Lineage-encoded genomic determinants of neonatal invasive Group B Streptococcal disease.

OBJECTIVES: Group B Streptococcus (GBS) is a leading cause of invasive infection in newborns and infants. Yet the genomic basis of invasiveness and disease severity remains incompletely defined. We aimed to identify bacterial genomic features associated with invasive GBS disease and to explore associations between bacterial genomics and severe clinical outcomes. METHODS: We performed PacBio long-read whole-genome sequencing of 381 GBS isolates, including 127 invasive isolates obtained from blood cultures of infants with GBS disease and 254 serotype-matched colonizing isolates obtained from re

Facing Antimicrobial Resistance and Virulence in Streptococcus agalactiae During Late Pregnancy: Evaluation of Lactobacilli as a Supportive Approach.

Streptococcus agalactiae (Group B streptococcus, GBS) remains a major cause of neonatal sepsis and meningitis, despite the success of intrapartum antibiotic prophylaxis. However, the rising trend of antimicrobial resistance and the emergence of hypervirulent lineages highlight the need for complementary, innovative prophylactic strategies. This study aimed to characterize the resistance and virulence repertoire of colonizing GBS isolates and to assess the antagonistic potential of selected Lactobacillus strains under conditions mimicking the vaginal niche. We performed integrated phenotypic, g

Late-Onset Group B Streptococcus Sepsis Through the Lens of 28-Week-Old Triplets.

Late-onset Group B Streptococcus (GBS) disease in infants presents with a wide range of clinical manifestations and can be challenging to diagnose in preterm infants. This vulnerable population is more likely to acquire late-onset GBS disease and has a significantly higher mortality rate compared with term infants. In this article, we discuss triplets born at 28 weeks' gestation who exhibited multiple complications associated with late-onset GBS disease. We also review the epidemiology, clinical presentation, and management of late-onset GBS through the lens of these infants.

A Review of Early- and Late-Onset Neonatal Infections by Group B Streptococcus: Pathophysiology and Vaccine Prospects.

Group B Streptococcus (GBS) remains a leading cause of neonatal morbidity and mortality worldwide. About 18% of pregnant women are colonized in the gastrointestinal or genitourinary tracts. Vaginal colonization can result in ascending intrauterine infection, enabling GBS to reach the amniotic fluid and infect the fetus during pregnancy or the newborn at delivery, leading to invasive GBS (iGBS) disease. Early-onset disease (EOD) typically manifests as sepsis, whereas late-onset disease (LOD) is more often associated with meningitis. Intrapartum antibiotic prophylaxis (IAP), based on screening a

Clinical characteristics, outcomes and molecular epidemiology of group B streptococcal disease in young infants in the Netherlands.

OBJECTIVES: Group B streptococcus (GBS) disease remains a major cause of morbidity and mortality in young infants. We conducted a prospective multi-centre cohort study to evaluate clinical outcomes, molecular epidemiology, and coverage of potential maternal vaccines candidates in the Netherlands. METHODS: Between January 1st, 2018, and December 31st, 2024, infants aged 0-89 days with culture-confirmed GBS sepsis or meningitis were identified through the Netherlands Reference Laboratory of Bacterial Meningitis. Clinical data, including GBS serotype, were collected for all cases. RESULTS: We i

Maternal 6-valent group B Streptococcus vaccine in non-pregnant and pregnant females: a randomized phase 1/2 trial.

Group B Streptococcus (GBS) disease is a major cause of infant morbidity and mortality. Capsular polysaccharide conjugate GBS vaccines have been evaluated for decades; however, none is licensed. This phase 1/2 trial evaluated an investigational maternal hexavalent polysaccharide-protein conjugate GBS vaccine (GBS6) in non-pregnant females and pregnant participants and their infants. In stage 1 (conducted in South Africa), healthy non-pregnant participants received GBS6, GBS6 + aluminum phosphate (AlPO4) or placebo; a subset received GBS6 + AlPO4 booster approximately 2 years later. Stage 2 (co

Rapid intrapartum PCR assay for group B streptococcus within a dutch risk-based strategy: diagnostic accuracy and optimization of antibiotic use in a single-centre cohort.

OBJECTIVE: To compare the diagnostic accuracy of a rapid intrapartum polymerase chain reaction (PCR) assay with standard intrapartum culture for detecting maternal group B streptococcus (GBS) carriage, and to explore potential effects on intrapartum antibiotic prophylaxis (IAP) within a Dutch risk-based strategy. BACKGROUND: GBS is a leading cause of early-onset neonatal sepsis. While many countries apply universal screening, the Netherlands follows a selective, risk-based prevention protocol with markedly lower use of IAP. Standard culture requires 24-48 h, limiting its value in urgent obste

ypmS promotes capsular polysaccharide shedding and in vivo fitness of Group B Streptococcus.

