Aflatoxins in liver biopsies from Sudanese children.

Aflatoxin analysis of 40 percutaneous needle liver biopsies in 27 children with protein-energy malnutrition and 13 children with miscellaneous liver disease in The Sudan is reported. Aflatoxins B1, B2 and aflatoxicol were detected in 5 of the 16 biopsies from kwashiorkor but in none of 11 biopsies from marasmus or marasmic kwashiorkor. Aflatoxins G1, G2 and M2 were detected in 5 of 12 children with chronic liver disease. A very high concentration of aflatoxicol was found in a breast-fed infant with neonatal hepatitis of unknown etiology.

Serum insulin-like growth factors I and II concentrations and growth hormone and insulin responses to arginine infusion in children with protein-energy malnutrition before and after nutritional rehabilitation.

Serum insulin, growth hormone (GH), insulin-like growth factors (IGFs) I and II, cortisol, and albumin concentrations were measured in 15 children with kwashiorkor, 15 with marasmic-kwashiorkor, and 21 with marasmus, before and in the survivors, after nutritional rehabilitation, as well as in 10 underweight and eight normal Egyptian children. We also evaluated arginine-induced insulin and GH secretion. IGF-I concentrations were reduced in the three severely malnourished groups (0.07 +/- 0.03, 0.05 +/- 0.03, and 0.09 +/- 0.09 U/ml, respectively) but returned to normal after refeeding. IGF-II co

Severe protein energy malnutrition in Lesotho, death and survival in hospital, clinical findings.

In Lesotho's central hospital 55 (25%) of 218 admissions for severe PEM died during 1981 and 1982. Most deaths (62%) occurred in the first week. The most important causes of death were acute GE and pneumonia in marasmus and kwashiorkor, respectively. The cause of death remained obscure in 16 children, however. In marasmus a poor prognosis was significantly associated with the finding on admission of a temperature less than 36.5 degrees C (P less than 0.05), apathy (P less than 0.01) and a depigmented skin (P less than 0.05), while in marasmic kwashiorkor only the finding of the latter was sign

Multiresistant Salmonella ohio infections at the University Hospital of the West Indies.

During 1982-83 there was a substantial increase in the number of S. ohio infections at the University Hospital of the West Indies, which coincided with the appearance of strains resistant to chloramphenicol, cotrimoxazole, ampicillin, neomycin and carbenicillin. Multiresistant strains of S. ohio accounted for 19.3% of all salmonella isolates during this period and all of 40 strains tested were able to transfer resistance determinants to E. coli K12 J 53-2. S. ohio was cultured from stool (60), blood (5), wounds and abscesses (4) and postmortem material (2). Eighty-six per cent of S. ohio infec

Serum aminotransferases and gamma-glutamyl transferase in protein energy malnutrition.

Forty six children suffering from Protein Energy Malnutrition (PEM) were classified according to the Wellcome classification. Their aspartate aminotransferase, alanine aminotransferase and gamma-glutamyl transferase were measured. Aspartate aminotransferase was raised in 20 patients (43.5%) and alanine aminotransferase was raised in 12 patients (26%). Y-glutamyl transferase was raised in only one patient suffering from marasmic kwashiorkor, who, in contrast to the rest of the patients had a marked rise in aminotransferases. The aminotransferase elevation correlated positively with a Severity I

The nutritional status and feeding practices among sick urban pre-school children admitted to a major hospital in Accra, Ghana.

In a prospective study of 652 sick pre-school children only 33% were found to be adequately nourished. Among the malnourished, 54.3% had first degree malnutrition while 32.3% and 13.4% had second and third degree malnutrition respectively. The majority of malnourished children (72.4%) had undernutrition: kwashiorkor (14.2%), marasmus 7.3%) and marasmic kwashiorkor (6.1%) was relatively less common. Furthermore, 58% of the underweight children were stunted, indicating malnutrition of some considerable duration. Malnutrition was essentially of postnatal origin and closely related to a high incid

The influence of aflatoxins on child health in the tropics with particular reference to kwashiorkor.

Aflatoxins are common environmental hazards in all the underdeveloped countries of the tropics where they commonly contaminate food. They are toxic to most species of animals and are among the most powerful carcinogenic agents known. The liver is the principal target for toxicity. Metabolic derangements caused by aflatoxins include depression of protein and enzyme synthesis, disorder of lipid metabolism and immunological suppression. The aetiology and pathogenesis of kwashiorkor remains somewhat obscure. Similarities in the geographical and climatic prevalence of kwashiorkor and aflatoxins and

Plasma and red blood cell fatty acid composition in children with protein-calorie malnutrition.

Plasma and red cell fatty acids were measured in 44 children with third degree malnutrition: 22 children with marasmus, 11 children with kwashiorkor, and 11 with marasmic kwashiorkor (MK). The presence of edema, dermatosis, hair changes, and hypoalbuminemia were positively correlated with the red cell percentage linoleic acid and negatively correlated with red cell percentage arachidonic acid. Red cell percentage linoleic acid in kwashiorkor was decreased from the control value of 15.3% to 10.9%. In marasmus and MK, there were significantly greater decreases, to 7.7% and 8.0%, respectively. Re

Weight-for-age of children admitted to the Ethio-Swedish Children's Hospital, Addis Ababa, 1978.

