Neonatal invasive meningococcal disease: Clinical case from Saudi Arabia and updated review of reported cases.

Neonatal invasive meningococcal disease (IMD) is rare but associated with severe morbidity and mortality. We report an 11-day-old male neonate who presented with fever, apnea, and poor feeding and was diagnosed with meningitis due to Neisseria meningitidis. Diagnosis was confirmed by blood culture and cerebrospinal fluid (CSF) multiplex PCR, although CSF culture remained negative. The patient developed ventriculitis, requiring prolonged antimicrobial therapy. To contextualize this case, we reviewed 71 previously reported neonatal IMD cases published between 1916 and 2024. The present case is d

Emergence of ciprofloxacin-resistant Neisseria meningitidis ST-175 in Tunisia: Clinical case description and genomic characterization.

We describe a case of meningococcal bloodstream infection complicated by purpura fulminans in a 49-year-old woman in Tunisia in May 2025, caused by a ciprofloxacin-resistant Neisseria meningitidis. The patient presented with rapidly progressive purpura and septic shock requiring intensive care and empirical cefotaxime therapy. Blood cultures, molecular testing, and whole-genome sequencing (WGS) were investigated to characterize the N. meningitidis isolate. The strain was non-groupable and belonged to ST-175 (cc175), with BAST profile 3645 and a MenDeVAR Index indicating insufficient evidence f

HIV-Specific vs Universal Immunization Coverage in US Children with HIV: A Multi-Site Study.

BACKGROUND: Immunization recommendations for children living with HIV (CLWH) differ from the general pediatric population due to increased risk of vaccine-preventable diseases. Adherence to these specific recommendations remains understudied in the United States. We assessed HIV-specific versus universal immunization coverage across multiple pediatric HIV clinics. METHODS: We conducted a retrospective, cross-sectional study of CLWH (0-18 years) receiving HIV care as of May 1, 2021, at 6 US pediatric HIV clinics. We evaluated coverage for universal and HIV-specific vaccines, including those th

Ability of the MenB-fHbp vaccine to provide immune protection against meningococcal serogroup B ST-1161 UK university "South West Outbreak" strain isolates.

In 2021/22, the UK Health Security Agency reported an excess in cases of invasive meningococcal disease (IMD) caused by a meningococcal serogroup B (MenB) ST-1161 strain in South West (SW) England, primarily affecting university students. Here, we present a phylogenetic analysis of the "SW outbreak" strain, assess potential coverage provided by MenB vaccines using sequence-based tools and, for three representative outbreak isolates, evaluate the breadth of coverage provided by bivalent fHbp MenB vaccine (MenB-fHbp; Trumenba®)-containing vaccines and the susceptibility of isolates to antisera o

Invasive meningococcal B disease: Spatiotemporal cluster identification using finetype data, the Netherlands, 2005-2023.

This study assessed whether systematically using finetype data in national surveillance of invasive meningococcal disease serogroup B (IMD-B) in the Netherlands could improve cluster detection in order to prevent further cases through public health actions. We analysed 2005-2023 data, including 1,642 IMD-B cases with complete finetype and municipality information (95%; N = 1729). Using a generalized linear model, we calculated expected baselines for each finetype, including temporal trends. Using SaTScan™, we applied Poisson scan-statistics with a 365-day window to identify spatiotemporal clus

Neisseria meningitidis carriage, antimicrobial resistance, and risk factors in UK men who have sex with men.

Urogenital infections caused by Neisseria meningitidis (Nm) are increasing globally, yet the prevalence, antimicrobial resistance profiles, and transmission dynamics of Nm in men who have sex with men (MSM) remain poorly defined. We conducted an oropharyngeal carriage study in 174 MSM attending a London sexual health clinic in 2023, prior to the implementation of 4CMenB vaccination and doxycycline post-exposure prophylaxis (doxyPEP). Nm was detected in 21.26% of participants, with carriage significantly associated with throat gonorrhoea, consistent with frequent co-colonisation. Whole-genome s

[Analysis surveillance results of the Neisseria meningitidis carriage rate among healthy population in certain regions of Qinghai Province in 2023].

In 2023, a total of 853 throat swab specimens were collected from healthy individuals across four counties/districts in Qinghai Province to isolate Neisseria meningitidis (Nm) and analyze its carriage rate and serogroup distribution. Among the 853 specimens, 45 were positive for Nm, yielding an overall carriage rate of 5.28%. The serogroup distribution included 27 strains of serogroup B (60.00%), 11 strains of serogroup C (24.44%), 3 strains of serogroup Y (6.67%), 2 strains of serogroup W (4.44%), and 2 non-groupable strains (4.44%). The carriage rates varied geographically, ranging from 1.87

Immune responses to glycoconjugate vaccines rely on a balance between polysaccharide length and glycosylation density.

