Double heterozygous PROK2 p.(Ile55Ter)-PROKR2 p.(Arg85Leu) variants: case report of an oligogenic case of congenital hypogonadotropic hypogonadism with anosmia.

INTRODUCTION: Delayed puberty in males requires differentiation between self-limited delay and congenital hypogonadotropic hypogonadism (CHH), including Kallmann syndrome (KS). CASE PRESENTATION: We report an adolescent male presenting with delayed puberty and anosmia, consistent with KS. Hormonal evaluation confirmed CHH, and MRI demonstrated olfactory bulb agenesis. Targeted next-generation sequencing identified two heterozygous variants in functionally related genes: a pathogenic nonsense variant in PROK2 and a missense variant in PROKR2 classified as a variant of uncertain significance. When the Uterus is Not Visualized on Ultrasonography in Adolescents and Young Adults with Primary Amenorrhea: Diagnostic Pitfalls and Clinical Implications.

OBJECTIVE: Uterine nonvisualization on ultrasonography in adolescents and young adults (AYAs) presenting with primary amenorrhea is frequently interpreted as Müllerian agenesis. However, in hypoestrogenic conditions or during the prepubertal period, the uterus may be small or difficult to detect on imaging, potentially leading to diagnostic uncertainty and misclassification. This study aimed to evaluate the diagnostic outcomes and clinical implications of uterine nonvisualization in AYAs with primary amenorrhea. MATERIALS AND METHODS: We retrospectively reviewed AYAs aged 13-20 years evaluate

Primary functional hypothalamic amenorrhoea in the context of feeding and eating disorders or excessive exercise: Insights from non-athletic and elite athletic populations.

Primary amenorrhoea is defined as the absence of menarche by the expected age, with or without delayed pubertal development in adolescents. It may result from anatomical, genetic, endocrine, and functional causes. Among the latter, we propose the term primary functional hypothalamic amenorrhoea (P-FHA) to describe primary amenorrhoea characterised by hypothalamic-pituitary-ovarian axis suppression in the absence of organic disease. P-FHA is frequently associated with low energy availability (LEA), particularly in individuals with feeding and eating disorders (FEDs) or excessive exercise, often

Clinical and immunological features of a patient exhibiting delayed puberty, microcephaly, scoliosis and epilepsy caused by a novel mutation in IGSF10.

Members of the immunoglobulin superfamily (IgSF) are cell-surface proteins essential for a wide range of biological processes, including molecular transport, morphoregulation, and cell differentiation. IGSF10, a member of this family, has been identified as a disease-associated gene in delayed puberty and cleidocranial dysplasia. In this study, a Chinese patient with delayed puberty, microcephaly, scoliosis, and epilepsy was investigated. The genetic defect and immune alterations were assessed using whole-exome sequencing, Sanger sequencing, and single-cell RNA sequencing. After variant filter

Clinical and Molecular Findings in Two Siblings with a Novel Homozygous DCAF17 Frameshift Variant and Review of the Literature: A Case Report.

INTRODUCTION: Woodhouse-Sakati syndrome (WSS) is an ultra-rare autosomal recessive multisystem disorder caused by biallelic pathogenic variants in DCAF17. The condition is characterized by endocrine dysfunction, ectodermal abnormalities, and variable neurological involvement, with considerable intra- and inter-familial phenotypic heterogeneity. CASE PRESENTATION: We report two siblings born to consanguineous parents who presented with short stature, delayed puberty, partial alopecia, and hypogonadism. The male sibling had hypogonadotropic hypogonadism and intermittent hyperglycemia, while the

From Amenorrhea to Pregnancy: Comprehensive Management of GnRH Pulse Therapy Achieving Pregnancy in a Woman with Idiopathic Hypogonadotropic Hypogonadism.

BACKGROUND: Idiopathic hypogonadotropic hypogonadism (IHH) is a rare endocrine disorder characterized by deficient gonadotropin-releasing hormone (GnRH) secretion or action, resulting in delayed puberty and infertility. Its clinical distinction from functional hypothalamic amenorrhea can be challenging because of overlapping features. Pulsatile GnRH therapy offers a physiological approach to fertility induction in affected women; although pregnancies following this treatment have been reported previously, detailed longitudinal documentation from diagnosis through pregnancy and delivery in wome

Does Body Fat Mass Influence the Timing of Puberty in Boys with Constitutional Delay of Growth and Puberty?

OBJECTIVE: In girls, a strong relationship exists between body fat percentage (BFP) and pubertal development, with a critical fat threshold considered essential for the initiation of puberty. In contrast, research addressing the impact of body composition on the timing of puberty in boys remains limited, and the available findings are inconsistent. To evaluate the association between body composition and delayed pubertal development in boys with constitutional delay of growth and puberty (CDGP) compared with healthy peers. METHODS: Thirty-one boys with CDGP and 31 age-matched healthy males (T

Pubertal delay in patients with paternally derived 15q11.2-q13 duplication syndrome and MKRN3 overexpression.

