BACKGROUND: Phlegmon is a severe skin and soft tissue infection (SSTI) that can escalate rapidly if not promptly recognized and treated. Its early presentation may appear deceptively mild, potentially delaying recognition of its severity. Without timely intervention, phlegmon can extend into deeper tissues and result in life-threatening systemic complications. Although SSTIs are common, progression from a seemingly minor scratch injury to a fulminant upper extremity infection complicated by compartment syndrome and multiorgan failure is uncommon. This case is reported to highlight the potentia

BACKGROUND: Tattooing is increasingly common and generally considered safe but can rarely result in infectious complications ranging from localized skin infection to severe systemic illness. Toxic shock syndrome (TSS) is a rare, yet life-threatening condition caused by toxin-producing Staphylococcus aureus or Streptococcus pyogenes. Tattoo-associated TSS is uncommon, and awareness among emergency physicians may be limited, potentially delaying diagnosis and treatment.
CASE REPORT: We describe a case of TSS in a previously healthy 21-year-old man following extensive tattooing of the upper arm.

Puerperal infection with group A streptococcus (GAS), Streptococcus pyogenes , is associated significant morbidity and mortality. When associated with toxic shock syndrome (TSS), mortality rates rise to approximately 50%. We present the case of a 32-year-old Para 2 reporting severe left distal lower extremity pain, fevers, and chills at 1 week following an uncomplicated vaginal delivery. The patient's clinical status rapidly decompensated to septic shock requiring transfer to the intensive care unit. She underwent anterior and lateral compartment fasciotomy of the left lower extremity for conc

PURPOSE: Invasive Streptococcus pyogenes (iGAS) infections have risen globally since the COVID-19 pandemic. This study aimed to characterize clinical-pathological patterns in iGAS presenting as unexpected deaths and to evaluate the role of post-mortem microbiology alongside clinical and autopsy findings in determining cause of death. METHODS: Anonymized clinical, microbiological, and pathological (macroscopy and/or histopathology) data from fatal cases with positive S. pyogenes cultures, reported in four ESGFOR laboratories between September 2020 and May 2023, were reviewed. RESULTS: Forty cas

OBJECTIVES: Parapneumonic pleural effusions/empyema (PPE/PE) are serious complications of community-acquired pneumonia in children. Although Streptococcus pneumoniae and Streptococcus pyogenes are established major pathogens, the clinical relevance of oral cavity bacteria (viridans streptococci and/or anaerobic bacteria [VS/AA]) in paediatric PPE/PE is largely unclear.
METHODS: A nationwide hospital-based surveillance study in Germany recorded children aged <18 years with PPE/PE from October 2010 to June 2023, with bacteria detected from pleural fluid and/or blood. We compared clinical charac

Streptococcal toxic shock syndrome in children is a severe complication of group A Streptococcus infection, characterized by acute shock and multiple organ dysfunction. It is a critical illness with high mortality, and early diagnosis and treatment are key to improving prognosis. Children with this syndrome have a higher risk of death, and the pathogenesis is complex. This review summarizes the pathogenesis and recent advances in the diagnosis and treatment of streptococcal toxic shock syndrome caused by group A Streptococcus infection in children, aiming to improve clinical outcomes and reduc

Tubo-ovarian abscess (TOA) is an exceptionally rare diagnosis in prepubertal girls and typically arises through mechanisms distinct from pelvic inflammatory disease. We report the case of a girl in her early childhood with severe obesity and clinical features suggestive of Prader-Willi syndrome who presented with acute abdominal pain and systemic sepsis. Diagnostic laparoscopy was undertaken for suspected perforated appendicitis and revealed a ruptured TOA caused by Group A Streptococcus Surgical drainage and targeted antimicrobial therapy resulted in complete recovery. This case highlights th

BACKGROUND: Intracranial infection caused by Streptococcus pyogenes is extremely rare in adults. We report a case of Streptococcus pyogenes combined with Epstein-Barr virus (EBV) infection in cerebrospinal fluid (CSF), which was diagnosed by targeted next-generation sequencing (tNGS). The findings were subsequently confirmed by capillary electrophoresis (CE) and quantitative real-time PCR (qPCR), respectively. Meanwhile, digital PCR (dPCR) detection indicated the presence of Streptococcus species DNA in the blood. Brain magnetic resonance imaging (MRI) suggested the possibility of meningitis.

