Narrative Review: Sugar and Rice and the Diabetes Epidemic in India-A Historical Context.

South Asians appear to be particularly susceptible to diabetes. India hosts 18 percent of the world's population but more than 25 percent of the world's diabetics, and individuals of South Asian descent carry this presumed increased risk for diabetes when they emigrate to other parts of the world. One conundrum is that the epidemic of diabetes began around Calcutta (modern day Kolkata) in east India well before it appeared in the United States and Europe, and this emergence occurred despite the frequent occurrence of famines and starvation in India. Here we review the history of diabetes in In

Kwashiorkor Mimicking Portal Hypertensive Ascites After Bariatric Surgery.

Kwashiorkor is a severe form of protein-calorie malnutrition. It is rarely observed in well-developed countries and is a potential complication of bariatric surgery. We report a case of kwashiorkor presenting as new-onset hepatic steatosis and high serum-ascites albumin gradient, low total protein ascites in an adult 2 years after Roux-en-Y gastric bypass with heavy alcohol use, and pancreatic insufficiency. The ascites resolved with parenteral nutrition, alcohol cessation, and pancreatic enzyme supplementation. This case highlights the diagnostic and pathophysiologic complexity of malnutritio

Predictors of Changes in Weight and Body Composition in Undernourished Children in Mozambique: A Longitudinal Study.

Undernutrition is a major public health challenge in Mozambique, requiring a better understanding of changes in weight and body composition. In the present study, changes in weight and body composition were analyzed in undernourished children, and predictors of these changes were identified during and after hospitalization. A longitudinal cohort study was conducted in Maputo, Mozambique (2018-2020), in children aged 6 to 58 months hospitalized for malnutrition. On hospital admission, clinical data, including anemia, edema, and HIV infection status, were collected. Anthropometric measurements,

Ultra-High-Burden TP53 Mutation (VAF 92%) in Acute Myeloid Leukaemia: A Fatal Case with Rapid Clinical Progression.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Micronutrient status in children aged 6-59 months with severe wasting and/or nutritional edema: implications for nutritional rehabilitation formulations.

Undernutrition remains a global struggle and is associated with almost 45% of deaths in children younger than 5 years. Despite advances in management of severe wasting (though less so for nutritional edema), full and sustained recovery remains elusive. Children with severe wasting and/or nutritional edema (also commonly referred to as severe acute malnutrition and part of the umbrella term "severe malnutrition") continue to have a high mortality rate. This suggests a likely multifactorial etiology that may include micronutrient deficiency. Micronutrients are currently provided in therapeutic f

The performance of upper arm circumference for age in diagnosing severe acute malnutrition in children aged 6 to 59 months in South Kivu, Eastern Democratic Republic of Congo: Lwiro Cohort.

BACKGROUND: The studies on the use and performance of the Mid-Upper Arm Circumference for age (MUACZ) for the diagnosis of severe acute malnutrition (SAM) are still rare. Our study aimed to analyze the performance of MUACZ for diagnosis of SAM in South Kivu, eastern DR Congo. METHODS: We analyzed a database of children admitted from 1987 to 2008 for management of SAM in the east of the DRC. Anthropometric indicators (z-score) were calculated and classified according to the standards of the World Health Organization (WHO). To evaluate the performance of MUACZ using the combination of weight-fo

Kwashiorkor during pregnancy: A rare complication of bariatric surgery with fetal health consequences.

Bariatric surgery, while beneficial for managing obesity and improving fertility, can lead to serious nutritional deficiencies that impact maternal and fetal health. We report the case of a 31-year-old woman with a history of gastric bypass who developed Kwashiorkor, a rare and severe complication of malnutrition, during the second trimester of her twin pregnancy. This condition was associated with severe intrauterine growth restriction. Kwashiorkor, typically seen in young children suffering from famine in low-income regions, is characterized by severe protein deficiency. In this case, parent

Understanding the medical challenges for the diagnosis and treatment of bilateral pitting oedema in children: a qualitative study.

Severely malnourished patients can present with bilateral pitting oedema, which is a common sign of Kwashiorkor. However, bilateral pitting oedema can also be an expression of other pathologies. In Mali and DRC, the number of children presenting with bilateral pitting oedema at MSF (Médecins Sans Frontiers/Doctors Without Borders) hospitals are up to 30% (Mali) and 49% (DRC) higher than in other countries, however, the reasons underlying this trend are unknown. Through this qualitative study, we aimed to explore the perspectives and lived experiences of health professionals on the diagnosis an

Aberrant One-Carbon Metabolism and Ancestral Genetics Underlie Edematous Severe Acute Malnutrition.

