Coexisting focal nodular hyperplasia and diffuse hepatic metastases from an ileal neuroendocrine tumor: a case report and literature review.

INTRODUCTION AND IMPORTANCE: Focal nodular hyperplasia (FNH) is a benign liver lesion usually associated with vascular abnormalities. Neuroendocrine tumors (NETs), on the other hand, frequently metastasize to the liver and typically appear as hypervascular lesions. The coexistence of FNH and diffuse hepatic metastases from an ileal NET is exceptionally rare and represents a major diagnostic challenge because both entities may present as hypervascular liver lesions, potentially leading to misdiagnosis and inappropriate management. CASE PRESENTATION: A 56-year-old woman with a history of epilep

Immunometabolic Reprogramming in Hepatocellular Carcinoma Mechanisms, Biomarkers, and Therapeutic Implications.

Hepatocellular carcinoma (HCC) develops in a chronically inflamed and dysregulated liver metabolism, in which tumor progression and resistance to treatment are orchestrated by the changes in cellular metabolism and immune control. Growing evidence recognizes immunometabolic reprogramming as the two-way interaction of metabolic processes and immune cell capabilities as one of the major determinants of immune evasion and heterogeneity of treatment response in HCC. The review aims to comprehensively evaluate immunometabolic reprogramming in hepatocellular carcinoma, with a focus on its role in tu

Ectopic Duodenal Varices Secondary to Portal Hypertension Caused by Primary Hepatic Neuroendocrine Tumor.

Primary hepatic neuroendocrine tumors (PHNETs) are rare, often presenting with abdominal pain, carcinoid syndrome, or incidental imaging findings. PHNET-associated portal hypertension is uncommon. A 45-year-old man with known PHNET presented with massive upper gastrointestinal bleeding from a duodenal ectopic varix and hemorrhagic shock. Emergent esophagogastroduodenoscopy achieved hemostasis with hemoclips. He subsequently developed high-serum-ascites albumin gradient (1.4 g/dL); imaging showed a 23-cm hepatic mass, portal vein dilatation, and splenomegaly. Liver biopsy confirmed a well-diffe

Carcinoid Heart Disease Presenting as Cardiogenic Shock: A Case Report.

Carcinoid heart disease (CHD) is a complication of carcinoid syndrome that results from chronic exposure of the endocardium to circulating vasoactive substances, particularly serotonin. It leads to progressive right-sided valvular dysfunction and right heart failure. Presentation as cardiogenic shock is uncommon and may delay recognition of the underlying disease. We present the case of a 44-year-old woman with no prior medical history who developed progressive dyspnea, ascites, and lower extremity edema over four months and was admitted with cardiogenic shock. Transthoracic echocardiography r

Intravenous adrenaline (epinephrine): its use in the critical care setting.

Adrenaline (epinephrine) is an endogenous catecholamine with potent β₁-adrenergic, moderate β₂-, and α₁-adrenergic activity, widely used in resuscitation and critical care. At lower infusion rates, it increases cardiac output and decreases systemic vascular resistance. While at higher doses it will result in greater inotropy and peripheral vasoconstriction, with potential adverse effects such as arrhythmias, lactic acidosis, and ischemia. This review synthesises contemporary evidence surrounding adrenaline's physiological and pharmacological profile, highlighting its role in cardiac arrest, pe

Carcinoid Heart Disease: Diagnostic Value of Cardiac MRI in a Patient With Metastatic Small‑Intestinal Neuroendocrine Tumor.

Carcinoid heart disease (CHD) is a serious and potentially life‑limiting complication of neuroendocrine tumors (NETs), resulting from prolonged systemic exposure to serotonin and other vasoactive substances, leading to fibrotic valvular degeneration. We report the case of a 71‑year‑old woman with metastatic small‑intestinal NET and associated carcinoid syndrome presenting with progressive exertional intolerance. Transthoracic echocardiography revealed severe tricuspid and moderate pulmonary regurgitation with right‑sided ventricular dilation. Cardiac MRI confirmed regurgitation and right‑heart

Oncological Outcomes after Elective and Emergency Resection of Small Intestinal Neuroendocrine Tumours.

PURPOSE: Small intestinal neuroendocrine tumour (siNET) has distinct features and disease course compared to other gastrointestinal neuroendocrine tumours. While they mostly present with subtle symptoms, they also can present acutely requiring emergency surgical intervention. We assessed the impact of surgical intervention timing on oncological outcomes of siNET. METHOD: A retrospective observational single centre cohort study of all patients diagnosed with siNET, and received surgical intervention at a tertiary level hospital between 2008 and 2025. RESULTS: A total of 66 patients were diagn

Oxalate nephropathy from untreated pancreatic insufficiency in a neuroendocrine tumour patient.

