen suppression often requires supraphysiologic glucocorticoid dosing with a risk of long-term morbidity. AREAS COVERED: This review discusses current evidence on pharmacotherapeutic strategies for 21OHD, including evolving glucocorticoid formulations and delivery methods, as well as both established and emerging non

Diagnosis and management of androgen excess presenting after menopause.

Postmenopausal hyperandrogenism is a rare yet clinically relevant condition. It can result from a wide variety of sources and etiologies, from benign functional disorders to life-threatening androgen-secreting tumors. Polycystic ovary syndrome is the most frequent cause of hyperandrogenism in postmenopausal patients. Ovarian or adrenal tumors are rare but must be promptly recognized because any delay in diagnosis is associated with a significant risk of mortality. Sudden onset and rapid progression of symptoms or virilizing features are clinical red flags that must lead to the use of imaging t

Diagnostic work-up in paediatric adrenocortical tumours: an international consensus.

OBJECTIVE: Paediatric adrenocortical tumours (pACTs) are rare but potentially life-threatening neoplasms, most of which are hormonally active and frequently present with androgen excess. Although a comprehensive evaluation is recommended, no consensus exists on the optimal diagnostic work-up. The goal of this document is to provide evidence-based recommendations to standardize the diagnostic evaluation of children with suspected pACTs. METHODS: We applied a modified three-step Delphi process with an international panel of pACT experts to develop consensus on the initial diagnostic work-up. Fo

Testosterone reference ranges in women, adolescent girls, and children: the 2024 PCOS Challenge-CDC stakeholder meeting.

CONTEXT: Testosterone measurement is fundamental to the management of many disorders in women, adolescent girls, and children, most notably androgen excess, including polycystic ovary syndrome (PCOS, aka polyendocrine metabolic ovarian syndrome [PMOS]) and congenital adrenal hyperplasia, and androgen deficiency disorders. Despite 2 decades of progress in testosterone assay standardization, pediatric and adult female reference intervals (RIs) remain highly variable across laboratories and assay systems, limiting diagnostic accuracy and clinical decision-making. OBJECTIVE: On August 26, 2024, P

Steroidogenic enzyme dysregulation in polycystic ovary syndrome: Mechanistic insights and emerging therapeutic strategies.

Polycystic ovary syndrome (PCOS) is the most prevalent endocrine-metabolic disorder in women of reproductive age, and is characterized by hyperandrogenism, anovulation, and polycystic ovarian morphology. Emerging molecular evidence has identified dysregulated ovarian steroidogenesis as a major contributing mechanism linking reproductive and metabolic phenotypes in PCOS, acting in concert with neuroendocrine and metabolic dysfunction. This review synthesizes current knowledge on enzymatic and regulatory perturbations driving androgen excess and estrogen deficiency in PCOS, emphasizing their mec

Influence of Biological Sex on Metabolic and Adipokine Responses after Metabolic-Bariatric Surgery: A Narrative Review.

PURPOSE OF REVIEW: Obesity is a multifactorial, chronic, and relapsing non-communicable disease characterized by excessive adipose tissue accumulation and metabolic dysfunction. Increasing evidence indicates that biological sex influences not only the development and distribution of adipose tissue but also responses to obesity treatments, including metabolic-bariatric surgery. This narrative review synthesizes current evidence on sex-related differences in metabolic outcomes after bariatric surgery. RECENT FINDINGS: Bariatric surgery induces substantial weight loss and marked improvement of o

Metabolic-Hormonal Interplay and the Case for a Reproductive-Metabolic Framework in the Glucagon-Like Peptide-1 (GLP-1) Era.

Polycystic ovary syndrome (PCOS), proposed by some investigators to be more accurately termed polycystic-morphology ovary syndrome (PMOS), reflecting its heterogeneous and not universally cystic pathophysiology, remains the most prevalent endocrine disorder among reproductive-age women worldwide. Despite this, its clinical management continues to be fragmented along specialty lines, with gynecologists addressing menstrual irregularity and contraception, endocrinologists managing insulin resistance and metabolic risk, and dermatologists treating hyperandrogenic manifestations. Bilateral giant myelolipomas in a female patient with untreated 21-hydroxylase deficiency.

Adrenal myelolipomas are benign neoplasms of the adrenal gland composed of mature adrenal and adipose tissue as well as myeloid elements. Although infrequent in the general population, their prevalence is markedly increased in patients with poorly controlled classical congenital adrenal hyperplasia (CAH), most likely attributable to chronic ACTH-mediated adrenal stimulation. We report the case of a 59-year-old female with a history of classic CAH due to 21-hydroxylase deficiency who did not receive treatment for over 30 years and had developed bilateral giant myelolipomas (left side: 24.5 cm ×

Potential role of maintaining physiological testosterone levels in improving glucose metabolism among normal-weight Japanese women: a pilot exploratory study.

BACKGROUND: Traditionally, elevated androgen levels in women have been associated with obesity and deteriorated glucose metabolism. However, most existing data are derived from patients with polycystic ovary syndrome (PCOS) or obesity. Consequently, the physiological role of maintaining testosterone levels in normal-weight women remains poorly understood. OBJECTIVE: To eliminate metabolic biases associated with obesity, this exploratory study investigated the relationship between serum total testosterone levels and parameters of body fat and glucose metabolism (HbA1c, fasting plasma glucose,

A single-nucleus multiomic study of preoptic area cells in prepubertal control vs prenatally androgenized female mice.

Polycystic ovary syndrome (PCOS) is a common endocrine disorder with developmental origins. While the etiology is unclear, current postulates include epigenetic programming. Cell-type-specific epigenetic changes by which prenatal androgen excess programs the neuroendocrine axis have not been defined. Using single-nucleus (sn) multiome sequencing (single-nucleus ribonucleic acid sequencing [snRNAseq] + single-nucleus assay for transposase-accessible chromatin using sequencing [snATACseq]) of the mouse preoptic area, we profiled transcriptional and chromatin accessibility landscapes across 31 ce

Prevalence of Congenital Adrenal Hyperplasia in South Indian Polycystic Ovarian Syndrome Women.

INTRODUCTION: Polycystic ovary syndrome (PCOS) is a prevalent endocrine disorder characterised by hyperandrogenism and ovulatory dysfunction. Nonclassic congenital adrenal hyperplasia (NCCAH) is an important differential diagnosis, but its prevalence in Indian women remains unclear. Previous North Indian studies reported variable rates using less specific assays. This study aimed to determine the prevalence and clinical characteristics of NCCAH among hyperandrogenemic South Indian women with PCOS. METHODS: This cross-sectional study was conducted at a tertiary care centre in South India (Janu

11-Oxygenated Androgens in Pcos: Impact of Pcos Phenotypes and First-Line Interventions.

CONTEXT: Androgen excess in polycystic ovary syndrome (PCOS)/polyendocrine metabolic ovarian syndrome (PMOS) is classically attributed to ovarian steroids, yet adrenal-derived 11-oxygenated androgens are potent contributors whose regulation and treatment responsiveness in PMOS remains poorly characterized. OBJECTIVE: To quantify classic and 11-oxygenated androgens across PMOS phenotypes versus controls and to evaluate their responses to oral contraceptive pills (OCPs), metformin, or lifestyle modification (LSM).
