At our hospital (Jefferson Davis Hospital, Houston, Tex.) since 1979, non-group D alpha-hemolytic streptococci have been isolated with increasing frequency from neonatal blood cultures with clinical findings of sepsis. A total of 47 such isolates were identified to the species level by the scheme of Facklam and were compared with 57 genital isolates from 167 maternity patients. Among the genital isolates, S. sanguis II and S. MG-intermedius accounted for 53 and 28%, respectively, and both were significantly less common in neonatal cultures (23 and 11%, respectively; P less than 0.05). Among ne

Studies of group A, M type 49 streptococci from England, Trinidad and Alaska indicate that isolates of this serotype often differ with respect to phage subtype from one geographical area to another, but are generally homogeneous in one place at one time. The findings support the conclusion that acute glomerulonephritis can be associated with a variety of phage subtypes of M type 49 streptococci. In outbreaks of skin sepsis without nephritis in England, the phage subtypes of M type 49 streptococci isolated from skin lesions of meat handlers were the same as those recovered from skin lesions of

In two patients seen recently in North Yorkshire, systemic infection with Streptococcus pyogenes appeared to originate from the upper respiratory tract; such infection is now rarely reported from the more developed countries. A study was arranged to detect bacteraemia in patients with sore throat: of 343 patients tested 93 yielded beta-haemolytic streptococci from the respiratory tract, but streptococcal bacteraemia was detected in none. These results suggest that clinically unsuspected streptococcal bacteraemia is uncommon in such patients.

In the lifespan of a human being the two extremes, the early childhood and old age are prone to septicaemia because of poorly functioning anti-infectious defensive mechanisms. In a newborn full term baby these mechanisms are all present but still unexperienced. The importance of both specific and nonspecific factors will be discussed. The etiologic agents, causing septicaemia in nurseries undergo continuous change. In Helsinki we have during the last 20 years experienced staphylococci to start with, changing then to Gram-negative rods, to streptococci group B and now we have increasing difficu

Thirty-two patients with skin infections were treated with Augmentin, a combination of amoxycillin with the beta-lactamase inhibitor clavulanic acid. These infections were primary skin sepsis (7), infected eczema (11), infected trauma (10) and leg ulcers (4). The majority of cases were caused by amoxycillin-resistant Staphylococcus aureus either alone or in combination with Streptococcus pyogenes. Thirty patients (94%) responded to treatment with only one withdrawal (for side effects). Side effects were limited to nausea (9%) diarrhoea (9%) and rash (3%). No patient with diarrhoea showed evide

Pneumococci have been shown to induce granulocytopenia and pulmonary leukostasis which might contribute to morbidity and mortality in pneumococcal sepsis. We studied whether other gram-positive species (groups A and B streptococci, Staphylococcus aureus, Bacillus cereus, and Clostridium perfringens) could also induce these phenomena. Rabbits were challenged with nonviable preparations of each species, and all five species induced profound granulocytopenia (mean decrease of 90%) and pulmonary leukostasis. In vitro studies of serum incubated with these species of bacteria showed a mean consumpti

Cefmenoxime (CMX) was evaluated in 25 children with a suspicion of bacterial infection. Of the 20 confirmed bacterial infections, 19 were cured by CMX therapy (effective rate, 95%). The diagnoses included acute pharyngotonsillitis (4), acute bronchitis (1), pneumonia (7), streptococcal dacryocystitis (1), infections accompanied with acute leukemia (4), and acute urinary tract infections (3). The etiologic pathogens were beta-hemolytic Streptococcus group A (1), and F (1), Staphylococcus aureus (4), Haemophilus influenzae (4), Escherichia coli (4), Klebsiella pneumoniae (2), etc. CMX was very e

In man streptococcal septicaemia is most often associated with the streptococci of Lancefield's groups, A, B and D, with pneumococci, with Streptococcus milleri and with viridans streptococci. The specific determinants of the ability of streptococci to establish infection and to invade the blood stream are only partially understood. Even if fully comprehended, they would provide an incomplete picture of the factors responsible for the capacity of streptococci to produce a myriad of disease states the clinical expression of which include such diverse manifestations as acute sore throat, impetig

