OBJECTIVE: To compare PBSCT with BMT in Thalassaemia patients in terms of rejection, non-rejection mortality, disease free survival and overall survival.
METHODS: Fifty six patients were transplanted from September 2000 - July 2005. Twenty nine underwent BMT and 27 received PBSCT. Most patients were intensely transfused to keep minimum haemoglobin of 12 gm/dl and received desferioxamine, 24 hours infusion, before transplantation. Pesaro class I (n-20) and class II (n-20) received conditioning with standard Bu/Cy. Of class III (n-16), ALG was added to standard Bu/Cy in 9 who received PBSCT and

Secondary hyperparathyroidism (SHPT) is a common complication of chronic kidney disease. Prolonged stimulation of the parathyroid glands can lead to severe mineral bone disorders. Uremic tumoral calcinosis (UTC) is a rare but debilitating manifestation of prolonged hyperparathyroidism characterized by periarticular calcium-phosphate deposits. This case describes the progression of SHPT to multifocal UTC in a patient with autosomal dominant polycystic kidney disease (ADPKD). A 46-year-old female with ADPKD-related end-stage renal disease (ESRD) was evaluated for renal transplantation and found

CONTEXT: Medical therapy for Cushing's syndrome (CS) is increasingly used. A potent adrenal steroidogenesis inhibitor, osilodrostat, has been rarely linked to prolonged adrenal insufficiency (AI).
OBJECTIVE: We hypothesized that osilodrostat-induced AI could be associated with adrenal gland shrinkage.
DESIGN: Noninterventional, retrospective, longitudinal, institutional review board-approved study of patients with CS treated at Oregon Health and Science University between January 1, 2000, and July 1, 2025.
SETTING: Ambulatory and inpatient, academic, quaternary medical center.

Ectopic adrenocorticotropic hormone syndrome is a distinct subtype of endogenous Cushing's syndrome characterized by excessive cortisol secretion, which increases susceptibility to opportunistic infections. Herein, we present the case of a male patient in his early 40s who was admitted with fatigue and bilateral lower-extremity edema. Laboratory tests revealed markedly elevated serum adrenocorticotropic hormone and cortisol levels. Ectopic adrenocorticotropic hormone syndrome was confirmed via dexamethasone suppression testing and somatostatin receptor-targeted positron emission tomography-com

Parathyroid glands are the smallest organs in our body, that regulate calcium metabolism, by producing parathyroid hormone (PTH). They are four, at least, as they might be either supernumerary or ectopic, especially in patients with chronic renal failure. Hyperparathyroidism (HPT) is a common endocrine disease, characterized by hormone imbalance, which leads to increased secretion of PTH. Primary hyperparathyroidism (PHPT) is caused by hyperfunctioning parathyroid glands, mainly by a solitary parathyroid adenoma and less frequently by parathyroid hyperplasia or double parathyroid adenomas. The

BACKGROUND: Recurrent disease due to ectopic parathyroid tissue remains a significant challenge following conventional total parathyroidectomy with autotransplantation (TPTX+AT) for refractory secondary hyperparathyroidism (SHPT). This retrospective cohort study aimed to evaluate the safety and medium-term efficacy of a standardized, extended surgical approach-clean parathyroidectomy with autotransplantation (CPTX+AT)-which systematically resects the central neck compartment to address this issue. We also clarified the exclusion criteria for kidney transplantation candidates and the applicatio

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a well-known cause of hyponatremia, frequently associated with small-cell lung carcinoma. Its association with myeloproliferative neoplasms (MPNs), however, is rare and underreported. We describe the case of an 81-year-old female with a history of chronic hyponatremia attributed to SIADH, requiring multiple hospitalizations. During her hospitalizations, she would be on fluid restriction and urea tablets. Her medical history included paroxysmal atrial fibrillation, a stable pulmonary nodule, and seropositive rheumatoid arthriti

Endogenous hypercortisolism remains a high-stakes diagnostic challenge in which delays or misclassification drive excess morbidity and mortality. This chapter synthesizes an evidence-based, physiology-anchored approach to the differential diagnosis of ACTH-dependent Cushing's syndrome, integrating clinical phenotyping, first-line biochemistry, dynamic testing, imaging, and invasive confirmation. We first outline bedside features that raise or lower the pre-test probability of Cushing disease (CD) versus ectopic ACTH syndrome (EAS). Then, the initial laboratory tests (nocturnal salivary cortiso

Cushing's syndrome comprises a heterogeneous group of disorders characterized by chronic excess of glucocorticoid release from adrenal glands, caused by altered adrenal function or by ACTH hypersecretion from pituitary or ectopic sources. Advances in high-throughput molecular profiling have substantially refined our understanding of the mechanisms underlying endogenous hypercortisolism. This chapter provides an integrated overview of the genetic, transcriptomic, and epigenetic alterations driving both ACTH-dependent and ACTH-independent forms of the disease. BACKGROUND: Cushing syndrome (CS) is a severe endocrine disorder caused by prolonged exposure to glucocorticoid excess. Among its diverse complications, reproductive and sexual disorders arehighly prevalent, impacting patients' quality of life. Despite this clinical significance, clinicalstudies specifically investigating the hypothalamic-pituitary-gonadal (HPG) axis status in femalepatients with CS remain insufficient, highlighting the urgent need for thorough evaluation.
METHOD: This retrospective cross-sectional study evaluated the HPG axis status in 137 women diagnosed with CS between 200

