Prolonged breast-feeding: no association with increased risk of clinical malnutrition in young children in Burkina Faso.

Reported are our findings from a case-control study of the association between prolonged breast-feeding and clinical malnutrition in an urban setting in West Africa. The cases were children aged 12-36 months who had been hospitalized with a diagnosis of clinical malnutrition. Children of a similar age who lived in neighbouring courtyards were recruited as controls. For 152 case-control pairs in which both children were receiving solid foods, non-breast-feeding was associated with an increased risk of clinical malnutrition (crude odds ratio = 2.37; 95% confidence interval = 1.24, 4.55). This as

Protein-losing enteropathy in Transkeian children with morbid protein-energy malnutrition.

A commercially available radial immunodiffusion assay was used to measure serum and faecal alpha 1-antitrypsin concentrations as well as alpha 1-antitrypsin clearance in 17 children with kwashiorkor, 11 children with marasmic kwashiorkor, 10 children with marasmus, and 16 normal children. Serum alpha 1-antitrypsin concentrations were significantly higher than normal in the marasmus and marasmic kwashiorkor groups, and significantly lower than normal in the kwashiorkor group. The intestinal clearance of alpha 1-antitrypsin was significantly higher than normal in the marasmus and marasmic kwashi

Elevated glutathione S-transferase activity in erythrocytes from malnourished children.

Glutathione S-transferases (GSTs) are principally involved in detoxification. These enzymes can be induced by an increased flux of substrate, such as occurs during pro-oxidative stress or antioxidant deficiency. We tested the hypothesis that the postulated oxidative stress in severe malnutrition would result in induction of GSTs in erythrocytes. Erythrocyte GST activity towards 1-chloro-2,4-dinitrobenzene (CDNB) was measured in 271 malnourished children (22 undernourished; 92 marasmic; 82 kwashiorkor; 75 marasmic-kwashiorkor) and 48 healthy children. GST activity in the malnourished children w

Bacteraemia in severely malnourished children.

During a 2-year period, 792 severely malnourished children (kwashiorkor, marasmus and marasmic kwashiorkor) were hospitalized (14.8% of all admissions). Community-acquired bacteraemia was recorded in 7.7% and nosocomial bacteraemia in 2.2% of malnourished children. The relative risk for bacteraemia on admission was 1.6 (95% confidence interval [CI] 1.3-1.9) and 2.0 for nosocomial bacteraemia (95% CI 1.4-3.0) in malnourished children as compared with well nourished children. Bacteraemia was 3.5 times more common in children with kwashiorkor than in marasmic children. Gram-negative enteric aerob

Increase in total serum triglyceride and phospholipid in kwashiorkor.

Fasting serum lipids in children presenting with protein-energy malnutrition were studied in comparison with those of sex- and age-matched well nourished controls. There was no difference in serum total cholesterol between malnourished and well nourished children. However, serum triglyceride and phospholipid concentrations were significantly higher in children with kwashiorkor or marasmic kwashiorkor than in well nourished controls. It appears that serum concentrations of triglycerides and phospholipids increase with increasing severity of malnutrition, and that fasting serum triglyceride and

Clinical and radiologic study of the frequency and presentation of chest infection in children with severe protein energy malnutrition.

The association between P.E.M. and frequent and severe life threatening infections including lower respiratory tract infections have been always reported. Lack of the usual general and local signs of infection in P.E.M. makes the diagnosis difficult and sometimes only postmortem. This study evaluated the frequency of chest infections as well as the sensitivity, specificity and predictivity of different signs and symptoms of the disease in 100 children with severe P.E.M. (marasmus, kwashiorkor, and marasmic kwashiorkor). Sixty two percent of the studied children had chest infection (33% pneumon

Evaluation of urinary tract infection in malnourished black children.

Urinary tract infection (UTI) is a well recognized complication in malnourished children. The need to investigate these patients for underlying renal pathology has not been clearly defined. Seventy-five children with malnutrition were evaluated for UTI by culture of urine obtained suprapubically prior to antibiotic therapy. All patients with UTI were investigated with renal ultrasonography, intravenous pyelography (IVP) and voiding cystourethrography (VCU). Haemoglobin, white cell count, serum urea, creatinine and electrolytes were determined in all the children. The mean age of the children w

Protein energy malnutrition (PEM), brain and various facets of child development.

Protein energy malnutrition (PEM) is a global problem. Nearly 150 million children under 5 years in the world and 70-80 million in India suffer from PEM, nearly 20 million in the world and 4 million in India suffer from severe forms of PEM, viz., marasmus, kwashiorkor and marasmic kwashiorkor. The studies in experimental animals in the west and children in developing countries have revealed the adverse effects of PEM on the biochemistry of developing brain which leads to tissue damage and tissue contents, growth arrest, developmental differentiation, myelination, reduction of synapses, synapti

Left ventricular mass and function in children with severe protein energy malnutrition.

We studied 25 children, aged 1-5 years (mean 2.65 +/- 0.8 years) with severe protein energy malnutrition, and compared their left ventricular mass and function to those of 26 healthy, age- and sex-matched normal children. The mean left ventricular mass in the patients was lower than that in the controls (25.75 +/- 8.09 g vs. 32.44 +/- 11.64 g; P less than 0.05, C.I. 2.08 to 11.30). However, left ventricular mass (g)/kg body weight was significantly increased in the patients (4.44 +/- 1.45 vs. 2.42 +/- 0.87; P less than 0.001, C.I. 1.28 to 2.76) suggesting relative cardiac "sparing". The exocrine pancreas in kwashiorkor and marasmus. Light and electron microscopy.

