Necrotizing fasciitis secondary to Wohlfahrtiimonas chitiniclastica in the absence of myiasis.

Wohlfahrtiimonas chitiniclastica is a rare Gram-negative bacillus associated with fly larvae, myiasis, chronic wounds, and severe polymicrobial infection. We report a 71-year-old woman with diabetes, peripheral neuropathy, severe peripheral vascular disease, and previous contralateral above-knee amputation who presented with septic shock secondary to an infected right hallux ulcer. Radiographs demonstrated soft-tissue gas extending into the distal calf. Emergency exploration revealed extensive necrosis and dishwater pus tracking proximally along the flexor hallucis longus tendon, consistent wi

Effects and Molecular Mechanisms of Heat-Killed Postbiotic Enterococcus faecalis EF-2001 on Muscle Volume and Grip Strength in Dexamethasone-Induced Muscle Atrophy in SD Rats.

The interaction between the gut microbiota and human health has gained increasing recognition, accelerating advances in microbiome research. While early studies have emphasized probiotics, concerns regarding antibiotic resistance and adverse effects, such as sepsis, have shifted research interest towards heat-treated microbial cells or postbiotics. This study investigated the therapeutic potential of heat-killed postbiotic Enterococcus faecalis EF-2001-one of the most widely used postbiotics worldwide-for the prevention and treatment of muscle atrophy. In vitro, mouse C2C12 myotubes were pretr

Transient Sepsis-Induced Cardiomyopathy in the Setting of Alcohol Withdrawal.

Sepsis-induced cardiomyopathy (SICM) is a reversible myocardial dysfunction that occurs in the setting of severe sepsis and septic shock, often complicating the management of patients with underlying cardiovascular disease. We report the case of a 62-year-old man with heart failure with improved ejection fraction (HFimpEF), paroxysmal atrial fibrillation, and severe alcohol use disorder who developed septic shock due to Enterococcus faecalis bacteremia, complicated by multiorgan failure and transient worsening of cardiac function. Myocardial dysfunction was attributed to SICM based on the temp

Rising burden of enterococcal bacteremia in Victoria, Australia: population-based incidence and antimicrobial resistance trends from three decades of surveillance.

Enterococcal bacteremia is a common healthcare-associated infection, associated with significant morbidity and mortality, with the emergence of vancomycin-resistant enterococci further complicating treatment and clinical outcomes. Despite this, long-term estimates of population-based incidence and antimicrobial resistance trends are limited. We aim to describe the burden of enterococcal bacteremia in Victoria, Australia (population 7.0 million), over a 35-year period. We conducted a retrospective analysis of laboratory-confirmed enterococcal bacteremia episodes voluntarily reported to the Vict

Trends in the prevalence of bloodstream infections in Spanish hospitals (2013-2023).

BACKGROUND: Bloodstream infections (BSI) represent a growing and significant issue due to their associated mortality, morbidity, and economic burden. This study aimed to assess the prevalence of BSI from 2013 to 2023, by type and onset, and to describe patients' characteristics and identified microorganisms. METHODS: Data from the point prevalence survey on infections and antimicrobial use in Spanish hospitals were analysed. Logistic regression was conducted to identify temporal trends and associations between different factors and hospital-acquired BSI (HA-BSI). Stepwise backward selection w

Clinical utility of the DENOVA score for predicting infective endocarditis in Enterococcus faecalis bacteraemia: external validation with decision curve analysis.

OBJECTIVES: The DENOVA score was developed to support risk stratification for infective endocarditis (IE) in patients with Enterococcus faecalis bacteremia (EfB) and to guide echocardiographic evaluation; external validation remains, however, limited. The objective of this study was to externally validate the DENOVA score in a large prospective international cohort of patients with EfB. METHODS: Prospective multicentre cohort study (2019-2024) conducted across 23 centres in six countries (Italy, Spain, Israel, Brazil, Switzerland, and Romania), including adult patients with monomicrobial EfB

Monotherapy vs. combination therapy for Enterococcus faecalis bacteraemia: a target trial emulation.

