X-linked hypophosphataemia diagnosed in adulthood with coexisting seronegative rheumatoid arthritis: a diagnostic pitfall.

INTRODUCTION: X-linked hypophosphataemia (XLH) is the most common inherited cause of renal phosphate wasting, typically diagnosed in childhood but occasionally identified in adulthood. CASE REPORT: We report a case of a 45-year-old woman with long-standing lower limb deformities, short stature, and diffuse joint pain. Laboratory evaluation revealed hypophosphataemia due to isolated renal phosphate wasting. Fibroblast growth factor 23 levels were markedly elevated. Imaging showed skeletal deformities and enthesopathic changes consistent with chronic XLH. Genetic testing identified a pathogenic

Phenotypic spectrum of 1q21.1 copy number variants in 42 children: genotype-phenotype correlations and clinical implications.

UNLABELLED: Chromosome 1q21.1 copy number variants (CNVs) are associated with a broad and variable clinical spectrum encompassing neurodevelopmental, anthropometric, and systemic anomalies. The incomplete penetrance and variable expressivity of these rearrangements pose significant challenges for clinical interpretation and genotype-phenotype correlation. We retrospectively analyzed 42 pediatric patients with 1q21.1 CNVs, identified by chromosomal microarray analysis (CMA), evaluated at a single tertiary center between 2019 and 2025. Patients were classified according to CNV type and localizat

Challenging Genetic Diagnosis in Siblings With Primordial Dwarfism, Muscle Wasting, Osteogenesis Imperfecta, Craniofacial Dysmorphism, and Ectodermal Anomalies.

Three siblings of consanguineous Pakistani parents have primordial dwarfism with microcephaly, failure to thrive, delayed developmental milestones, thin and lean physique, dysplastic limb joints, ectodermal anomalies, and craniofacial anomalies including dolichocephaly, micrognathia, and irregular dentition. Variable findings are muscular dystrophy and osteogenesis imperfecta. We hypothesized a novel syndrome and launched a disease gene search. At the gene locus identified by single-nucleotide polymorphism-based linkage mapping, genome sequencing detected a very rare intergenic variant g.13276

Height, Head Circumference, Sitting Height, Leg Length, and Body Proportions Growth References to the Argentinian and Spanish Population for Hypochondroplasia.

Hypochondroplasia (HCH) is a rare autosomal dominant skeletal dysplasia caused by gain-of-function variants in FGFR3, leading to disproportionate short stature. Available growth references are limited, and body proportion data is lacking. We conducted a retrospective cohort study of 131 children with molecularly confirmed HCH (72 males, 59 females) from Argentina and Spain. Longitudinal anthropometric data (height, sitting height, head circumference) from birth to 18 years were analyzed. Leg length and body proportion indices were derived. Age- and sex-specific centiles were constructed using

Long-Acting PEG-rhGH in Short Children Born SGA in China: 4-Year Results from a Randomized Phase 2 Study.

Introduction Children born small for gestational age (SGA) who failed to achieve catch-up growth are at risk of persistent short stature and associated metabolic complications. Daily recombinant human growth hormone (rhGH) therapy improves growth but is limited by adherence challenges. This study evaluated the long-term efficacy and safety of once-weekly PEGylated recombinant human growth hormone (PEG-rhGH; Jintrolong) in short children born SGA. Methods This multicenter, randomized, open-label, dose-response phase 2 study, comprised a 52-week main phase and an ongoing extension (NCT02375620),

Genotype-phenotype variability in TRMT10A deficiency: divergent developmental outcomes associated with biallelic loss-of-function variants.

