[Adult female acne : when and why to consider a hormonal assessment?].

Adult female acne (AFA) is a common reason for medical consultation and due to the hormonal imbalances, particularly hyperandrogenism. As the primary cause of AFA, hyperandrogenism may be associated with underlying conditions such as polycystic ovary syndrome. However, isolated acne is not a reliable indicator of androgen excess. This article discusses the clinical manifestations that should raise suspicion of such hormonal abnormalities and warrant specific investigations, allowing for the implementation of tailored treatments more specifics to AFA.

The Molecular Basis of Polycystic Ovary Syndrome and Its Cardiometabolic Correlates: Exploring the Intersection and Its Clinical Implications-A Narrative Review.

Recent studies have highlighted the association between polycystic ovary syndrome (PCOS) and cardiometabolic diseases, leading to an improved understanding of the underlying mechanistic factors. PCOS significantly increases cardiovascular risk by predisposing individuals to various subclinical and clinical conditions, including atherosclerosis and type 2 diabetes mellitus. Additionally, it interacts synergistically with other traditional cardiovascular risk factors, such as obesity, hyperlipidemia, and insulin resistance. Several molecular mechanisms involving genetics, epigenetics, adipokine

Production of 11-ketotestosterone in childhood adrenal tumors with virilization or peripheral precocious puberty: Dominant expression of 11β-hydroxysteroid dehydrogenase type 2.

11-Oxygenated androgens are important components of the androgen pool in humans. Among them, 11-ketotestosterone (11-KT), a potent 11-oxygenated androgen primarily produced in peripheral tissues outside the adrenal glands, has garnered research interest for its crucial role in several diseases associated with androgen excess. This study aimed to investigate the biosynthesis of 11-oxygenated androgens, particularly 11-KT, in childhood adrenocortical tumors (ACTs) presenting with symptoms of androgen excess. This retrospective study included three patients, aged 6 months, 2 years, and 12 years,

Ovarian-Specific Cyp17A1 Overexpression in Female Mice: A Novel Model of Endogenous Testosterone Excess.

Excessive androgen levels can severely affect female health. However, most existing models of androgen excess rely on exogenous androgen administration, which does not fully capture the effect of elevated local ovarian testosterone on reproductive and metabolic functions. Here, we report the development of a novel hyperandrogenic mouse model, Cyp17TM-625, generated by combining CRISPR-Cas9 and a Tet-On doxycycline system to induce Cyp17A1 overexpression in ovarian theca-interstitial cells. As a result, Cyp17TM-625 mice exhibited significantly elevated Cyp17A1 messenger RNA and protein levels,

Anti-Müllerian hormone in PCOS: Molecular regulation and emerging therapeutic strategies.

Anti-Müllerian hormone (AMH), a glycoprotein belonging to the transforming growth factor-beta (TGF-β) superfamily, is a key regulator of ovarian folliculogenesis. Dysregulated AMH expression is a hallmark of polycystic ovary syndrome (PCOS), a common endocrine and metabolic disorder characterized by hyperandrogenism, anovulation, and polycystic ovarian morphology. Elevated AMH levels in PCOS impair follicle-stimulating hormone (FSH) sensitivity, disrupt follicular maturation, and contribute to androgen excess-creating a feedback loop that exacerbates ovarian dysfunction. This review explores t

Adrenocortical carcinoma with dual androgen and cortisol secretion.

Adrenocortical carcinoma (ACC) is a rare endocrine cancer that originates in the adrenal cortex, known for its capacity to produce hormones such as cortisol, aldosterone, estrogens, or androgens. These hormonal imbalances lead to a diverse array of clinical manifestations. This case report describes a middle-aged male patient presenting with a dual-hormone secreting ACC, characterized by the secretion of both androgens and cortisol. This hormonal profile resulted in Cushing syndrome along with symptoms of androgen excess, including bilateral lower limb edema, prolonged fatigue, and altered men

Adrenocortical Carcinoma With 2 Distinct Syndromes From Secretion of Insulin-Like Growth Factor 2 and Steroid Hormones.

