A Tumultuous Journey of Metastatic Pancreatic Neuroendocrine Tumor with Carcinoid Syndrome.

A 25-year-old woman presented with metastatic pancreatic neuroendocrine tumor with carcinoid syndrome. She was refractory to octreotide and did not respond well to chemotherapy. Although surgical debulking remains the primary approach for managing these tumours, it entails inherent risks, including potentially exacerbating carcinoid syndrome. We strategically delivered the one Peptide Receptor Radionuclide Therapy cycle before tumor debulking, a decision that yielded a remarkable response, stabilizing her condition.

PFO closure in a patient with hypoxia due to tricuspid valve regurgitation and flow-driven shunt.

A female patient in mid-60s presented with progressive dyspnoea over several months and persistent hypoxia, requiring high-flow supplemental oxygen due to type 1 respiratory failure. Imaging investigations revealed metastatic carcinoid syndrome affecting the tricuspid valve, which led to severe tricuspid regurgitation (TR) directed towards a patent foramen ovale (PFO). A bubble study demonstrated a significant spontaneous right-to-left shunt through the PFO. Right heart catheterisation ruled out significant elevation of right-sided cardiac pressures as a cause of the right-to-left shunt. Thus,

Red Emitting Fluorescent EuDPA Complex as a Probe for the Detection of Serotonin, A Neurotransmitter.

Serotonin, a neurotransmitter and also a biomarker for Alzheimer's as well as Carcinoid Syndrome. It has two important roles in the human body - central as well as peripheral. In the central nervous system, it acts as a neurotransmitter and controls all the brain functions and as a peripheral hormone, it controls the functions of several organs in the body. Herein, we developed a Europium dipicolinic acid (EuDPA) based lanthanide luminescent complex with bright red fluorescence. This fluorescence is further used for the detection of Serotonin (ST). On adding ST, the fluorescence of the probe i

Hiding in Plain Sight: Pulmonic Valve Involvement in Carcinoid Heart Disease.

A 50 year old male with medical history of hypertension and carcinid heart disease was initially seen in valve clinic for planned tricuspid valve replacement. A repeat echocardiogram demonstrated flow acceleration accross the pulmonic valve concerning for pulmonic stenosis, which was not seen on prior echocardiograms. Follow up CT imaging confirmed pulmonic stenosis and the surgical plan was changed to include both tricuspid and pulmonic valve replacements. A high clinical index of suspicion is requried for evaluation pulmonic involvement in carcinoid heart disease.

Atypical Presentation of Small Bowel Neuroendocrine Carcinoma Leading to Acute Obstruction.

Rare neoplasms called neuroendocrine tumors (NETs) develop from intestinal enterochromaffin cells. They commonly produce symptoms through the secretion of serotonin along with other vasoactive peptides, resulting in carcinoid syndrome, characterized by cutaneous flushing and diarrhea. NETs are a very aggressive type of cancer, for which prognostic factors are lacking. They are also rarely found in males and young adults. Carcinoid tumors make up an atypical and intricate disease spectrum with various clinical features. The combination of etoposide and carboplatin can enhance overall survival i

Impact of treatment on quality of life in neuroendocrine neoplasm survivors.

Neuroendocrine neoplasms (NENs) are a heterogeneous group of tumors, ranging from well-differentiated, slow-growing neuroendocrine tumor (NET) with long-term survival, to aggressive high-grade neuroendocrine carcinoma (NEC). Advances in treatment and earlier diagnosis have led to improved survival outcomes; however, data regarding patients' quality of life (QoL) during therapy and in the post-treatment phase remain limited. This narrative review aims to analyze available data on QoL in NEN patients undergoing medical therapies, including nutritional considerations, to improve and personalize t

A deep foundation model for electrocardiogram interpretation: enabling rare disease detection through transfer learning.

In healthcare, scarcity of high-quality human-adjudicated labelled data may limit the potential of deep neural networks (DNNs). Foundation models provide an efficient starting point for deep learning that can facilitate effective DNN training with fewer labelled training examples. In this study, we leveraged cardiologist-confirmed labels from a large dataset of 1.6 million electrocardiograms (ECGs) acquired as part of routine clinical care at UCSF between 1986 and 2019 to pre-train a convolutional DNN to predict 68 common ECG diagnoses. To our knowledge, this model is one of the most comprehen

A Rare Discovery of Neuroendocrine Tumor: Cardiac Mass.

