Unraveling the Complexity of Polycystic Ovary Syndrome: Biomarkers for Diagnosis, Prognosis, and Treatment.

Polycystic ovary syndrome is distinguished by alterations in ovarian morphology, ovulatory failure, and increased androgen levels. The National Institutes of Health (NIH) defines it as ovulatory dysfunction accompanied by hyperandrogenism. Women with PCOS may have obesity, type 2 diabetes, anxiety, hypertension, insulin resistance, and pregnancy-related complications. PCOS is additionally linked with a greater chance of cardiovascular and metabolic disorders. Several factors, including LH/FSH ratio, FAI levels, and ovarian USG, should be considered when diagnosing PCOS. The Rotterdam criterion

Impact of female sexual dysfunction on assisted reproductive technology outcome.

BACKGROUND: Despite its high prevalence in infertile women and known impact on natural conception, the specific influence of female sexual dysfunction (FSD) on assisted reproductive technology (ART) outcomes, which bypass intercourse, remains unclear. AIM: To investigate the association between female sexual function and ART outcomes, including live birth. METHODS: In this prospective cohort study, 300 women undergoing their first ART cycle completed the Female Sexual Function Index (FSFI) prior to treatment. Multivariable regression models were used to assess the association between FSFI me

BRCA1 alleviates inflammation, oxidative stress, and ovarian granulosa cell apoptosis by inhibiting endoplasmic reticulum stress, thereby ameliorating polycystic ovary syndrome.

Polycystic ovary syndrome (PCOS) is a prevalent endocrine disorder characterized by metabolic imbalance, oxidative stress, and granulosa cell (GC) dysfunction. Given the established role of BRCA1 in maintaining genomic stability and regulating stress responses, its potential involvement in PCOS pathogenesis warrants investigation. We examined the impact of BRCA1 on PCOS using a dehydroepiandrosterone (DHEA)-induced mouse model and testosterone-stimulated KGN cells. Lentiviral vectors overexpressing BRCA1 were administered by ovarian injection in PCOS mice or used to infect KGN cells. The IL-1β

Gut microbiota modulation by novel synbiotic improves production performance and ovarian function in aged laying hens via gut-ovary axis.

The management of oxidative stress and ovarian dysfunction associated with aged laying hens, which is highly involved in gut microbiota, has been suggested as a feasible approach to improve production performance. Here, we investigated the effects of a novel synbiotic (Bacillus amyloliquefaciens + inulin, BAI), a gut microbiota regulator, on the improvement of production performance in aged laying hens, and dissected the underlying mechanisms using multi-omics analysis. Our findings showed that, compared to the control, high-dosage BAI supplementation significantly improved production performa

AMH in PCOS and Beyond-Rare Case Series.

Background and Clinical Significance: Anti-Müllerian hormone (AMH) is a dimeric glycoprotein secreted from the granulosa cells of the preantral and small antral follicles, which has entered routine clinical practice as a valuable tool for the diagnosis of different ovarian disorders. Increased AMH levels have been recommended as a criterion for polycystic ovary syndrome (PCOS). However, its widespread use remains limited due to analytical diversity and contradictory age-specific thresholds, among other factors that modulate AMH levels. Case Presentation: Herein, we present a rare case series o

Luteolin alleviates PCOS by inhibiting AR/STAT3/NLRP3-mediated granulosa cell pyroptosis.

BACKGROUND: Polycystic ovary syndrome (PCOS), a common endocrine-metabolic disorder, is driven by hyperandrogenism and chronic low-grade inflammation that impair follicular development. Granulosa cell pyroptosis is increasingly recognized as a key pathogenic mechanism in PCOS. Luteolin (LUT), a natural flavonoid found in many traditional medicinal plants, exhibits potent anti-inflammatory properties. However, its role in regulating granulosa cell pyroptosis within the context of PCOS has not been elucidated. METHODS: We established a dehydroepiandrosterone (DHEA)-induced PCOS rat model to eva

Neutrophils and NETosis in polycystic ovary syndrome: unraveling the immuno-metabolic thromboinflammatory axis.

Polycystic ovary syndrome (PCOS), traditionally defined by its endocrine and reproductive hallmarks-hyperandrogenism, oligo-anovulation, and polycystic ovarian morphology-is increasingly viewed as a complex disorder associated with chronic low-grade immuno-metabolic inflammation. Emerging evidence suggests that neutrophils, the most abundant innate immune cells, may play a contributory role in linking metabolic stress, inflammatory signaling, and vascular dysfunction in PCOS. In women with PCOS, hyperandrogenism, insulin resistance, and adipose-derived inflammatory cues have been associated wi

[Neuroendocrine regulation of the reproductive system: integration of research of neuropeptide and experimental pathophysiology of amenorrhea of different genesis (literature review)].

Neuroendocrine regulation of reproductive function represents a complex system based on the integration of signals between the central nervous system and peripheral organs. In recent years, particular attention has been given to the role of neuropeptides - such as kisspeptin, brain-derived neurotrophic factor (BDNF), and orexins - in the pathogenesis of disorders associated with menstrual irregularities. This review provides a detailed analysis of the molecular mechanisms underlying neuropeptide regulation in functional hypothalamic amenorrhea (FHA), primary ovarian insufficiency (POI), and po

Affective Sensitivity to Ovarian Steroid Hormone Flux Across the Menstrual Cycle: Manifestations and Biopsychosocial Risk Factors.