Non-islet cell tumor hypoglycemia as a result of insulin-like growth factor (IGF)-2 secretion is rare. A 59-year-old woman was referred with postmenopausal bleeding due to endometrial hyperplasia. Serum testosterone, estradiol, and adrenal androgens were elevated with suppressed gonadotropin concentrations. Cross-sectional imaging demonstrated a large left adrenal mass. The patient subsequently presented acutely with hypoglycemia. During a supervised fast, symptomatic hypoglycemia occurred within 5 hours. Serum samples drawn prior to hypoglycemia correction revealed an elevated IGF-2:IGF-1 rat

PCOS and Obesity: Contraception Challenges.

Polycystic ovary syndrome (PCOS) is one of the most prevalent endocrine disorders affecting women of reproductive age, with an estimated prevalence of 5-10%. Women with PCOS are at increased risk for metabolic disturbances. A significant proportion of women with PCOS, ranging from 40 to 85%, are either overweight or obese. Oral contraception is the standard first line treatment for PCOS. However, certain conditions associated with PCOS, such as obesity, must be considered when deciding to prescribe combined oral contraception. It seems that there is no clinical advantage in using high-dose eth

Society for Endocrinology Clinical Practice Guideline for the Evaluation of Androgen Excess in Women.

CONTEXT: Androgen excess is common in women and refers to clinical or biochemical evidence of elevated androgenic steroids such as testosterone. It is associated with underlying polycystic ovary syndrome in the majority of cases. However severe androgen excess is less common and may indicate the presence of underlying adrenal or ovarian neoplasms, genetic disorders or severe insulin resistance syndromes. Currently there are few consensus guidelines to assist clinicians with a standardised management approach to the patient with severe androgen excess. DESIGN: Clinical practice guideline. MET

The Impact of Metformin on Plasma Prolactin Levels in Antipsychotics-Treated Men With Hyperprolactinemia and Early-Onset Androgenic Alopecia.

OBJECTIVE: Metformin decreases elevated levels of pituitary hormones. Little is known about the association between the reproductive axis and hormonal effects of this drug. The purpose of our study was to investigate whether activity of the hypothalamic-pituitary-gonadal axis determines metformin action on prolactin levels in men with prolactin excess. DESIGN/SUBJECTS/MEASUREMENTS: This prospective cohort study included two groups of men with drug-induced hyperprolactinemia and type 2 diabetes or prediabetes: men with normal hair growth (group A, n = 23) and men with late-onset androgenic alo

Unilateral pubic hair growth: a paracrine finding indicative of underlying leydig cell tumor in a prepubertal boy.

Leydig cell tumors (LCTs) are rare in children and typically present with testicular masses and signs of early puberty. We report a case of a 6-year-old boy presenting solely with precocious pubic hair growth. Ultrasound revealed a small right testicular mass and elevated serum testosterone. Intra-operatively, hair growth was more prominent on the affected hemi-scrotum. Testis-sparing surgery confirmed a benign LCT with adjacent increased spermatogenesis. This is the first known report of ipsilateral pubic hair as a presenting sign of LCT. Localized androgen excess may be an early clinical ind

Genetics of primary congenital hypothyroidism: three decades of discoveries and persisting etiological challenges.

Androgen excess is thought to play a crucial role in the onset and progression of polycystic ovary syndrome (PCOS), although the underlying mechanism remains unclear. Using our mild dihydrotestosterone (DHT)-exposed rat model, which more closely reproduces human PCOS phenotypes than conventional models, we examined whether the presence of ovaries is essential in the pathophysiology of PCOS induced by androgen excess. At 26 days of age, female rats were divided into two primary groups: bilaterally ovariectomized (OVX) and sham-operated (intact). Each group was further divided into PCOS (implant

Endothelial dysfunction in congenital adrenal hyperplasia due to 21-hydroxylase deficiency: current knowledge and novel biomarkers.

Congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency (21OHD) is a complex endocrine disorder characterized by impaired cortisol synthesis and androgen excess. Beyond its hormonal and metabolic implications, CAH has been increasingly associated with an elevated risk of cardiovascular complications, including endothelial dysfunction, a critical precursor to atherosclerosis and a risk factor for cardiovascular and metabolic diseases. This review explores the current knowledge on endothelial function in patients with CAH, focusing on the interplay between chronic hormonal imbalanc