INTRODUCTION: Necrotizing fasciitis (NF) following minor trauma in healthy individuals is rare and poses a considerable diagnostic challenge. Due to its nonspecific early presentation, NF is associated with high rates of misdiagnosis and mortality, underscoring the critical need for early recognition and intervention. CASE PRESENTATION: A previously healthy 37-year-old male developed rapidly progressive necrotizing fasciitis of the scrotum and right lower extremity following minor bicycle-related abrasions. Upon admission, the patient presented with a high-risk Laboratory Risk Indicator for Ne

A global increase in severe group A Streptococcus (GAS) infections has been reported following the COVID-19 pandemic, but data from Asia remain limited. We examined epidemiology and clinical characteristics of severe paediatric GAS infections across 86 Japanese hospitals, focusing on patients under 18 years of age, hospitalized between 1 January 2019 and 31 March 2024. Severe GAS infection was defined by the isolation of GAS from sterile sites, or from non-sterile sites under specific conditions, such as streptococcal toxic shock syndrome (STSS). A total of 83 cases were analysed. Cases increa

Invasive group A streptococcal disease (iGAS) can present with severe manifestations, including sepsis, necrotizing fasciitis, and streptococcal toxic shock syndrome. Although β-lactam antibiotics are considered standard therapy, the role of adjunctive treatments, particularly clindamycin and intravenous immunoglobulin (IVIG), in children with these severe manifestations remains debated. Clindamycin is effective irrespective of pathogen load and is suggested to inhibit toxin production, while IVIG is hypothesized to modulate the immune response through multiple mechanisms, including neutraliza

INTRODUCTION: Streptococcus pyogenes (S. pyogenes) can cause streptococcal toxic shock syndrome (STSS), especially in immunocompromised individuals. Early diagnosis is the key to treat STSS effectively. This study presents a fatal case of STSS caused by the emerging M1(UK) S. pyogenes lineage in a patient with type 2 diabetes mellitus (T2DM), alongside a comprehensive literature review. CASE PRESENTATION: A 65-year-old man with T2DM presented with altered consciousness, necrotizing soft tissue injury and was diagnosed with STSS. Bacterial cultures were obtained from right lower limb exudate, r

Purpura fulminans (PF) is a rare, life-threatening emergency characterized by rapidly progressive purpura evolving into hemorrhagic bullae and skin necrosis due to dermal microvascular thrombosis, typically accompanied by shock and disseminated intravascular coagulation (DIC). We report two fatal adult cases of infection-associated PF caused by Streptococcus pyogenes (group A streptococcus). Both patients presented with early gastrointestinal symptoms and rapidly developed septic shock, severe coagulopathy compatible with overt DIC, and extensive lower-limb purpura with bullae and necrosis. A Group A Streptococcus (GAS, Streptococcus pyogenes) is an exclusively human pathogen whose virulence is driven by a diverse array of surface structures, secreted toxins, and immune evasion mechanisms. Central to its pathogenicity is the M protein, a surface-anchored molecule that inhibits phagocytosis by interfering with complement deposition and binding host factors such as fibrinogen. GAS also secretes a wide range of toxins and enzymes that damage tissues and disrupt host defences. Streptolysin O and streptolysin S are potent cytolysins that lyse immune cells and contribute to tissue necr

Necrotising fasciitis (NF) is a rapidly progressing and life-threatening soft tissue infectious disease characterised by extensive necrosis of the fascia and subcutaneous tissue and is often accompanied by systemic toxic shock and multiple organ dysfunction. In recent years, the incidence rate of invasive Streptococcus pyogenes (GAS)infection has increased, and some strains exhibit high virulence and drug resistance, increasing the difficulty of clinical treatment. We aim to provide more precise and efficient nursing solutions for patients with necrotising fasciitis through the whole process m

BACKGROUND: Group A streptococcus (GAS) could lead to various disease types in children, but central nervous system (CNS) infections are uncommon. In this paper, we analyzed the clinical features of a GAS case with meningitis and subdural empyema, and characterized the GAS clone. CASE PRESENTATION: A thirteen-year-old boy complained of fever, headache, and left hemiplegia. Physical examination also showed central facial palsy of left side. The examinations of blood and cloudy cerebrospinal fluid (CSF) showed bacterial meningitis. Blood cultures and metagenomic sequencing (mNGS) of CSF showed G

In spring 2024, 2 otherwise healthy men were diagnosed with invasive Group A Streptococcus (iGAS) meningitis at autopsies performed at 2 separate southern Wisconsin (WI) offices. As iGAS is a rare cause of bacterial meningitis, this raised concern for an outbreak and/or increased organism virulence. The 2 offices retrospectively searched their databases for iGAS deaths from 2002 to 2024 and collaborated with the WI State Laboratory of Hygiene (WSLH) and WI Department of Health Services (WI-DHS) to assess corresponding statewide iGAS activity. The search revealed a total of 19 iGAS deaths (age