BACKGROUND: Accurate localization of adrenocorticotropic hormone (ACTH) secretion is crucial for the surgical management of ACTH-dependent Cushing's syndrome. While magnetic resonance imaging (MRI) and biochemical testing aid diagnosis, bilateral inferior petrosal sinus sampling (BIPSS) remains a reference standard in diagnostically challenging cases. In the absence of corticotropin-releasing hormone (CRH), vasopressin may serve as an effective alternative corticotroph stimulant during BIPSS; however, its additional benefit in lateralizing corticotroph adenomas remains uncertain.
OBJECTIVE: T

INTRODUCTION: Cushing's disease is caused by pituitary tumors that secrete excess adrenocorticotropic hormone (ACTH). Metyrapone is widely used for medical management as a bridge to transsphenoidal pituitary surgery.
CASE REPORT: An 80-year-old man presented with severe hypercortisolemia (101 μg/dL [reference range, 5-23 μg/dL]) and elevated plasma ACTH, 228 pg/mL [reference range, 7.2-63.3 pg/mL]). Initial high-dose dexamethasone suppression and corticotropin-releasing hormone (CRH) tests suggested ectopic ACTH syndrome; however, extensive imaging failed to identify a source. Metyrapone was

CONTEXT: Clinical presentation of inactivating parathyroid hormone (PTH)/PTH-related protein (PTHrP) signaling disorders (iPPSDs, historically pseudohypoparathyroidism [PHP]) exhibits pronounced age-dependence. Indeed, main features, including PTH resistance and brachydactyly (BD), develop during late childhood, while other features (ectopic ossifications, obesity, and hypothyroidism) are most prevalent in toddlers. The latter are included among minor diagnostic criteria; therefore, a substantial diagnostic delay has been reported.
OBJECTIVE: The aim of this work is to describe the early natu

BACKGROUND: According to the World Health Organization, air pollution is closely associated with climate change and, in particular, with global warming. In addition to melting of ice and snow, rising sea level, and flooding of coastal areas, global warming is leading to a tropicalization of temperate marine ecosystems. Moreover, the effects of air pollution on airway and lung diseases are well documented as reported by the World Allergy Organization.
METHODS: Scientific literature was searched for studies investigating the effect of the interaction between air pollution and climate change on

Cytoplasmic male sterility (CMS) is widely used in rapeseed hybrid breeding. The Polima (pol) CMS system is associated with the mitochondrial chimeric gene orf224, yet how it disrupts early anther differentiation and leads to sterility remains unclear. We combined cytological analyses, ultrastructural observations, transcriptomics, proteomics, and metabolomics to examine young anther development in pol CMS, maintainer, and restorer lines, with particular focus on mitochondrial function, metabolism, and hormone homeostasis. In pol CMS, L2-derived anther cells failed to differentiate into the mi

BACKGROUND: Limited availability of corticotropin-releasing hormone (CRH) currently complicates the differentiation of adrenocorticotropin (ACTH)-dependent Cushing's syndrome (CS). The diagnostic value of common screening tests in distinguishing Cushing's disease (CD) from ectopic CS (ECS) remains unclear.
OBJECTIVE: To assess the diagnostic performance of screening tests, alone and in combination, in differentiating CD from ECS.
METHODS: Retrospective multicenter study enrolling patients with confirmed ACTH-dependent CS and available screening tests at diagnosis. Data are expressed as multi

Autosomal recessive hypophosphatemic rickets type 2 (ARHR2) is a rare metabolic bone disorder caused by biallelic loss-of-function variants in ENPP1, the gene encoding for Ectonucleotide pyrophosphatase/phosphodiesterase 1 (ENPP1), an enzyme essential for generating extracellular pyrophosphate, a key endogenous inhibitor of ectopic calcification. The clinical spectrum of ENPP1 deficiency varies widely, and recognition in adulthood is challenging due to phenotypic heterogeneity and overlap with more common metabolic bone and parathyroid disorders. We describe two brothers in their late 40s with

The spontaneous adventitious bud (SAB) mutant of sugarcane, which forms clustered buds on the stem, provides a unique system for elucidating the mechanisms underlying in vivo regeneration and plant meristem differentiation anomalies. The present study investigated the mechanism underlying SAB formation in the sugarcane mutant 'YZ2018-81' through an integrated approach combining histological analysis, endogenous hormone profiling, and RNA-sequencing. Histological analysis revealed that the adventitious buds originated from the dedifferentiating cortical parenchyma cells, following a development