Pancreatic exocrine insufficiency (PEI) is an under-recognised cause of chronic diarrhoea in patients with neuroendocrine tumours (NETs). If left untreated or undertreated, PEI can lead to complications such as oxalate nephropathy (ON), which may result in loss of kidney function and potentially limit oncologic treatment options, including peptide receptor radionuclide therapy (PRRT). We present a man in his late 60s with a small bowel NET who developed persistent, uncontrolled diarrhoea for several years. The diarrhoea was attributed to multiple factors, including secretory NET (ie, carcinoid

Determining Prognosis in Patients With Carcinoid Syndrome: A Retrospective Single-Center Cohort Study.

BACKGROUND: Carcinoid syndrome (CS), the most prevalent neuroendocrine tumor (NET)-related hormonal syndrome, is associated with impaired survival. However, the contribution of NET-specific death in patients with this syndrome is currently unknown. This study aimed to evaluate overall survival (OS), prognostic factors, and causes of death in patients with CS. PATIENTS AND METHODS: We retrospectively included patients with CS treated between 1995 and 2021 at our ENETS Center of Excellence. We used Kaplan-Meier curves and log-rank tests to perform survival analyses, and a Cox proportional hazar

Recurrent ventricular asystole during octreotide infusion in a patient with pancreatic neuroendocrine tumor: a case report.

BACKGROUND: Somatostatin analogs such as octreotide are frequently used in the prevention of carcinoid syndrome in the perioperative period following neuroendocrine tumor resection. This is the first reported case of ventricular asystole associated with postoperative intravenous octreotide infusion in a patient with previous congenital cardiac surgery. CASE PRESENTATION: We present the case of a 62-year-old white British male patient with a history of previous congenital cardiac surgery who sustained multiple episodes of ventricular asystole and associated loss of cardiac output during octreo

Real-World Observational Study of Somatostatin Analogs and Rescue Medication Usage for Neuroendocrine Tumors in Canada.

INTRODUCTION: Neuroendocrine tumors (NETs) are uncommon malignancies with increasing incidence in Canada. Long-acting somatostatin analogs (SSA) such as Lanreotide Autogel® (LAN-ATG) and Octreotide Long-Acting Release (OCT-LAR) are first-line treatments for well-differentiated gastroenteropancreatic neuroendocrine tumors either with or without carcinoid syndrome. Despite their efficacy, many patients require short-acting SSA for breakthrough symptoms. We investigated the impact of switching between long-acting SSAs on the usage of breakthrough medications and the incidence of above maximum rec

Clinical and Pathological Features of EC-cell Familial Small Intestine Neuroendocrine Tumors: A Nationwide Cohort From the French GTE-RENATEN Network.

EC-cell familial small intestine neuroendocrine tumors (EC-cell F-SINET) are a recently described but poorly characterized entity. We aimed to describe their clinical and pathological features, and to compare them to patients with a sporadic form (EC-cell S-SINET). We constituted a nationwide cohort including (retrospectively patients diagnosed before 2012 and prospectively from 2012 to 2022) all patients with F-SINET (histologically proven SINET in ≥ 2 first or second-degree relatives) managed in the French GTE-RENATEN network. Clinical and pathological data were described and compared to the

Impact of Symptom Phenotype on Surgical Quality and Survival in Small-Bowel Neuroendocrine Tumours: A Quality Metrics Analysis.

BACKGROUND: There is debate regarding the optimal management of small-bowel neuroendocrine tumours (SBNETs), particularly concerning upfront resection in various clinical presentations. While symptom phenotypes are known to influence survival, their impact on technical surgical quality, especially in the emergency setting, remains poorly defined. This study evaluates whether symptom phenotype compromises the delivery of guideline-concordant surgical care. METHODS: A retrospective analysis of 108 consecutive SBNET resections (2000-2023) at a specialized tertiary centre was performed. Patients

Neuroendocrine tumors and pregnancy: the French PregNET study.

OBJECTIVE: The rising incidence of neuroendocrine tumors (NETs) and delayed motherhood raise concerns about pregnancy outcomes and management, yet data remain scarce. METHODS: This national cohort study, conducted through the GTE and RENATEN-ENDOCAN networks, included 17 women with NETs who experienced 26 pregnancies, resulting in 27 births. Maternal and fetal outcomes were analyzed based on tumor status, presence of carcinoid syndrome, and treatments during pregnancy. Early postpartum tumor evolution was evaluated using mRECIST on CT scans at 1 and 6 months after delivery. RESULTS: Median m

Pellagra From Tryptophan Depletion in Carcinoid Syndrome due to a Pulmonary Atypical Carcinoid Neuroendocrine Tumor.

Pellagra may complicate functioning pulmonary atypical carcinoid and present as a subtle acral dermatosis rather than the classical photo-distributed rash. In neuroendocrine tumor patients with chronic diarrhea and new skin changes, clinicians should suspect niacin deficiency and consider empiric nicotinamide, even when niacin assays are unavailable.
