Impact of Symptom Phenotype on Surgical Quality and Survival in Small-Bowel Neuroendocrine Tumours: A Quality Metrics Analysis.

BACKGROUND: There is debate regarding the optimal management of small-bowel neuroendocrine tumours (SBNETs), particularly concerning upfront resection in various clinical presentations. While symptom phenotypes are known to influence survival, their impact on technical surgical quality, especially in the emergency setting, remains poorly defined. This study evaluates whether symptom phenotype compromises the delivery of guideline-concordant surgical care. METHODS: A retrospective analysis of 108 consecutive SBNET resections (2000-2023) at a specialized tertiary centre was performed. Patients

Neuroendocrine tumors and pregnancy: the French PregNET study.

OBJECTIVE: The rising incidence of neuroendocrine tumors (NETs) and delayed motherhood raise concerns about pregnancy outcomes and management, yet data remain scarce. METHODS: This national cohort study, conducted through the GTE and RENATEN-ENDOCAN networks, included 17 women with NETs who experienced 26 pregnancies, resulting in 27 births. Maternal and fetal outcomes were analyzed based on tumor status, presence of carcinoid syndrome, and treatments during pregnancy. Early postpartum tumor evolution was evaluated using mRECIST on CT scans at 1 and 6 months after delivery. RESULTS: Median m

Pellagra From Tryptophan Depletion in Carcinoid Syndrome due to a Pulmonary Atypical Carcinoid Neuroendocrine Tumor.

Pellagra may complicate functioning pulmonary atypical carcinoid and present as a subtle acral dermatosis rather than the classical photo-distributed rash. In neuroendocrine tumor patients with chronic diarrhea and new skin changes, clinicians should suspect niacin deficiency and consider empiric nicotinamide, even when niacin assays are unavailable.

The Clinical Impact of Patent Foramen Ovale in Patients With Carcinoid Heart Disease.

BACKGROUND: Carcinoid heart disease (CaHD) typically affects the right-sided valves. Rarely, CaHD affecting the left-sided valves has been observed in the setting of patent foramen ovale (PFO). However, the impact of PFO on valve disease progression and prognosis in CaHD is not well understood. OBJECTIVES: The purpose of this study was to evaluate the impact of PFO on CaHD progression and clinical outcomes. METHODS: Patients diagnosed with neuroendocrine tumor and carcinoid syndrome (CS), and with serial transthoracic echocardiograms, were included. The primary endpoint was progression of le

Survival trends in the management of carcinoid heart disease among patients with neuroendocrine neoplasms.

BACKGROUND: Nearly 20% of patients with neuroendocrine tumors (NET) develop carcinoid syndrome (CS), and 20-25% of those with CS develop carcinoid heart disease (CHD). Contemporary risk factors for CHD and prognostic variables in NET management remain limited. This study aimed to characterize the clinical and imaging factors associated with the development, and prognostic impact of carcinoid heart disease in patients with neuroendocrine neoplasms. METHODS: A single-institution, retrospective analysis of well-differentiated NET patients from January 2010 to December 2024 was conducted. Cox pro

Acute neurologic crisis after bronchoscopic biopsy: Carcinoid-like presentation of pulmonary mucoepidermoid carcinoma.

INTRODUCTION: Bronchoscopic evaluation of endobronchial tumors rarely triggers systemic complications. While bronchial carcinoid tumors can precipitate carcinoid syndrome or crisis through serotonin release, classic carcinoid syndrome occurs in under 5% of bronchial carcinoids. We describe a patient who developed acute neurologic and autonomic crisis following bronchoscopy of an unknown endobronchial tumor, initially mimicking carcinoid crisis but ultimately attributed to cerebral air embolism with final pathology revealing pulmonary mucoepidermoid carcinoma. CASE PRESENTATION: A 52-year-old

Nonmetastatic Pulmonary Carcinoid Presenting With Carcinoid Syndrome Despite Negative 5-HIAA: A Case Report.

