Thyroid status was estimated serially by measuring triiodothyronine (T3), thyroxine (T4) and thyrotrophin (hTSH) in 20 patients suffering from meningococcal meningitis, typhoid fever and other acute febrile illnesses. Significantly low T3 and only slightly increased T4 were observed in all the patients. hTSH was normal in all of these. A significant reciprocal relationship was found between the degree of fever and fall in T3 concentrations. T3 tended to rise in patients who recovered but in those who deteriorated or died, T3 remained persistently low.

Cascade enzyme inhibitors (C1-esterase inhibitor, C3b inactivator, antithrombin III) and other major proteolytic enzyme inhibitors (alpha 1 trypsin inhibitor, alpha 1 chymotrypsin inhibitor, inter-alpha-trypsin inhibitor, alpha 2 macroglobulin) as well as C3 and alpha 1 acid glycoprotein, have been examined in the sera of Nigerian patients suffering from meningococcal infection of varied severity. Patients with meningococcaemia had lower serum concentrations of important inhibitors than did patients with localised meningitic infection. Within the coccaemic group, those who died had the lowest

Immunisation may induce myocardial complications. In this pilot study clinical, electrocardiographic, chemical and immunological findings have been studied during a six weeks' follow-up after routine immunisation (mumps, polio, tetanus, smallpox, diphtheria and type A meningococcal disease) among 234 Finnish conscripts at the beginning of their military service. Serial pattern of ECG changes suggestive of myocarditis was recorded in eight of the 234 conscripts one to two weeks after vaccination against smallpox and diphtheria. Changes were mainly minor ST segment elevations and T wave inversio

In 9 cases (6 men, 3 women) during 1975 we have isolated N. meningitidis from routine samples from urogenital/rectal locations to be examined for gonococci by culture. One of the men harboured meningococci in the rectum, the other 5 in the urethra. Two of the women had meningococci in the cervix and one in the urethra. Of the meningococci isolated, one belonged to serogroup A, 6 to group B and 2 to group C. Six of the patients, 4 men and 2 women, who attended the Department of Venereal Diseases, are described in greater detail. Of these, only one had symptoms. The meningococci disappeared spon

A surveillance program to determine group C meningococcal acquisition in military personnel during an epidemic was made. Data from fortnightly nasopharyngeal cultures show that individuals who were admitted with meningococcal disease acquired the organism within a week of disease onset. There were 24 company/platoons in which a single case occurred. Six company/platoons had 2 cases each. While other conditions which could act as predisposing factors were not ruled out, the data here favor acute respiratory disease as a predisposing factor(s) that may cause the increased incidence of meningococ

103 cerebrospinal fluid (CSF) samples from 55 patients with bacteriologically proven meningitis (caused mainly by Neisseria meningitidis group A and Haemophilus influenzae type b) and from 29 patients with unproved meningitis or other diseases were studied using the latex agglutination (LA) test to demonstrate bacterial antigen in CSF. The tests for N. meningitidis groups A and C and H. ineluenzae type b were found to be rapid, reliable and specific for the serological group of the organism. The demonstration of N. meningitidis group B antigen has not succeeded with the test. Negative results

The encouraging results of an earlier controlled field trial of the serogroup A meningococcal polysaccharide vaccine in the prevention of clinical disease prompted this study, the aim of which was to evaluate further the effectiveness of another lot of this type of vaccine, the duration of immunity, and the effectiveness against meningococcal carriage. A controlled field trial was carried out in early 1973 on 176 646 schoolchildren 6-15 years of age, of whom half received the serogroup A polysaccharide vaccine and the other half tetanus toxoid as a control. The incidence of cerebrospinal menin

Four patientw with acute polyarthritis due to meningcoccemia are described. One had skin manifestations suggesting vasculitis, another had transient nodular lesions, but there were none of these features in the other two. Synovial fluid cultures were negative and cells counts did not suggest septic arthritis. The diagnosis was not suspected until positive blood cultures were reported in three cases and meningitis developed in the fourth. In spite of diagnostic delay due to slow growth in cultures (six to 11 days) there was prompt response to antibiotic treatment. Meningococcemia should be susp

Within a period of only three months meningococci, Sero B, C and also Y, were detected among the patients of a hospital. The meningococci were isolated as sole pathogen, or in association with other bronchopathogens in five cases of purulent respiratory tract infection, most of whom were suffering from acute or chronic bronchitis. The pathogens were initially identified falsely as "facultative" Neisseria subflava. It is stressed that meningococci must be considered as possible pathogens in such infections if large numbers of the socalled facultative Neisseria have been detected with correspond

