BACKGROUND: Sepsis remains a leading cause of morbidity and mortality in intensive care units (ICUs), with antimicrobial resistance (AMR) complicating therapeutic decisions. This study aimed to characterize pathogen distribution, AMR patterns, and prognostic indicators in ICU patients with sepsis at Jiaxing First Hospital.
METHODS: A retrospective analysis of 352 septic patients (2021-2024) was conducted. Clinical outcomes, pathogen profiles, and antimicrobial susceptibility were evaluated. Multivariable logistic regression and ROC curve analysis identified predictors of non-recovery.

Enterococcus faecalis is a gram-positive, facultative anaerobic bacterium that commonly colonizes the gastrointestinal and genitourinary tracts. Although frequently implicated in urinary tract infections and bacteremia, it is also a well-recognized cause of subacute infective endocarditis. We present a case of subacute infective endocarditis caused by Enterococcus faecalis, initially masked by presumed urinary sepsis in a patient who had undergone transurethral resection of the prostate (TURP) less than two months prior. The patient presented with four days of fever and generalized weakness, a

Gastrostomy tube (G-tube) replacement is often routine; however, in children with complex medical needs, misplacement can rapidly lead to life-threatening complications. We describe the case of a 16-year-old male patient with neuronal ceroid lipofuscinosis who developed fulminant septic shock after accidental G-tube dislodgement and malposition. At an outside hospital, reinsertion attempts required tract dilation; bloody aspirate and inconclusive radiographs were overlooked, and feeds were resumed at home. After 48 hours, the patient presented to a community hospital in profound shock (blood p

BACKGROUND: Bleeding from an ileal conduit after radical cystectomy is uncommon and is usually attributed to tumor recurrence, infection, stones, or local stomal trauma. Portal hypertensive stomal varices are rare. FOLFIRINOX adds further diagnostic complexity because oxaliplatin may cause sinusoidal endothelial injury and noncirrhotic portal hypertension, whereas irinotecan may damage gastrointestinal mucosa. We report a case of massive hemorrhage from a long-functioning Bricker ileal conduit during FOLFIRINOX therapy.

The failure of endodontic procedures such as root canal therapy is primarily indicated by persistent microbial infections. Root canal therapy involves the removal of decaying dental pulp (internal blood vessels and nerves of teeth), sanitizing the canal, and filling the space with biocompatible materials. Improper cleaning or the breakdown of these materials can lead to secondary infections. These infections, if left untreated, can lead to severe pain, bone and tooth loss, and potentially systemic infection. The bacterium Enterococcus faecalis is one of the most commonly associated organisms w

BACKGROUND: Bacterial infection at the maternal-fetal interface contributes to adverse pregnancy outcomes, including preterm birth, chorioamnionitis, and sepsis. Despite evolving obstetric practices that may influence microbial patterns, recommended clinical algorithms for prophylaxis and treatment rely on outdated data. Furthermore, the pathogenic organisms associated with adverse outcomes remain incompletely characterized.
METHODS: We conducted a retrospective study of placental cultures from a high-risk obstetric cohort from a single hospital system between 2014 and 2019 (n = 714). Placent

Spontaneous esophageal rupture (Boerhaave syndrome) is a rare and life-threatening condition associated with high morbidity and mortality, particularly when diagnosis and source control are delayed. We report the case of a 73-year-old man presenting with vomiting, chest pain, and dyspnea. Computed tomography revealed extensive pneumomediastinum, and endoscopy confirmed a distal esophageal perforation with mediastinopleural contamination. The patient developed septic shock requiring intensive care support, including mechanical ventilation and vasopressors. Endoscopic vacuum therapy (EVT) was in

BACKGROUND: Infection is a common and often fatal complication in cancer patients. Notably, cancer patients account for up to 21% of all sepsis cases. Furthermore, sepsis is present in 30% of all deaths among those hospitalized with cancer. However, comprehensive antimicrobial resistance (AMR) surveillance data specific to this population are lacking. This gap hinders the effectiveness of antimicrobial stewardship programs (ASPs).
METHODS: We investigated bacterial distribution and AMR patterns in Chinese cancer patients to inform empirical therapy and ASPs. This multicenter retrospective stu

INTRODUCTION: Enterococci are commensal bacteria of the digestive and genitourinary flora of humans and animals. In humans, enterococci can cause sepsis of urinary or abdominal origin. Compared to Enterococcus faecium and Enteroccocus faecalis, Enterococcus thailandicus is one of the least characterized members of the genus Enterococcus. E. thailandicus has been rarely associated with human pathology.
METHODS: We describe the first report of E. thailandicus in respiratory samples, and the third one in an intra-abdominal infected collection. Clinical isolates of E. thailandicus were cultured o

Enterococci are common commensals of the human gut and important opportunistic pathogens, with Enterococcus faecium and Enterococcus faecalis being the most clinically prevalent species. A significant epidemiological shift has emerged with an increasing clinical burden of E. faecium. To compare genomic evolution of E. faecium and E. faecalis, we performed whole-genome sequencing on 93 E. faecium and 32 E. faecalis isolates causing bloodstream infections at a single hospital (2022-2024). Analysis of patient demographics revealed that E. faecium infections originated from fewer sources than E. f

Purpose: Bloodstream infections (BSIs) require prompt, targeted antimicrobial therapy, but traditional blood culture methods may delay optimal treatment. The BIOFIRE® Blood Culture Identification 2 (BCID2) panel provides rapid pathogen identification and mayreduce time to targeted therapy. This study evaluated the impact of BCID2 implementation at a community teaching hospital. Methods: A retrospective, quasi-experimental pre/post study was conducted at a for-profit community teaching hospital in adults aged ≥18 years with BSIs. Eligible patients had bacteremia, blood cultures collected, and e

BACKGROUND: Batracylin is a heterocyclic arylamine topoisomerase inhibitor with preclinical anticancer activity. Marked species differences in sensitivity to the toxicity of batracylin were observed and attributed to differential formation of N-acetylbatracylin by N-acetyltransferase. A Phase I trial of batracylin in cancer patients with slow acetylator genotypes identified a dose-limiting toxicity of hemorrhagic cystitis. To further explore the metabolism of batracylin and N-acetylbatracylin across species, detailed studies using human, rat, and dog liver microsomal and hepatocyte preparation

Objective: To determine whether temporal interference (TI) ultrasound can enhance microbubble (MB) cavitation and provide transient modulation of blood-brain barrier (BBB) permeability. Impact Statement: By integrating a time-dependent multiphysics MB-vessel model with in vivo validation, this study links TI-enhanced cavitation dynamics to vessel-wall mechanical responses and a transient BBB permeability window, providing a mechanistic basis for pressure-efficient ultrasound-mediated brain delivery. Introduction: Focused ultrasound combined with MBs can transiently increase BBB permeability, b

BACKGROUND: Group B Streptococcus (GBS) is a major cause of invasive bacterial diseases, including sepsis, meningitis, and pneumonia, particularly in newborns and infants. Pregnant adults, those with pre-existing conditions, and older adults are particularly susceptible. Ongoing research is focused on developing various vaccines utilizing different antigens, including capsular polysaccharides and alpha-like proteins (Alps). Epidemiological data on these antigens in GBS is essential for predicting the effectiveness of these vaccines. However, no epidemiological studies on Alps genotype have bee