Glycoconjugate vaccines are effective against bacterial infections. Key characteristics for optimal immunogenicity are the carbohydrate-to-protein ratio and consequent glycosylation density. Directing the conjugation to specific sites is beneficial when the protein acts as carrier and antigen, however a caveat of this approach is the limited number of conjugated glycans attached which might reduce the anti-glycan immune response. We investigated if polysaccharide length is crucial for optimizing immunogenicity in single-site selective conjugates. We tested conjugates from meningococcal serogro

Efficacy of the serogroup B Neisseria meningitidis vaccine (4CMenB) in preventing experimental Neisseria gonorrhoeae urethral infection: a double-blind randomised controlled human challenge study protocol.

INTRODUCTION: Rates of sexually transmitted infections are on the rise globally, including those caused by Neisseria gonorrhoeae. Retrospective analyses across clinical settings around the world have indicated that rates of gonorrhoea declined following mass campaigns with vaccines made with outer membrane vesicles (OMV) from N. meningitidis serogroup B, a pathogen closely related to N. gonorrhoeae. The US Food and Drug Administration (FDA)-approved N. meningitidis serogroup B vaccine (4CMenB) contains N. meningitidis OMV and five recombinant protein components: factor H-binding protein (fHbp)

"Why should I pay for this vaccine?" - Norwegian adolescents views on meningococcal vaccination.

BACKGROUND: As one of few countries in Europe, Norway does not offer vaccination against invasive meningococcal disease (IMD) as part of the National Immunization Program for Children (NIP). Instead, adolescents aged 16-19 are recommended to consider having the meningococcal ACWY conjugate vaccine (MCV4) if they participate in activities that increase the risk of IMD. Meningococcal vaccination rates in Norway are low compared to vaccines offered through the NIP. Whereas all vaccinations in the NIP are free and organized through schools or municipal health clinics, there is no uniform system in

Do the protein-based meningococcal group B vaccines, 4CMenB and MenB-fHbp, elicit serum bactericidal antibody against meningococcal serogroup X clonal complex 181?

OBJECTIVES: Bexsero (4CMenB) and Trumenba (MenB-fHbp) were designed and licensed to protect against meningococcal serogroup B disease; however, the antigens in these vaccines are shared among other meningococcal serogroups. This study assessed the immunogenicity of the two licensed serogroup B vaccines against a hyperinvasive serogroup X strain. METHODS: Sera from adults immunized with either 4CMenB (before and after one, two, or three doses of vaccine) or MenB-fHbp (before and after two doses) were assayed in the serum bactericidal antibody (SBA) assay against serogroup X strain BF 2/97. Disease burden of Streptococcus pneumoniae and Neisseria meningitidis meningitis in children and adolescents: A population-based study.

Bacterial meningitis remains a major, life-threatening infection in children and adolescents, with Streptococcus pneumoniae meningitis (SPM) and Neisseria meningitidis meningitis (NMM) accounting for most cases. This study aimed to quantify the global burden and spatiotemporal patterns of SPM and NMM in individuals aged 0 to 19 years from 1990 to 2021, and to provide evidence for integrated, cross-pathogen prevention and improved clinical pathways. Using Global Burden of Disease 2021 estimates for 204 countries and territories, we extracted deaths and disability-adjusted life years (DALYs) for

Beyond Vaccination: Persistent Meningococcal Risk in Anti-C5-Treated aHUS-Case Report and Review of Literature.

Background/Objectives: Atypical hemolytic uremic syndrome (aHUS) is a rare, life-threatening thrombotic microangiopathy caused by dysregulation of the alternative complement pathway, often related to genetic mutations or autoantibodies. The introduction of complement C5 inhibitors, such as eculizumab and ravulizumab, has significantly improved renal and overall outcomes. However, complement inhibition impairs host defense against encapsulated bacteria, markedly increasing the risk of invasive infections, particularly Neisseria meningitidis. Vaccination against meningococcal groups ACWY and B,

Factor H-related proteins as a protective evolutionary mechanism against microbial infections.

Factor H (FH) is the main regulator of the complement alternative pathway, and it is essential to prevent complement-mediated autologous damage. Many pathogens express FH-binding proteins on their surface that recruit human FH as a strategy to avoid or limit their elimination by the host complement. The FH-Related Proteins (FHR-1, FHR-2, FHR-3, FHR-4A, and FHR-5) share sequential and structural homology with FH that result from their common ancestry, but none of them can regulate complement like FH. While the precise contribution of FHRs in human complement is still not completely understood,

Immunogenicity, reactogenicity, and safety of a reduced-interval 4CMenB primary immunisation schedule in UK infants: a multicentre, open-label, assessor-blinded, phase 4 randomised controlled trial.

BACKGROUND: Until July, 2025, infants in the UK received the four-component meningococcal B vaccine (4CMenB) at ages 8 weeks and 16 weeks, boosted at 1 year. 4CMenB introduction was associated with large reductions in serogroup B (MenB) invasive meningococcal disease in childhood (cases now peak at age 3 months vs age 5-6 months before routine immunisation). A shorter interval between 4CMenB priming doses could reduce cases further. This study aimed to compare the immunogenicity, reactogenicity, and safety of 4CMenB priming schedules.