Formerly, death due to burns was usually caused by shock in 50% of the cases. Today only 5% of the deaths are caused by shock; 95% of all deaths are due to infection and half of these deaths are due to septicaemia. The treatment of infections depends on two factors: -- Prevention of contamination -- Prevention of bacterial growth. Contamination can be prevented by early skin grafts, a no-touch method when applying dressings, nursing in ventilated rooms and local antiseptics. The history of the use of various antiseptics starting with penicillin V and ending with cerium nitrate is discussed. Fo

One of two children admitted with septicemia due to group A beta-hemolytic streptococcus died following a very fulminant course. This organism may cause overwhelming disease in newborn infants, as well as in children with diseases which compromise the immune system. Group A streptococcus, though very sensitive to penicillin, can cause severe and rapidly progressive illness even in previously normal children, unless recognized and treated promptly.

During a 4-year period, 48 obstetric patients with aerobic streptococcal bacteremia were identified. The isolates were group B (31); group D, enterococci (5); group A (4); group D, not enterococci (3); Streptococcus pneumoniae (3); and viridans streptococci (2). The most common diagnoses were endometritis (36) and chorioamnionitis (8). For all streptococcal genital infections, the clinical presentation included early-onset fever, often with few localizing signs. Only one patient had a chronic debilitating disease. There was an excellent correlation between genital and bloodstream isolates (36

Six cirrhotic patients are described in whom peritonitis developed from 10 days to 15 months after the insertion of a LeVeen shunt. The presenting clinical features of fever, increasing ascites and deterioration in mental status resembled these previously reported for cirrhotic patients but with spontaneous peritonitis without shunts. Important differences were observed, however, in microbiology, incidence of bacteremia and therapy. Staphylococcus aureus, a rare cause of spontaneous peritonitis, was found in one half of our cases, suggesting that perioperative contamination during shunt placem

The data presented in this study indicate that the Staphylococcus aureus is related casually to the toxic shock syndrome and that the dominant phage type was 29. This type has occurred singly as well as in combination with phage type 52 and some other group I phage types. The 29, 52 complex was present in approximately two-thirds of a series of 159 isolates of Staphylococcus aureus obtained from patients with toxic shock syndrome, while another fourth was nontypable. In further support of this concept was the evidence that 94.1 per cent of the isolates of the 29, 52 phage complex tested were c

We studied the possibility of detecting beta-hemolytic streptococcal infections by determining antibodies against the group specific polysaccharides from streptococci of groups A, A-variant, C and G. The methods used were radio-immunoassay with these four polysaccharides and hemagglutination of group A polysaccharide sensitized erythrocytes. Significant increases in these antibodies were rare in tonsillitis patients, whereas high concentrations of antibodies could often be observed in association with severe systemic infections or rheumatic fever. Both strictly immunogen-restricted and crossre

Outbreaks of wound infection with Streptococcus pyogenes and Staphylococcus aureus occurred in a abattoir and a pork-processing factory in Autumn 1980. Investigations showed that staff handling the meat before de-boning were particularly affected and that bone was the cause of 48% of the wounds which became clinicially infected. Of the total isolates of Strep. pyogenes and Staph. aureus from wounds of known cause 75% were from lesions caused by bone. In one outbreak streptococcal infection was seen principally in newly-employed staff. Group L streptococci were found in several sites in the por

Group A streptococcal pyrogenic exotoxins (SPEs) A, B, and C and alpha-amanitin enhance host susceptibility to lethal endotoxin shock. The capacity of SPE C and alpha-amanitin to prepare rabbits for the enhancement phenomenon required pretreatment of the animals 1 to 2 h before giving endotoxin. Endotoxin clearance from the circulation of rabbits pretreated with either SPE C or alpha-amanitin was reduced. Even at the time of death, significant amounts of endotoxin remained in the circulation. It is proposed that the SPE and alpha-amanitin inhibit ribonucleic acid synthesis in Kupffer cells wit
