Umbilical Cord Quantitative Polymerase Chain Reaction and Sepsis Complications for Neonates.

IMPORTANCE: Sepsis occurring in the first 72 hours of life is associated with high mortality and complications such as intraventricular hemorrhage (IVH), particularly in preterm infants. Although blood culture is the diagnostic standard, its low detection rate may underestimate the clinical burden of infection. OBJECTIVE: To determine whether molecular detection of bacterial DNA in umbilical cord tissue improves identification of preterm infants at risk of sepsis-related death or grade IV IVH. DESIGN, SETTING, AND PARTICIPANTS: Samples were collected from infants with a birth weight less tha

Neonatal sepsis and meningitis caused by Group B Streptococcus or Escherichia coli in twins. A systematic review.

OBJECTIVES: Neonatal sepsis in one twin poses a dilemma in managing the healthy-appearing sibling. Shared exposures and genetic factors increase risk, and early signs of sepsis can be subtle with rapid deterioration. However, the magnitude of this risk for the healthy twin is unclear, and management strategies vary considerably. METHODS: We systematically searched PubMed and EMBASE on June 10th 2025 for multiples in which at least one infant (0-89 days) developed culture-confirmed Group B Streptococcus or Escherichia coli early-onset or late-onset sepsis or meningitis. RESULTS: Of 151 sets o

Navigating towards resilient relationships with kelp.

BACKGROUND: Batracylin is a heterocyclic arylamine topoisomerase inhibitor with preclinical anticancer activity. Marked species differences in sensitivity to the toxicity of batracylin were observed and attributed to differential formation of N-acetylbatracylin by N-acetyltransferase. A Phase I trial of batracylin in cancer patients with slow acetylator genotypes identified a dose-limiting toxicity of hemorrhagic cystitis. To further explore the metabolism of batracylin and N-acetylbatracylin across species, detailed studies using human, rat, and dog liver microsomal and hepatocyte preparation

Precise Manipulation of Bubble Cavitation and Blood-Brain Barrier Modulation using Temporal Interference Ultrasound.

Objective: To determine whether temporal interference (TI) ultrasound can enhance microbubble (MB) cavitation and provide transient modulation of blood-brain barrier (BBB) permeability. Impact Statement: By integrating a time-dependent multiphysics MB-vessel model with in vivo validation, this study links TI-enhanced cavitation dynamics to vessel-wall mechanical responses and a transient BBB permeability window, providing a mechanistic basis for pressure-efficient ultrasound-mediated brain delivery. Introduction: Focused ultrasound combined with MBs can transiently increase BBB permeability, b

The Stratified Association of Left Atrial Volume Index and Dynamic Change of Pulmonary Arterial Pressure on Prognosis After Catheter Ablation in Patients With Persistent Atrial Fibrillation.

BACKGROUND: Group B Streptococcus (GBS) is a major cause of invasive bacterial diseases, including sepsis, meningitis, and pneumonia, particularly in newborns and infants. Pregnant adults, those with pre-existing conditions, and older adults are particularly susceptible. Ongoing research is focused on developing various vaccines utilizing different antigens, including capsular polysaccharides and alpha-like proteins (Alps). Epidemiological data on these antigens in GBS is essential for predicting the effectiveness of these vaccines. However, no epidemiological studies on Alps genotype have bee

Prevalence of Candidate Vaccine Targets and Genomic Features of Pediatric Invasive Streptococcus Agalactiae in Japan.

BACKGROUND: Streptococcus agalactiae (Group B Streptococcus, GBS) is a leading cause of invasive neonatal and infant infections, including sepsis and meningitis. This study aimed to estimate the vaccine coverage and characterize the genomic features of pediatric invasive GBS in Japan. METHODS: We conducted a nationwide, multicenter, retrospective genomic surveillance study involving 237 GBS isolates from sterile specimens of children aged ≤15 years across 35 hospitals in Japan between 2004 and 2023. Serotyping, antimicrobial susceptibility testing, and whole-genome sequencing were performed.

Personalizing Modern Heart Failure Therapy in HFrEF: Bridging Evidence, Clinical Practice, and Future Perspectives.

BACKGROUND: Heart failure with reduced ejection fraction (HFrEF) remains a major cause of morbidity, mortality, and healthcare utilization worldwide despite substantial therapeutic advances. Over the past three decades, randomized clinical trials have established four foundational pharmacologic pillars of guideline-directed medical therapy (GDMT): renin-angiotensin system inhibition or angiotensin receptor-neprilysin inhibition, evidence-based beta-blockers, mineralocorticoid receptor antagonists, and sodium-glucose cotransporter 2 inhibitors. However, real-world implementation of these therap

Characterization of a novel monoclonal antibody candidate that targets bacterial GAPDH and protects neonatal mice from infections caused by Streptococcus pneumoniae or Streptococcus agalactiae.