Hormone sensitivity is a heterogeneous phenomenon involving multiple dimensions of sensitivity to estrogen and progesterone fluctuations across the menstrual cycle. While the majority of menstruating individuals do not experience any significant impact from these hormone changes on mood or behavior, rates of hormone sensitivity in clinical populations, particularly affective disorders, are substantially elevated, suggesting potential shared psychosocial or physiological mechanisms or moderators of risk. In this review, we provide an overview of menstrually related mood disorders and dimensions

Mechanisms of Phthalate-Induced Accelerated Ovarian Aging in Experimental Models.

PURPOSE OF REVIEW: Accelerated ovarian aging is associated with early infertility as well as other adverse health outcomes. Little is known about the factors that accelerate ovarian aging, but several studies indicate that exposure to phthalates accelerates ovarian aging. This is a significant human health concern because humans are ubiquitously and unavoidably exposed to phthalates. Thus, it is imperative to study the mechanisms of phthalate-induced accelerated ovarian aging so that strategies can be developed to prevent phthalate-induced ovarian aging. This review focuses on the mechanisms b

Hodgkin Lymphoma-The Effect of Chemotherapy on Gonadal Function and Fertility Is Strongly Related to the Treatment Regimen, Age, and Sex: A Systematic Review and Meta-Analysis.

Background/Objectives: Hodgkin lymphoma (HL) primarily affects individuals of reproductive age, making gonadal dysfunction after chemotherapy a critical survivorship concern. While fertility preservation options including gamete and gonadal tissue cryopreservation are available before treatment, evidence-based counseling requires regimen-specific risk estimates accounting for patient age and sex. Therefore, a meta-analysis was performed to assess presumed infertility in HL patients, stratified by chemotherapy regimen, age, and sex. Methods: This systematic review and meta-analysis, conducted w

Access to Fertility Preservation Counselling for Young Women with Haematological Malignancies: Incidence-Adjusted Trends from the Italian PreFerIta Network (2015-2023).

Background: Preserving fertility in young women with cancer is crucial for comprehensive care. Based on GBD 2023 estimates, approximately 1000 women aged 15-39 are diagnosed with haematological malignancies annually in Italy. Guidelines recommend timely fertility preservation (FP) counselling for all at-risk patients, yet real-world access data remain limited. Methods: This multicentre, retrospective observational study analysed FP counselling for women aged 15-39 with haematological malignancies from 2015 to 2023. Counselling data were extracted from the Italian Assisted Reproductive Technolo

Hydrogen-generating nanomodulators for metabolic repair and functional recovery in ovarian tissue transplantation.

Ischemia-reperfusion (I/R) injury occurring during ovarian tissue cryopreservation and transplantation (OTC-T) induces mitochondrial dysfunction and oxidative stress, leading to pronounced follicular loss and compromised graft performance-key challenges that limit transplantation success. Here, we present a hydrogen-releasing nanomodulator composed of potassium borohydride nanoparticles embedded within a thermosensitive hydrogel (KBH₄@Gel), designed to achieve sustained in situ hydrogen (H₂) generation for metabolic repair and ovarian function restoration. Upon local administration, the thermo

Impact of adjuvant anti-PD-1 immunotherapy on ovarian function in female patients with resectable melanoma: A prospective observational study.

INTRODUCTION: Immune checkpoint inhibitors (ICIs) have transformed cancer treatment and are increasingly used in the adjuvant and neoadjuvant setting for malignancies such as melanoma, which often affect younger adults. With improving survival, concerns about long-term safety, including potential effects on fertility, are growing. METHODS: We conducted a national, multicenter, prospective observational study of female patients < 45 years treated with adjuvant anti-PD-1 therapy for resectable stage III-IV melanoma from 2018 to 2024. Ovarian function was assessed at baseline and at 3, 6, 12, an

Postoperative Management After Risk-Reducing Salpingo-Oophorectomy for Hereditary Breast and Ovarian Cancer Syndrome: A Narrative Review on Balancing Oncologic Benefit and Quality of Life.

Risk-reducing salpingo-oophorectomy (RRSO) is the most effective intervention for reducing ovarian cancer risk in women with hereditary breast and ovarian cancer syndrome (HBOC) associated with pathogenic BRCA1 and BRCA2 variants. RRSO has been shown to markedly reduce the risk of ovarian cancer and to confer a protective effect against breast cancer, thereby contributing to improved overall survival. However, when performed before natural menopause, RRSO induces abrupt menopause, which is associated with vasomotor symptoms, sexual dysfunction, accelerated bone loss, adverse cardiovascular cha

Ovarian function suppression decision-making and uptake in premenopausal women with breast cancer: a mixed methods analysis.

PURPOSE: Ovarian function suppression (OFS) reduces the risk of recurrence of hormone receptor-positive breast cancer but increases the likelihood of toxicity and nonpersistence with endocrine therapy. In addition, rates of OFS utilization are lower than expected. To increase understanding of these issues, we sought to identify patient factors associated with the use of OFS injections, as well as treatment decision-making and education needs. METHODS: In this convergent mixed methods designed study, patients receiving OFS, who started then discontinued OFS injections, and who never initiated
