AIMN, ITANET, SIE joint position paper for the recognition, prevention and management of NET carcinoid syndrome and crises during peptide receptor radionuclide therapy.

PURPOSE: Carcinoid syndrome (CS) is the most frequent neuroendocrine tumor (NET)-related syndrome with variable presentations related to hormonal discharge and long-term complications, such as carcinoid heart disease (CHD). Carcinoid crisis (CC) is an acute and serious CS complication, due to a massive release of vasoactive hormones and biologically active substances, which may occur during stressogenic procedures such as peptide receptor radionuclide therapy (PRRT). CC is an emergency and life-threatening condition that needs to be prevented, promptly recognized, and managed. The purpose of t

Paltusotine versus octreotide: different effects on radioligand uptake in neuroendocrine tumours.

OBJECTIVE: Somatostatin receptor analogues are well-established in the treatment of metastatic gastro-enteropancreatic neuroendocrine tumours (GEP-NETs), especially for symptom control in patients with the carcinoid syndrome, and to control tumour growth. However, they need to be discontinued before peptide receptor radionuclide therapy (PRRT) as they may saturate the somatostatin receptor 2 (SSTR2) and prevent binding of the radioactive ligand. DESIGN: We evaluated the effects of the novel somatostatin analogue paltusotine on 18F-SiTATE radioligand uptake and on GEP-NET cell viability in com

Uncommon Origin, Common Presentation: A Rare Case of Mesenteric Carcinoid Tumor Mimicking Functional Gastrointestinal Disorders.

Carcinoid tumors, a subtype of well-differentiated neuroendocrine neoplasms, are frequently misdiagnosed due to their indolent course and nonspecific gastrointestinal (GI) symptoms that mimic more common disorders. Here we report a rare case of a 59-year-old woman with hepatitis C and a history of intravenous drug use who presented with progressive abdominal pain, nausea, vomiting, diarrhea, and weight loss. Initial evaluations were inconclusive, but further imaging revealed a central mesenteric mass-an uncommon primary site-along with hepatic lesions, raising concern for metastatic disease. E

Metastatic neuroendocrine neoplasms of the breast: a systematic review.

Neuroendocrine neoplasms (NENs) originate mainly in the digestive system and lungs and are classified as neuroendocrine carcinomas (NECs) or neuroendocrine tumors (NETs). Breast metastases from NENs are rare, accounting for only 1% of breast neoplasms, but may be more frequent than expected. This study aimed to characterize patients with breast metastatic NENs through a literature review. A systematic review was conducted using PubMed, Embase, LILACS, and OpenGrey databases through July 2024. Eligible studies included case reports, case series, or cross-sectional studies documenting at least o

Paraneoplastic Neurological Syndromes in Oncology: Clinical Features, Mechanisms, and Educational Perspectives.

Paraneoplastic neurological syndromes (PNS) represent a rare but clinically significant group of disorders that arise as indirect effects of cancer through immune-mediated mechanisms. They differ from direct tumor invasion, metastasis, or treatment-related toxicity, and often precede cancer diagnosis, making early recognition essential. This review outlines the distinctions between carcinoid syndrome, paraneoplastic endocrine/metabolic syndromes, and PNS, highlighting their clinical relevance in oncology education. We summarize current knowledge on the pathophysiology, clinical spectrum, diagn

Exploring the carcinoid crisis: insights from a cancer-specific centre.

OBJECTIVE: To assess the prevalence, pre-procedure biomarkers, and management of carcinoid crisis (CC) in a cancer-specific hospital. DESIGN: Retrospective observational study of subjects with carcinoid syndrome (CS) undergoing invasive procedures or systemic therapies over 5 years. METHODS: Electronic haemodynamic parameters and patient records were extracted. CC incidence was assessed using strict (SBP <80 mmHg, >10 min) or broad criteria (severe hypotension, hypertension, arrhythmias, >1 min). RESULTS: 48 patients with CS underwent 71 procedures (35 invasive procedures, 36 systemic thera

Ovarian carcinoid tumor with carcinoid heart syndrome: A case report and literature review.

