Biochemical control in young women with congenital adrenal hyperplasia taking oral contraceptives: a prospective observational study.

OBJECTIVE: Androgen excess in 21-hydroxylase deficiency (21OHD) remains difficult to manage in adolescent females. Combined oral contraceptive(s) (OC) containing ethinylestradiol and progestin modulate ovarian and adrenal steroidogenesis, but their specific impact on 21OHD has not been systematically evaluated. We therefore examined how combined OCs influence the steroid metabolome in young women with classic 21OHD. DESIGN: Prospective observational study. METHODS: Young women aged 11-24 years with classic 21OHD who planned to initiate combined OC were enrolled. The study comprised 3 visits

Physical activity, sedentary behavior, and metabolic-reproductive health in women with PMOS (PCOS): a narrative review of mechanisms, clinical evidence, and individualized intervention strategies.

Polyendocrine Metabolic Ovarian Syndrome (PMOS) is a common endocrine-metabolic disorder in reproductive-aged women, characterized by clinical heterogeneity and an increased susceptibility to insulin resistance, type 2 diabetes mellitus, dyslipidemia, non-alcoholic fatty liver disease, and cardiometabolic risk. Although PMOS is often recognized through reproductive manifestations such as menstrual irregularity, ovulatory dysfunction, hyperandrogenism, and infertility, metabolic dysfunction is closely linked to reproductive outcomes through effects on androgen excess, ovulatory function, oocyte

RNA m⁶A modification and its regulatory factors are altered in granulosa cells from women with PMOS.

PURPOSE: Polyendocrine metabolic ovarian syndrome (PMOS), an endocrine disorder with unknown aetiology is the leading cause of anovulatory infertility. N6-methyladenosine (m6A), the most abundant internal modification in eukaryotic mRNA, regulates multiple aspects of RNA metabolism and reproductive function. However, the contributions of m6A modification and its regulatory factors including microRNAs to the pathophysiology of PMOS remain poorly understood. We therefore investigated global m6A abundance, expression of key m6A regulatory genes, and selected experimentally validated m6A-associate

Increased levels of long-chain acylcarnitines and polyunsaturated fatty acids in girls with premature adrenarche: An exploratory longitudinal metabolomics study.

Premature adrenarche (PA) has been associated with accelerated growth, increased adiposity, and reduced insulin sensitivity traceable to early life, yet the mechanisms underlying these metabolic traits remain unclear. This study investigated whether PA is associated with distinct circulating metabolite signatures. Untargeted metabolomics was used to analyze fasting serum samples from a community-based cohort. Sixteen girls with PA at age 7 were compared with 27 age-matched controls before adrenarche at the same age ("age-matched" comparison) and again when controls developed adrenarche at age

Incidental discovery of neglected 11β-hydroxylase deficiency causing 46,XX disorder of sex development in a 35-year-old adult: A radiologically driven diagnosis.

11β-hydroxylase deficiency (11β-OHD) is the second most common form of congenital adrenal hyperplasia (CAH), characterized by androgen excess, mineralocorticoid-driven hypertension, and hypokalemia. We report a 35-year-old individual with a 46,XX karyotype, registered and raised male since birth, in whom a disorder of sexual development (DSD) had remained unrecognized. Abdominal MRI performed for nonspecific abdominal pain revealed bilateral giant adrenal myelolipomas on macronodular adrenal hyperplasia and a retro-vesical uterus-like structure. On examination, external genitalia showed comple

Molecular mechanisms of Polyendocrine Metabolic Ovarian Syndrome and identification of novel druggable targets for drug discovery and repositioning.

Polyendocrine Metabolic Ovarian Syndrome (PMOS) is a heterogeneous disorder that mainly affects women of reproductive age. Genetic predisposition, insulin resistance, androgen excess, inflammation, mitochondrial dysfunction, and changes in gut microbial composition were notable contributors to the prognosis of PMOS. Several genetic variations, including mutations, polymorphisms, and variations in gene regulation, have been identified that predispose individuals to PMOS. A complex network involving metabolic, hormonal, and inflammatory pathways plays a major role in disease progression. To iden

A Cross-Sectional Exploratory Study of Endothelial Biomarkers in Non-Obese Women with and Without Polyendocrine Metabolic Ovarian Syndrome.

Endothelial dysfunction has been reported in polyendocrine metabolic ovarian syndrome (PMOS) and is associated with obesity. We hypothesised that circulating biomarkers of endothelial dysfunction would not differ between non-obese women with PMOS and age- and BMI-matched controls in the absence of major between-group differences in insulin resistance (IR), systemic inflammation or vitamin D status. Plasma concentrations of 20 endothelial-associated proteins were measured using Slow Off-rate Modified Aptamer (SOMAscan) technology in 29 non-obese women with PMOS and 29 age- and BMI-matched contr

A Biological Disease-Staging Model of Polyendocrine Metabolic Ovarian Syndrome: From Plasticity-Dominant States to Pathological Remodeling.