BACKGROUND: Carcinoid syndrome from pulmonary carcinoids without hepatic metastases is rare and diagnostically challenging when biochemical markers are normal. CASE PRESENTATION: A woman in her mid-60 s presented with an incidental right adrenal mass during evaluation for dyspnea and elevated D-dimer. She reported a 20-year history of paroxysmal tachycardia and 5 years of episodic flushing, profuse sweating, exertional dyspnea, and fine tremors triggered by minimal physical activity. Imaging revealed a 3.6 cm lipid-rich adrenal adenoma and an 8 × 10 mm left upper lobe pulmonary nodule. Compre

A Combination of the Tryptophan Hydroxylase Inhibitor Telotristat with the mTOR Inhibitor Everolimus as an Effective Strategy Against Neuroendocrine Tumors.

BACKGROUND AND OBJECTIVES: Neuroendocrine tumors are a rare and heterogeneous group of neoplasms that frequently cause carcinoid syndrome, characterized by diarrhea, flushing, and carcinoid heart disease. A recent therapy for carcinoid syndrome involves inhibition of tryptophan hydroxylase, the rate-limiting enzyme in serotonin synthesis, using telotristat ethyl. Telotristat has demonstrated clinical control of carcinoid syndrome; however, its potential antitumoral effects remain unclear. This study aimed to evaluate the antitumor activity of telotristat ethyl alone and in combination with eve

A Grade 1, Metastatic, Well-Differentiated Neuroendocrine Tumor Presenting as Closed-Loop Small Bowel Obstruction With Ischemic Bowel.

Small bowel neuroendocrine tumors (NETs) are typically indolent neoplasms that often present with vague abdominal symptoms and are frequently diagnosed only after regional or metastatic spread has occurred. Mesenteric nodal metastases may provoke a desmoplastic reaction resulting in bowel tethering, luminal narrowing, mesenteric vascular compromise, obstruction, and intestinal ischemia. We report the case of a 49-year-old man with no prior abdominal surgery who presented with three months of intermittent abdominal pain followed by abrupt worsening to diffuse severe abdominal pain and emesis. Primary Ovarian Neuroendocrine Neoplasm With Carcinoid Syndrome and Carcinoid Heart Disease.

BACKGROUND: Neuroendocrine neoplasms commonly arise in the midgut, lungs, or pancreas, while primary ovarian neuroendocrine neoplasms are rare and can present with carcinoid syndrome and carcinoid heart disease even in the absence of hepatic metastases. CASE REPORT: A 70-year-old female with coronary artery disease presented with 3 months of dyspnea, leg edema, diarrhea, and facial flushing. Examination suggested right-sided heart failure with a palpable lower abdominal mass. Her 24-hour urinary 5-hydroxyindoleacetic acid level was markedly elevated (106.0 mg), and serum chromogranin A was al

Transcatheter Tricuspid Valve Replacement in Carcinoid Heart Disease With Transjugular 3D Intracardiac Echocardiography Guidance.

BACKGROUND: Transcatheter tricuspid valve replacement (TTVR) with the EVOQUE system was the first Food and Drug Administration-approved TTVR therapy for clinical use in patients with symptomatic severe tricuspid regurgitation. Successful implantation is largely dependent on both fluoroscopic and transesophageal echocardiography (TEE) guidance. Imaging by TEE may be inadequate for confident and successful TTVR, which has led to adjunctive imaging with intracardiac echocardiography (ICE). CASE SUMMARY: We present a unique case of TTVR with the EVOQUE system using transjugular 3-dimensional (3D)

Outcomes of liver resections for neuroendocrine tumor liver metastases in carcinoid heart disease.

Carcinoid heart disease (CHD) is associated with advanced neuroendocrine tumor liver metastases (NETLM) and may preclude surgical cytoreduction. We assessed perioperative and long-term outcomes of hepatectomy in patients with CHD. We retrospectively analyzed 311 patients undergoing cytoreductive hepatectomy for intestinal NETLM: non-functional (n = 163), carcinoid syndrome (CS) without CHD (n = 110), and CHD (n = 38), including patients undergoing pre-hepatectomy valve replacement. CHD patients more frequently had >10 liver metastases (78%) and larger lesions (median 9.5 cm) and required major

Quadrivalvular Carcinoid Heart Disease Via Patent Foramen Ovale as Initial Presentation of Metastatic Neuroendocrine Tumor.

