An unusual cutaneous manifestation of group A streptococcal bacteremia.

A case of streptococcal bacteremia presenting with disseminated nodular lesions is presented. Although the incidence of group A streptococcal bacteremia has declined, the disease may affect both normal and immunologically compromised children. Early recognition and prompt antibiotic therapy are required to ensure a favorable outcome.

[Preliminary results of a bacteriologic study of the blood and urine of children with nephritis].

Single examination of the urine and blood by the method described in this paper was carried out in 14 children suffering from glomerulonephritis. L-form cultures were isolated in 11 cases. The cultures were isolated both at the active phase and during the remission. Three L-form cultures reverted into streptococci spontaneously. The revertants showed a sharp difference from the L-forms by the cell morphology and also by the colour and morphology of the colonies.

[Clinical and experimental studies of suppurative surgical infection in cattle].

Investigated were a total of 30 cattle 6 of which had experimental surgery infection and 24 showed spontaneously developing suppurative infection. The animals were studied in terms of general status, blood picture, quality of urine and synovia, and rumen content. The dynamis was followed up of the development of sepsis and purulent artritis. It was found that in case of suppurative surgical infections in cattle the general status was invariably affected, the severity of the process depending on its localization and developmental phase. Changes were observed also in the eye fundus and in the ur

Serological characterization of group-A streptococci associated with skin sepsis in meat handlers.

A series of outbreaks of skin sepsis among meat handlers in England during 1974 and 1975 afforded an opportunity to study the group-A streptococci commonly isolated from the lesions. Few of these streptococci could be M typed with existing antisera. Intensive study of strains from two outbreaks of sepsis in one abattoir in Shropshire led to the recognition of three new provisional M types. The streptococci were first sorted according to T-typing pattern and ability to produce opacity factor. Opacity-factor producing strains with the same T pattern were then screened for inhibition of opacity p

Insuring effective antimicrobial therapy: laboratory evaluation.

Specialized tests for the susceptibility of pathogenic microorganisms are not indicated for most clinical infections. Primary attention should be focused on proper collection of representative culture material and subsequent accurate identification of the offending organism. Specific antimicrobial therapy can then be empirically instituted based upon previously known susceptibility patterns. An overview of the broth dilution, agar dilution, and agar (disk) diffusion techniques is presented. The absolute indications for these tests are as follows: (1) infections with gram-negative bacilli; (2)

[Interrelationship between staphylococcal sensitization, the protective function of leukocytes and septic complications in the burned].

The state of staphylococcus sensitization and the index of completeness of staphylococcus phagocytosis was studied in 42 persons with severe burns. Development of staphylococcus complications was connected with marked specific microbial sensitization and disturbance of intracellular digestion of staphylococci by the patients' blood leukocytes. Detection of unfavourable interraltions of the antistaphylococcus immunity and allergy in the course of the disease can be of diagnostic and prognostic significance.

Group a beta-hemolytic streptococcal pneumonia: clinical course and complications of management.

Three childhood cases of beta-hemolytic streptococcal pneumonia are presented with a review of the literature. The disease, though uncommon in childhood, results in severe local and systemic effects. Hematologic fluid and electrolyte problems are frequent. Pericardial disease and secondary bacterial invasion are added dangers encountered in management. Early and vigorous drainage of the pleural cavity is essential to successful outcome. It appears that predisposing viral disease, including influenza, is essential for acquisition of this form of bacterial pneumonia.

Pulmonary changes in neonatal sepsis to group B beta-hemolytic Streptococcus: relation of hyaline membrane disease.

The radiographic changes of hyaline membrane disease were seen in seven of eight infants with group B beta-hemolytic streptococcal sepsis. Hyaline membranes were found throughout the lungs of all six infants who were examined after death. The most striking finding was the presence of group B streptococci within the membranes of five infants. Streptococci were so numerous in one instance that they comprised the bulk of the membrane. Another infant uas thought to have classical hyaline membrane disease until numerous group B streptococci were found within the membranes when lung sections were ex

Antibacterial activity of oxidized regenerated cellulose.

It has been demonstrated that oxidized regenerated cellulose promptly and markedly reduces the bacterial census of ten different strains of common pathogens when exposed to them in vitro. This was not true of two other hemostatic agents tested, namely, absorbable gelatin sponge and topical thrombin. Wounds in guinea pigs in which oxidized regenerated cellulose was placed and infected with one of three pathogens had healing per primum in 39 of 40 instances. In similar control wounds and incisions in which absorbable gelatin sponge was placed, sepsis of the wound developed in 19 of 20 of the con

Serious group A streptococcal diseases in children.

The illnesses of 40 patients with diagnoses of septicemia, cellulitis with bacteremia, pneumonia empyema, and meningitis caused by Streptococcus pyogenes, Group A, are described. Twenty-five of 27 patients (93%) without underlying disease survived, whereas only seven of 13 children (54%) with underlying disease survived. Nine of the 25 patients who were otherwise normal and who survived these infections had prolonged, complicated illnesses. Four of these patients, and one who died, had septicemia without a focus of infection at the time of admission. Streptococcus pyogenes, Group A, although v

Bacteremic infections due to clindamycin-resistant streptococci.

We have recently observed four patients with bacteremic infections due to group A streptococci that were highly resistant to clindamycin and only moderately susceptible to erythromycin. Two of the infections developed while the patients were receiving clindamycin. The organisms were not beta-hemolytic under standard conditions nor were they M or T typable. We urge that all streptococci isolated from patients with serious infections be grouped serologically and be tested for antibiotic susceptibilities, especially if therapy with antibiotics other than penicillin is to be considered.

Streptococci and aerococci associated with systemic infection in man.

A collection of 820 isolates of streptococci and aerococci from human blood or internal organs was classified by means of a set of cultural and biochemical tests. Of these, 719 (712 streptococci and 7 aerococci) were from patients believed to be suffering from a systemic infection: endocarditis 317; a purulent lesion in an internal organ ("purulent disease") 152; bacteriaemia 250. The endocarditis streptococci included members of all recognisable taxa, but only certain of these were common: Streptococcus sanguis, 16-4%; dextran-positive and usually mannitol-fermenting strains of S. bovis (S. b