OBJECTIVES: Optimal treatment for Enterococcus faecalis bloodstream infection (EF-BSI) remains a topic of debate. We aim to evaluate the effectiveness of combination therapy compared with monotherapy in patients with EF-BSI and no endocarditis. METHODS: This was a target trial emulation based on a prospective, multicentre, international dataset collected in 24 international centres from January 2019 to December 2024. We included all adult patients with monomicrobial EF-BSI with negative echocardiography within 7 days from BSI onset. Exclusion criteria were diagnosis of endocarditis, not recei

Enterococcus endocarditis in an infant with congenital renal duplex anomaly: A case report.

Infective endocarditis is extremely rare in neonates and early infants but is associated with substantial morbidity and mortality. We report a sudden unexpected death in a 5-week-old male infant who was born at term in good condition. At one week of age, he presented with Enterococcus faecalis sepsis confirmed by blood culture. Ultrasound revealed a previously undiagnosed congenital renal duplex anomaly. Intravenous ampicillin-based beta-lactam antibiotics were administered after which the baby demonstrated clinical improvement. The baby was discharged after 2 weeks and was given antibiotic pr

Enterococcus faecalis Bacteremia Following Transurethral Resection of the Prostate (TURP): Urinary Sepsis or Infective Endocarditis?

Enterococcus faecalis is a gram-positive, facultative anaerobic bacterium that commonly colonizes the gastrointestinal and genitourinary tracts. Although frequently implicated in urinary tract infections and bacteremia, it is also a well-recognized cause of subacute infective endocarditis. We present a case of subacute infective endocarditis caused by Enterococcus faecalis, initially masked by presumed urinary sepsis in a patient who had undergone transurethral resection of the prostate (TURP) less than two months prior. The patient presented with four days of fever and generalized weakness, a

Massive Septic Shock and Complex Survivorship in a Medically Complex Pediatric Patient Following Gastrostomy Tube Malposition With Operative Findings of Feed Within the Retrorectus Space.

Gastrostomy tube (G-tube) replacement is often routine; however, in children with complex medical needs, misplacement can rapidly lead to life-threatening complications. We describe the case of a 16-year-old male patient with neuronal ceroid lipofuscinosis who developed fulminant septic shock after accidental G-tube dislodgement and malposition. At an outside hospital, reinsertion attempts required tract dilation; bloody aspirate and inconclusive radiographs were overlooked, and feeds were resumed at home. After 48 hours, the patient presented to a community hospital in profound shock (blood p

Massive bleeding from a Bricker ileal conduit during FOLFIRINOX therapy in noncirrhotic portal hypertension: suspected oxaliplatin-associated sinusoidal injury and presumed irinotecan-associated conduit mucositis: a case report.

BACKGROUND: Bleeding from an ileal conduit after radical cystectomy is uncommon and is usually attributed to tumor recurrence, infection, stones, or local stomal trauma. Portal hypertensive stomal varices are rare. FOLFIRINOX adds further diagnostic complexity because oxaliplatin may cause sinusoidal endothelial injury and noncirrhotic portal hypertension, whereas irinotecan may damage gastrointestinal mucosa. We report a case of massive hemorrhage from a long-functioning Bricker ileal conduit during FOLFIRINOX therapy. CASE PRESENTATION: A 72-year-old man with prior radical cystoprostatectom

Phage mediated growth inhibition and biofilm disruption of the endodontic pathogen Enterococcus faecalis.

The failure of endodontic procedures such as root canal therapy is primarily indicated by persistent microbial infections. Root canal therapy involves the removal of decaying dental pulp (internal blood vessels and nerves of teeth), sanitizing the canal, and filling the space with biocompatible materials. Improper cleaning or the breakdown of these materials can lead to secondary infections. These infections, if left untreated, can lead to severe pain, bone and tooth loss, and potentially systemic infection. The bacterium Enterococcus faecalis is one of the most commonly associated organisms w

Placental Pathogens Associated With Adverse Maternal and Neonatal Outcomes.

BACKGROUND: Bacterial infection at the maternal-fetal interface contributes to adverse pregnancy outcomes, including preterm birth, chorioamnionitis, and sepsis. Despite evolving obstetric practices that may influence microbial patterns, recommended clinical algorithms for prophylaxis and treatment rely on outdated data. Furthermore, the pathogenic organisms associated with adverse outcomes remain incompletely characterized. METHODS: We conducted a retrospective study of placental cultures from a high-risk obstetric cohort from a single hospital system between 2014 and 2019 (n = 714). Placent