Histological sections of pancreas and liver from 65 cases of children dying from childhood malnutrition were reviewed. The extent of pancreatic atrophy and fibrosis was compared with fatty change in the liver. Pancreatic atrophy was common, and often associated with severe fatty change in the liver, but also occurred in marasmic children with scanty liver fat. Pancreatic fibrosis, when present, was only of mild degree. Among 16 patients with marasmus, fibrosis was only seen in one pancreas. Fibrosis was recorded in 8/25 cases of kwashiorkor, and in 7/24 cases diagnosed as marasmic-kwashiorkor.

Nosocomial bacterial infections among children with severe protein energy malnutrition.

The incidence of hospital acquired acute bacterial infections among 164 severely malnourished children admitted to the paediatric wards at the Muhimbili Medical Centre in Dar es Salaam were studied. On admission, ninety two per cent of the patients had at least one form of bacterial infection. During the subsequent two weeks hospital stay, 49% of the patients acquired a new infection. Septicaemia and urinary tract infection (UTI) were the commonest infections. Staphylococcus aureus was the commonest organism in the former, while gram negative organisms, Escherichia coli and Klebsiella species,

[Management of malnourished children. Cases from the Protestant Hospital of Dabou].

Malnutrition due to lack of nutritional resources unfortunately remains an endemic which ravages Third World countries. Not only is malnutrition a result of continuous droughts and wars, but it is a reflection of the difficult economic circumstances which persist in these countries. Malnutrition should be specifically treated in each region with the least costly locally available means. The experience of the Protestant Hospital of Dabou proves that this manner of care is possible in the midst of a rural area with very limited resources. Hospital surveillance, nutritional education and a stay i

Some factors contributing to protein-energy malnutrition in the middle belt of Nigeria.

A number of risk factors leading to malnutrition were investigated among 400 mothers of malnourished children in the middle belt of Nigeria. Poverty, family instability, poor environmental sanitation, faulty weaning practices, illiteracy, ignorance, large family size and preventable infections are the main factors responsible for malnutrition. The strategies for intervention are in the area of health education emphasizing the importance of breastfeeding, family stability, responsible parenthood and small family sizes through culturally acceptable family planning methods. There is need to impro

Bone turnover in malnourished children.

Pyridinoline (PYD) and deoxypyridinoline (DPD) are cross-linking aminoacids of collagen that are located mainly in bone and cartilage. When bone matrix is resorbed these cross-links are quantitatively excreted in the urine and therefore represent specific markers. We have measured the urinary excretion rate of PYD and DPD in 46 severely malnourished boys to assess their skeletal turnover and to relate this to their subsequent rate of growth. The children were aged 13 months (SD 6), and height-for-age was -3.6 (1.6) Z-score, and weight-for-height was -2.4 (0.8) Z-score. PYD excretion when malno

Prevalence of HIV-1 infection and symptomatology of AIDS in severely malnourished children in Dar Es Salaam, Tanzania.

In Dar es Salaam, Tanzania, 200 children with severe malnutrition and controls matched for age, sex, and area of residence were screened for serological evidence of infection with the human immunodeficiency virus type 1 (HIV-1) over 5 months in 1988. The prevalence of HIV-1 antibodies in the malnourished group was 25.5% (51 of 200) compared with 1.5% (three of 200) in the controls. The seroprevalence rate was equally high in malnourished children above the age of 18 months (26 of 102; 25.5%), as in those below this age (25 of 98; 25.5%). The prevalence rate was higher in children with marasmus

Chronic diarrhea and malnutrition--histology of the small intestinal lesion.

The purpose of this study was to quantitate the jejunal lesion in Gambian children with chronic diarrhea-malnutrition syndrome. There were 40 subjects (20 male, 20 female) with a mean age of 19.7 months. All were severely malnourished, with marasmus in 30, marasmic kwashiorkor in 9, and kwashiorkor in 1. Of subjects tested, 70% were anergic to intradermal challenge with either purified protein derivative or candidin. Jejunal biopsies, performed on every subject after admission to hospital, were studied by computerised image analysis and immunocytochemistry. A spectrum of mucosal changes that v

Effects of defective in vivo synthesis of mitochondrial proteins on cellular biochemistry and physiology of malnourished rats.

Investigations were carried out to find the biochemical and bioenergetic implications of a defective in vivo synthesis of mitochondrial proteins during dietary-protein depletion. 3.4% dietary protein was fed to 21-day-old weanling rats for 30 days (experimental), while 21.0% dietary protein was fed to controls. A close simulation of marasmic-kwashiorkor syndrome was obtained in the experimental group. Rat liver mitochondria were isolated, and the functional and structural integrity was determined using biochemical standard parameters. Results show that the respiratory control ratio (RCR) is re

Elevated level and underdeveloped fatty acid composition of brain cholesterol esters in malnourished children.

The content and fatty acid composition of cholesterol esters were measured in the forebrain, cerebellum, and brain stem of children who died within the first 2 years of life from severe malnutrition syndrome of kwashiorkor, marasmus, or marasmic kwashiorkor. All three brain regions of the malnourished children had elevated concentrations of cholesterol esters, compared to those of apparently normal, well-nourished children of comparable ages. This was particularly true for the brain stem, in which the elevation in some cases was 10-fold or more. In this brain region, and less pronouncedly also