Neisseria meningitidis was recovered from the urethra of nine and from the anal canal of seven men, and from the cervix of two women. Twelve of the 16 men were admitted homosexuals. Seven men had no symptoms, while the remainder had mild to moderate symptoms. One woman was asymptomatic and the other was in hospital with acute salpingitis. Reports of previous isolations of meningococci from the urethra, cervix, or anal canal are reviewed. It is concluded that in men, these infections are usually mild and self-limited, but in women, meningococcal genital infections frequently proceed to severe d

Crebrospinal fluid (CSF) from 142 patients was tested for the presence of Haemophilus influenzae, pneumococcal and meningococcal antigens by counter-immuno-electrophoresis with commercial antisera. Group- or type-specific antigen was detected in the CSF of 67% of 64 patients with meningitis proved by culture to be due to these organisms, and in 10 of 25 patients with purulent meningitis but negative cultures. No false positive results were obtained in 24 normal CSF specimens, or in the CSF of 29 patients with meningitis caused by other organisms. The diagnostic usefulness of this specific, rel

An analysis is presented of the duration of hospitalization of the patients with acute bacterial (purulent) meningitis which occurred at Boston City Hospital during 12 selected years between 1935 and 1972. Considerable differences in the length of stay in the hospital were noted depending on the bacterial etiology, whether the infection was present on admission or acquired within the hospital, and, of course, whether the patient survived or died. Most of the deaths occurred within the first few days after admission or after the diagnosis was first established, but even among fatal cases, the s

The therapy of the bacterial purulent meningitis supports itself - at least in the causative organisms appearing most frequently - meningococci, pneumococci - on the application of a sufficiently high and sufficiently long penicillin therapy. In other causative organisms - pseudomonas, E. coli - the therapy demands the aimed use of other antibiotics (gentamycin, carbenicillin, polymyxin). The pneumococcal meningitis with pronounced reference to age shows the highest letality. The urgent admission to hospital must be demanded also when there is the slightest suspicion on the existence of a meni

In 104 patients with bacterial meningitis admitted to departments of general medicine, the diagnosis before admission and the effect of previous antibiotic treatment were studied. Antibiotic therapy begun before admission in 30 patients did not affect the ability to make a bacteriological diagnosis. The duration of illness before admission was longer in the treated than in the untreated group. About 50% of the patients were admitted with diagnoses other than meningitis, although half these patients had distinct meningeal signs on arrival at hospital. In the whole series, 76% had meningeal sign

Two cases of post-meningococcal inflammatory arthritis with a relapsing course in one case are reported. The authors then recall the characteristics of the joint manifestations during meningococcal infections in the light of other cases in the literature. These arthropathies are generally aseptic and their resistance to antibiotics is remarkable, whilst non- steroid anti-inflammatory drugs, even used alone as in one of the cases reported here, are remarkably effective. The pathogenesis of these cases of arthritis is not clear: it seems however according to Greenwood and Whittle that they are m

The cerebrospinal fluid of 120 subjects, 80 of whom were suffering from a purulent meningitis, were examined comparatively by classical bacteriological techniques (direct examination and culture) and by electro-immunodiffusion. This procedure makes it possible to detect not only viable bacteria, but also capsular polysaccharide antigens. It revealed the diagnosis in 96.9% of cases of meningococcal, pneumococcal and Haemophilus meningitis, as against 80.3% positive results using classical bacteriological techniques, for the same organisms. Electro-immunodiffusion provides a rapid answer (20 min

Eighty four cases of meningococcal infections are reviewed. Fifty seven cases presented themselfs as meningococcal meningitis, twelve cases as sepsis with moderate hypotension and 15 cases were sepsis with septic shock. A brief course of the disease, shock, echymosis, absence of meningeal signs, leucopenia and intravascular coagulation were findings more frequent in the group of patients with hiperacute sepsis, whereas other signs as fever, headaches, vomiting and petechiae were present with equal frequency in the three groups. N. meningitis was isolated in 73% of the cases. Shock (18.85%) and

A report is given on 3 cases of arthritis after cerebrospinal meningitis caused by B meningococci, with a relapsing course in 1 case. The observations are compared with the literature and the characteristics of articular manifestations of meningococcal infections are outlined. Three types are distinguished. Type I is characterized by banal arthralgias masked by the symptoms of meningitis. Type II encompasses septic arthritides preceding or concurring with the meningitis and like the latter sensitive to antibiotics. Type III includes postmeningococcal arthritides with sterile articular fluid re
