Likelihood of lung transplantation before and after introduction of the lung composite allocation score.

BACKGROUND: Continuous distribution (CD), implemented in March 2023, marked the most substantial revision to United States (U.S.) lung allocation since the lung allocation score (LAS) was introduced in 2005. We evaluated whether inequities observed under LAS changed after CD. METHODS: We conducted a retrospective study of 5887 adult candidates listed for lung transplant in the U.S. during the pre-CD (March 8, 2021-March 8, 2022) and post-CD (March 9, 2023-March 8, 2024) eras using Organ Procurement and Transplantation Network data. Multivariable competing risk models identified factors associ

Anesthesia management of thoracic giant tumor resection: two case reports.

Surgical excision remains the primary therapeutic approach for giant tumors located within the thoracic cavity. However, these tumors frequently induce compression of critical structures such as the airways, lungs, heart, and vena cava, thereby posing significant challenges for perioperative respiratory and hemodynamic management. This report details the anesthetic management strategies employed in two patients undergoing resection of giant thoracic tumors: one patient with dwarfism and another with severe respiratory and circulatory insufficiency. These cases offer important strategies for ai

Integrating next-generation sequencing with biochemical screening to optimize first-tier newborn screening systems.

Newborn screening (NBS) is a critical public health initiative aimed at the early detection of congenital disorders. The emergence of next-generation sequencing (NGS) holds the potential to expand the spectrum of diseases detectable at birth. To evaluate the feasibility of integrating NGS into existing biochemical screening workflows, we conducted a prospective pilot study involving 1,810 newborns. In parallel with routine biochemical screening, we employed two NGS strategies—multiplex PCR-NGS (M-NGS) and target-capture NGS (T-NGS)—to compare their performance. A total of 32 cases were confirm

Longitudinal functional lung imaging in children with Post-COVID-19 syndrome.

BACKGROUND: Respiratory distress and COVID-19-related symptoms persist as Post-COVID-19 syndrome (PCS) in a proportion of children and adolescents. We aimed to determine whether ventilation or perfusion defects constitute a possible cause for PCS. RESULTS: Mean ventilation was lower at baseline in vaccinated than in PCS (p=0.04) and V/Qmatchnon-defected increased from baseline to follow-up in vaccinated (p=0.03). In post-hoc comparison with historic data, V/Qmatchnon-defected improved in recovered (historic: 63.5±18.7%; baseline: 88.8±9.0%, p<0.0001; follow-up: 88.1±10.6%; p=0.0002) and PCS (h

Oral immunization with recombinant Lactococcus lactis expressing VP3 and VP3-IFNα fusion protein protects ducklings against novel goose parvovirus infection.

The novel goose parvovirus (NGPV), an emerging variant of goose parvovirus, is the causative agent of short beak and dwarfism syndrome (SBDS) in ducks, posing a substantial threat to the global waterfowl industry. There is an urgent need for safe and convenient vaccines that can effectively induce mucosal immunity. This study aimed to develop and evaluate the immunogenicity and protective efficacy of an oral vaccine based on recombinant Lactococcus lactis (L. lactis) expressing the major capsid protein VP3 of NGPV, with or without fusion to duck interferon-alpha (IFNα). Trajectory-based analysis of growth in congenital adrenal hyperplasia: therapeutic implications for final height preservation.

UNLABELLED: Congenital adrenal hyperplasia (CAH) is an autosomal recessive genetic disorder resulting in a dysfunction of the steroidogenesis pathway and glucocorticoid deficiency. Proper care demands not only preventing acute complications but also optimizing growth and sexual development. This study was set out to analyze final height metrics and the factors affecting them as a long-term outcome measure in CAH patients. This observational retrospective study evaluated almost 3000 visits of 63 CAH patients treated in one center between 1996 and 2025, who reached their near-final height (nFH).

Evaluating the Effectiveness of Early Genetic Screening for Fanconi Anemia in High-Risk Pediatric Populations.

INTRODUCTION: Fanconi anemia (FA) is the most prevalent inherited disorder leading to bone marrow failure, resulting from a rare autosomal recessive genetic condition that affects all three types of blood cells. A key characteristic of FA is the body's heightened sensitivity to DNA-damaging agents, particularly those that induce crosslinking, which serves as an important diagnostic marker. Children at higher risk-such as those with unexplained growth delays, congenital defects, or a family history of FA-can significantly benefit from genetic testing. METHODS: This study involved 140 pediatric

Consensus statement on the management of Achondroplasia in the Middle East.

Achondroplasia is the most common skeletal dysplasia and presents as a lifelong, multi-system condition characterized by disproportionate short stature, rhizomelic limb shortening, macrocephaly with frontal bossing, and a spectrum of neurological, respiratory, orthopedic, and psychosocial complications. Effective management requires anticipatory surveillance, coordinated multidisciplinary care, and early recognition of life-threatening complications. Therapeutic options have expanded in recent years with the introduction of vosoritide, a C-type natriuretic peptide analog that targets the under

Thermospermine signaling in vascular development and stress adaptation: from translational regulation to climate-resilient crops.

Thermospermine (tSpm) is an asymmetric structural isomer of spermine found to accumulate in vascular plants and certain thermophilic bacteria. Since identification of ACAULIS5 (ACL5) as the enzyme responsible for tSpm biosynthesis in Arabidopsis thaliana, tSpm has emerged as a critical post-transcriptional regulator of vascular development, balancing xylem vessel differentiation and maintenance of procambial cell identity. Loss-of-function acl5 mutants display severe dwarfism and excessive xylem proliferation, whereas complementary gain-of-function approaches show that tSpm restrains auxin-dri

Prototypical transfer learning framework for small-sample 1H NMR metabolomics: Accurate and interpretable classification of short stature.

Short stature (SS) is a common pediatric endocrine disorder, primarily including growth hormone deficiency (GHD) and idiopathic SS (ISS). Nuclear magnetic resonance (NMR)-based metabolomics can reveal metabolic signatures associated with SS. However, the limited availability of clinical data results in small sample sizes and high-dimensional features, which constrain the stability and generalizability of traditional methods. In this study, a prototypical transfer learning approach for small-sample metabolomics data is proposed. By integrating a shared feature encoder, cross-domain feature alig
