Genomic Typing of Meningococcal Carriage Isolates in an Urban Sexual Health Clinic.

Asymptomatic pharyngeal Neisseria meningitidis (Nm) carriage is seen in >30% of sexually transmitted infection (STI) clinic attendees. With increasing reports of Nm urethritis and STI-related invasive disease outbreaks, longitudinal assessment and genomic characterization of Nm among patients at an urban STI clinic population in Columbus, Ohio, was undertaken. This study determined the genomic basis of oropharyngeal, urogenital and rectal Nm isolates carried by patients presenting for care. Cultures using media selective for Neisseria spp., Nm-specific PCR screening of colonies with oxidase-po

HIV Infection: What Is New in Prevention and Treatment?

HIV remains a significant public health challenge despite highly effective prevention and treatment strategies. Screening for HIV infection is recommended for individuals ages 15 to 65 years. The status neutral approach to care includes preexposure prophylaxis for HIV-negative patients and antiretroviral therapy for patients with HIV. The preexposure prophylaxis options approved by the US Food and Drug Administration include oral and injectable formulations, which reduce HIV acquisition risk by 99% when taken consistently. Same-day antiretroviral therapy initiation is the standard of care for

Vaccine-preventable disease outbreaks in schools, part 2: pertussis, meningococcal disease, diphtheria, poliomyelitis, hepatitis A, and rotavirus infection.

Schools are essential elements during childhood and adolescence, not only for providing education, but also for promoting the personal development and socialization of students. School attendance almost always takes place in classrooms and in close proximity for several hours daily. Students also interact with students from other classrooms and schools through participation in sports and extracurricular activities. These characteristics render the schools ideal environments for the transmission of vaccine-preventable diseases. We reviewed 57 articles published from 2000 to June 2, 2025, presen

Self-reported vaccine uptake across six infectious diseases among gay, bisexual, transgender, and non-binary individuals in Australia: findings from a 2024 national survey.

BACKGROUND: Gay and bisexual men who have sex with men (GBMSM) and trans and gender diverse people (TGD) are disproportionately affected by several vaccine-preventable diseases. However, research on vaccine uptake for both sexually transmitted and non-sexually transmitted infections among GBMSM and TGD remains limited. Understanding differences in uptake between selective (e.g. hepatitis A; meningococcal; human papillomavirus; HPV; mpox) and universal (e.g. influenza, COVID-19) is important for informing future immunisation policy and strategies. METHODS: We surveyed past vaccine uptake among

Complement C5 inhibitors as add-on rescue treatment in severe AQP4-antibody neuromyelitis optica spectrum disorder attacks: a case series and literature review.

Acute attacks of aquaporin-4 antibody neuromyelitis optica spectrum disorder (AQP4-Ab NMOSD) are typically managed with intravenous methylprednisolone, with therapeutic plasma exchange (PLEX) reserved for severe episodes. Despite these interventions, a subset of patients fails to respond and may sustain significant neurological disability. Complement C5 inhibitors, currently approved for long-term relapse prevention, have recently gained attention as a potential rescue therapy by rapidly inhibiting the terminal complement cascade, thus preventing downstream astrocytic injury. We report three p

Purulent meningococcal pericarditis with cardiac tamponade in a Hajj pilgrim: A diagnostic challenge of anchoring bias on MERS-CoV.

We report a rare case of purulent pericarditis with cardiac tamponade due to Neisseria meningitidis serogroup W135 in a 15-year-old pilgrim returning from the Hajj. Initial suspicion of Middle East respiratory syndrome coronavirus (MERS-CoV) was prompted by pneumonia and camel milk exposure, misdirecting the diagnosis. The patient developed cardiac tamponade requiring urgent pericardiocentesis; 16S rRNA PCR confirmed N. meningitidis W135 despite negative cultures. He recovered fully with ceftriaxone and colchicine. This case highlights diagnostic anchoring bias in travel medicine and the need

Epidemiology and public health response to a meningitis outbreak in Ghana's Upper West Region, 2025: a descriptive cross-sectional study.

Bacterial meningitis remains a recurrent public health threat in the African meningitis belt despite vaccination efforts. Following the introduction of the Meningococcal Africa vaccine, shifts in causative pathogens have been observed. In early 2025, a meningitis outbreak occurred in Ghana´s Upper West Region, necessitating a detailed epidemiological and response assessment. We conducted a retrospective descriptive cross-sectional analysis of surveillance data from all 11 districts of the Upper West Region, Ghana. Records of suspected meningitis cases reported between epidemiological weeks 1 a

Immunoglobulin A vasculitis in association with invasive meningococcal disease.

Immunoglobulin A vasculitis is uncommon in adults and is frequently associated with an identifiable trigger, most often infection. Invasive meningococcal disease is a rare but life-threatening infection caused by Neisseria meningitidis. We report the case of an 18-year-old Indigenous Australian woman who presented with fever, purpuric rash, ankle synovitis, abdominal pain, and systemic inflammation. A skin biopsy demonstrated leukocytoclastic vasculitis with immunoglobulin A deposition on immunohistochemistry, and blood cultures subsequently identified N. meningitidis serogroup B. Cerebrospina

Meningococcal serogroup W ST-11 causing meningitis, endogenous endophthalmitis, and septic arthritis in a child.

BACKGROUND: Invasive meningococcal disease (IMD) remains a major global health concern because of its epidemic potential, unpredictable presentation, and rapid progression. Although IMD classically presents as meningitis, meningococcemia, or both, Neisseria meningitidis can also cause extra-meningeal manifestations. Serogroup W (MenW), particularly the hyperinvasive ST-11 lineage, has increasingly been associated with atypical and invasive clinical phenotypes. We report an 11-year-old previously healthy boy with IMD due to MenW ST-11 who developed the exceptional triad of meningitis, endogenou

Meningococcemia in a Boy with Dense Deposit Disease Receiving the C5 Complement Inhibitor Ravulizumab: A Case Report.

INTRODUCTION: Dense deposit disease, also known as C3 glomerulopathy, is a rare renal disorder caused by abnormal complement deposition in the glomerular basement membrane. Patients often require long-term immunosuppressive therapy and, in some cases, complement inhibitors such as ravulizumab. While effective at limiting renal damage, complement blockade significantly increases susceptibility to invasive infections from encapsulated bacteria, particularly Neisseria meningitidis. Despite immunization and antimicrobial prophylaxis, these patients remain incompletely protected. We describe a case

Genetic basis of meningococcal capsular switching between serogroups A and B belonging to ST-7.

OBJECTIVES: Capsular switching from serogroup A to serogroups C or X has been documented, whereas it is rarely observed from serogroup A to serogroup B. This study aims to report and characterize a rare capsular switching event from serogroup A to serogroup B belonging to ST-7. METHODS: Comparative genomic analyses were performed on ST-7 isolates across serogroups A, B, C, and X. Recombination analysis of the cps locus was conducted to determine the recombination boundaries. The fitness costs of capsular switching were assessed by comparing bacterial growth assay, adhesion and invasive capaci
