Local cluster of gonorrhoea strains harbouring the Neisseria meningitidis porA gene: a re-emerging diagnostic escape mutant.

OBJECTIVES: To investigate Neisseria gonorrhoeae specimens that exhibit diagnostic escape when the gonococcal porA pseudogene is employed as the target of nucleic-acid amplification tests (NAATs). METHODS: Eight NAAT specimens, from five patients, were identified as potential N. gonorrhoeae diagnostic escape mutants over a 7-month period (2023-2024) in Scotland. All specimens tested positive for N. gonorrhoeae on the first-line NAAT, which uses the opa gene target. However, all tested porA negative with confirmatory testing using a NAAT which included both opa and porA targets. To investigate

Clinical and Microbiological Characteristics of Meningococcal Eye Infections: Retrospective National Surveillance in England, 2010-2022.

OBJECTIVES: Neisseria meningitidis is primarily associated with severe systemic infections but can infect the eye and periocular tissues. Most meningococcal eye infections have a mild prognosis, but there is a significant increase in the risk of invasive disease. UK public health guidelines recommend chemoprophylaxis for meningococcal eye infection cases and contacts. This study involved a clinical and microbiological analysis of meningococcal eye infections in England over a 13-year period. METHODS: The analysis included all English cases of eye infection with confirmed isolation of N. menin

Epidemiology and Clinical Outcomes of Meningococcal Infections in Japan: A Nationwide Inpatient Database Study From 2010 to 2023.

BACKGROUND: Meningococcal infection remains a life-threatening disease despite advances in early recognition and therapy. While the incidence of meningococcal infection is extremely low in Japan, nationwide data on its clinical outcomes have been lacking. METHODS: We conducted a retrospective cohort study using the Japanese Diagnosis Procedure Combination (DPC) inpatient database from July 2010 to March 2023. Patients with a main diagnosis of meningococcal infection (International Classification of Diseases, 10th Revision code A39) were included. The primary outcome was in-hospital mortality.

Genotypic and phenotypic comparison of Neisseria meningitidis carriage and invasive disease isolates contemporaneously collected in the Netherlands.

Neisseria meningitidis (Nm), a commensal that colonizes the nasopharynx of 4.5%-24% of healthy humans, can cause invasive meningococcal disease (IMD). We hypothesized that distinct genotypic and/or phenotypic signatures might be found in carriage vs. invasive isolates. Carriage isolates were cultured from nasopharyngeal swabs (n = 267) collected from healthy students (aged 13-21 years) during the 2013 and 2014 school years in the Netherlands. Invasive isolates (n = 214) were cultured from all reported disease cases in the Netherlands from 2012 to 2014. Whole core genome sequences were determin

Patient Preferences for Neuromyelitis Optica Spectrum Disorder (NMOSD) Treatments: Results from a Discrete Choice Experiment Study in the USA.

INTRODUCTION: Benefit-risk profiles and mode/frequency of administration vary among the four US-approved treatments for anti-aquaporin-4 antibody-positive neuromyelitis optica spectrum disorder (AQP4-Ab+ NMOSD), but no published studies have reported treatment preferences among patients with NMOSD in the USA. Therefore, the objective of this study was to quantify preferences and predict treatment choices among patients with AQP4-Ab+ NMOSD in the USA. METHODS: A cross-sectional, web-based discrete choice experiment survey was administered to patients with self-reported AQP4-Ab+ NMOSD. Responde

Sustained salivary IgG but short-lived IgA responses following primary and booster MenACWY-TT vaccination in older adults.

Carriage of Neisseria meningitidis is essential in meningococcal transmission and is a prerequisite for invasive meningococcal disease (IMD). Meningococcal conjugate vaccines have been suggested to reduce acquisition of carriage in adolescents and young adults, contributing to herd protection. Mucosal immunity is considered key in protection against colonization. While mucosal antibody responses following parenteral meningococcal conjugate vaccination have been described in infants, adolescents and young adults, data in older adults, who are at increased risk for infectious diseases including

The impact of introducing meningococcal C/ACWY booster vaccination among adolescents in Germany: a dynamic transmission modelling study.

BACKGROUND: In Germany, primary vaccination against invasive meningococcal disease (IMD) serogroup C aims to reduce the highest burden of IMD in infants aged 12–23 month. Due to another IMD-peak in adolescents, we modelled the potential impact of introducing adolescent boosters with conjugate meningococcal C or ACWY (MenC/MenACWY) vaccines. METHODS: We built an age- and serogroup-structured dynamic-transmission model for Germany, which we calibrated to national surveillance data in 2005–2019. We simulated five vaccination scenarios of either continuing with the current MenC primary vaccination

Gene exchange between Neisseria meningitidis and Neisseria gonorrhoeae.

Genetic exchange between Neisseria meningitidis (NM) and Neisseria gonorrhoeae (NG) has not been well studied. This study aimed to investigate evidence of genetic exchanges between these two species. All coincident paired NM and NG isolates cultured from pharyngeal swabs collected from a sexual health clinic in Sydney in 2021 underwent whole-genome sequencing. A gene-by-gene analysis of the 47 NM-NG pairs identified 184 instances where the ancestry of the gene revealed intermixing between the two species. Incorporating the gene phylogenies demonstrated that these events occurred across a wide

Assessing the potential impact of the 20-valent (PCV20) and an adult 21-valent (aPCV21) pneumococcal conjugate vaccine on invasive pneumococcal disease in England.

BACKGROUND: Several higher-valent pneumococcal conjugate vaccines (PCVs) have been licensed recently, including a 20-valent PCV (PCV20) for children and adults and 21-valent PCV (aPCV21) licensed for adults. We assessed the potential for aPCV21 to further reduce the burden of invasive pneumococcal disease (IPD) in England. METHODS: IPD cases are electronically reported to the UKHSA and pneumococcal isolates are routinely submitted to the UKHSA national reference laboratory for confirmation and serotyping. We used national enhanced surveillance data for all IPD cases confirmed in England (July

Parental knowledge, attitudes, and acceptability of meningococcal vaccination in Istanbul, Turkey: A cross-sectional survey.

Meningococcal infections range from asymptomatic carriage to fulminant sepsis with tissue necrosis. As the disease progresses rapidly, vaccination remains the most effective preventive strategy. Parental acceptance of vaccination is closely linked to awareness, knowledge, attitudes, and behaviors. This study explores these factors and offers targeted recommendations to enhance vaccine uptake. This cross-sectional, descriptive, and comparative study was conducted via a questionnaire administered to parents of pediatric patients seen or hospitalized at the University of Health Sciences Haydarpaş

[Molecular Detection of Neisseria meningitidis Serogroup C in an Adolescent with Atypical Presentation: A Silent Threat].

Neisseria meningitidis is an important pathogen that can cause invasive meningococcal diseases, especially with serogroups A, B, C, W and Y, which can be seen endemic or sporadic. Early diagnosis and treatment of invasive meningococcal disease is life-saving. Delay in diagnosis of meningitis infection, especially in the adolescent age group, which initially presents with atypical symptoms, can further increase the risk of morbidity and mortality. In this case report the clinical evaluation and diagnostic process of a 15-year-old female patient who admitted to the emergency department with abdo