BACKGROUND: Primary ovarian carcinoid tumors are rare neuroendocrine neoplasms comprising <0.1% of ovarian tumors and 1% of all carcinoid tumors. Up to 30% present with carcinoid syndrome, and approximately one-quarter of these develop carcinoid heart syndrome. Unlike gastrointestinal carcinoid tumors, ovarian lesions can cause carcinoid syndrome without hepatic metastases due to direct venous drainage into the systemic circulation. CASE: A 46-year-old woman presented with abdominal discomfort, new onset hypertension, and lower extremity edema. Imaging revealed a 16.8 cm right adnexal mass wi

Contrast-Enhanced Ultrasound and Somatostatin Receptor Scintigraphy Unveil an Occult Neuroendocrine Tumor With Carcinoid Syndrome and Presumed Small Intestinal Origin - A Case Report.

A 68-year-old man with postprandial flushing and palpitations was referred for multiple hepatic lesions. Computed tomography showed hypoattenuating metastases without a detectable primary, whereas contrast-enhanced ultrasound revealed arterial hyperenhancement and Kupffer-phase defects, indicating hypervascularity typical of neuroendocrine tumors (NETs). Urinary 5-hydroxyindoleacetic acid elevation, together with liver biopsy findings, confirmed a well-differentiated NET (grade 2), presenting with carcinoid syndrome. Somatostatin receptor scintigraphy demonstrated diffuse hepatic uptake and a

The exceptionally rare phenomenon of well-differentiated colon neuroendocrine tumors.

Colonic neuroendocrine tumors (NETs), excluding rectal NETs, are often described as relatively common and aggressive, with inferior median survival compared with other gastrointestinal (GI) primary sites. However, epidemiological databases may conflate well-differentiated NETs with poorly differentiated neuroendocrine carcinomas (NECs), leading to a lack of precise data on the prevalence, clinical behavior, and prognosis of well-differentiated colonic NETs. We analyzed a large institutional database to identify patients with well-differentiated NETs originating in the colon, excluding rectal N

Carcinoid Tumor of the Duodenum Presenting as Iron Deficiency Anemia.

Carcinoid tumors are slow-growing tumors that typically occur in the gastrointestinal (GI) tract, lungs, and other endocrine glands. They are commonly discovered incidentally and usually cause no symptoms. When symptomatic, they tend to present with vague abdominal pain and may lead to carcinoid syndrome after metastasis to the liver. Such tumors rarely present with GI bleeding and thus rarely lead to iron deficiency anemia. In this report, we present an uncommon case of a 71-year-old male who presented with fatigue, weakness, intermittent rectal bleeding, and black stools. He was vitally stab

A Rare Case of a Neuroendocrine Tumor With Intracardiac Metastasis: Management and Literature Review.

Neuroendocrine tumors (NETs) are rare neoplasms originating from the neuroendocrine system. While they can develop sporadically throughout the body, they are mostly found in the gastrointestinal tract, respiratory tract, pancreas, and thymus. NETs can be classified into low-grade indolent tumors, intermediate-grade tumors, or high-grade aggressive cancers. These tumors have the potential to metastasize to various parts of the body, with the liver being the primary site for metastases. However, it is extremely rare for NETs to metastasize to the heart. Here, we present a unique case of intracar

Gut microbial and functional alterations lead to metagenomic signatures for midgut neuroendocrine tumor patients and for carcinoid syndrome.

SUMMARY: We report the case of a 41-year-old Japanese woman with visual field disturbances during late pregnancy. At 39 weeks of gestation, she was diagnosed with bitemporal hemianopsia at the ophthalmology department. An MRI revealed a symmetrical pituitary gland enlargement, compressing the optic chiasm. An emergency cesarean section was performed immediately, resulting in the delivery of a male infant weighing 3,112 grams. Laboratory tests indicated low serum free thyroxine (T4), thyroid-stimulating hormone (TSH), cortisol, luteinizing hormone, and follicle-stimulating hormone. The patient

Nutritional Management of Functioning GEP-NENs.