BACKGROUND: Carcinoid heart disease typically affects right-sided valves. Quadrivalvular involvement is rare. CASE SUMMARY: A 52-year-old woman presented with florid heart failure. Multimodality imaging demonstrated severe multivalvular regurgitation, and a large patent foramen ovale. She underwent bioprosthetic triple valve replacement with atrial septal defect patch closure. Histology revealed fibromyxoid degeneration and postoperative evaluation confirmed a metastatic small bowel neuroendocrine tumor. Following somatostatin analog therapy as a bridge, she underwent definitive oncologic res

A silent tumor, a loud valve: carcinoid heart disease as the first manifestation of a pelvic neuroendocrine tumor.

Carcinoid heart disease (CaHD) is a well-recognized complication of functional neuroendocrine tumors (NETs), typically occurring in the presence of hepatic metastases that permit vasoactive substances to reach the systemic circulation. Its occurrence in the absence of liver involvement is uncommon and may delay diagnosis. We report a 46-year-old woman who was incidentally found to have severe tricuspid regurgitation during preoperative evaluation for uterine fibroids. Further evaluation revealed elevated 24-hour urinary 5-hydroxyindoleacetic acid and markedly increased chromogranin A levels, r

Clinical experience with the diagnosis and treatment of middle ear adenomatous neuroendocrine tumors.

Middle ear adenoma, also known as middle ear adenomatous neuroendocrine tumors (MEANTs), is a rare neoplasm, accounting for < 2% of all middle ear tumors. It is characterized by slow growth, non-invasive behavior, and nonspecific symptoms, which often complicate preoperative diagnosis. This review summarizes our clinical experience in the diagnosis and treatment of 11 cases of MEANT. In this cohort, four cases were preoperatively diagnosed as cholesterol granuloma, five as cholesteatoma, and two as middle ear tumors. The median age at diagnosis was 36.7 years, with all patients (100%) experien

Carcinoid syndrome revealed by chronic diarrhea and isolated severe tricuspid regurgitation.

Carcinoid syndrome is an uncommon manifestation of metastatic neuroendocrine tumor (NET) and may present without classic flushing or bronchospasm. We describe a woman in her 50s with hypertension treated with hydrochlorothiazide who presented with confusion, severe hyponatremia with a serum sodium concentration of 114 mmol/L (reference range, 135-145 mmol/L), chronic watery diarrhea, bilateral leg edema, and 40-lb weight loss. Computed tomography (CT) revealed a calcified mesenteric mass and multiple hepatic lesions. Serum chromogranin A was 1434 ng/mL (SI: 1434 µg/L) (reference range, <100 ng

[Anesthesia in carcinoid syndrome for tricuspid and pulmonary valve replacement: case report].

Carcinoid syndrome is characterized by the presence of a carcinoid tumor that can release chemical mediators that, if not adequately controlled, can even lead to carcinoid crisis. In this condition occurs hemodynamic instability, with distributive shock, bronchospasm, and cardiac arrhythmias. Carcinoid tumors have an incidence of 1-2 cases per 100,000 patients, and 20-30% of them present episodic vasomotor symptoms such as bronchospasm, hypotension and diarrhea secondary to therelease of vasoactive amines. Because it is an infrequent situation, we present the anesthetic management of a patient

Cutaneous manifestations of carcinoid tumor and syndrome.

Carcinoid syndrome refers to the signs and symptoms a patient experiences secondary to a carcinoid tumor or another well-differentiated neuroendocrine tumor, which secretes serotonin and other peptides that enter the bloodstream; only 10% of patients with carcinoid tumors experience carcinoid syndrome. Common findings include facial flushing, tachycardia, and shortness of breath. Carcinoid tumors usually originate in the gastrointestinal tract. They are slow growing but can metastasize to the liver, lymph nodes, and elsewhere. Primary or metastatic cutaneous carcinoid tumors present as pink, f

Approach to the Patient: Theranostics as precision medicine for neuroendocrine tumors.

CONTEXT: Theranostics has emerged as a precision medicine paradigm in neuroendocrine tumors (NETs), integrating receptor-based imaging with targeted radionuclide therapy to individualize treatment. This approach is particularly valuable in functional and biologically heterogeneous NETs, where hormonal control and variable clinical behavior dominate therapeutic priorities. CASE DESCRIPTION: We present a three-case series illustrating the application of theranostics across diverse NET contexts. Case 1 describes a patient with metastatic insulinoma with refractory hypoglycemia, in whom multimoda