Group B Streptococcus (GBS) is a leading cause of neonatal sepsis and meningitis worldwide. Capsular polysaccharide (CPS) is a major virulence factor that aids GBS in colonizing the gastrointestinal (GI) and vaginal tracts, surviving in whole blood and evading the immune system. CPS is also the basis for a GBS CPS-protein conjugate vaccine candidate. In addition to its function on the surface of the bacteria, CPS can also be released and shed into the environment. In Streptococcus pneumoniae, shed CPS has been shown to absorb antimicrobial peptides and CPS-specific antibodies, allowing the bac

Time to rethink prevention of neonatal group B streptococcal disease.

Group B Streptococci (GBS) are both natural colonizers and important invasive pathogens in newborns. The introduction of intrapartum antibiotic prophylaxis has led to a significant decrease in the incidence of early onset sepsis. In contrast, late onset sepsis, which accounts for up to 50% of cases, has not declined. IAP impacts microbiome development during infancy, specifically it reduces the abundance of Bifidobacterium and Bacteroides spp., which might be associated with long-term health outcomes such as asthma development and obesity. This review highlights the challenges and risks associ

The novel prophage lysin Lys1459 exhibits broad-spectrum antibacterial activity via triple-binding domain.

UNLABELLED: Phage-encoded lysins represent a promising class of antimicrobial agents against drug-resistant bacteria due to their ability to degrade bacterial cell walls. This study characterizes a novel prophage-derived lysin, Lys1459, from Streptococcus uberis. We expressed and purified the full-length Lys1459 protein, along with its three individual domains (CHAP, A2, and CBD), to systematically evaluate its antibacterial spectrum, biochemical properties, and therapeutic efficacy. The full-length Lys1459 demonstrated potent lytic activity, achieving a 3-log reduction in Streptococcus agalac

Maternal and neonatal outcomes and histological chorioamnionitis in preterm premature rupture of membranes complicated by group B streptococcus colonization: a retrospective analysis.

BACKGROUND: Histological chorioamnionitis (HCA) is frequently identified in placental examinations following preterm premature rupture of membranes (PPROM). Colonization of the genital tract by Group B Streptococcus (GBS) represents a major risk factor for serious neonatal infections. This study evaluated the influence of GBS on both maternal and neonatal outcomes in pregnancies complicated by PPROM before 34 weeks’ gestation managed expectantly. METHODS: We conducted a retrospective analysis of clinical records from pregnant women experiencing spontaneous PPROM between 23 + 0 and 33 + 6 weeks

Recurrent Group B Streptococcus Neonatal Invasive Infections, France, 2007-2021.

Recurrence is a rare complication of group B Streptococcus (GBS) neonatal infections. We conducted a retrospective observational study on GBS neonatal invasive infections in France from 2007 to 2021. A total of 1527 cases were reported, of which 36 (2.36%) were recurrent. Recurrence mainly concerned preterm (68%) and low-birth-weight (72%) infants and was associated with the hypervirulent GBS clonal complex 17 (83%; odds ratio, 2.86 [95% confidence interval, 1.18-6.92]). No β-lactam-tolerant strains were identified, and bacterial whole-genome sequencing could not reveal any specific feature as

A case of penicillin-resistant group B Streptococcus isolated from a patient in the UK.

OBJECTIVES: In England, group B streptococci (GBS; Streptococcus agalactiae) are considered universally susceptible to penicillin. Reports from Africa, Asia, North America and a few European countries have described GBS isolates with penicillin MICs above the epidemiological cut-off (0.125 mg/L). Our aim was to characterize a penicillin-resistant GBS (PRGBS) isolate recovered in 2016 from a patient treated with long-term antimicrobials in the UK. METHODS: Antibiotic susceptibility of a referred isolate from a discharging sinus overlying a chronic prosthetic joint infection was determined usin

Early onset neonatal bloodstream infections in South African hospitals.

BACKGROUND: Neonatal sepsis is a leading cause of death in low- and middle- income countries (LMIC). Increasing antibiotic resistance in early onset (< 72 h of life) bloodstream infection (EO-BSI) pathogens in LMIC has reduced the effectiveness of the recommended empiric antibiotic regimen (ampicillin plus gentamicin). METHODS: We retrospectively analysed blood culture-confirmed EO-BSI episodes at nine neonatal units from three central and six peripheral hospitals in the Western Cape Province, South Africa between 1 January 2017 and 31 December 2018.