Loss-of-function variants in TRMT10A are known to cause a rare autosomal recessive disorder primarily characterized by microcephaly, short stature, and impaired glucose metabolism. However, the extent of phenotypic variability and the factors influencing clinical heterogeneity remain poorly understood. Here, we describe two individuals harboring distinct biallelic loss-of-function variants in TRMT10A who exhibited divergent developmental phenotypes. The first patient, a 34-year-old man with a homozygous early frameshift variant, presented with preserved cognitive function and previously unrepo

Diagnostic utility of IGF-1/IGFBP-3 ratio and the insulin tolerance test in diagnosing growth hormone deficiency.

PURPOSE: Diagnosing Growth Hormone Deficiency (GHD) is challenging due to pulsatile GH secretion and high false-positive rates in traditional stimulation tests. This study evaluates the diagnostic utility of the IGF-1/IGFBP-3 molar ratio as a complementary tool to the gold-standard Insulin Tolerance Test (ITT). METHODS: A retrospective study analyzed 225 pediatric patients (aged 2-18) evaluated for short stature between 2015 and 2025. Confirmed GHD required a peak GH < 7 ng/mL across two provocation tests and a positive response to rhGH therapy. Serum IGF-1 and IGFBP-3 levels were quantified

Etiological Diagnoses Versus Descriptive Developmental Diagnoses.

Jane is a 7-year-old girl who presented for a developmental-behavioral pediatrics (DBP) evaluation because of parental concern about developmental delays and atypical behavior. Jane is a former term infant who was small for gestational age. She experienced some initial feeding difficulties and was discharged home from the nursery at 5 days of age. As a toddler, Jane was mildly delayed in acquiring gross motor developmental milestones, but she was more significantly delayed in acquiring fine motor and adaptive developmental milestones. She began receiving private occupational therapy (OT) servi

Final adult height in patients with congenital adrenal hyperplasia due to 21-hydroxylase deficiency.

PURPOSE: Congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency (21OHD) can impair linear growth and reduce final adult height (FAH). However, long-term growth outcomes have not been well characterized in Asian populations. This study evaluated FAH and identified clinical and treatment-related factors associated with short stature and height gain in Korean patients with 21OHD. METHODS: This retrospective multicenter cohort study included 119 patients with 21OHD (59 males, 60 females) who reached FAH. Logistic regression identified risk factors for short stature. The effect of g

Pediatric Fabry disease revealed by early neuropathic symptoms: a case report.

Fabry disease (FD) is a rare X-linked lysosomal storage disorder caused by α-galactosidase A deficiency, resulting in progressive glycosphingolipid accumulation and multisystem morbidity. Early pediatric manifestations are frequently overlooked, delaying treatment and increasing the risk of irreversible organ damage. We describe a 14-year-old boy presenting with acroparesthesia, recurrent abdominal pain, exercise intolerance, and anhidrosis since early childhood. Clinical evaluation revealed peripheral neuropathy and short stature. Biochemical testing demonstrated nearly absent α-galactosidase

Molecular Control of Replication Fork Speed: dNTP Availability, Phosphorylation, PARylation, and UFMylation.

The regulation of replication fork (RF) speed is essential for faithful genome duplication and the maintenance of genome stability, and its dysregulation contributes to carcinogenesis and developmental disorders. Although many anticancer therapies perturb DNA replication, the molecular mechanisms by which RF speed is controlled remain incompletely understood. A major limitation has been the frequent omission of direct measurements of cellular deoxynucleotide triphosphate (dNTP) pools, despite their strong and immediate influence on DNA polymerase kinetics. In this review, we first summarize ho

Multidisciplinary pathogenicity assessment of congenital hypothyroidism-associated SLC5A5 gene variants.

Next-generation sequencing (NGS) has markedly expanded the mutational spectrum of monogenic congenital hypothyroidism (CH), but has also led to the identification of numerous variants of uncertain significance, limiting the clinical utility of genetic testing. We previously developed an SLC5A5-specific machine learning-based predictor that improved the interpretation of missense variants associated with CH. Here, we systematically evaluated the pathogenicity of all CH-associated SLC5A5 variants reported in the literature using an integrated multidisciplinary framework based on the American Col