CONTEXT: Duplications of the 15q11.2-q13 region are primarily associated with neurodevelopmental disorders, while their endocrine implications remain poorly defined. The imprinted gene MKRN3, located within this locus, inhibits pubertal onset, and loss-of-function variants cause central precocious puberty. However, the clinical impact of increased MKRN3 dosage remains unclear. This study investigates whether paternally derived 15q11.2-q13 duplications lead to MKRN3 overexpression and are associated with delayed pubertal maturation. MATERIALS AND METHODS: Four patients with confirmed paternal

Crohn's Disease: Diagnosis and Management.

Crohn's disease is a chronic inflammatory condition that affects the gastrointestinal tract and often causes extraintestinal manifestations. Diarrhea, abdominal pain, fatigue, weight loss, and fever are common symptoms. Children may present with changes in growth velocity or delayed puberty. Initial evaluation for suspected Crohn's disease includes endoscopy and cross-sectional imaging. Fecal calprotectin testing is useful for determining whether ileocolonoscopy and imaging are needed in uncertain cases. These tests are critical in monitoring treatment effectiveness because mucosal inflammatio

Deletion of glucocorticoid receptors in Lepr-expressing neurons attenuates the effects of negative energy balance on female reproduction.

BACKGROUND: Negative energy balance increases glucocorticoids (GC) and inhibits reproduction; however, the neurocircuitry mediating these effects remains poorly understood. We hypothesized that GC signaling in leptin receptor (Lepr)-expressing neurons mediates reproductive disruption caused by metabolic stress. METHODS: We generated a mouse model with selective deletion of glucocorticoid receptor (GR) in Lepr-expressing cells (LeprGRKO). We characterized pubertal development, reproductive phenotype, ovarian folliculogenesis, and hypothalamic gene expression in female LeprGRKO mice and in GRlo

Sixteen Years of Clinical Data Including Genetic Analysis to Explain Delayed Puberty in a Chinese Boy with 21-Hydroxylase Deficiency: A Case Report.

INTRODUCTION: In 21-hydroxylase deficiency (21-OHD), impaired 21-hydroxylase activity causes 17-hydroxyprogesterone (17-OHP) accumulation and androgen excess, typically manifesting as hyperandrogenism. The POR gene encodes cytochrome P450 oxidoreductase (POR), the essential electron donor for all microsomal cytochrome P450 enzymes. While POR deficiency impairs multiple steroidogenic enzymes, its phenotypic impact on 21-OHD patients remains poorly characterized. CASE PRESENTATION: We report a male 21-OHD patient with homozygous CYP21A2 variant (c.293-13C>G) who presented atypically with absent

Case Report of Initial Manifestation of Hypogonadotropic Hypogonadism Based on Unoccluded Growth Plates in a 26-Year-Old Vietnamese Patient.

Hypogonadotropic hypogonadism is a rare endocrine disorder characterized by deficient secretion of gonadotropin-releasing hormone (GnRH), leading to reduced levels of luteinizing hormone (LH) and follicle-stimulating hormone (FSH), ultimately resulting in impaired sexual maturation and pubertal development. While typically diagnosed during adolescence, rare cases present in adulthood as incidental findings. This case report describes an unusual presentation of hypogonadotropic hypogonadism discovered incidentally during treatment of a traumatic tibial fracture in a 26-year-old male patient, hi

Timing of puberty and psychiatric and somatic diseases in childhood: a population-based study.

OBJECTIVE: To determine if childhood diseases are associated with the timing of puberty. DESIGN: Our population-based study (6347 girls and 6833 boys born 1997-2002) modelled associations between age at peak height velocity (PHV) and ICD-10 diagnosis codes obtained before age 18. METHODS: Linear regression was used. The data were adjusted for gestational age at birth, birth weight standard deviation score, body mass index at age 6, maternal smoking status during pregnancy, and mother's working status. RESULTS: Earlier pubertal timing (lower age at PHV) was associated with a subsequent diagn

Late Recognition of Aromatase Deficiency Following 16 Years of Misdiagnosis as Congenital Adrenal Hyperplasia: A Case with Coexisting Gonadoblastoma and Dysgerminoma.

Aromatase deficiency (AD) is a rare autosomal recessive disorder caused by pathogenic variants in the CYP19A1 gene, resulting in impaired conversion of androgens to estrogens. Clinical manifestations in 46, XX individuals include ambiguous genitalia, delayed puberty, and reproductive dysfunction. Both sexes may develop metabolic abnormalities, delayed epiphyseal closure, eunuchoid body proportions, and low bone mass. Because of its rarity and overlap with congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency (21OHD), diagnosis may be delayed or missed. Gonadal tumors have not b