BACKGROUND: Functioning gastroenteropancreatic neuroendocrine neoplasms (GEP-NENs) are rare tumors that secrete biologically active hormones, leading to complex clinical syndromes such as carcinoid syndrome, VIPoma, glucagonoma, gastrinoma, insulinoma, and somatostatinoma. These syndromes frequently induce profound metabolic, gastrointestinal, and nutritional disturbances. OBJECTIVE: This review aims to provide a comprehensive overview of the physiopathology of malnutrition in functioning GEP-NENs and to highlight nutritional and supportive care strategies, including how medical, surgical, an

Echocardiographic Evaluation of Carcinoid Heart Disease.

Carcinoid heart disease (CaHD) is a complication that occurs in patients with metastatic neuroendocrine tumors (usually to the liver) and carcinoid syndrome. Hormonal release causes endocardial thickening, typically affecting right-sided cardiac valves. Symptomatic patients with CaHD have a poor prognosis, and management of valvular heart disease is complicated by metastasis and other associated conditions and carries a higher risk than management of other acquired valve disease. Clinical and biomarker assessment are used for screening and echocardiography is the diagnostic imaging backbone us

Diagnostic Dilemma of a Neuroendocrine Tumour Complicated by Simultaneous Retroperitoneal Fibrosis and Carcinoid Heart Disease in a Perimenopausal Woman.

Carcinoid tumours are rare, slow-growing neuroendocrine neoplasms that often remain asymptomatic until metastatic spread or the development of carcinoid syndrome. Carcinoid syndrome is characterised by flushing, diarrhoea, and bronchospasm due to the secretion of vasoactive hormones. These tumours commonly arise in the gastrointestinal tract but can also occur in other organs, namely, the lungs, genitourinary tract, and pancreas. Retroperitoneal fibrosis (RPF), a rare inflammatory disease, involves chronic inflammation leading to fibrous scarring and compression of surrounding structures like

A Royal Flush: Carcinoid Heart Disease Complicated by Severe Tricuspid and Pulmonic Valve Regurgitation.

Carcinoid heart disease (CHD) is a severe complication of metastatic neuroendocrine tumors (NETs), leading to fibrotic degeneration of right-sided heart valves. A 38-year-old male presented with progressive dyspnea, fatigue, abdominal bloating, diarrhea, and facial flushing. Imaging revealed hepatic metastases and mesenteric lymphadenopathy, and biochemical markers confirmed a NET of likely gastrointestinal origin. Echocardiography showed torrential tricuspid regurgitation, severe pulmonary insufficiency, right ventricular dilation, and a patent foramen ovale (PFO). The patient was started on

Carcinoid Heart Disease Revealing the Burden of a Neuroendocrine Tumour: A Case Report.

Carcinoid heart disease (CHD) is a rare but serious complication of carcinoid syndrome (CS), typically arising in patients with metastatic neuroendocrine tumours (NETs). Prolonged exposure of the right side of the heart to vasoactive substances such as serotonin leads to progressive valvular fibrosis, predominantly affecting the tricuspid and pulmonary valves, and often culminates in right-sided heart failure. We report the case of a 67-year-old woman with a metastatic small bowel NET who developed CS and later presented with worsening exertional dyspnoea and peripheral oedema. Echocardiograph

Diagnosis and Treatment of Right-Sided Valve Disease in Cardiac Carcinoid Syndrome: A Systematic Review.

INTRODUCTION: Carcinoid heart disease (CaHD) is a severe complication of neuroendocrine tumors, characterized by progressive fibrotic involvement of right-sided heart valves due to serotonin and other vasoactive substances. This condition significantly worsens prognosis and quality of life, yet therapeutic strategies remain heterogeneous and based on low-level evidence. The objective of this study was to systematically review diagnostic approaches and therapeutic options for right-sided valvular disease in CaHD, assessing indications, advantages, limitations, and outcomes. METHODS: A systemat