Glyceraldehyde-3-phosphate dehydrogenase (GAPDH) is a highly conserved bacterial enzyme essential for glycolysis, yet it also plays important non-metabolic, "moonlighting" roles, including host immunomodulation. Here, we describe the generation and characterization of a monoclonal antibody (mAb01) that targets extracellular GAPDH from Streptococcus agalactiae (GBS) and Streptococcus pneumoniae. Using bio-layer interferometry (BLI) and surface plasmon resonance (SPR), we demonstrate that mAb01 binds with high affinity, exhibiting dissociation constants in the low nanomolar range for both antige

Vaccines preventing neonatal sepsis: focus on Group B Streptococcus, Klebsiella pneumoniae and Escherichia coli to combat rising antimicrobial resistance.

INTRODUCTION: Neonatal sepsis, a systemic infection occurring in infants within their first 28 days of life, is a leading cause of mortality globally. The burden is especially severe in low- and middle-income countries (LMICs), where incidence and mortality rates are higher than in high-resource settings. Antimicrobial resistance, driven by multidrug-resistant pathogens prevalent in LMICs, further complicates its effective management. Vaccination offers a promising strategy to prevent infections caused by common neonatal sepsis pathogens, potentially reducing sepsis incidence and mitigating re

Intrapartum group B streptococcus (GBS) detection by intrapartum PCR versus antenatal culture at 35-38 weeks of gestation: Justification for a crossover, cluster randomized controlled trial, and protocol summary.

The incidence of neonatal group B streptococcus (GBS) related to early onset sepsis (EOS) has decreased dramatically with antenatal culture screening and intrapartum antibiotic prophylaxis (IAP) implementation during the 1990s. However, while 30 % of women are currently exposed to intrapartum antibiotics, 52 to 82 % of newborns with GBS-EOS were born to mothers with negative antenatal GBS culture screening who did not receive IAP, because of possible intermittent maternal GBS colonization. Intrapartum GBS screening by Polymerase Chain Reaction (PCR) has been suggested as a suitable tool to imp

Genomic adaptation in group B Streptococcus following intrapartum antibiotic prophylaxis and childbirth.

Through vaginal colonization, group B Streptococcus (GBS) causes severe outcomes including neonatal sepsis and meningitis. Although intrapartum antibiotic prophylaxis (IAP) has reduced neonatal disease rates, GBS can persist in the genitourinary tract even after antibiotic administration. To determine if IAP selects for genomic signatures that enhance GBS survival and persistence, we compared the isolates from individuals before (prenatal) and after (postpartum) IAP/childbirth. Among 34 of the paired strains from participants with persistent colonization, 31 (91.2%) clustered together in a cor

Maternal immunization against group B Streptococcus: Immune correlates, microbiome trade-offs, and global implementation challenges.

Group B Streptococcus (GBS) remains a leading cause of neonatal sepsis, meningitis, stillbirth, and long-term neurodevelopmental impairment worldwide. Although intrapartum antibiotic prophylaxis (IAP) has substantially reduced early-onset GBS disease in high-income settings and remains an effective and evidence-based intervention, it does not prevent late-onset disease, maternal colonization, or GBS-associated stillbirths, and its implementation is challenging in many low- and middle-income countries (LMICs). Maternal vaccination has therefore emerged as a promising complementary strategy to a

Overwhelming post-splenectomy infection caused by Streptococcus agalactiae, sequence type 10 with capsular serotype Ib encapsulating ST10/1b occurring 30 years after splenectomy: a case report.

Overwhelming post-splenectomy infection (OPSI) is a rapidly progressive and potentially fatal condition that affects individuals without a spleen. Encapsulated bacteria are among the most common pathogenic microorganisms of OPSI. Streptococcus agalactiae (group B streptococci; GBS) is an encapsulated Gram-positive pathogen. Although it causes various invasive infections in adults, including bacteremia, septic shock, and toxic shock syndrome, it has rarely been associated with OPSI. We describe a case of GBS-associated OPSI in a 72-year-old man who developed septic shock 30 years after undergoi

A covR promoter mutation enhances Group B Streptococcus virulence by upregulating pgaC to reinforce capsular surface anchoring.

Group B Streptococcus (GBS) is a major zoonotic pathogen responsible for neonatal sepsis and cerebrospinal meningitis. A single adenine (T) insertion in the covR promoter, identified in a clinical isolate, has been shown to enhance pathogenicity. However, the mechanistic basis remains unclear. In this study, we demonstrated that this insertion downregulates covR expression and enhances hemolysin production. Unexpectedly, we also found that it leads to a thicker and more compact capsule based on dot blot and flow cytometry analyses. Transcriptome analysis performed on the covR promoter mutant r
