Therapeutic potential of a newly isolated bacteriophage against multi-drug resistant Enterococcus faecalis infections: in vitro and in vivo characterization.

BACKGROUND: In nosocomial settings, vancomycin-resistant Enterococcus faecalis is a major health threat leading to increased morbidities, mortalities, and treatment costs. Nowadays, several approaches are under investigation to enhance the activity of or replace the traditional antibiotics. Bacteriophage therapy was sought as a potential approach for combating E. faecalis infections. The present study focuses on isolating and characterizing bacteriophage against clinical multi-drug resistant (MDR) E. faecalis strain Lb-1492. The phage stability, lytic activity, host-range, latent period, burst

Clinical characteristics, therapy and outcome of bloodstream infections caused by vancomycin-resistant enterococci: a multicentre clinical experience.

OBJECTIVES: Bloodstream infections (BSIs) due to vancomycin-resistant Enterococcus spp. (VRE) are considered a predictor of mortality among frail patients. The aim of this study was to evaluate the risk factors associated with 30 day mortality and relapse of infection in enterococcal BSI caused by VRE and to evaluate the impact of antibiotic regimens in targeted therapy. METHODS: We conducted a retrospective study of consecutive hospitalized patients in six teaching hospitals from August 2016 to August 2022 in Italy. All adult patients with a documented VRE BSI were included. RESULTS: We enr

Disrupting Biofilms on Human Kidney Stones-A Path Toward Reducing Infectious Complications During Stone Surgery.

Kidney stones are a common disorder associated with significant morbidity and often requires surgical intervention. Pathogenic bacteria are found in almost 40% of stones, where they form biofilms that are protected from systemic antibiotic treatments. Stone surgeries disperse biofilms resulting in up to 30% of patients developing postoperative urinary tract infections and 15% developing sepsis. This work is based on the hypothesis that chitosan, an antimicrobial polymer, can eradicate bacterial biofilms present in the stone and potentially serve as an adjunct to irrigation during stone surgery

Impact of rapid identification by MALDI-TOF MS from positive blood cultures in Enterococcus spp. bloodstream infections.

PURPOSE: Regarding bloodstream infections (BSI) Enterococcus spp. rank among the top five most common organisms. Due to enterococci intrinsic resistance, empiric antibiotic therapy is often inappropriate and early identification becomes crucial. Our objective was to assess the clinical impact of MALDI-TOF identification directly from positive blood cultures (BC) in Enterococcus spp. BSI (E-BSI). METHODS: A retrospective cohort study included all adult patients with E-BSI from 2010 to 2017 in a tertiary hospital. ID consultation within 48 h and MALDI-TOF identification directly from BC within

Antibiotic resistance of urinary pathogens after kidney transplantation: a 10-year single-center survey in Germany.

PURPOSE: Urinary tract infections (UTIs) are common complications after kidney transplantation (KT), often resulting in severe outcomes like acute graft failure and sepsis. Factors such as diabetes, age, sex, and type of transplantation significantly influence disease progression. Rising antibiotic resistance complicates treatment, emphasizing the importance of Antimicrobial Stewardship (AMS), particularly during the post-transplant immunosuppression phase. Recent changes in treatment protocols, including a shift away from treating asymptomatic bacteriuria and modifications in antibiotic presc

Accurate identification of Enterococcus lactis causing bacteraemia by matrix-assisted laser desorption ionization-time of flight mass spectrometry.

Introduction. Enterococcus faecium clade B has recently been re-classified as Enterococcus lactis. Although E. lactis was previously associated with food products and probiotics, the recent re-classification has prompted the need for the accurate identification of this species and re-interpretation of its disease-causing ability. Since the re-classified E. lactis can currently only be identified by molecular techniques such as whole-genome sequencing, we constructed a MALDI Biotyper® custom database to rapidly identify and differentiate E. lactis causing bacteraemia from E. faecium.Hypothesis/

The relationship between blood culture time-to-positivity and Enterococcus faecalis infective endocarditis.

Infective endocarditis (IE) can be difficult to diagnose due to non-specific symptoms and the imperfect sensitivity of blood cultures and echocardiography. The interval between blood culture collection and the first detection of bacterial growth is known as time-to-positivity (TTP) and is considered a proxy for bacterial load. Several studies have demonstrated a shorter TTP in staphylococcal IE compared to staphylococcal bacteraemia of other sources; however, evidence in enterococcal IE is limited. We characterise the relationship between blood culture TTP and a diagnosis of Enterococcus faeca
