INTRODUCTION: Xerophthalmia, caused by vitamin A deficiency, is a significant public health problem that can lead to severe ocular surface damage, including keratomalacia and permanent blindness if not promptly treated.
CASE: A 26-day-old male neonate presented with bilateral corneal clouding ten days after birth. Ocular examination revealed corneal xerosis in both eyes and a corneal perforation in the right eye, which was sealed with iris tissue. The mother reported a history of night blindness during the third trimester of pregnancy, suggestive of maternal vitamin A deficiency.

BACKGROUND: The influence of pre-pregnancy body mass index (BMI) and gestational weight gain (GWG) on perinatal outcomes of women with polycystic ovary syndrome (PCOS) remains unclear. Therefore, we explored how the above indicators influence pregnancy outcomes in women with PCOS.
METHODS: A retrospective study was conducted involving the baseline characteristics, laboratory data, and pregnancy outcomes of 722 pregnant women with PCOS. Subjects were grouped in a way to find out risks in their pregnancy outcomes. Multivariable logistic regression analysis was performed to investigate how BMI a

Vitamin A deficiency is an uncommon but serious complication of malabsorptive bariatric surgery in developed nations, which can manifest several years after the procedure. It can easily be overlooked, as patients often present with nonspecific signs and symptoms early in the disease course, and ocular manifestations typically appear once the deficiency reaches a severe stage. A female patient in her early 40s presented with acute vision loss 8 years after undergoing biliopancreatic diversion with duodenal switch bariatric surgery. Ophthalmic examination revealed Bitot's spots and progressive k

Vitamin A deficiency (VAD) is a nutritional disorder that is predominantly observed in developing countries due to malnutrition. However, it can also occur in developed countries, particularly in children with neurodevelopmental disorders associated with avoidant and restrictive food intake disorder (ARFID) or malabsorption. The clinical manifestations of VAD include ocular complications, such as night blindness, xerophthalmia, and keratomalacia, as well as systemic effects, including impaired immunity, growth retardation, and increased susceptibility to infections. While there has been notabl

Vitamin A deficiency (VAD) can manifest as xerophthalmia, a spectrum of eye conditions ranging from night blindness to severe outcomes like keratomalacia and corneal scarring. We report two cases of a seven-year-old girl and a 13-year-old boy in Japan with xerophthalmia caused by VAD associated with autism spectrum disorder (ASD). The first case demonstrates severe complications of prolonged VAD, including optic disc edema and irreversible corneal damage, which then results in blindness. The second case identified and treated earlier in the stage of corneal xerosis shows favorable prognosis. T

PURPOSE: To describe the histological findings and results of anterior-segment optical-coherence-tomography (AS-OCT) in patients with severe keratomalacia and corneal perforation due to vitamin-A deficiency (VAD).
OBSERVATIONS: Four patients (3 female, 1 male) with moderate to severe VAD were included in this single-center case series. Keratomalacia/corneal perforation was diagnosed by slit-lamp examination. The findings were documented using photographs and AS-OCT imaging (CASIA-2, Tomey corporation, Nagoya, Japan). Ocular and general medical findings including causes of VAD are reported. VA

While working as an ophthalmologist in a hospital in Tanzania in the 1980s, I became interested in why so many children were blind due to corneal scarring. Keratomalacia (drying and clouding of the cornea) due to vitamin A deficiency had been reported in Asia, but Bitot's spots – normally associated with keratomalacia – were rarely seen in African children. The children's parents often said the eye problems and blindness was due to measles. In addition, herpes simplex keratitis had been reported as a cause of corneal ulceration after measles in Nigeria.