The weight-for-age of 1235 hospital-admitted children aged between one month and 14 years was assessed. Sixty per cent (764) of child admissions in all age groups had weights less than 80% of the Harvard reference weight-for-age value; 27% (339) had weights less than 60% of the Harvard reference weight-for-age value. Of children who were diagnosed to suffer from protein-energy malnutrition on admission, marasmus was diagnosed most frequently, followed by marasmic-kwashiorkor and kwashiorkor. Seventy-five per cent of child admissions who died in hospital had weights less than 80% of the referen

The effect of dietary lactose on the early recovery from protein-energy malnutrition. I. Clinical and anthropometric indices.

To assess the advisability of using lactose-containing formulas in the rehabilitation of severely malnourished children, indices of clinical recovery, growth and restoration of body proteins and gastrointestinal function were measured longitudinally during the initial 45 days of hospitalization in 20 male, preschool children with kwashiorkor and marasmic-kwashiorkor. All patients received a diet based on cows' milk, but half were allocated to a formula pretreated with beta-galactosidase to hydrolyze the lactose, while the others received the untreated, intact milk. The groups were identical wi

Carbohydrate malabsorption in children with severe protein energy malnutrition.

To determine the incidence of carbohydrate malabsorption, particularly lactose malabsorption in Basotho children with severe PEM during treatment with a dry skim milk (DSM)-sucrose-oil mixture, 105 children with PEM were submitted to a Hydrogen Breath Test (HBT) after administration of the mixture. Carbohydrate malabsorption occurred more frequently in children with kwashiorkor (28/58) than in those with marasmus (5/33), marasmic kwashiorkor (3/15) and healthy controls (8/34). The positive HBT appeared to be due to lactose malabsorption in at least two thirds of the children with kwashiorkor a

Thyroid status in various degrees of protein-calorie malnutrition in children.

The thyroid functional parameters of 102 children with protein-calorie malnutrition (PCM) classified as undernourished, marasmic, kwashiorkor and marasmic-kwashiorkor were studied by measuring the serum T4, T3, T3 percentage uptake (T3%UT), free thyroxine index (FTI), and (in a few subjects) TSH. Total plasma proteins, albumin and globulin were also estimated. T4 was increased markedly in the undernourished children and significantly decreased in kwashiorkor and marasmic-kwashiorkor groups. There was a consistent and significant fall in T3 levels of all PCM children. From normal values, the fa

Moderate and severe protein energy malnutrition in childhood: effects on jejunal mucosal morphology and disaccharidase activities.

The enzymatic activities of maltase, sucrase, and lactase were determined in jejunal biopsies obtained from 24 children with gastrointestinal symptoms and different degrees of protein energy malnutrition (PEM) to see if these differences were related to the extent of malnutrition or concomitant small intestinal mucosal injury. Even in patients with moderate PEM, lactase activities were significantly lower than control values. The decrease in lactase activity seemed to depend on the severity of malnutrition. In contrast, maltase and sucrase activities were decreased only in second and third deg

Is the adult protein-energy malnutrition syndrome the same as that described in the infant?

Protein-energy malnutrition, a multi-factorial disease, has been described predominantly in the infant. It was the aim of this research to give a biochemical assessment of the adult form and to compare it to the infantile syndrome within the same socio-cultural context of central Zaïre (Kwilu region). Thirty-four children, 22 women and 2 men suffering from marasmic kwashiorkor at the hospital of Yasa-Bonga (Kwilu) were submitted to a complete set of 7 anthropometric and 60 biochemical tests. The control values were taken from healthy well-fed children and adults from Yasa-Bonga; for certain pa

Measles vaccination in severely malnourished Sudanese children.

The clinical and serological responses to attenuated measles virus vaccine were compared in 35 severely malnourished and 35 well-nourished children. A third group of severely malnourished children, who received an injection of vitamin B12, served as controls. The children were observed for three weeks following vaccination. Paired sera were collected from each child before and after administration of the vaccine or vitamin B12 and measles antibody titres were estimated using HAI technique. The malnourished children who received measles vaccine developed higher fever of longer duration and had

Infections in children with severe protein-energy malnutrition.

The infections occurring in 68 black children admitted to hospital with kwashiorkor, marasmus or marasmic-kwashiorkor were studied prospectively. Fifteen episodes of bacteraemia, most commonly due to Gram-negative enteric bacilli, occurred in 13 children (19%), six of these episodes being nosocomial. Urinary tract infection, diagnosed on suprapubic urine specimens and all due to Escherichia coli, occurred in five out of 16 cases (31%). In the 48 cases admitted with gastro-enteritis and 18 with pneumonia, a causative organism was found in the minority. Fifteen episodes of nosocomial infection o