Severe acute malnutrition (SAM) contributes to the death of millions of children under age five annually. SAM is clinically classified as non-edematous SAM (NESAM) or the more severe edematous SAM (ESAM), which is more common in east-central Africa and the Caribbean. The reason some children develop ESAM while others develop NESAM remains unclear; however, recent studies have identified aberrant one-carbon metabolism (OCM) in ESAM relative to NESAM. Here, we assess genetic variants at 103 loci known to influence OCM, and determine their association with ESAM in 711 samples from Jamaica and Mal

Extensive striae and cellophane-like skin in an adolescent case of restrictive anorexia nervosa: a rare dermatologic manifestation of extreme malnutrition.

BACKGROUND: Anorexia nervosa (AN) is a severe psychiatric disorder associated with extreme malnutrition and multi-organ complications, including cutaneous manifestations. CASE PRESENTATION: We report an 11-year-old girl with restrictive-type AN who presented with a critically low BMI (7.7-9.0 kg/m2), who presented with striking cutaneous findings. These included widespread striae distensae with a translucent, "cellophane-like" appearance, desquamative lesions reminiscent of the "flaky paint" dermatosis seen in kwashiorkor, and sacral pressure ulcers. Laboratory findings revealed severe protei

Duration of recovery from severe acute malnutrition and associated factors in children aged 6-59 months: a retrospective cohort study.

Malnutrition, particularly severe acute malnutrition (SAM) in children under five, is a global crisis affecting 25-35 million children, especially in Africa and Southeast Asia. In Ethiopia, 21% are underweight, 36% stunted, and 7% wasted. This study assesses recovery time from SAM in children admitted to public hospitals in Jimma Town. We conducted a retrospective study with 323 participants, collecting data from medical charts and analyzing it in Stata version 15. Used Kaplan-Meier for survival analysis and Cox regression for predictors of hospital stay, considering p-values < 0.05 as signifi

Model-Informed Dose Selection for a Novel Human Immunoglobulin G4 Derived Monoclonal Antibody Targeting Proprotein Convertase Kwashiorkor Type 9: Insights from Population Pharmacokinetics-Pharmacodynamics and Systems Pharmacology.

Monoclonal antibody drugs targeting proprotein convertase kwashiorkor type 9 (PCSK9) have recently demonstrated remarkable success in lipid-lowering therapies. Specifically, antibodies derived from immunoglobulin G1 (IgG1, alirocumab) and IgG2 (evolocumab) have been successfully utilized for this purpose. Recently, a novel recombinant fully human anti-PCSK9 monoclonal antibody, originally derived from IgG4 and designated as SAL003, was developed. This study aimed to explore the pharmacokinetics, efficacy, and safety of SAL003 in both single and multiple administrations. The investigation inclu

Arm circumference for age, arm circumference and weight-for-height z-score for the evaluation of severe acute malnutrition: a retrospective cohort study in eastern Democratic Republic of Congo.

BACKGROUND: Little is known about the use of mid-upper arm circumference for age (MUACZ) for diagnosing of severe acute malnutrition (SAM) and its correlation with WHZ (weight-for-height Z-score) in an area endemic for severe acute malnutrition (SAM) and with a high prevalence of kwashiorkor. Our study aims to analyze the concordance between the diagnostic criteria of SAM in a region presenting these characteristics. METHODS: We analyzed a database of children admitted from 1987 to 2008 for the management of SAM in Eastern Democratic Republic of Congo. Anthropometric indicators (z-score) were

Childhood growth during recovery from acute illness in Africa and South Asia: a secondary analysis of the childhood acute illness and nutrition (CHAIN) prospective cohort.

BACKGROUND: Growth faltering is well-recognized during acute childhood illness and growth acceleration during convalescence, with or without nutritional therapy, may occur. However, there are limited recent data on growth after hospitalization in low- and middle-income countries. METHODS: We evaluated growth following hospitalization among children aged 2-23 months in sub-Saharan Africa and South Asia. Between November 2016 and January 2019, children were recruited at hospital admission and classified as: not-wasted (NW), moderately-wasted (MW), severely-wasted (SW), or having nutritional oed

Determinants of Undernutrition among Children Admitted to a Pediatric Hospital in Port Sudan, Sudan.

Severe acute undernutrition (SAU) is still a crucial global health issue in the 0-59 months population, increasing the risk of mortality as well as of long-term consequences. In Sudan, 3.3 million children suffered from acute malnutrition between 2018 and 2019. This study was planned to evaluate, in the area of Port Sudan, the prevalence of acute undernutrition after the COVID-19 pandemic and to identify the most important factors favoring the development of acute undernutrition. The available clinical records of all the under-five children (n = 1012) admitted to the Port Sudan Emergency Pedia