Polyendocrine metabolic ovarian syndrome (PMOS) is a heterogeneous endocrine-metabolic disorder characterized by reproductive dysfunction, metabolic abnormalities, and hormonal dysregulation. Although the Rotterdam criteria are widely used for phenotypic classification, accumulating evidence suggests that disease heterogeneity reflects varying contributions of insulin resistance, hyperandrogenism, chronic inflammation, mitochondrial dysfunction, autophagy dysregulation, and cellular senescence. To review the role of integrated intracellular signaling networks in PMOS and propose a conceptual m

Genetic risk for PCOS/PMOS is associated with hyperandrogenism but not irregular menses in adolescents and young adults.

CONTEXT: Diagnosing polycystic ovary syndrome, recently renamed polyendocrine metabolic ovarian syndrome (PMOS), during adolescence is challenging because menstrual irregularity and clinical signs suggesting hyperandrogenism are common. Recent international recommendations highlighted an "at risk for PMOS" category, defined by either menstrual regularity or hyperandrogenism, to guide surveillance; however, its prevalence and genetic correlates remain unknown. OBJECTIVE: To estimate the prevalence of PMOS and "at risk for PMOS" in adolescence and young adulthood and to assess associations with

Ketogenic diet combined with probiotics for polyendocrine metabolic ovarian syndrome: research progress from the perspective of the gut microbiota-host metabolic axis.

Polyendocrine metabolic ovarian syndrome (PMOS), formerly known as polycystic ovary syndrome (PCOS), is a common endocrine-metabolic disorder characterized by insulin resistance, androgen excess, chronic inflammation, and gut microbial dysbiosis. Increasing evidence suggests that the gut microbiota-metabolite-host axis contributes to the interaction between metabolic dysfunction, immune imbalance, and reproductive endocrine abnormalities. The ketogenic diet (KD) may improve PMOS by restricting carbohydrate intake, promoting ketone production, reducing body weight, enhancing insulin sensitivity

Critical Function of SIK3 in Androgen-Producing Ovarian Theca Cells for Female Reproductive Competence.

Infertility affects over 10% of women of reproductive age, with ovulatory dysfunction being the leading cause. Despite its widespread prevalence, the mechanisms behind infertility and anovulation are not fully understood. Recently, we reported infertility, ovulation failure, and severe ovarian abnormalities in mice lacking salt-inducible kinase 3 (SIK3). However, this phenotype was not fully replicated in mice in which SIK3 was deleted in granulosa cells. Since SIK3 is also expressed in theca cells, we generated SIK3 knockout mice specifically targeting SIK3 loss in androgen-producing theca ce

Serum 11-ketotestosterone is associated with overall disease-control status but adds limited information beyond androstenedione in children with 21-hydroxylase deficiency.

INTRODUCTION: Monitoring children with 21-hydroxylase deficiency (21-OHD) requires suppression of adrenal androgen excess while avoiding glucocorticoid overtreatment. However, the clinical value of serum 11-oxygenated androgens beyond established biomarkers remains uncertain. We evaluated whether serum 11-ketotestosterone (11KT) is associated with overall disease-control status and whether it provides incremental information beyond androstenedione (A4). METHODS: We conducted a retrospective, single-center study of 67 children with classic 21-OHD and 67 healthy controls. Steroids were measured

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Malignant transformation of a testosterone-secreting ovarian steroid cell tumor: a case report.

BACKGROUND: Ovarian Steroid Cell Tumors (SCT) are rare sex cord-stromal tumors accounting for less than 0.1% of ovarian neoplasms. Most are hormonally active and present with symptoms of androgen excess. Steroid cell tumor, not otherwise specified (SCT-NOS), represents the majority of cases and carries the greatest malignant potential, although most tumors are benign and cured with surgical excision. Malignant transformation of an initially benign SCT-NOS is rarely reported, and the molecular mechanisms underlying progression remain poorly characterized. CASE: A 41-year-old woman presented wi

Case Report: Androgen-secreting pediatric adrenocortical tumor of uncertain malignant potential presenting with virilization in a 19-month-old girl.

BACKGROUND: Virilization in girls younger than 2 years should prompt careful evaluation for pathological androgen excess and should be interpreted as a practical clinical reminder rather than as a stand-alone diagnostic tool. Pediatric adrenocortical tumors (ACTs) are rare but are usually hormonally active, and their interpretation requires pediatric-specific clinicopathological criteria rather than direct application of adult adrenal carcinoma criteria. CASE PRESENTATION: A 19-month-old girl presented with a 6-month history of progressive voice deepening, pubic hair growth, and clitoromegaly