Carcinoid tumors are rare, slow-growing neoplasms of neuroendocrine origin commonly affecting the gastrointestinal or respiratory system. They metastasize to lymph nodes and the liver based on their lymphatic and venous drainage of their tissue of origin. Metastasis to the heart is exceedingly rare accounting for <2 % of cases. Exceedingly rare is the incidental detection of cardiac carcinoid metastasis as the initial presentation of carcinoid syndrome. Here, we describe a case of metastatic cardiac carcinoid detected during a routine echocardiogram done for evaluation of a systolic murmur.

A Rare Case of Idiopathic Angioedema Associated With Amlodipine Use.

Angioedema is a self-limited, non-pitting edema of subcutaneous or submucosal tissues often affecting the face, lips, oral cavity, and airway. It may be histamine or bradykinin-mediated and is most commonly triggered by allergic reactions, medications, or systemic conditions. Drug-induced angioedema is frequently associated with angiotensin-converting enzyme (ACE) inhibitors, whereas calcium channel blockers (CCBs) like amlodipine are rarely implicated. This case report describes a rare case of recurrent angioedema in a hypertensive patient, ultimately attributed to amlodipine, and underscores

Concomitant carcinoid heart disease and patent foramen ovale: a case report and review of literature.

BACKGROUND: Carcinoid Heart Disease (CHD) primarily affects the right heart valves, while left heart involvement is rare and often associated with a patent foramen ovale (PFO). Early identification of a PFO in CHD can be critical to patient outcomes. A 61-year-old woman with metastatic neuroendocrine tumor presented with worsening breathlessness and hypoxemia. Imaging excluded pulmonary embolism and lung metastases. Transthoracic echocardiography revealed severe tricuspid regurgitation due to carcinoid valve involvement. Persistent hypoxemia prompted transesophageal echocardiography, which dem

Carcinoid syndrome mimicking irritable bowel syndrome: don't fall into the trap.

INTRODUCTION: Carcinoid syndrome (CS) is a complex condition caused by the systemic release of bioactive substances from neuroendocrine neoplasms (NENs), particularly small bowel tumors (sbNENs). Its symptoms-flushing, abdominal pain, and diarrhea-often resemble irritable bowel syndrome (IBS), resulting in misdiagnosis and delayed therapy. AREAS COVERED: This review examines the pathophysiology of CS, especially serotonin overproduction and its effects on gut motility and secretion. The overlap between CS and IBS is analyzed, with emphasis on diagnostic pitfalls and warning signs. Distinctive

Clinical presentation of bone metastases in neuroendocrine tumors: a retrospective cohort study.

Bone metastases (BMs) are a rare and late event in patients with neuroendocrine tumors (NETs). The aim of our study was to investigate the clinical presentation and outcome of BMs in a large cohort of patients with NETs. A retrospective study was performed at two referral centers of Northern Italy (IRCCS Humanitas Research Hospital in Milan and Santa Maria della Misericordia University Hospital in Udine). Three hundred fifty-two consecutive patients with either gastroenteropancreatic or non-gastroenteropancreatic NETs were included: 52 patients with synchronous or metachronous BMs (BM-positive

Case report of carcinoid syndrome with multi-organ involvement.

BACKGROUND: Carcinoid syndrome (CS) is a rare complication of neuroendocrine tumors (NETs). Characterized by cutaneous flushing, diarrhea, abdominal pain, and long-term risk for valvular heart disease, these components occur in a fraction of patients with advanced disease. Early recognition through history taking, physical and laboratory examination, and clinical imaging is paramount to successful management. CASE DESCRIPTION: We report a case of CS in a 21-year-old male patient found to have a metastatic gastrointestinal, well differentiated (Ki67 <3%) NET. The patient presented with cutaneo

Telotristat ethyl affects tumour-fibroblast crosstalk in small intestinal neuroendocrine tumours.

Small intestinal neuroendocrine tumours (SI-NETs) are associated with mesenteric fibrosis, which causes significant morbidity and mortality. Telotristat ethyl was developed to treat carcinoid syndrome in SI-NET patients. Recent studies indicated telotristat ethyl could have anti-tumour activity; however, the mechanism remains unclear. This study aimed to investigate the effects of telotristat ethyl on SI-NET-fibroblast crosstalk in tumour progression and mesenteric fibrosis. A co-culture paracrine model with GOT1 (tumour) cells and LX2 (stromal) cells was optimized. Cells were treated with con