We report a case of a patient with autism spectrum disorder (ASD) and perforated keratomalacia secondary to vitamin A deficiency. A 6-year-old boy complained of difficulty in opening the eyelids. The ocular conjunctiva was hyperemic and keratinized with purulent ocular (eye) discharge. Both corneas showed epithelial defects with hypopyon. The serum vitamin A level was ≤5 IU/dL (normal 97-316), leading to a diagnosis of xerophthalmia and keratomalacia due to vitamin A deficiency. Intramuscular injection of vitamin A (50,000 IU/day), as well as oral administration of multivitamin (containing 2,5

Enamel hypoplasia (EH) is a prevalent developmental defect of teeth that can result from various insults, including prenatal nutrient deficiencies. This study aimed to evaluate the association between prenatal serum retinol deficiency and EH in the deciduous teeth of offspring at 2-y of age. A cohort of 1,450 pregnant women was enrolled, and their prenatal nutritional status was assessed between 12 and 14 wk of gestation. Maternal serum retinol, serum 25-hydroxyvitamin D (25OHD), hemoglobin, body mass index, and birth outcomes, infant feeding practices, family socioeconomic status, and demogra

We report the manifestations of vitamin A deficiency (VAD) in three children with underlying autism of different stages. These children were under developmental paediatrician follow-up for autism, and the VAD was not detected until these children presented to Ophthalmology screening for varying stages of signs and symptoms. On further assessment, all of our patients have VAD secondary to poor dietary intake, as autistic patients are associated with having selective eating habits. In our case series, we discuss the spectrum of xerophthalmia presentations, which can be mild and can manifest as p

Mucormycosis is a rare but serious opportunistic fungal disease characterized by rhino-orbito-cerebral and pulmonary involvement. It is mainly seen in people with secondary immunosuppression, isolated vitamin A deficiency, measles, and AIDS patients. It showed a rise during the second wave of the COVID-19 epidemic in the spring of 2021 in India, especially in diabetic COVID-19 patients. Vitamin A deficiency is known to cause nutritional immunodeficiency and hence leading the way to increased opportunistic fungal, bacterial, and viral infections. In the eye, it causes keratitis, night blindness

BACKGROUND: Keratomalacia refers to an ocular destructive dryness of the cornea that results from severe vitamin A deficiency. It is an ocular condition, usually affecting both eyes. It affects infants, children and women of reproductive age. In developing countries, vitamin A deficiency in the diet and associated keratomalacia are a major cause of childhood blindness.
PURPOSE: To report the clinical finding of a patient with bilateral keratomalacia arising from severe vitamin A deficiency from a malnourished diet.
PATIENTS AND METHODS: Case report.
RESULTS: A two-year-old female with sever

While vitamin A deficiency is a leading cause of blindness globally, it is uncommon in the developed world. Here we describe the unique presentation of a young man in the United States with keratomalacia from vitamin A deficiency related to pancreatic insufficiency. The patient presented with bilateral blurry vision that persisted for two weeks, significant unintentional weight loss, orthostatic hypotension, and profuse diarrhea. Upon slit-lamp examination, bilateral corneal opacities were appreciated. After completing additional testing, it became clear that the patient's corneal opacities we

BACKGROUND: The retinol level and retinol delivery to the placenta may vary depending on various factors involving the mother and newborn. The present study evaluates the factors affecting retinol levels in newborns and the transplacental retinol passage in preterm newborns.
METHODS: In this prospective cohort study, the retinol and retinol binding protein (RBP) in the umbilical cord blood of 44 preterm infants with a gestation age of <30 weeks were studied. Serum retinol and RBP levels were determined using an enzyme-linked immunosorbent assay, and the rate of transplacental retinol passage

Xeropthalmia refers to a range of ocular symptoms caused by vitamin A deficiency (VAD), ranging from night blindness and Bitot's spots to corneal xerosis, ulceration, and keratomalacia, which can lead to blindness. We report two cases of xerophthalmia in children with intellectual disabilities. Ocular examination revealed generalized conjunctival xerosis, corneal xerosis, and dense superficial punctate keratopathy. Both share a history of a strict self-selective diet of mainly rice and noodles. Serum vitamin A levels for these children showed a very low level (<0.10 µmol/L) and were categorize
